Epilepsy in children with a history of febrile seizures

Purpose: Febrile seizure, the most common type of pediatric convulsive disorder, is a benign seizure syndrome distinct from epilepsy. However, as epilepsy is also common during childhood, we aimed to identify the prognostic factors that can predict epilepsy in children with febrile seizures. Methods: The study comprised 249 children at the Korea University Ansan Hospital who presented with febrile seizures. The relationship between the subsequent occurrence of epilepsy and clinical factors including seizure and fever-related variables were analyzed by multivariate analysis. Results: Twenty-five patients (10.0%) had additional afebrile seizures later and were diagnosed with epilepsy. The subsequent occurrence of epilepsy in patients with a history of febrile seizures was associated with a seizure frequency of more than 10 times during the first 2 years after seizure onset (P<0.001). Factors that were associated with subsequent occurrence of epilepsy were developmental delay (P<0.001), preterm birth (P=0.001), multiple seizures during a febrile seizure attack (P=0.005), and epileptiform discharges on electroencephalography (EEG) (P=0.008). Other factors such as the age at onset of first seizure, seizure duration, and family history of epilepsy were not associated with subsequent occurrence of epilepsy in this study. Conclusion: Febrile seizures are common and mostly benign. However, careful observation is needed, particularly for prediction of subsequent epileptic episodes in patients with frequent febrile seizures with known risk factors, such as developmental delay, history of preterm birth, several attacks during a febrile episode, and epileptiform discharges on EEG.

<P>A 6-year-old castrated male Maltese weighing 4.8 kg was presented with a non-healing wound exhibiting purulent discharge after surgery on scar tissue of a chronic twelve-month-old bite wound on the left caudal abdominal region. The dog had previously undergone four surgeries and had been on continuous antibiotic therapy for eight months. Following radiographic and ultrasonographic examinations, the problem was diagnosed as an enterocutaneous fistula of a herniated bowel loop under the skin. Surgical resection of the fistula involving the bowel loop resolved all symptoms.</P>
휨 진동법에 의한 옹이가 있는 소나무재의 비파괴적 인장강도 성능평가

<P>이 연구에서는 마그네틱 드라이버의 휨 진동법에 의하여 옹이가 있는 소나무재의 휨 강도성능을 예측한 전보에 계속하여, 전보와 동일한 비파괴 평가법인 휨 진동법에 의하여 옹이가 있는 소나무재의 인장강도성능을 예측하였다. 옹이를 가진 소나무재의 정목면과 판목면에 대해 마그네틱 드라이버의 휨 진동에 의해 얻어진 공진주파수로부터 계산된 동적탄성율은 정적 인장시험을 통해 얻어진 인장강도성능과 높은 상관관계를 나타내었다. 그 상관계수는 인장탄성율과의 관계에서 유의성 1% 수준의 0.766, 인장강도와의 관계에서 유의성 1% 수준의 0.812의 높은 값을 나타내었다. 그리고 상관계수의 정목면과 판목면사이의 차이는 매우 적었다. 이 값들은 ASTM의 인장면의 폭에 대한 옹이의 직경비율 KO(%)와 인장강도성능과의 관계에 대한 값(r=0.402~0.472)뿐만 아니라 본 실험에서 계산한 시험편 전체 폭에 대한 각 면에 존재한 모든 옹이 직경의 총합의 비율KT(%)와 인장강도성능과의 관계에 대한 값(r=0.649~0.735) 보다 더 높은 값을 나타내었고, 정적인장탄성율과 인장강도사이의 상관계수(r=0.873)보다 약간 적은 값을 나타내었다. 이 결과는 마그네틱 드라이버의 휨 진동에 의해 야기된 공진주파수로부터 얻어진 동적탄성율은 옹이가 있는 소나무재의 인장강도성능을 예측하기 위해 효과적으로 사용가능한 것을 나타낸다.</P>
Biomimetic Liquid-Sieving through Covalent Molecular Meshes

<P>The porin pores of biological cell membranes enable molecules to be sieved out selectively while water molecules traverse the channel in a single file. Imitating this streaming mechanism is a promising way to create artificial liquid-sieving membranes, but ultrathin molecular pores need to be produced in a large membrane format to be functional under high transmembrane pressures. Here we show that a membrane composed of a covalent molecular mesh can filter mixtures of small molecules in a liquid by the porin-like mechanism. Tetrahedral network formers are polymerized layer-by-layer on a nanoporous substrate to yield a thin layer of a covalent molecular network containing an array of molecular meshes grown by a pore-limited mechanism. Each of the meshes exhibits high water permeability, estimated to be greater than 2500 Lm(-2) h(-1). Glucose or larger molecules are selectively sieved out while the solvent and solutes smaller than glucose traverse the mesh.</P>

Background/Aims: Data on the immunoprotective status against measles, mumps, rubella, varicella zoster virus (VZV), hepatitis A virus (HAV), and Epstein-Barr virus (EBV) infection in patients with inflammatory bowel disease (IBD) are still lacking. Therefore, we investigated the seropositivity rates for viral infectious diseases and the associated factors in Korean patients with IBD. Methods: In this retrospective cohort study, serum immunoglobulin G antibody positivity rates against measles virus, mumps virus, rubella virus, VZV, HAV, and EBV viral capsid antigen (VCA) were measured in patients with Crohn’s disease or ulcerative colitis (UC) who first visited the IBD clinic. Seropositivity rates and their associated factors were analyzed. Results: Between January 2016 and December 2018, 263 patients were enrolled (male, 167 [67.3%]; UC, 134 [50.9%]). The median age at serological test was 30 years (interquartile range, 22 to 46). The seropositivity rates were 84.0%, 85.2%, 66.5%, 87.4%, 50.0%, and 93.7% for measles, mumps, rubella, VZV, HAV, and EBV, respectively. Younger age at serological test was associated with seronegative status for measles (adjusted odds ratio [aOR], 0.92; 95% confidence interval [CI], 0.88 to 0.96), VZV (aOR, 0.83; 95% CI, 0.74 to 0.93), and HAV (aOR, 0.93; 95% CI, 0.91 to 0.95). Furthermore, IBD type-UC was associated with seronegative status against VZV (aOR, 0.33; 95% CI, 0.11 to 0.99). Conclusions: Seropositivity rates for common viral infectious diseases in Korean patients with IBD were similar to those of the general population. In the younger age group, protective immunity against measles, VZV, and HAV is required, with proper vaccination, as necessary.
