Hyun Soo Kim, Jong Woon Cheon, Min Su Kim, Chang Kil Jung, Kyung Rok Kim, Jae Won Choi, Dong Woo Kang and Sun Young Kim1 Departments of Internal Medicine and Pathology1, DongKang Medical Center, Ulsan, Korea Actinomycosis is a chronic suppurative disease and caused by Actinomycosis species, principally Actinomyces israelii, which are part of the normal inhabitant on the mucous membrane of the oropharynx, gastrointestinal tract, and urogenital tract. It usually affects cervicofacial, thoracic and abdominal tissue. Cervicofacial type has the highest percentage of occurrence with 50%. Actinomycosis frequently occurs following dental extraction, jaw surgery, chronic infection or poor oral hygiene. It may also be considered as an opportunistic infection in immunocompromised patients such as malignancy, human immunodeficiency virus infection, diabetes mellitus, steroid usage or alcoholism. But, actinomycosis rarely occurs in adults with normal immunity and rare in the esophagus. We report an unusual case of esophageal actinomycosis which was developed in a patient with normal immunity and improved by therapy with intravenous penicillin G followed oral amoxicillin, and we also reviewed the associated literature.

Original Articles : Clinical significance of occult hepatitis B virus infection in chronic hepatitis C patients

Background/Aims: We investigated the frequency of occult hepatitis B virus (HBV) infection in anti-hepatitis C virus (HCV)-positive individuals and the effects of occult HBV infection on the severity of liver disease. Methods: Seventy-one hepatitis B virus surface-antigen (HBsAg)-negative patients were divided according to their HBV serological status into groups A (anti-HBc positive, anti-HBs negative; n=18), B (anti-HBc positive, anti-HBs positive; n=34), and C (anti-HBc negative, anti-HBs positive/negative; n=19), and by anti-HCV positivity (anti-HCV positive; n=32 vs. anti-HCV negative; n=39). Liver biopsy samples were taken, and HBV DNA was quantified by real-time PCR. Results: Intrahepatic HBV DNA was detected in 32.4% (23/71) of the entire cohort, and HBV DNA levels were invariably low in the different groups. Occult HBV infection was detected more frequently in the anti-HBc-positive patients. Intrahepatic HBV DNA was detected in 28.1% (9/32) of the anti-HCV-positive and 35.9% (14/39) of the anti-HCV-negative subjects. The HCV genotype did not affect the detection rate of intrahepatic HBV DNA. In anti-HCV-positive cases, occult HBV infection did not affect liver disease severity. Conclusions: Low levels of intrahepatic HBV DNA were detected frequently in both HBsAg-negative and anti-HCV-positive cases. However, the frequency of occult HBV infection was not affected by the presence of hepatitis C, and occult HBV infection did not have a significant effect on the disease severity of hepatitis C. Bayesian Estimation of the Ratio of Two Means using Dirichlet Process Prior

In the past, there have been computational difficulties with the model which adapted the Dirichlet process prior. However, using the MCMC, computational difficulties can be solved. In this paper, the populations of interest is the ratio of two exponential populations and two Poisson populations. The Dirichlet process prior is used to provide a nonparametric Bayesian estimate of a ratio of two means. The proposed procedure is applied to some real and artificial examples, and compare it with classical estimator.

율곡 이이(李珥)의 삶에 나타난 가치관과 효정론(孝情論) - 주로 유·소년기를 중심으로 -

율곡은 한국역사에 보기드문 여성삼대(女性三代)로 이어진 사임당(師任堂)의 태교(胎敎)로 천재성이 일찍 드러났으며 가장 인간다운 바탕을 타고 났다. 세 살 때엔 말을 배우면서 글을 익혔고, 다섯 살 때에 천부적인 효자임이 만천하에 널리 알려졌다. 6∼7세에 이르면서 사서(四書) 등 유교경전을 모친으로부터 집중적으로 지도받아 이미 이때에 한문의 문리가 텃다. 7세때에 이웃의 친척이요, 권세가인 진복창을 ‘정의(正義)’에 입각한 의리정신으로 심하게 비판한 순 한문의 『진복창전(陳復昌傳)』을 지어냈다. 그 문장력으로 ‘화석정시(花石亭詩)’도 지었다. 9세에 『이륜행실도(二倫行實圖)』를 읽고 『봉부모형제도(奉父母兄弟圖)』도 그려서 맹세하였다. 11세때에 부친 이원수(李元秀)의 병(病)이 위독했을 때에 외조부(外祖父) 사당에 들어가서 신령(神靈)에게 기도드렸다. 늙고 불쌍한 아버지 목숨을 젊고 재주있는 자신의 목숨으로 바꿔가기 바란다고 간청하면서 기도 드렷다. ‘지성감신(至誠感神)’이란 말이 있듯이 율곡의 효성에 감동한 신령이 역력히 나타타서 하는 말이 “장한 아들을 두었소. 이 아이는 장차 ‘동방의 대유학자(大儒學者)’가 되니, 잘 길러야 하오. 단, 내일 아침에 날이 새면 ‘王’ 변에 ‘耳’ 자(字)를 넣어서 ‘이(珥)’자로 이름(名)을 지으시오.” 과연 그 이튿날에 그대로 실행 하였더니 씻은 듯이 부친의 병(病)은 나았다. 율곡가(栗谷家)의 가족들은 신령에게 감사드리고 이이(李珥)가 유학 공부만 열심히 하면 반드시 대성(大成)할 날이 올 것이라고 그 날을 기다리면서 희망찬 나날을 보냈다. 율곡은 부모에게 효도하고 자신의 성장을 위해서 열심히 공부하였다. 13세에 ‘진사초시’에 장원급제 하였다. 놀라운 일이다. 그러나 율곡은 관직보다는 학문의 길을 걷고 싶었다. 그러나 16세 때에 평소에 우상처럼 생각했고 유학(儒學)을 가르쳐주고 따뜻한 마음(心)을 심어주고 정성 함양에 도움을 준 사임당이 돌연히 별세한다. 삼년상(三年喪-心喪 포함)을 모두 마치고 관례(冠禮)도 마쳤지만 실성(失性)할 정도로 마음의 안정을 찾을 수 없었다. 이 불안심리를 억제치 못하던 차에 강남의 봉은사(奉恩寺)에 가서 불서(佛書)를 읽고 인생의 ‘생사설(生死說)’에 마음이 끌려 금강산에 입산(入山)해서 불교에 귀의하여 철학적으로 심층연구하여 ‘모친의 죽음’에 대해서 알고 싶어서 결국은 효를 위하여 금강산에 입산하였다. Yulgok was the most humane basis as a preacher of Saimdang(師任堂). At the age of three, he learned to learn words, and when he was 5 years old, he was known to be a natural Hyoja(孝子). At the age of six, he instructed Confucian scriptures from his mother and awakened him to Chinese. At the age of seven, he wrote ‘Jinbokchangjeon(陳復昌傳)’ and then ‘Faseokjungsi(花石亭詩)’. At the age of nine, he read the ‘Eryunhangsildo(二倫行實圖)’ and painted ‘Bongbumohyungjedo(奉父母兄弟圖)’. When his father, Lee Won-soo(李元秀), at the age of 11, was ill, he went to his maternal grandfather's temple and prayed to the spirit. He wanted to change the life of his old and miserable father to his own young life. As if there is a saying, “Heavenly willingness will impress the heavens(至誠感天)”, a spirit inspired by Yulgok’s Hyosung appeared. “I have a long son. This child will become a ‘Great Confucian of the East’ in the future and should be raised well. And if the day shall come to pass in the morning, make it ‘Ei(珥)’ your name, and it shall be on the next day, and your father 's sickness shall be cleansed. The people of the Yulgok family gave thanks to the spirit and spent hopeful days. Yulgok worked for his parents and worked hard. At the age of thirteen, he passed the first grade to ‘Jinsachosi(進士初試)’. It was a surprise. But his mother, whom he respected at the age of sixteen, suddenly died. After three years, He became an adult, but he could not find his heart stable enough. When this uneasy mind could not be restrained, He went to Bongeunsa(奉恩寺) Temple in Gangnam(江南), read Buddhist books, went to Mt. Geumgang(金剛山) and rebelled against Buddhism.

An integrated analysis of elbasvir/grazoprevir in Korean patients with hepatitis C virus genotype 1b infection

Background/Aims: In the Republic of Korea, an estimated 231,000 individuals have chronic hepatitis C virus (HCV) infection. The aim of the present analysis was to evaluate the safety and efficacy of elbasvir/grazoprevir (EBR/GZR) administered for 12 weeks in Korean patients who were enrolled in international clinical trial phase 3 studies. Methods: This was a retrospective, integrated analysis of data from patients with HCV genotype (GT) 1b infection enrolled at Korean study sites in four EBR/GZR phase 3 clinical trials. Patients were treatment-naive or had previously failed interferon-based HCV therapy, and included those with human immunodeficiency virus coinfection or Child- Pugh class A cirrhosis. All patients received EBR 50 mg/GZR 100 mg once daily for 12 weeks. The primary endpoint was sustained virologic response at 12 weeks after completion of therapy (SVR12, HCV RNA <15 IU/mL). Results: SVR12 was achieved by 73 of 74 (98.6%) patients. No patients had virologic failure and one discontinued from the study after withdrawing consent. SVR12 rates were uniformly high across all patient subgroups. A total of 16 patients had nonstructural protein 5A resistance-associated substitutions at baseline (16/73, 22%), all of whom achieved SVR12. Adverse events (AEs) reported in >5% of patients were fatigue (6.8%), upper respiratory tract infection (5.4%), headache (5.4%), and nausea (5.4%). Thirteen patients (17.6%) reported drug-related AEs, two serious AEs occurred, and two patients discontinued treatment owing to an AEs. Conclusions: In this retrospective analysis, EBR/GZR administered for 12 weeks was well-tolerated and highly effective in Korean patients with HCV GT1b infection.

서남해안 2.5GW 해상풍력단지 건설의 경제효과분석

MOTIE(Ministry of Trade, Industry & Energy) aims to build a Southwest 2.5GW offshore wind farm. The job will require help from the government, electric operator and manufacturers. The domestic offshore wind farm market will boost global competitiveness up until 2019. This research analyzes the economic impact of the offshore wind farm project. The local acceptance in terms of production, employment and value added are expected to be maximized. According to the quantitative analysis using the extended 2010 input-output table, we calculated that the production inducement impact will reach up to 13.5 trillion won. 11.07 trillion won from the offshore wind farm industry and 2.4 trillion won from others. Furthermore the impact of increase in employment is expected to reach 22,662. 9,883 will be from the wind farm industry and 12,779 from other fields. The impact is expected to be greater especially during the middle of the jobless growth period, Lastly, the value added inducement impact is expected to be 4.24 trillion won. Regarding economic impacts, strategies required for increasing local acceptance are as follows. First, in order to diversify and sophisticate the primary industry into a secondary or tertiary one, we need to develop the nearby wind farm site into a wind farm thema park. Attracting the wind farm production, installing and maintaining/repairing firms and establishing an experience center for K-12 students by providing them an education center for offshore wind energy can be specific means of development. Also, during the operation and after the destruction of facilities, the government should lay out the sub-structure of the wind farm as a sea ranch. Thirdly, the level of local acceptance regarding the community ownership of the offshore wind farm should increase to those of Middelgrunden(Denmark) and Navarra(Spain). Considering the economic condition of Buan, Gochang and the Yeonggwang area, community ownership is one of best solutions for the increment of local tax revenue. At last, considering the supports system of areas adjacent to power plants and ``Partnership management with local community’, the offshore wind farm operator may need to provide assistant policies to nearby communities. The policies should be based on Corporate Social Responsibility(CSR) such as education scholarship projects, local economy cooperative projects, sports/culture projects.

전기 방열기가 국화재배온실의 난방에 미치는 영향

경상대학교 교내에 설치되어 있는 1-2W형 온실을 대상으로 전기 방열기를 이용하여 국화 재배온실의 난방효과를 검토한 결과는 다음과 같다. 실험기간동안 최고, 평균 및 최저 외기온은 각각 -3.8~21.3℃, -5.2~16.1℃ 및 -12.5~14.4℃ 정도의 범위로 나타났으며, 온실 내외의 평균상대습도 각각 43.5~98.6% 및 35.2~100%로 나타났다. 12월 중순부터 2월 상순까지 최저 외기온은 대략 -5.0~-10.0℃ 전후로 나타나 진주기상대의 최근 자료와 비교하면 상대적으로 최저기온이 낮게 나타나는 경향이 있었다. 야간의 경우, 방열기 직하부의 엽온이 방열기 중간 지점에서 측정한 엽온 보다 크게 2~3℃정도 높게 나타나거나 또는 미미하지만 약간 높게 나타나는 경향이 있었다. 근권부의 경우, 직하부나 중간 지점에서의 온도 차이는 거의 없는 것으로 나타났고, 근권부의 최고온도와 기타 최고 온도의 발생 시점을 보면, 약 2시간정도의 지체현상이 있음을 알 수 있었다. 그리고 실험기간동안 난방에 소요된 총 소비전력량, 공급에너지 및 총 난방비는 각각 2,800kWh, 2,408,000kcal 및 112,000won 정도였다. 화석연료인 경유로 난방할 경우, 총 난방비는 224,500won 정도였다. 방열기를 이용하여 난방할 경우, 난방비를 약 50% 정도 줄일 수 있을 것으로 판단되었다. An analysis in heating effects of an electric radiator located in a 1-2W type chrysanthemum (3 cultivars) cultivation greenhouse installed in Gyeongsang National University drew the following conclusions. During the experiment period, the highest, average, and the lowest outside temperatures were in the ranges of -3.8~21.3℃, -5.2~16.1℃ and -12.5~14.4℃, respectively, and the average relative humidity inside and outside the greenhouses were in the ranges of 43.5~98.6% and 35.2~100%, respectively. From mid-December to early February, the lowest outside temperature was recorded as approximately -5.0~-10.0℃, which showed that it tended to be relatively lower than the temperatures recorded at the Jinju Meteorological Observatory. During the night, the leaf temperature measured directly under the radiator tended to be higher by 2~3℃ than that those at the middle point of the radiator, or higher by anegligible amount. In the case of root zone temperature, it was found that there was almost no difference between temperatures of the part directly under and the middle point, and the time when the highest temperature of root zone and other highest temperatures took place showed that there was about a 2-hour delay phenomenon. The total electricity consumption, energy supply and total heating cost during the experiment period were 2,800 kWh, 2,408,000 kcal and 112,000 won, respectively. When diesel, a kind of fossil fuel, was used as heating oil, the total heating cost was around 224,500 won. It was estimated that the total heating cost could be reduced by around 50% if a radiator was used.

일본산 (日本産) 모시목의 (毛翅目) 연구

Studies on aquatic insects along the river in Oo Ma Da Mountain Stream in Yo Shi No Gun of Nara, Japan. It had been studied from April in 1973 to April in 1974 by Jae Won, KIM. teacher of biology at Po Sung high school in Seoul, Korea. Of the sample of aquatic insects, the Larvae and pupa and adults belonging to Trichoptera were 65 species and following species were unrecorded by the other authors up to the present. 1. Rhyacphila sp. RJ(Larvae: new mention species) 2. Adults of Rhyacophila sp. RJ are Rhyacophila kuwamurae TSUDA. 3. Adults of Rhyacophila sp. RG are Rhyacophila impar MARTYNOV. syn. Rhyacopthila tacita TSUDA. 4. Adults of Rhyacophila sp. RI are Rhyacophila kisoensis TSUDA. 5, I have known Genitalia sturucture from pupa of Neophilax sp. NC. 6. Apatania sp.
