혈액투석 환자에게서 발생한 재생불량성 빈혈 1예

Aplastic anemia is a disease characterized by pancytopenia in the peripheral blood and hypocellular bone marrow with the fatty infiltration. The causes of the acquired aplastic anemia were usually related to drugs and immune-related diseases. A 34-years old man was admitted due to general weakness. He has been going hemodialysis for 2 years because of end stage renal disease (ESRD) due to chronic glomerulonephritis. At admission, laboratory tests showed hemoglobin 6.2 g/dL, hematocrit 18.0 %, WBC 5,710mm (neutrophil 71.8%, lymphocyte 19.1%, monocyte 5.9%), reticulocyte 1.9%, platelet 93,000/mm, Fe 12.8 ug/dL, TIBC 204 ug/dL, ferritin 941.47 ng /ml, haptoglobin 0.72 g/L, vitamin B12 508.17 pmol/L, folate 24 ng/mL, total protein 6.54 g/dL, albumin 3.76 g/dL, Alk. pohsphatase 79 IU/L, AST 30.0 IU/L, ALT 39.7 IU/L, total bilirubin 0.57 mg/dl, BUN 79.4 mg/dl and creatinine 10.18 mg/dl. Peripheral blood smear showed the normocytic normochromic anemia with anisocytosis. Anemia was diagnosed but it was irresponsive to the recombinant human erythropoietin (EPO) and blood transfusion. In the bone marrow aspiration smear were there the hypoplastic myeloid and erythroid precursors. The bone marrow biopsy section showed the hypoplasia of all components (10-20%) and the fatty infiltration. We have experienced one case of the idiopathic aplastic anemia in a patient going through hemodialysis and we just report it with documentary records. 저자들은 혈액 투석 중인 환자에서 약물, 면역계 이상등의 원인을 발견할 수 없었음에도 재생 불량성 빈혈이 발생한 1예를 경험하여 문헌 고찰과 함께 보고하는 바이다.

색상 측정 기기를 이용한 복합레진 적층 수복과 단일 수복의 색상 비교 분석

Objectives: This study analyzed the difference in color caused by different thickness in enamel layer of composite resins when applied with single and layering placement technique, and evaluated if the results agreed with the shade guide from the manufacturers to verify reliability of the color matching process of the manufacturers. Materials and Methods: For single composite resin samples, 6 mm diameter and 4 mm thickness cylindrical samples were fabricated using Ceram-X mono (DENTSPLY DeTrey) and CIE L*a*b* values were measured with spectrophotometer. Same process was done for layering compositie resin samples, making 3 dentinal shade samples, 4 mm thickness, for each shade using Ceram-X duo (DENTSPLY DeTrey) and enamel shade resins were layered in 2 mm thickness and CIE L*a*b* values were measured. These samples were ground to 0.2 mm thickness each time, and CIE L*a*b* values were measured to 1 mm thickness of enamel shade resin. Results: Color difference (△E*) between single and layering composite resin was 1.37 minimum and 10.53 maximum when layering thicknesses were between 1 mm and 2 mm and 6 out of 10 same shade groups suggested by manufacturer showed remarkable color difference at anythickness (△E* > 3.3). Conclusion: When using Ceram-X mono and duo for composite resin restoration, following the manufacturer's instructions for chossing the shade is not appropriate, and more accurate information for Ceram-X duo is needed on the variation and expression of the shades depending on the thickness of the enamel. This study was conducted to investigate the effects of horticultural activity types on the development of young children's sociality and creativity. The experimental unit consisted of the experimental group A and B of 26 children, respectively, who are in P University Kindergarten and control group of 26 who are in G kindergarten. They were 5 years old in the same area. The experimental group A was provided a horticultural program for growing plant and group B, for creating. The programs for group A and B were taken twice a week, total 14 times, and standard curriculum for control group was provided at C kindergarten. As of the results, the group A significantly increased the level of sociality compared to the control group, and the group B was significantly increased the level of creativity compared to the control group.

Anti-apoptotic Activity of Ginsenosside Rb₁ in Hydrogen Peroxide-treated Chondrocytes

Chondrocyte apoptosis has been recognized as an important factor in the pathogenesis of osteoarthritis (OA). Hydrogen peroxide(H₂O₂), which produces reactive oxygen species, reportedly induces apoptosis in chondrocytes. The ginsenoside Rb₁(G-Rb₁) is the principal component in ginseng and has been shown to have a variety of biological activities, such as anti-arthritis, anti inflammation, and anti-tumor activities. In this study, we evaluated the effects of G-Rb₁ on the mitochondrial permeability transition(MPT) and caspase-3 activity of chondrocyte apoptosis induced by H₂O₂. Cultured rat articular chondrocytes were exposed to H₂O₂ with or without G-Rb₁ and assessed for viability, MPT, Bcl-xL/Bax expression, caspase-3 activity, and apoptosis. The co-treatment with G-Rb₁ Showed ann inhibition of MPT, caspase-3 activity, and cell death. Additionally, the levels of the apoptotic protein Bax were significantly lower and the levels of the anti-apoptoic protein Bcl-xL were higher compared with H₂O₂ treatment alone. The result of this study demonstrate that G-Rb₁ protects chondrocytes against H₂O₂-induced apoptosis, at least in part via the inhibition of MPT and caspase-3 activity. These results demonstrate that G-Rb₁ is a potentially useful drug fot the treatment of OA patients.

Autogenous tooth transplantation for replacing a lost tooth: case reports

The autogenous tooth transplantation is an alternative treatment replacing a missing tooth when a suitable donor tooth is available. It is also a successful treatment option to save significant amount of time and cost comparing implants or conventional prosthetics. These cases, which required single tooth extraction due to deep caries and severe periodontal disease, could have good results by transplanting non-functional but sound donor tooth to the extraction site.

Protective effect of Rhus verniciflua Stokes extract in an experimental model of postmenopausal osteoporosis

Post-menopausal osteoporosis (PMO) is a major global human health concern. Owing to the need for therapeutic drugs without side effects, natural extracts containing various polyphenolic compounds that may exert estrogenic effects have been studied in depth. Rhus verniciflua Stokes (RVS), which has been used as a traditional herbal medicine for centuries in Korea, was recently revealed to exert estrogenic effects attributable to its bioactive ingredients sulfuretin and butein, which have strong estrogen receptor–binding affinities. In this study, the protective potential of RVS in PMO was evaluated by using an experimental animal model of PMO, which was established by ovariectomy (OVX) of female Sprague Dawley rats. The oral administration of RVS at 20 mg/kg or 100 mg/kg for 8 weeks markedly protected against OVX-induced atrophy of the uterine tube and reversed the elevation in the ratio of serum receptor activator of nuclear factor-kB ligand to osteoprotegerin, which is a marker of disease severity. In addition, RVS inhibited OVX-induced tibia bone loss, activated osteogenic activity, and suppressed osteoclastic activity in the tibial epiphyseal plate, a region of bone remodeling. Collectively, these factors indicated that the oral intake of RVS might be beneficial for the prevention of PMO.

Background: This study aimed to evaluate the effect of iodine restriction on thyroid function in subclinical hypothyroidism (SCH) patients in an iodine-replete area. Subjects & Methods: The study consisted of 146 patients who were diagnosed with SCH. Urinary iodine concentration (UIC) was measured in 82 patients; of these, 20 patients with UIC <300 μg/L were excluded, and 62 patients with UIC =300 μg/L were educated about the restriction of iodine-rich foods. Following the first follow-up visit, these patients were divided into 2 groups based on the UIC level: group A (well controlled iodine intake, UIC <300 μg/L, n=40) and group B (poorly controlled iodine intake, UIC =300 μg/L, n=22). The remaining 64 patients did not restrict iodine rich foods (group C). The 82 patients with measured UICs were reevaluated every 3-6 months. Thyroid function and UIC were measured at each visit. The correlation between serum TSH level and UIC was determined for the 82 patients in whom UIC was measured. Results: Following 3-6 months of iodine restriction, the serum TSH levels significantly decreased in group A (9.0 mU/L to 4.7 mU/L, p <0.01). In addition, the serum free T4 levels in group A significantly increased (1.11 ± 0.23 ng/dL to 1.18 ± 0.17 ng/dL, p <0.05). However, there were no significant changes in serum TSH or free T4 levels in group B and C. Serum TSH levels were significantly correlated with UIC (r=0.33, p <0.01). Conclusion: Iodine restriction may decrease serum TSH levels in SCH patients, and serum TSH levels are strongly correlated with UIC. Therefore, restriction of iodine intake could be a primary treatment option in SCH patients in an iodine-replete area.

As leisure time is extended, staying time at one shopping space is increased, and complex shopping mall are taking center stage as a new type of retail channels by family-focused shopping culture, because the whole family enjoys pleasures at the same time owing to coexistence of shopping space and entertainment facilities in them. As these complex shopping mall have a new concept that is a combination of traditional retail trade and the elements of entertainment, they become a solution of consumers` different needs, changed buying patterns, and the competitive distribution market. Complex shopping malls are evolving into the space of life that offers goods, service, entertainment facilities, and a variety of experience. Moreover, companies using SNS that is a new marketing communication tool have been on the rise. Companies can appeal to consumers, and cut marketing costs through SNS. Companies use SNS as an important means of marketing in order to form, keep and develop relations with consumers. These social network service marketing activities influence complex shopping mall, and the mall form relations with consumers by using Twitter, Facebook, and Blogs. The final purpose of the study is to suggest a marketing strategy of complex shopping mall by departmentalizing consumers depending on customer equity and lifestyle, and finding their characteristics through users` mall image and awareness who use SNS of complex shopping mall. The study conducted an online survey in followers of Twitters operated by A mall and B mall. The questionnaire survey was conducted in October, 2011 in Twitter followers of A mall and the 220 questionnaires returned by followers of A mall were used in statistical analysis. And the 261 questionnaires returned by Twitter followers of B mall from October, 2011 to November, 2011 were used in statistical analysis. The data of this present study was analyzed using Amos 18, SPSS 18.0 program. Confirmatory factor analysis was conducted in order to analyze the validity and reliance of the study parameters and reliance analysis was conducted in order to confirm the consistency of the questions in the criteria. The structured equation model was used for hypothesis testing and the cluster analysis was conducted in order to analyze the lifestyle of SNS users and separate groups and the multiple group analysis was conducted in order to investigate the relationship of each parameter in each cluster. The outcome of the study is summarized as follows. First, Survey of social network service users of complex shopping mall was performed to identify the mall awareness and image. Second, As a result of confirming the effect of mall image on self-congruity, mall image appeared to have had effect on self-congruity. Third, as a result of verifying that positive mall awareness has effect on mall commitment, mall awareness proved not to have had effect on mall commitment. Fourth, as a result of confirming that self-congruity has positive effect on commitment into shopping mall, it appeared to have has significant positive effect. Fifth, as a result of testing of the hypothesis that the higher the customer`s commitment into shopping mall the higher the loyalty, it proved the theory was correct. Sixth, as a result of testing if the loyalty has effect on customer equity, loyalty proved to have had effect on customer equity. Seventh, as a result of testing if awareness of shopping mall has effect on customer equity, awareness was found to have had effect on customer equity. Eighth, this study classified customer group into three categories of fashion-oriented, sociability-oriented and rationality- oriented according to the lifestyle of users of multipurpose shopping mall`s SNS. Implications obtained from the study include the following; In a bid to provide assistance in developing the marketing strategy for complex shopping mall using social network service, social network service users were categorized by characteristic and the difference in using and aware of SNS was identified. This study was aimed at reviewing based on previous studies on SNS and lifestyle at home and abroad but the questions on lifestyle focusing on fundamental SNS users have yet to be developed, causing difficulties in fully describing the lifestyle of SNS users. Thus further study to develop the more specific and reliable questions on lifestyle of SNS users shall continue.
