Sjögren syndrome (SS) is a chronic autoimmune disease characterized by dryness of the mouth and eyes due to lymphocytic infiltration of the exocrine glands. In American European Consensus Group (AECG) criteria, abnormal salivary gland scintigraphy (SGS) result is one of the objective signs of SS and it has been proposed as a valid and non-invasive alternative approach to functional evaluation of salivary gland, especially in the case when unstimulated whole salivary flow is more than 1.5 mL in 15 minutes or other AECG criteria is unmet. Patients with SS are more likely to have the thyroid disease (TD), but this association remains controversial. We present a case of the use of SGS for diagnosis of primary SS and TD in patients with dry mouth and burning sensation of tongue. Through this case, we suggest the usefulness of salivary scintigraphy for screening TD in addition to diagnosis of SS.
가로수 식재 시나리오에 따른 기온 및 미세먼지 저감 효과 분석

This study aims to establish a scenario based on the spacing and arrangement of the roadside trees to reduce heat waves and fine dust in cities that occurred during the urbanization process and to quantitatively analyze the degree of reduction. The ENVI-met 5.0.2v model, a micro-climate simulation program, was used to analyze the degree of improvement in the thermal environment and fine dust according to the roadside tree scenario. As a result of temperature analysis according to street tree spacing, the narrower the distance between roadside trees, the lower the temperature during the day as the number of planted trees increased, and a similar pattern was shown regardless of the distance between roadside trees in the morning and evening. In the case of fine dust emitted from the road, the concentration of fine dust increased slightly due to the increase in roadside trees, but the concentration of sidewalks where people walk increased slightly or there was no difference because of blocking fine dust on trees. The temperature according to the arrangement of street trees tended to decrease as the number of planted trees increased as the arrangement increased. However, not only the amount of trees but also the crown projected area was judged to have a significant impact on the temperature reduction because the temperature reduction was greater in the scenario of planting the same amount of trees and widening the interval of arrangement. In terms of the arrangement, the fine dust concentration showed a difference from the results according to the interval, suggesting that the fine dust concentration may change depending on the relationship between the main wind direction and the tree planting direction. By quantitatively analyzing the degree of thermal environment and fine dust improvement caused by roadside trees, this study is expected to promote policies and projects to improve the roadside environment efficiently, such as a basic plan for roadside trees and a project for wind corridor forests.
Fungal Osteomyelitis of Temporomandibular Joint and Skull Base Caused by Chronic Otitis Media

Chronic otitis media (COM) is a chronic inflammatory disease which affects the middle ear, mastoid cavity. It presents hearing loss, ear pain, dizziness, headache, temporomandibular joint (TMJ) inflammation and intracranial complication. Intracranial complications such as skull base osteomyelitis (SBO) may occur secondary to COM due to transmission of infection by a number of possible routes. SBO is an uncommon condition with a significant morbidity and mortality if not treated in the early stages. We report a-67-year-old male patient with diabetes and untreated COM who presented atypical severe TMJ, periorbital and postmandibular pain. By computerized tomography (CT), magnetic resonance imaging (MRI) and whole body bone scan (WBBS), he was diagnosed with SBO spreading from untreated COM via infective arthritis of TMJ. Through this case, we suggest proper utilization of diagnostic imaging, especially CT or MRI for the early detection of SBO in the case of COM accompanying with the greater risk of infection developments such as diabetes.

Background : The purpose of this study was to evaluate the toxicity and efficacy of high-dose chemotherapy with busulfan, thiotepa and melphalan (BTM) as a myeloablative regimen in allogeneic bone marrow transplantation (allo-BMT) for patients with acute myelogenous leukemia (AML). Methods : Twenty-seven patients with AML were enrolled; Sixteen patients had standard risk (SR) diseases (first complete remission (CR1) and de novo AML) and eleven patients had high risk (HR) diseases (second, or subsequent remission, secondary AML, relapsed, or refractory AML, CR marrow with persisting extramedullary manifestation (chloroma), or hypoplastic acute leukemia). The conditioning regimen included busulfan 4 mg/kg/day for a total dose of 12 mg/kg; thiotepa 250 mg/m2/day for a total dose of 500 mg/m2; and melphalan 50 mg/m2/day for a total dose of 100 mg/m2. Cyclosporine A and short-course methotrexate were used for graft-versus-host disease (GVHD) prophylaxis. Results : The median time to recovery a granulocyte count of 0.5 x 109/L was 14 days (range 10∼25 days) and platelet transfusion independence was 30 days (range 12∼49 days). The major regimen-related toxicities were gastrointestinal-related symptoms including oral mucositis, nausea, vomiting, and diarrhea. All patients experienced oral mucositis (≥ grade 1) and the patients with oral mucositis of equal and greater than grade 3 were 44% in SR and 45% in HR. The toxicities associated with lung, skin, heart and brain were minimal. Three (11%) patients had severe or fatal veno-occlusive disease (VOD). There were five treatment-related death (19%) (hepatic VOD with multiorgan failure (n=3), pneumonia and ARDS (n=2)) within the first 100 days after allo-BMT. There was not a significant difference between SR and HR group (p=0.167). The incidence of acute GVHD equal or greater than grade II was less than 10%. The actual survival at 2 year was 70.4%(95% confidence interval (CI), 54.7%∼86.1%)(SR; 81.3% (95% CI; 63.4∼99.1%) vs HR; 54.6% (95% CI; 28.7∼80.4%), p=0.154). After a median follow-up of 630 days, 18 of 27 (67%, 355∼1062 days) patients are alive without evidence of disease. Three of the 27 patients relapsed (SR; 0% vs HR; 55.6% (95% CI; 19.6∼71.3%), p=0.004). Conclusion : The BTM regimen followed by allo-BMT is associated with acceptable toxicity and appears to have significant activity in patients with AML. It should be used with caution in patients with prior hepatopathy or refractory state who have an increased risk of severe VOD. Busulfan, thiotepa, and melphalan is an effective and alternative myeloablative regimen for patients with AML.(Korean J Med 60:156-166, 2001)

<P>Parkinson's disease (PD) is a well known neurodegenerative disorder characterized by selective loss of dopaminergic neurons in the substantia nigra pars compact (SN). Although the exact mechanism remains unclear, oxidative stress plays a critical role in the pathogenesis of PD. DJ-1 is a multifunctional protein, a potent antioxidant and chaperone, the loss of function of which is linked to the autosomal recessive early onset of PD. Therefore, we investigated the protective effects of DJ-1 protein against SH-SY5Y cells and in a PD mouse model using a cell permeable Tat-DJ-1 protein. Tat-DJ-1 protein rapidly transduced into the cells and showed a protective effect on 6-hydroxydopamine (6-OHDA)-induced neuronal cell death by reducing the reactive oxygen species (ROS). In addition, we found that Tat-DJ-1 protein protects against dopaminergic neuronal cell death in 1-methyl-4-phenyl-1,2,3,6,-tetrahydropyridine (MPTP)-induced PD mouse models. These results suggest that Tat-DJ-1 protein provides a potential therapeutic strategy for against ROS related human diseases including PD.</P>
