ABCB1 c.2677G>T/c.3435C>T diplotype increases the early‑phase oral absorption of losartan

Losartan has been shown to be a substrate of thedrug-efflux transporter MDR1, encoded by the ABCB1 gene. ABCB1 c.2677G>T and c.3435C>T variants are knownto be associated with reduced expression and function ofP-glycoprotein (P-gp). We investigated the effects of ABCB1diplotype on the pharmacokinetics of losartan. Thirty-eighthealthy Korean volunteers with different ABCB1 diplotypes[c.2677G> T and c.3435C>T; carriers of GG/CC(n = 13), GT/CT (n = 12) and TT/TT (n = 13) diplotype]were recruited and administered a single 50 mg oral doseof losartan potassium. Losartan and its active metaboliteE-3174 samples in plasma and urine were collected up to10 and 8 h after drug administration, respectively, and theconcentrations of both samples were determined by HPLCmethod. Significant differences were observed in Cmax oflosartan and losartan plus E-3174 (Lo + E) among the threediplotype groups (both P < 0.01). However, the power ofthe performed test is less than the desired power (0.800). The tmax of losartan and E-3174 in three diplotype groups were also significantly different (both P < 0.01). The AUCvalues of Lo + E were significantly different among thethree diplotype groups until 6 h after losartan administration(P < 0.01). On the contrary, AUC at the periods of 8–10 hand 10 h-infinity of Lo + E were significantly lower in theTT/TT group than in the GG/CC group. Urinary excretionof losartan until 4 h after losartan administration in the TT/TT group was higher than that of the GG/CC group. Theseresults suggest that c.2677G>T/c.3435C>T diplotypes ofABCB1 may significantly increase the early-phase absorptionof losartan, but not the total absorption.

Oxidative stress is closely associated with various diseases and is considered to be a major factor in ischemia. NAD(P)H: quinone oxidoreductase 1 (NQO1) protein is a known antioxidant protein that plays a protective role in various cells against oxidative stress. We therefore investigated the effects of cell permeable Tat-NQO1 protein on hippocampal HT-22 cells, and in an animal ischemia model. The Tat-NQO1 protein transduced into HT-22 cells, and significantly inhibited against hydrogen peroxide (H<sub>2</sub>O<sub>2</sub>)-induced cell death and cellular toxicities. Tat-NQO1 protein inhibited the Akt and mitogen activated protein kinases (MAPK) activation as well as caspase-3 expression levels, in H<sub>2</sub>O<sub>2</sub> exposed HT-22 cells. Moreover, Tat-NQO1 protein transduced into the CA1 region of the hippocampus of the animal brain and drastically protected against ischemic injury. Our results indicate that Tat-NQO1 protein exerts protection against neuronal cell death induced by oxidative stress, suggesting that Tat-NQO1 protein may potentially provide a therapeutic agent for neuronal diseases. Significance of Inflammatory Markers in Diabetic Patients with Stable Coronary Artery Disease

Background/Aims: Patients with diabetes are prone to coronary artery disease (CAD); however, the majority of diabetic patients show normal coronary arteries. We examined differences in the clinical aspects of diabetic patients with insignificant and with significant stenosis of the coronary artery. Methods: A total of 418 consecutive diabetic patients with stable angina who had undergone coronary angiography from January 2004 to March 2007 were included in this study. Patients were subdivided into control and CAD groups and then clinical characteristics and CAD-associated factors were evaluated. Results: A total of 92 (22%) patients were assigned to the control group and 326 (78%) patients were assigned to the CAD group. Using univariate regression analysis, we found that patients with CAD were significantly older (control vs. CAD; 59±21 vs. 64.7±33.7, years, p<0.001), had a longer duration of diabetes (8.2±21.8 vs. 10.2±29.8, years, p=0.027), higher titers of high sensitivity C-reactive protein (hsCRP; 0.3±6.79 vs. 0.9±12.6, mg/dL, p=0.015), and increased hemoglobin A1c (HbA1c) levels (7.1±3.8 vs. 7.5±4.8, %, p=0.007) compared to control patients. Multivariate regression analysis showed that only differences in age, hsCRP, and HbA1c were statistically significant. When patients were subdivided into groups based on hsCRP levels (208 patients in the low group [49.8%], 210 patients in the high group [50.2%]), we found that patients with higher hsCRP levels showed more frequent multivessel disease. Conclusions: In diabetic patients, age, hsCRP, and HbA1c were associated with stable CAD. Among these factors, hsCRP levels were significantly correlated with multivessel involvement in diabetic CAD. Therefore, high hsCRP levels may be a strong predictor for atherosclerotic progression of the coronary arteries in diabetic patients, suggesting that regular screening tests should be performed. This study was conducted to investigate whether treatment with Korean medicine might help to improve symptoms of patients diagnosed with Ramsay Hunt Syndrome. A patient diagnosed with Ramsay Hunt Syndrome was hospitalized for 15 days from October 4, 2018 to October 18, 2018 and treated using Korean medicine (Sinbaro3 pharmacopuncture, acupuncture, electroacupuncture, and herbal medicine). Therapeutic effects were evaluated using the Numerical Rating Scale (NRS), House-Brackmann grade, Yanagihara's score survey, and Digital Infrared Thermographic Imaging (DITI). After treatment, the Numerical Rating Scale score (6 at admission down to 2 points at discharge), House-Brackmann grade (Grade V down to Grade II), Yanagihara's score (6.0 up to 22.0), and Digital Infrared Thermographic Imaging showed improvement from admission to discharge. Treatment with traditional Korean medicine, may help improve symptoms of Ramsay Hunt Syndrome such as facial nerve paralysis, neuralgia after paraplegia, hearing difficulties, and dizziness.

Purpose: Previous studies suggest that the concentration of 25-hydroxyvitamin D [25(OH)D] in cord blood may show an inverse association with respiratory tract infections (RTI) during childhood. The aim of the present study was to examine the influence of 25(OH)D concentrations in cord blood on infant RTI in a Korean birth cohort. Methods: The levels of 25(OH)D in cord blood obtained from 525 Korean newborns in the prospective COhort for Childhood Origin of Asthma and allergic diseases were examined. The primary outcome variable of interest was the prevalence of RTI at 6-month follow-up, as diagnosed by pediatricians and pediatric allergy and pulmonology specialists. RTI included acute nasopharyngitis, rhinosinusitis, otitis media, croup, tracheobronchitis, bronchiolitis, and pneumonia. Results: The median concentration of 25(OH)D in cord blood was 32.0 nmol/L (interquartile range, 21.4 to 53.2). One hundred and eighty neonates (34.3%) showed 25(OH)D concentrations less than 25.0 nmol/L, 292 (55.6%) showed 25(OH)D concentrations of 25.0-74.9 nmol/L, and 53 (10.1%) showed concentrations of ${\geq}75.0$ nmol/L. Adjusting for the season of birth, multivitamin intake during pregnancy, and exposure to passive smoking during pregnancy, 25(OH)D concentrations showed an inverse association with the risk of acquiring acute nasopharyngitis by 6 months of age (P for trend=0.0004). Conclusion: The results show that 89.9% of healthy newborns in Korea are born with vitamin D insufficiency or deficiency (55.6% and 34.3%, respectively). Cord blood vitamin D insufficiency or deficiency in healthy neonates is associated with an increased risk of acute nasopharyngitis by 6 months of age. More time spent outdoors and more intensified vitamin D supplementation for pregnant women may be needed to prevent the onset of acute nasopharyngitis in infants.

A tick survey was conducted to monitor ticks using tick traps attached dry ice and a flagging method at each four sites in Busan, Ulju, and Geoje areas of Gyeongnam Province from April to November, 2018. Haemaphysalis longicornis was the most frequently collected, representing 99.8% in Ulju. A total of 27,247 ticks were collected as H. longicornis (Trap Index; TI 283.2), H. flava (TI 0.3), and Ix. nipponensis (TI 0.3). In the collection environment, a total number of 15,672, 5,798, 4,876, and 901 ticks were collected from glassland, tump, tomb, and mountain path, respectively. On the other hand, H. longicornis and Ix. nipponensis were collected with flaggings at the four sites in Ulju, Busan, and Geoje in May and June, 2018. A total of 1,517 ticks collected H. longicornis (Flagging Index; FI 9.3) and Ix. nipponensis (FI 0.1) in Ulju. Haemaphysalis longicornis were collected 2,183 (FI 13.6) and 1,596 (FI 10.0) in Geoje and Busan, respectively. In the results of the isolation of Severe Fever with Thrombocytopenia Syndrome (SFTS) from the ticks, no pathogens detected from RNA of 557 pools of the ticks using a Polymerase Chain Reaction method in 2018.
