Gitelman's syndrome is a rare disorder with a characteristic set of metabolic abnormalities. These include hypokalemia, metabolic alkalosis, hypocalciuria and hypomagnesemia. The clinical manifestations of Gitelman's syndrome are very similar with those of Bartter's syndrome and diuretic abuse. Gitelman' syndrome and Bartter's syndrome can be distinguished by renal clearance study after administration of furosemide and thiazide, since the primary defect in each of these disorders is an impairment in sodium reabsorption in the distal tubule and loop of Henle, respectively. Pseudo-Bartter's syndrome due to diuretic abuse can be distinguished by history of diuretic use and a positive urine assay for diuretics. We have experienced a case of pseudo-Gitelman's syndrome in 56-year old woman with unexplained hypokalemic metabolic alkalosis, hypocalciuria, and hypomagnesemia. The patient denied ingestion of diuretic medication. The results of renal clearance study using furosemide and thiazide in this case suggested the presence of the primary defect of sodium reabsorption in the distal tubule rather than in the loop of Henle. These clinical and laboratory findings were consistent with those of Gitelman's syndrome. But, this case was confirmed as pseudo-Gitelman's syndrome since diuretics were detected by a urine assay for diuretics. Furosemide and hydrochlorothiazide were contained in the pill that she had been taking due to constipation. Herein, we report the case of pseudo-Gitelman's syndrome by abuse of cathartics containig diuretics presented as Gitelman's syndrome in renal clearance study.

어머니가 인식한 부부관계가 유아의 자아탄력성에 미치는 영향에서 양육스트레스의 매개효과

본 연구는 어머니가 인식한 부부관계가 유아의 자아탄력성에 영향을 미치는 과정에서 어머니의 양육스트레스가 매개역할을 하는지 알아보고자 하였다. 연구대상은 경기도에 위치한 유아교육기관 6곳에 재원하고 있는 만 3세 유아 167명과 그들의 어머니였다. 주요 연구결과는 다음과 같다. 첫째, 어머니가 인식한 부부관계와 어머니의 양육스트레스, 어머니의 양육스트레스와 유아의 자아탄력성 간에는 부적 상관이 나타났으며, 어머니가 인식한 부부관계와 유아의 자아탄력성 간에는 정적 상관이 나타났다. 그리고 어머니가 인식한 부부관계는 양육스트레스와 자아탄력성에 유의미한 영향을 미치고 있었으며, 어머니의 양육스트레스도 유아의 자아탄력성에 유의미한 영향을 미치고 있었다. 둘째, 매개변인인 양육스트레스는 어머니가 인식한 부부관계가 유아의 자아탄력성에 미치는 영향력 사이에서 완전매개 역할을 하는 것으로 나타났다. 이러한 결과를 바탕으로, 유아의 자아탄력성이 발달될 수 있도록 어머니가 긍정적인 부부관계를 갖고 양육스트레스가 감소될 수 있도록 도울 수 있는 유아교육기관의 역할에 대해 논의하였다. The purpose of this study was to determine whether the marital relationship perceived by mothers while it affects the ego resilience of the children may mediate the parenting stress from mothers` child breeding. This study`s subjects were 167 three-year-old children attending 6 early childhood education institutions located in Kyeonggi-Do and their mothers. The instruments used in this study were a `relationship inventory`, `parenting stress index/short form`, and `Devereux early childhood assessment`. The main findings of this study were as follows: First, negative correlations were observed between the marital relationship recognized by mothers and the parenting stress from mothers` child breeding, and also between the parenting stress from mothers` child breeding and the ego resilience of the children. By contrast, positive correlations were found between the marital relationship recognized by mothers and the ego resilience of the children. In sequence, the marital relationship recognized by mothers significantly affects the parenting stress from mothers` child breeding and the ego resilience of the children. Second, the parenting stress from mothers` child breeding, a mediator variable, acts as a complete mediator between the marital relationship recognized by mothers and the ego resilience of the children. Based on these findings, we summarized in this study that, in order to develop children`s ego resilience, early childhood education institutions need to be a mediator for reducing parenting stress from mothers` child breeding and improving marital relationship.
급성 신부전이 합병된 심한 횡문근융해증을 유발한 Doxylamine 중독증

Doxylamine succinate is an over-the-counter drug widely used for treating. We experienced a case of severe rhabdomyolysis complicating acute renal failure after doxylamine overdose in a 24-year-old male. The maximum values of creatine kinase and creatinine level during hospitalization were 264,141 IU/L and 8.4 ㎎/dL, respectively. Oliguria on the sixth hospital day and sere treated with hemodialysis. Then, he recovered without any sequelae. To the best of our knowledge, the maximum creatine kinase level of 264,141 IU/L in the present case is the highest value anong the case reports on doxylamine-induced rhabdomyolysis and this is the first case report in Korea of doxylamine-induced severe rhabdomyolysis accompanying oliguric acute renal failure and requiring treatment with hemodialysis.

BACKGROUND:: Bevacizumab plus doublet chemotherapy has become one of the standard treatments for patients with metastatic colorectal cancer (mCRC). We have investigated the efficacy and safety of bevacizumab plus capecitabine and oxaliplatin as first-line chemotherapy for Korean patients with mCRC. METHODS:: Patients were treated with bevacizumab 7.5 mg/kg and oxaliplatin 130 mg/m on day 1 and capecitabine 2000 mg/m/d on days 1 to 14. After 9 cycles, patients were entered into the maintenance treatment, consisting of bevacizumab and capecitabine. Treatment was repeated every 3 weeks and response was evaluated every 2 cycles. RESULTS:: Of the 49 patients (median age, 57 y), 29 (59%) had primary tumors in the colon, and 31 (63%) had metastases in 2 or more organs. Overall response rate was 71.4% (95% confidence interval [CI], 58.7%-84.1%), median progression-free survival was 10.4 months (95% CI, 8.2-12.5 mo), and median overall survival was 20.0 months (95% CI, 16.7-23.4 mo). Frequent grade 3 toxicities included neuropathy (9, 18.4%), neutropenia (8, 16.3%), diarrhea (6, 12.2%), and thrombocytopenia (3, 6.1%). Bevacizumab-related grade 3 or 4 toxicities included proteinuria (1, 2.0%) and bowel perforation (1, 2.0%). CONCLUSIONS:: Bevacizumab plus capecitabine and oxaliplatin was well tolerated and showed promising antitumor activity in Korean patients with mCRC.
