The development of reliable methods to diagnose acute kidney injury is essential to allow the adoption of early therapeutic interventions and evaluate their effectiveness. Based on the fact that kidney injury molecule-1 (KIM-1) expression levels in kidneys are markedly upregulated early after a damage event, here we developed a noninvasive KIM- 1-based molecular imaging technique to detect kidney injury. First, we took advantage of a phage-display platform to select small peptides demonstrating a specific high binding affinity to KIM-1. The promising candidate was conjugated with fluorescent probes, and its imaging potential was validated in vitro and in vivo. This peptide, with the sequence CNRRRA, not only showed a high imaging potential in vitro, allowing a strong detection of KIM-1 expressing cells by microscopy and flow cytometry but also generated a strong kidney-specific signal in live-imaging in vivo experiments in the context of a drug-induced kidney-injury mouse model. Our data overall suggest that the CNRRRA peptide is a promising probe to use in the context of in vivo imaging for the detection of KIM-1 overexpression in damaged kidneys.

Purpose: The purpose of this study was to understand the degree of depression, sleep quality, job stress and their association among office workers. Method: A self-administered questionnaire was distributed to 147 workers employed in 5 big enterprises, and 5 small and medium enterprises from October 21 to November 21, 2012. The questionnaires included socio-demographic and health-related characteristics, job-related characteristics, sleep quality, job stress (KOSS) and depression (CES-D). Result: Among all subjects, 23.1% was in the depression group (21 points and over in the CES-D score). Depressive level was positively correlated with job stress and quality of sleep. The adjusted odd ratio for the effects of sleep quality and job stress on depression significantly increased in the depression group compared to that of the normal group. Multiple regression analysis revealed that the following factors of influence had combined explanatory powers on depression: BMI, life satisfaction, quality of sleep, and job stress. Conclusion: The study revealed that complicated influences were exerted on the level of depression by variable factors, as well as socio-demographic characteristics, health-related characteristics, quality of sleep, and job stress. Specifically, the level of depression was influenced by the quality of sleep and job stress.

Abstract

Aspergillus tracheobronchitis (ATB) represents a rare disease entity accounting for 3.5% to 5.0% of cases of invasive aspergillosis. Particularly, mass-forming ATB is extremely rare, and clinical reports are limited. Given that the patency of the trachea and bronchi are essential for maintaining sufficient airflow, a central airway mass originating from an Aspergillus infection is likely to be associated with severe clinical manifestations and fatal outcomes. Although several therapeutic options for ATB have been suggested, including medication and airway interventions, the optimal choice of treatment in diverse clinical conditions remains under discussion.

We report a case of ATB that initially manifested as severe dyspnea and total atelectasis of the left lung in a patient with newly detected diabetes.

Radiographic study, bronchoscopy, and pathologic findings of the lesion revealed mass-forming type of ATB.

Interestingly, our patient's symptoms dramatically resolved with voriconazole without further invasive intervention.

This clinical experience highlights the beneficial role of voriconazole in the treatment of rare and potentially fatal cases of ATB.
