        Albumin: A Multi-talented Clinical and Pharmaceutical Player

        Albumin is one of the copious and important plasma proteins which takes an active part in transporting therapeutic agents, neutralization of toxins, protection from inflammation, and maintenance of colloidal intravascular osmotic pressure. Moreover, the albumin is also involved in modulating the acid-base balance, maintenance of vascular endothelial integrity, and binding to a range of exogenous and endogenous compounds. The binding capability of the albumin helps in the transportation, delivery, and clearance of drugs and compounds outside of the body. Due to the marked biological and biochemical properties, the albumin is used in critical care and other clinical conditions as a plasma volume expander, replacement of fluids and nutrients loss, and restoration of albumin levels in hypoalbuminemic individuals. Clinical and pharmaceutical scientists are studying it rigorously to address various pathological conditions. The use of human serum albumin in the management of diseased conditions still needs to be more clarified which can be done by following the institutional clinical guidelines established after the mutual consensus of the clinical experts belonging to different medical backgrounds. In this review, the evidence-based clinical utilization of the albumin in different pathological conditions like hemorrhagic shock, decompensated liver cirrhosis, sepsis and septic shock, abdominal and cardiac surgery, acute brain injury, acute respiratory distress syndrome, and Alzheimer’s disease has been discussed. Furthermore, in addition to the role of albumin in cell therapy, the special features of the albumin as a pharmaceutical vehicle like cancer-specific targeting capability, surface modifiability, stealth property, and its use as a formulation stabilizer have been reviewed.

        Anomalies of the Healthcare Sector Using Workplace Safety and Job Satisfaction: A Case Study of Pakistan

        This study aims to investigate the impact of workplace safety on doctors' retention and turnover intention along with job satisfaction as the mediator. A questionnaire-based survey was conducted with 394 medical doctors working in thirty-five hospitals in Pakistan using a structural equation modeling. Results of the study showed that: 1) workplace safety has a significant positive relationship with employee retention; 2) workplace safety has a significant negative relationship with turnover intention; 3) workplace safety has a significant positive relationship with job satisfaction; 4) job satisfaction has a significant positive relationship with employee retention; 5) job satisfaction has a significant positive relationship with turnover intentions; 6) job satisfaction mediates between workplace safety and employee retention; while 7) job satisfaction failed to mediate between workplace safety and turnover intentions. The findings of the study suggest that in a fear-free and safe environment, employees' chance to stay will increases. The study also suggests that dissatisfied employees do not need to leave the organization. There can be other factors that can be explored in future studies. This study also provides a practical implication for the doctors' low retention and high turnover, specifically in the healthcare sector of Pakistan by providing guidelines to the human resource executives to focus on the strategic implementation of workplace safety.

        ON WEAKLY CYCLIC GENERALIZED B-SYMMETRIC MANIFOLDS

        The object of the present paper is to introduce a type of non-flat Riemannian manifold, called a weakly cyclic generalized $B$-symmetric manifold $(WCGBS)_{n}$. We obtain a sufficient condition for a weakly cyclic generalized $B$-symmetric manifold to be a generalized quasi Einstein manifold. Next we consider conformally flat weakly cyclic generalized $B$-symmetric manifolds. Then we study Einstein $(WCGBS)_{n}$ $(n>2)$. Finally, it is shown that the semi-symmetry and Weyl semi-symmetry are equivalent in such a manifold.

        Outcomes of Primary Unilateral Cheiloplasty in Same-Day Surgical Settings

        Background Financial, clinical, and psychological considerations have made same-day surgery an attractive option for a variety of procedures. This article aimed to analyse the postoperative results of same-day primary unilateral cleft nasolabial repair. Methods This study was performed from 2011 to 2014. Unilateral cleft lip patients fulfilling the inclusion criteria were preoperatively classified as mild, moderate, and severe. All patients underwent same-day surgery and were discharged after satisfying the appropriate clinical criteria, receiving thorough counselling, and the establishment of a means of communication by phone. Postoperative outcomes were assessed and stratified according to preoperative severity and the type of repair. Results A total of 423 primary unilateral cleft lip patients were included. Fisher's anatomical subunit approximation technique was the most common procedure, followed by Noordhoff's technique. The postoperative outcome was good in 89.8% of cases, fair in 9.9% of cases, and poor in 0.2% of cases. The complication rate was 1.18% (n=5), and no instances of mortality were observed. The average hospital stay was 7.5 hours, leading to a cost reduction of 19% in comparison with patients who stayed overnight for observation. Conclusions Mild unilateral cleft lip was the most common deformity for which Fisher's anatomical subunit approximation technique was performed in most of the cases, with satisfactory postoperative outcomes. Refinements in the cleft rhinoplasty techniques over the course of the study improved the results regarding cleft nasal symmetry. Single-day primary unilateral cleft cheiloplasty was found to be a cost-effective procedure that did not pose an additional risk of complications.

      • Heat transfer analysis of Carreau–Yasuda nanofluid flow with variable thermal conductivity and quadratic convection

        Brownian motions and Thermophoresis are primary sources of nanoparticle diffusion in nanofluids, having substantial implications for the thermo-physical characteristics of nanofluids. With such a high need, the 2D, laminar MHD (Magnetohydrodynamic) quadratic convective stream of Carreau–Yasuda nano liquid across the stretchy sheet has been reported. The flow is caused by surface stretching. The principal purpose of this extensive study is to enhance thermal transmission. The effects of variable thermal conductivity and heat source are considered as well. The governing boundary layer equations are transmuted using similarity parameters into a series of non-linear ODEs (ordinary differential equations). The bvp4c algorithm is adopted to fix the translated system numerically. The effects of prominent similarity variables over the temperature, velocity and concentration field are graphically visualized and verified via tables. It explored that fluid’s speed diminishes for the more significant inputs of the magnetic coefficient, Brownian motion coefficient and Prandtl number. The thermal efficiency is improved for larger values of thermophoretic constant, varying thermal conductance and heat-generating parameters. The concentration field has proved to be a decreasing function of nanofluid constants.

        Magnetic Field and Permeability Effects on Jeffrey Fluid in Eccentric Tubes Having Flexible Porous Boundaries

        In the present study, we recapitulated the magnetic field and porous medium effects on the peristaltic mechanism of a Jeffrey fluid model observed between two electrically conducting eccentric annuli. The inner annulus is assumed to be rigid and to contain constant velocity across the longitudinal direction of the enclosure; the outer one is considered to be flexible and to experience peristaltic waves travelling down its walls. Moreover, the magnetic field is examined in the direction normal to the pattern in the presence of a porous medium. The flow, meanwhile, is incompressible and follows in an unsteady format. The flow’s descriptive equations are reduced by anticipation of long wave length and small Reynolds number approximations. The resulting governing equations are then handled using analytical techniques. The achieved expressions for the considerable functions are manipulated graphically to determine the influences of various appertaining parameters. Finally, the results are compared with those of previous studies to confirm the validity of the present solutions through tables and graphs. The graphs indicated that the magnetic field increases the pumping rate but that the porosity reduces it.

        ON GENERALIZED W<sub>3</sub> RECURRENT RIEMANNIAN MANIFOLDS

        The object of the present work is to study a generalized W<sub>3</sub> recurrent manifold. We obtain a necessary and sufficient condition for the scalar curvature to be constant in such a manifold. Also, sufficient condition for generalized W<sub>3</sub> recurrent manifold to be special quasi-Einstein manifold are given. Ricci symmetric and decomposable generalized W<sub>3</sub> recurrent manifold are studied. Finally, the existence of such a manifold is ensured by a non-trivial example.

        Measurement of Traffic-Related Air Pollution in Peshawar, Pakistan - A Pilot Study

        This pilot study measured Traffic-Related Air Pollution (TRAP) and calculated the corresponding Air Quality Index (AQI) in Peshawar. Using Libelium wireless sensors, the research measured outdoor TRAP and monitored indoor air quality for 48 days. The maximum outdoors daily mean concentration was 47 μg m-3 for PM1, 90 μg m-3 for PM2.5, 356 μg m-3 for PM10, 258 ppb for SO2, and 219 ppb for NO2, respectively. This corresponds to PM2.5 AQI of 158 (Unhealthy), PM10 AQI of 148 (Unhealthy for Sensitive Groups; USG), SO2 AQI of 181 (Unhealthy), and NO2 AQI of 123 (USG). The maximum daily average concentration for the indoor condition was 31 μg m-3 for PM1, 49 μg m-3 for PM2.5, 78 μg m-3 for PM10, 465 ppb for SO2, and 247 ppb for NO2, respectively. The corresponding AQI was 135 (USG) for PM2.5, 62 (Moderate) for PM10, 254 (Very Unhealthy) for SO2, and 129 (USG) for NO2. Data analysis shows that about 73% of the overall indoor AQI falls in the category of “USG”, while SO2 was the largest contributor to overall AQI. The study concludes that indoor AQI was slightly better than outdoor AQI because of the distance and height from the outdoor location. Moreover, Pakistan’s AQI for PM2.5 exceeds WHO’s 24-hours limit; however, it was relatively better by 23%, 65%, and 170% compared to China, India, and Bangladesh, respectively. In contrast, AQI for SO2 and NO2 was poor as compared to the same countries. The concentration and AQI for traffic-related air pollutants remain unhealthy and sometimes becomes hazardous, which means the sensitive groups are at greater risk.
