        급성심근경색증의 조기진단에 있어서 Myoglobin의 진단적 의의

        Background: Exact and early diagnosis of acute myocardial infarction (AMI) is essential for the subsequent routine management of this frequent cardiovascular disease. Currently, AMI has been diagnosed using the combination of the history, electrocardiogram(ECG), and biochemical markers of myocardial necrosis. At present, many biochemical markers are used to diagnose AMI. In this study, the predictive values of serum myoglobin and creatine kinase-MB(CK-MB) were compared in the emergency department. Methods: Fifty-four consecutive patients who presented within 12 hours from onset of chest pain of presumed cardiac origin were enrolled into the study. Patients with trauma or renal failure were excluded. The serial serum myoglobin and CK-MB levels were obtained prospectively at admission and 2, 4, 6, 12, 24 and 48 hours after admission. We compare the levels of serum myoglobin and CK-MB within 2, 4, 6, 8, 10, 12, 24, and 48 hours after symptom onset respectively. And we compare the sensitivity, specificity, positive predictive value, and negative predictive value of myoglobin and CK-MB. Results: using World Health Organization criteria, 28 AMI patients were identified. Mean time from symptom onset to presentation was 4.1±1.3 hours. The predictive values of serum myoglobin were better than those of CK-MB within 6 hours after symptom onset. But, 6 hours after symptom onset, the predictive values of CK-MB were better than those of serum myoglobin. The false positive cases of serum myoglobin were 3-one was lung cancer with pleural effusion and the others were unstable angina. The false positive cases of CK-MB were 6-one case was viral myocarditis and the ohters were unstable angina. Conclusion: To compare the serum myoglobin and CK-MB in the diagnosis of AMI, serum myoglobin had better predictive values than CK-MB within 6 hours after symptom onset and useful in the early diagnosis of AMI. But, 6 hours after symptom onset, CK-MB had better predictive values than serum myoglobin and useful in following up.

        자기베어링의 회전정밀도에 영향을 미치는 인자로 PWM 전력증폭기, 위치 센서 등과 같은 자기베어링 구성 장치의 동특성 및 정밀도, 시스템의 정확한 모델링, 제어기법, 런아웃 등이 있다. 본 연구에서는 능동 자기베어링을 제어하기 위해 자기베어링의 PWM 전력증폭기와 회전축을 모델링하고 이를 바탕으로 능동 자기베어링 제어를 위한 PID 제어기를 구성하였으며, 변위 센서의 부착위치 및 회전축의 진원도의 영향으로 발생하는 주기적인 런아웃 요소를 첨가하여 런아웃의 영향을 확인하였으며, 런아웃(Runout)에 의해 발생하는 에러(Error)를 효과적으로 제어하여 자기베어링의 제어 정밀도를 향상시키기 위한 방법으로 기본적인 PID 제어기에 최소평균자승(Least Mean Square, LMS) 알고리즘을 적용한 적응 피드포워드 제어기를 구성하여 자기베어링의 능동 제어에서 발생하는 주기적인 런아웃을 효과적으로 제어할 수 있음을 MATLAB을 통한 시뮬레이션을 통해 확인하였다.

      • CadC Has a Global Translational Effect during Acid Adaptation in Salmonella enterica Serovar Typhimurium

        <B>ABSTRACT</B><P>In <I>Salmonella enterica</I> serovar Typhimurium, the membrane-localized CadC is a transcriptional activator of the <I>cadBA</I> operon, which contributes to the acid tolerance response. Unlike in <I>Escherichia coli</I>, in which transcription of <I>cadC</I> is constitutive, in <I>S. enterica</I> serovar Typhimurium <I>cadC</I> expression is induced by low pH and lysine. Inactivation of <I>cadC</I> suppresses the acid-sensitive phenotype of a <I>cadA</I> mutation, suggesting the existence of other CadC-dependent genes in addition to the <I>cadBA</I> operon. Using a proteomic approach, we identified 8 of the putative CadC-induced proteins and 15 of the putative CadC-repressed proteins. The former include porin proteins OmpC and OmpF. The latter include proteins involved in glycolysis, energy production, and stress tolerance. To better understand the altered levels of OmpC and OmpF, we compared expression of <I>ompR</I> in <I>S. enterica</I> serovar Typhimurium wild-type and <I>cadC</I> mutant strains and determined that CadC exerted a negative influence on <I>ompR</I> transcription. Taken together, our findings strongly suggest that CadC may be a global regulator involved in the OmpR regulatory system during acid adaptation.</P>

        Serum levels of carbohydrate antigen 125 in combination with N-terminal pro-brain natriuretic peptide in patients with acute decompensated heart failure

        Background/Aims: Carbohydrate antigen 125 (CA-125) is an emerging prognostic biomarker for heart failure. We aimed to test the long-term prognostic value of CA-125 in combination with N-terminal pro-brain natriuretic peptide (NT-proBNP) in patients with acute decompensated heart failure (ADHF). Methods: This observational study included a total of 413 patients (64.1 ± 15.6 year-old, 214 men) with ADHF. All-cause mortality during the 2-year follow-up was investigated for the prognosis. Results: During the follow-up (mean follow-up, 591 ± 233 days), 109 deaths (26.0%) were recorded. In the multivariable analysis model, CA-125 was an independent factor associated with all-cause mortality (log CA-125: hazard ratio, 1.23; 95% confidence interval, 1.02 to 1.48; p = 0.030) together with age, sex, New York Heart Association class, β-blocker, and NT-proBNP. The Kaplan-Meier survival analysis demonstrated that the group with both low marker levels showed the best 2-year survival (87.9%) followed by the group with low NT-proBNP and high CA-125 (76.1%), high NT-proBNP and low CA-125 (64.7%) and high NT-proBNP and high CA-125 levels (54.3%) (p < 0.001). Addition of CA-125 in combination with NT-proBNP and established risk factors further increased the predictive power for mortality in patients with ADHF. Conclusions: CA-125 was an independent factor associated with all-cause mortality in patients with ADHF. Combination of CA-125 with NT-proBNP significantly improved the prediction of mortality in patients with ADHF.

        16S rRNA를 이용한 다금바리, 자바리, 능성어 판별법 개발

        다금바리, 자바리 및 능성어는 농어목(Perciformes Order) 바리과(Serranidae Family)에 속하며, 고급횟감으로 인기가 높은 어종이다. 자바리 및 능성어의 경우 몸통부분에 줄무늬가 선명하게 있으나 성어가 되면서 점차 불분명해지는 특징이 있어 무늬의 유무로 인하여 다금바리와 구별하기는 쉽지 않다. 또한 자바리는 제주도에서 다금바리라는 명칭으로 불리고 있으며, 일부 유통업자들이 자바리 및 능성어를 다금바리로 유통시키는 사례가 있어 종 특이 프라이머를 이용한 판별법을 마련하게 되었다. 종 특이 프라이머를 설계하기 위하여 유전자은행(www.ncbi.nlm.nih.gov)에 등록되어있는 다금바리(Accession No. AY947575), 자바리(Accession No. JN603832), 능성어(Accession No. AY947559), 우럭(Accession No. DQ678295) 등의 16S DNA부위의 염기서열을 대상으로 하였으며 비교 및 분석에는 소프트웨어인 BioEdit ver.7.0.9.0 프로그램을 사용하였다. 그 결과 다금바리, 자바리, 능성어를 판별할 수 있는 각각의 NS-003-F/NS-005-R(136 bp), EB-001-F/EB-002-R(181 bp), ES-001-F/ES-001-R(123 bp) 프라이머를 개발하였으며, PCR 조건을 확립하였다. 또한 적용성 검토를 위하여 우럭, 참돔을 대상으로 실시한 결과 비 특이적 밴드가 형성되지 않는 것을 확인하였다. 따라서 본 연구에서 개발된 다금바리 등에 대한 판별법은 시중에 불법적으로 유통 가능성이 있는 제품을 신속하고 과학적으로 판별할 수 있어 식품안전관리에 활용도가 매우 클 것으로 기대된다. Niphon spinosus, Epinephelus bruneus, and Epinephelus septemfasciatus are involved in the Perciformes Order and Serranidae Family. When E. bruneus and E. septemfasciatus are fully grown, the striped pattern on the body gradually disappears. Therefore, morphological classification of adult fishes is quite difficult to identify the differences to N. spinosus. In this study, we investigate the method to differentiate those using PCR. To design the primers, 16S rRNA region of N. spinosus, E. bruneus, and E. septemfasciatus registered in the GeneBank (www.ncbi.nlm.nih.gov) have been used and for the analysis, Bio Edit ver. 7.0.9.0 was used. As a result, it was design NS-003-F/NS-005-R (136 bp), EB-001-F/EB-002-R (181 bp), and ES-001-F/ES-001-R (123 bp) primers for the differentiation of each 3 different fishes. Therefore, the species-specific primer sets would be a useful tool for scientific and speedy differentiation against the illegal distribution for consumer protection.

      • AKR2A-mediated import of chloroplast outer membrane proteins is essential for chloroplast biogenesis

        In plant cells, chloroplasts have essential roles in many biochemical reactions and physiological responses. Chloroplasts require numerous protein components, but only a fraction of these proteins are encoded by the chloroplast genome. Instead, most are encoded by the nuclear genome and imported into chloroplasts from the cytoplasm post-translationally. Membrane proteins located in the chloroplast outer envelope membrane (OEM) have a critical function in the import of proteins into the chloroplast. However, the biogenesis of chloroplast OEM proteins remains poorly understood. Here, we report that an Arabidopsis ankyrin repeat protein, AKR2A, plays an essential role in the biogenesis of the chloroplast OEM proteins. AKR2A binds to chloroplast OEM protein targeting signals, as well as to chloroplasts. It also displays chaperone activity towards chloroplast OEM proteins, and facilitates the targeting of OEP7 to chloroplasts in vitro. AKR2A RNAi in plants with an akr2b knockout background showed greatly reduced levels of chloroplast proteins, including OEM proteins, and chloroplast biogenesis was also defective. Thus, AKR2A functions as a cytosolic mediator for sorting and targeting of nascent chloroplast OEM proteins to the chloroplast.

        Pancreaticopleural fi stula is a rare complication of chronic pancreatitis. The increased pressure of pancreatic duct is caused to posterior disruption of pancreatic duct into retroperitoneal space, leading to the formation of fistulous tract between pleural cavity and pancreas through aortic and esophageal hiatus. One of therapeutic method of pancreaticopleural fi stula is temporary ductal decompression as inserting pancre-atic stent. Pancreatic stent decreases ductal pressure and indirectly reduces the fl uid through pancreaticopleural fi stula. Plastic stent of pancreatic stents is used for ductal decompression and ductal bridging during short period because it is easilly controlled by endoscopist and has less complications. But, it uncommonly makes the complica-tions of occlusion, migration, fracture and so on. The plastic stent fracture of many complications is a serious problem during insertion or removal. When the fractured fragmented tip is continuously remained in pancreatic duct, it is caused to stent oclussion or other complication because of luminal narrowing and increased ductal pressure. we report that iatrogenic fractured fragment of pancreatic plastic stent was suseccfully retrieved with only endoscopic devices.

        Differences in Urine Cadmium Associations with Renal Damage Markers According to the Adjustment with Specific Gravity or Urinary Creatinine

        일반적으로 요중 카드뮴 농도는 요비중 또는 요중 크레아티닌 농도로 보정한 값을 사용해왔다. 그러나 어떤 보정방법이 더 타당한지에 대한 논란은 계속되고 있다. 본 연구에서는 비교적 큰 규모의 일반인구집단을 대상으로 요중 카드뮴농도와 각종 신장손상지표들과의 관련성을 평가함에 있어 요비중 보정 방법과 요중 크레아티닌 보정 방법 중 어느 방법이 더 타당한지 비교 평가하였다. 1,086명의 자원자 중 신장질환의 질병력이 있는 사람을 제외한 862명이 최종적으로 연구대상에 포함되었다. 대상자들로부터 측정한 요중 카드뮴 농도 및 malondialdehyde (MDA), N-acetyl-β-D-glucosaminidase 농도, 혈중 크레아티닌을 이용하여 산출한 사구체여과율 등의 신장손상지표들간의 관련성을 평가하였다. 연구 결과, 요중 크레아티닌 농도보다는 요비중으로 보정한 카드뮴 농도가 각종 신장손상지표와 높은 상관성이 있음을 보여주었다. 특히, 요비중 보정 카드뮴 농도는 요중 MDA 농도와 양의 상관관계를, 사구체여과율과는 음의 상관관계를 보여주었다. 이러한 결과는 일반인구집단에서 카드뮴 노출이 많아질수록 사구체여과율이 감소함을 의미하며 이러한 기전에서 산화적스트레스가 관여하고 있음을 보여준다. 또한, 사구체여과율이 카드뮴 노출에 의한 유용한 신장손상지표 중 하나로 사용될 수 있음을 의미한다. In general, specific gravity (SG) and urinary creatinine (CR) have been used to adjust urinary cadmium (Cd) concentrations. However, the validity of correction methods has been controversial. We compared the two adjustments to evaluate associations between urinary Cd and various renal damage markers and to evaluate the relationship between urinary Cd concentration and renal disease markers, such as estimated glomerular filtration rate (eGFR), in a relatively large general population sample. Among the 1,086 volunteers who were enrolled in this study, 862 healthy volunteers who did not have kidney disease were included in the final analysis. Urinary Cd, malondialdehyde (MDA), and N-acetyl-β-D-glucosaminidase (NAG) concentrations were measured, the creatinine-based eGFR was calculated, and the relationships between these markers were subsequently analyzed. This study showed the use of urinary Cd concentration adjusted with SG rather than with urinary creatinine may be appropriate in studies evaluating renal function based on Cd exposure. Urinary Cd concentration adjusted with SG had a positive correlation with urinary MDA levels and a negative correlation with eGFR. This relationship was relatively stronger in women than in men. This study showed that urinary Cd level was associated with decreased eGFR in the general population, and oxidative stress was likely to act as an intermediator in this process. These results suggest that eGFR can be a very good indicator of kidney damage caused by Cd exposure in the general population.
