Primary malignant melanoma of the esophagus is an extremely rare disease which counts for approximately 0.1-0.2% of all esophageal malignancies. It is also a very aggressive disease with a poor prognosis, with its 5yr survival rate ranging from 2.2% - 37.5%. Here we report a case of primary malignant melanoma of the esophagus in a patient whose diagnosis was made relatively early and thus total resection by an Ivor Lewis procedure was possible. A 51 year old Asian woman visited our hospital with epigastric discomfort. The patient had no noticeable medical history. She complained of mild nausea and dyspepsia, but there was no vomiting or weight loss. Physical examination showed no palpable lymph nodes in the head and neck region. There was no tenderness or rebound tenderness on the abdomen. For evaluation of her upper gastrointestinal symptoms, she had the upper endoscopy done. Endoscopy identifi ed a few dark pigmented, polypoid lesions scattered from 22cm to 30cm from the incisor tooth. The lesions were various in size, ranging from 0.2cm to 4-5cm and were fl at, slightly elevated with a irregular base. Biopsy done from two spots showed a well demarcated, slightly elevated and black pigmented tumor. For stage work-up, additional examinations were performed. The chest CT showed no abnormal thickening of the esophagus or any enlarged lymph nodes in the mediastinum. The PET-CT showed a mild focal abnormal uptake in the upper thoracic esophagus. The patient underwent an Ivor-Lewis procedure in June, 2013. Pathologic examination revealed spindle shaped malignant melanocytes with hyperchromated nucleis and black pigmentation. The lesion had invaded the submucosal layer. Thus the patient was diagnosed as esophageal malignant melanoma, stage T2N0M0. She is now under close observation at the out-patient department and has been disease free for 13 months.e

Objective The purpose of this study was to develop a Brooding Scale (BS) and to confirm its psychometric properties. Methods A preliminary questionnaire was developed based on a literature review and face-to-face interviews with healthy subjects. To evaluate reliability and construct validity, a 15-item BS was administered to 124 healthy subjects. Convergent validity was tested by assessing the relationship between the BS and the Ruminative Response Scale (RRS). Discriminant validity was confirmed in 58 patients with schizophrenia. Results The internal consistency for the BS was excellent. An exploratory factor analysis yielded two factors: the emotional (six items) and cognitive (five items) domains, which explained 33.83% and 23.69% of the variance, respectively. The BS total score and scores for factors 1 and 2 showed significant positive correlations with the RRS. The total score and sub-factor scores of the BS were significantly higher in patients with schizophrenia than in healthy subjects. Conclusion The BS can be used as a reliable and valid tool to assess brooding in healthy adults. In addition, it had good discriminant validity for patients with schizophrenia.

The mulberry silkworm, Bombyx mori has been one of the most important domestic animals which have provided with silk fibers for weaving fabrics and a food for a protein and lipid source. In addition, various health improvement effects of diverse silkworm larval powders were reported. Recently we found that steamed and freeze-dried mature silkworm larval powder (SMSP) generated using white-jade (also known as Backokjam) silkworm variety extended healthspan and increased resistance to Parkinson's disease in animal models. Because the colors of cocoons in silkworm varieties were caused by altered signal transduction pathways transporting phytochemicals from intestinal lumens to silk glands, we performed the proximate, amino acid, mineral, carbohydrates, fatty acid, and cholesterol composition analyses of SMSPs of 3 silkworm varieties which were weaving light yellow, golden, and red cocoons. Although most of nutrient compositions among 3 SMSPs were similar, there were significant differences in certain amino acids, minerals, and fatty acid compositions. Red silk silkworm (RS)-SMSP had higher contents of crude proteins and total amino acids than other SMSPs. In addition, the ratio of n-3/n-6 unsaturated fatty acids were higher than the other SMSPs. In contrast Golden silk silkworm (GS)-SMSP had higher ratio of potassium/sodium than the other SMSPs. These nutrient analysis results suggested that 3 SMSPs might have common and unique health improvement effects. Thus, further studies in the functionalities of the 3 SMSPs will reveal unknown their health improvements effects.

Objective. This study aimed to evaluate the relationship between serum immunoglobulin G4 (IgG4) levels and the presence and disease activity of rheumatoid arthritis (RA). Methods. The study enrolled 128 participants (RA, 96; healthy controls, 17; osteoarthritis, 11; and IgG4-related disease, 4) between March 2014 and July 2017. Blood samples were collected prior to the commencement of treatment, and serum IgG4 levels were determined using a nephelometric assay (levels ≥135 mg/dL were considered elevated). The levels of serum IgG4 and the ratio of IgG4/total IgG in patients with RA were compared with those in healthy controls, patients with osteoarthritis, and patients with IgG4-related disease. Furthermore, the relationship between serum IgG4 levels and RA disease activity was evaluated. Results. Among the 96 patients with RA, the mean (±standard deviation) serum IgG4 level was 48.0±45.4 mg/dL; 6 (6.3%) patients had elevated serum IgG4 levels. However, none of the healthy controls or patients with osteoarthritis had elevated serum IgG4 levels. The mean serum IgG4/IgG ratio in patients with RA was 3.5%±2.8% (range, 0.2%∼16.9%). Using Spearman’s correlation coefficient analysis, a significant correlation was found between serum IgG4 levels and the Disease Activity Score-28 with erythrocyte sedimentation rate (r, 0.245; p=0.016). Conclusion. There was an increased frequency of elevated serum IgG4 levels in patients with RA, and the levels were correlated with RA disease activity.

<P>Green and efficient extraction of natural products from biomass is considered an important area of interest in the pharmaceutical and biochemical industries. Recently, deep eutectic solvents (DESs) have been gaining increasing interest as sustainable and safe solvents. In this study, we aimed to provide a practical example using a popular traditional Chinese medicine, <I>Flos sophorae</I>, showcasing the tuneability of DESs as designer solvents to selectively and efficiently extract bioactive compounds from biomass. As a result, a solvent called PG-1 that was tailor-made from a 2 : 5 mixture of <SMALL>L</SMALL>-proline and glycerol using a freeze-drying method was more effective than methanol for extraction of quercetin, kaempferol, and isorhamnetin glycosides from <I>Flos sophorae</I>. With PG-1-based ultrasound-assisted extraction (UAE), operational conditions including the DES content in the extractant, the extractant-to-sample solid ratio, and the ultrasound irradiation time for UAE were statistically optimized using a central composite design combined with a response surface methodology. The resulting extraction method in which 50 mg of sample powder was extracted by UAE for 45 min using 1.00 mL of an aqueous solution containing 90% w/w PG-1 was found to be a greener and more efficient process than common extraction methods such as methanol-based UAE and heat reflux extraction that are generally environmentally harmful. Based on the antioxidant activity measured by the DPPH assay, the tailor-made extractant exhibited an additive activity arising from its component, <SMALL>L</SMALL>-proline. Recovery of the extracted flavonoids from the DES, which was assessed using rutin, since it is the major flavonoid extracted, was 75% with the use of water as an anti-solvent, and could reach as high as 92% with the simple application of C18 solid phase extraction (SPE). In comparison, the recovery efficiency of the anti-solvent method was significantly reduced for the flavonoid glycosides from the real <I>Flos sophorae</I> extracts, while the efficiency of the SPE method was reasonably high (81–87%).
