Outcomes of Palliative Chemotherapy for Ampulla of Vater Adenocarcinoma: A Multicenter Cohort Study

Background/Aims: Palliative chemotherapy (PC) is not standardized for patients with advanced ampulla of Vater adenocarcinoma (AA). This multicenter, retrospective study evaluated first-line PC outcomes in patients with AA. Methods: Patients diagnosed with AA between January 2010 and December 2020 who underwent PC were enrolled from 10 institutions. Overall survival (OS) and progression-free survival (PFS) according to the chemotherapy regimen were analyzed. Results: Of 255 patients (mean age, 64.0±10.0 years; male, 57.6%), 14 (5.5%) had locally advanced AA and 241 (94.5%) had metastatic AA. Gemcitabine plus cisplatin (GP) was administered as first-line chemotherapy to 192 patients (75.3%), whereas capecitabine plus oxaliplatin (CAPOX) was administered to 39 patients (15.3%). The median OS of all patients was 19.8 months (95% confidence interval [CI], 17.3 to 22.3), and that of patients who received GP and CAPOX was 20.4 months (95% CI, 17.2 to 23.6) and 16.0 months (95% CI, 11.2 to 20.7), respectively. The median PFS of GP and CAPOX patients were 8.4 months (95% CI, 7.1 to 9.7) and 5.1 months (95% CI, 2.5 to 7.8), respectively. PC for AA demonstrated improved median outcomes in both OS and PFS compared to conventional bile duct cancers that included AA. Conclusions: While previous studies have shown mixed prognostic outcomes when AA was analyzed together with other biliary tract cancers, our study unveils a distinct clinical prognosis specific to AA on a large scale with systemic anticancer therapy. These findings suggest that AA is a distinct type of tumor, different from other biliary tract cancers, and AA itself could be expected to have a favorable response to PC. (Gut Liver 2024;18:729-736)

Coronary stent dislodgement is a rare complication of percutaneous coronary intervention. We report a rare case of dislodgement of two intracoronary stents. On withdrawal of two balloon catheters, one with a guide wire was mechanically distorted from the left main (LM) to the proximal left anterior descending artery (LAD) while the other was dislodged from the LM to the ostial left circumflex ar-tery. The stent in the LAD could not be retrieved into the guide catheter using a Goose neck snare, because it was caught on a previously deployed stent at the mid LAD. A new stent was quickly deployed from the LM to the proximal LAD, be-cause the patient developed cardiogenic shock. Both stents, including a distorted and elongated stent, were crushed to the LM wall. Stent deployment and crushing may be a good alternative technique to retrieving a dislodged stent.

<P><B>Abstract</B></P><P><I>Brucella abortus</I>, the causative agent of brucellosis, can survive and replicate within host cells. Understanding bacterial virulence factors and bacteria–host cell interactions is critical for controlling brucellosis, yet very little is known about the virulence strategies and signaling pathways activated in phagocytes during infection to ensure their growth and survival. <I>B. abortus</I> was mutagenized by mini-Tn5Km2 transposon mutagenesis to identify virulence genes related to the internalization and intracellular replication of the bacteria. Of the total 2300 mutants used to infect HeLa cells, 23 mutants defective for intercellular growth and the mutated genes were identified. Sequence analysis of DNA flanking the transposon showed various insertion sites in bacterial genes that might be associated with virulence, including genes associated with lipoproteins, amino acid metabolism, translation, transcription, carbohydrate transport, coenzyme transport, inorganic ion transport, energy metabolism, membrane transport, and cell wall/membrane biogenesis. Moreover, mutants were classified into class I, class II and class III as higher, similar, and lower internalization, respectively, into HeLa cells. Furthermore, defective mutants for intracellular growth in HeLa cells were found to be defective in RAW 264.7 cells. Taken together, we suggest that the identified virulence associated genes might contribute to the intracellular growth and survival of <I>B. abortus</I> in phagocytes.</P>
Electron beam scattering device for FLASH preclinical studies with 6-MeV LINAC

In this study, an electron-scattering device was fabricated to practically use the ultra-high dose rate electron beams for the FLASH preclinical research in Dongnam Institute of Radiological and Medical Sciences. The Dongnam Institute of Radiological and Medical Sciences has been involved in the investigation of linear accelerators for preclinical research and has recently implemented FLASH electron beams. To determine the geometry of the scattering device for the FLASH preclinical research with a 6-MeV linear accelerator, the Monte Carlo N-particle transport code was exploited. By employing the fabricated scattering device, the off-axis and depth dose distributions were measured with radiochromic films. The generated mean energy of electron beams via the scattering device was 4.3 MeV, and the symmetry and flatness of the off-axis dose distribution were 0.11% and 2.33%, respectively. Finally, the doses per pulse were obtained as a function of the source to surface distance (SSD); the measured dose per pulse varied from 4.0 to 0.2 Gy/pulse at an SSD range of 20-90 cm. At an SSD of 30 cm with a 100-Hz repetition rate, the dose rate was 180 Gy/s, which is sufficient for the preclinical FLASH studies.

Bovine mastitis is an important disease causing serious economic loss in dairy production and food poison in public health. The major causative agents of bovine mastitis include Escherichia coli (E. coli), Streptococcus agalactiae (S. agalactiae), Staphylococcus aureus (S. aureus). These bacteria were found in milk and environmental condition such as feces, water, soil and so on. Recently, many cases of mastitis are derived from environmental contamination of micro-organisms, which important factors for the spread of this disease in farm. Ultraviolet irradiation (UV) has been used as disinfection for waste and water in clinical and industrial facilities. Moreover the UV irradiation has been used as useful bactericidal agents to remove bacterial biofilms in environmental condition. In this study, we determined the bacterial replication in different percentage of water content (PWC) in sterilized saw dust and feces complexes from farm, and results showed that slightly decreased growth pattern of E. coli and S. agalactiae but increased growth pattern of S. aureus in various PWC (200, 400 and 600%) until 144 h incubation. In the bacteriocidal effect of UV irradiation to bacteria in saw dust and feces complex, the results showed that bacteriocidal effect was depended on the UV irradiation time, irradiation distance and PWC. Especially the antibacterial activity of UV irratiation is stronger in low PWC (50%), long time irradiation (50 sec), and short distance (5 cm) than other condition of this study. Furthermore UV irradiation with stirring showed increased the bactericidal effect compared without stirring. These results suggested that bovine mastitis causing agents may survive long time in environmental condition especially saw dust and feces complexes in farm and can cause a various disease including mastitis. Moreover, these data can be used as basis for application and development of UV disinfection to control of bovine mastitis from environmental contaminated bacteria in dairy farm.
