Purpose: To evaluate the prognostic value of preoperative neck lymph node (LN) assessment with $^{18}F$-fluorodeoxyglucose positron emission tomography ($^{18}F$-FDG PET), computed tomography (CT), and magnetic resonance imaging (MRI) in oral cavity squamous cell carcinoma (OSCC) patients with pathologically positive LN. Materials and Methods: In total, 47 OSCC patients with pathologically positive LN were retrospectively reviewed with preoperative $^{18}F$-FDG PET and CT/MRI. All patients underwent surgical resection, neck dissection and postoperative adjuvant radiotherapy and/or chemotherapy between March 2002 and October 2010. Histologic correlation was performed for findings of $^{18}F$-FDG PET and CT/MRI. Results: Thirty-six (76.6%) of 47 cases were correctly diagnosed with neck LN metastasis by $^{18}F$-FDG PET and 32 (68.1%) of 47 cases were correctly diagnosed by CT/MRI. Follow-up ranged from 20 to 114 months (median, 56 months). Clinically negative nodal status evaluated by $^{18}F$-FDG PET or CT/MRI revealed a trend toward better clinical outcomes in terms of overall survival, disease-free survival, local recurrence-free survival, regional nodal recurrence-free survival, and distant metastasis-free survival rates even though the trends were not statistically significant. However, there was no impact of neck node standardized uptake value ($SUV_{max}$) on clinical outcomes. Notably, $SUV_{max}$ showed significant correlation with tumor size in LN (p < 0.01, $R^2$ = 0.62). PET and CT/MRI status of LN also had significant correlation with the size of intranodal tumor deposit (p < 0.05, $R^2$ = 0.37 and p < 0.01, $R^2$ = 0.48, respectively). Conclusion: $^{18}F$-FDG PET and CT/MRI at the neck LNs might improve risk stratification in OSCC patients with pathologically positive neck LN in this study, even without significant prognostic value of $SUV_{max}$.

<P><B>Abstract</B></P> <P> <I>Streptococcus pneumoniae</I> is a major respiratory pathogen that can cause pneumonia, meningitis, and otitis media. Although capsular polysaccharide-based vaccines are commercially available, there is a need for broad-spectrum, serotype-independent, and cost-effective vaccines. Recently, an intranasal vaccine formulated with gamma-irradiated nonencapsulated <I>S. pneumoniae</I> whole cells has been developed and its immunogenicity is under investigation. Since innate immunity influences the subsequent adaptive immunity, in the present study, we investigated the immunostimulatory activity of gamma-irradiated <I>S. pneumoniae</I> (r-SP) in the human bronchial epithelial cell-line, BEAS-2B, by comparing with heat-inactivated <I>S. pneumoniae</I> (h-SP) and formalin-inactivated <I>S. pneumoniae</I> (f-SP). r-SP potently induced interleukin (IL)-6 and IL-8 at both mRNA and protein levels in a dose- and time-dependent manner, whereas h-SP and f-SP poorly induced them. Of note, the mRNA levels of IL-6 and IL-8 were approximately two-fold higher when cells were stimulated with 3 × 10<SUP>7</SUP> CFU/ml of r-SP for 3 h, while the protein levels of IL-6 and IL-8 were approximately five-fold higher after stimulation with 3 × 10<SUP>7</SUP> CFU/ml of r-SP for 24 h. Furthermore, r-SP exhibited potent activation of Toll-like receptor 2 compared with h-SP or f-SP. The expression of IL-6 and IL-8 induced by r-SP was mediated through the activation of mitogen-activated protein kinases. Remarkably, when r-SP was further treated with heat or formalin, there was a decrease in the aforementioned activities. Taken together, we suggest that r-SP stimulates the human respiratory epithelial cells to produce the cytokines IL-6 and IL-8, which might influence the induction of adaptive immune responses.</P> <P><B>Highlights</B></P> <P> <UL> <LI> r-SP induces IL-6 and IL-8 expression more potently than h-SP or f-SP in BEAS-2B cells. </LI> <LI> r-SP phosphorylates MAP kinases more potently than h-SP or f-SP in BEAS-2B cells. </LI> <LI> r-SP strongly activates TLR2 compared with h-SP or f-SP. </LI> <LI> Soluble components of r-SP are responsible for the potent immunostimulatory activity. </LI> </UL> </P>

Objective: The purpose of this study was to increase success rate of sealant targeting C Product/3M Espe, T Product/Kuraray, U Product/Ultradent, and H Product/Ivoclar Vivadent AG as the light-cured sealants with high use frequency in the clinical. Methods: It polymerized halogen curing unit ULTRA-LIRE 5 TUBO (ROLENCE ENTERRISE INC), plasma arc curing unit LIEXTN 685 (3M Espe, USA), and LED curing unit Elipar Freelight 2 (3M Espe, USA), evaluated the compressive strength and micro-leakage in each material according to light wave length, compared difference between each curing unit and material, and then carried out this experiment in order to be conducive to selecting proper material. Results: U Product showed the highest compressive strength in light-source Plasma arc and LED. U Product and C Product showed high compressive strength in Halogen (p<0.001). As for a result of measuring the micro-leakage according to light source in tooth surface depending on a kind of sealant, T Product and U Product showed low micro-leakage in Plasma arc (p<0.001). As for a result of measuring the micro-leakage in tooth surface of sealant according to light source, Plasma arc showed the lowest micro-leakage in C Product, T Product, and U Product. Plasma arc and Halogen showed low micro-leakage in H Product (p<0.001). Conclusion: In consequence of the above experiment, the patients with strong occlusal force in the clinical application of sealant are recommended to polymerize U Product with LED. The patients as children with high probability of micro-leakage are recommended to use T Product as Plasma arc.

<P>Maintaining an appropriate balance between subsets of CD4<SUP>+</SUP> helper T cells and T regulatory cells (Tregs) is a critical process in immune homeostasis and a protective mechanism against autoimmunity and inflammation. To identify the role of vitamin A-related compounds, we investigated the regulation of interleukin (IL)-17-producing helper T cells (Th17 cells) and Tregs treated with all-trans-retinal (retinal). CD4<SUP>+</SUP>T cells or total cells from the spleens of C57BL/6 mice were stimulated under Treg-polarizing (anti-CD3/CD28 and TGF-β) or Th17-polarizing (anti-CD3/CD28, TGF-β, and IL-6) conditions in the presence or absence of retinal. To analyze their suppressive abilities, retinal-induced Tregs or TGF-β-induced Tregs were co-cultured with responder T cells. Collagen-induced arthritis (CIA) was established in interferon (IFN)-γ knockout mice. On day 13, retinal-induced Tregs were adoptively transferred to mice with established CIA after second immunizations. Compared with TGF-β-induced Treg cells, retinal-induced Tregs showed increased Foxp3 expression and mediated stronger suppressive activity. Under Th17-polarizing conditions, retinal inhibited the production of IL-17 and increased the expression of Foxp3.Retinal-induced Tregs showed therapeutic effects in IFN-γ knockout CIA mice. Thus, we demonstrated that retinal reciprocally regulates Foxp3<SUP>+</SUP> Tregs and Th17 cells. These findings suggest that retinal, a vitamin A metabolite, can regulate the balance between pro- and anti-inflammatory immunity. A better understanding of the manipulation of Foxp3 and Tregs may enable the application of this tremendous therapeutic potential in various autoimmune diseases.</P>
Effects of Water Deficit and UV-B Radiation on Accumulation of Functional Metabolites in Crops

With increasing social concerns for healthy food, the studies on the cultivation of crops to increase accumulation of functional metabolites in crops have been investigated. Accumulation of the metabolites in crops is highly affected by various types of stress, such as nutrient deficiency, water deficit (WD), extreme temperature and UV-B radiation as well as their own life cycle. This review summarizes the previous studies on the effects of environmental stresses, especially WD and UV-B radiation, on accumulation of functional metabolites in crops. UV-B radiation and WD during specific period (mainly at maturation stage) activates the adaptation and/or defense system in crops, thereby increasing biosynthesis of the metabolites. Although WD and UV-B radiation tend to decrease in crop yield, the decrease can be compensated by the production of high value crops having high content of functional metabolites.

Background/Aims: Osteoprotegerin (OPG) and fetuin-A are vascular calcification regulators that may be related to high cardiovascular (CV) mortality in hemodialysis (HD) patients. We evaluated the relationship between OPG, fetuin-A, and pulse wave velocity (PWV), a marker of vascular stiffness, and determined whether OPG and fetuin-A were independent predictors of CV events in HD patients. Methods: We conducted a prospective observational study in 97 HD patients. OPG and fetuin-A were measured at baseline and arterial stiffness was evaluated by PWV. All patients were stratified into tertiles according to serum OPG levels. Results: A significant trend was observed across increasing serum OPG concentration tertiles for age, HD duration, systolic blood pressure, cholesterol, triglycerides, and PWV. Multiple linear regression analysis revealed that diabetes (β= 0.430, p = 0.000) and OPG levels (β= 0.308, p = 0.003) were independently associated with PWV. The frequency of new CV events was significantly higher in the upper OPG tertiles compared with those in the lower OPG tertiles. In Cox proportional hazards analysis, upper tertiles of OPG levels were significantly associated with CV events (hazard ratio = 4.536, p = 0.011). Conclusions: Serum OPG, but not fetuin-A, levels were closely associated with increased vascular stiffness, and higher OPG levels may be independent predictors of new CV events in HD patients.
The Relation between Fertilization Practices and Functional Metabolites of Crops

Various researches on the effects of fertilization levels on functional metabolites in crop have been conducted. This review summarizes the previous studies on the relation between fertilization supply and accumulation of metabolites (phenolics, carotenoids, ascorbic acid and glucosinolates) which function as antioxidants in crop. The accumulation of phenolic compounds is related to the activation of phenylalanine ammonia lyase (PAL) in phenylpropanoid pathway. Most of the previous studies discuss that low nitrogen (N) supply activates PAL, thereby increasing the synthesis of phenolics. Similarly, high N supply leads to a decrease in ascorbic acid because of the shading effect derived from the accelerated vegetative growth under high N level. Unlike the phenolics and ascorbic acid, carotenoids are accumulated with increasing N supply. In this regard, the previous studies explain that N is a main element closely associated with formation of key enzyme for the synthesis of carotenoids. Glucosinolates are generally increased under decreasing N supply and increasing S supply. Although the previous studies show similar trends about the accumulation of metabolites by nutrient level, they also suggest that many other factors including crop types, cultivars, cultural environment (water, temperature, light, etc.) influence the accumulation of functional metabolites in crop.
