Weekly cisplatin or gemcitabine concomitant with radiation in the management of locally advanced carcinoma cervix: results from an observational study

        Arun K Verma,Ashok Kumar Arya,Milind Kumar,Anuj Kumar,Sweety Gupta,DN Sharma,GK Rath 대한부인종양학회 2009 Journal of Gynecologic Oncology Vol.20 No.4

Objective: The use of non-platinum drugs in concurrent chemoradiation in carcinoma cervix has not been well explored and hence a two arm study was planned to compare the outcome of concomitant cisplatin or gemcitabine in locally advanced carcinoma cervix. Methods: Thirty six patients were evaluated in this study for response rates and complications. These patients were divided into two arms, sixteen patients in the cisplatin arm and twenty patients in the gemcitabine arm. Cisplatin and gemcitabine were given as i.v. infusion at doses of 40 mg/㎡ and 150 mg/㎡ respectively for five weeks concomitant with radiotherapy. All patients had received pelvic radiotherapy to a dose of 50 Gy/25 fraction/5 weeks by four field box technique followed by high-dose-rate brachytherapy (3 sessions, each of 7.5 Gy to point A). Results: Median follow up was of 10.4 months (range, 3 to 36 months) and 10.9 months (range, 2 to 49 months) in the cisplatin and gemcitabine arms, respectively. At first follow up, 68.8% in the cisplatin arm and 70% in the gemcitabine arm had achieved complete response (p=0.93). Similar response rates were noted in different stages in both arms. None of the patients except one developed grade 4 toxicity. Similar toxicity profiles were observed in both arms. Local disease control, distant disease free survival and overall survival was 68.8% vs. 70%, 93.8% vs. 85%, 68.8% vs. 60% in the cisplatin and gemcitabine arms, respectively. Conclusion: Weekly gemcitabine had similar disease control and tolerable toxicity profile with cisplatin. Gemcitabine may be used as an alternative to cisplatin in patients with compromised renal function. Ruptured mirror DACA aneurysm: A rare case report and review of literature

Deepak Kumar Singh,Prevesh Kumar Sharma,Arun Kumar Singh,Vipin Kumar Chand 대한뇌혈관외과학회 2023 Journal of Cerebrovascular and Endovascular Neuros Vol.25 No.3

Bilateral distal anterior cerebral artery (DACA) aneurysms also called “kissing aneurysms” or “mirror aneurysm” are extremely rare, accounting for only 0.2% of all intracranial aneurysms. There have only been a few examples of mirror DACA aneurysms reported in the literature. Here, we report a rare case of mirror DACA aneurysm in a middle aged female with its successful clipping. Patient was admitted with severe headache and altered sensorium. Computed tomography (CT) head was suggestive of anterior inter-hemispheric hematoma. Digital subtraction angiography (DSA) was done which was suggestive of two distal anterior cerebral artery aneurysms located at same anatomical position. It was treated through microsurgical clipping. Mirror image DACA aneurysms are rare occurrence. All patients with ruptured DACA aneurysms should have angiography with 3D reconstruction studies. This aids in determining the aneurysm’s morphology and planning treatment accordingly.

Beyond Growth: Does Tourism Promote Human Development in India? Evidence from Time Series Analysis

SHARMA, Manu,MOHAPATRA, Geetilaxmi,GIRI, Arun Kumar Korea Distribution Science Association 2020 The Journal of Asian Finance, Economics and Busine Vol.7 No.12

The present study aims to investigate the impact of tourism growth on human development in Indian economy. For this purpose, the study uses annual data from 1980 to 2018 and utilizes two proxies for tourism growth - tourism receipt and tourist arrivals - and uses human development index calculated by UNDP. The study uses control variables such as government expenditure and trade openness. The study employs auto regressive distributed lag (ARDL) approach to investigate the cointegrating relationship among the variables in the model. Further, the study also explores the causal nexus between tourism sector and human development by using the Toda-Yamamoto Granger non-causality test. The result of ARDL bounds test reveals the existence of cointegrating relationship between human development indicators, government expenditure, trade openness, and tourism sector growth. The cointegating coefficient confirms a positive and significant relationship between tourism sector growth and human development in India. The causality result suggests that economic growth and tourism have a positive impact while trade openness has a negative impact on human development in India. The major findings of this study suggest that tourism plays an important role in the socio-economic development of Indian economy in recent years and the country must develop this sector to achieve sustainable development.

An efficient finite element analysis model for thermal plate forming in shipbuilding

        S.L. Arun Kumar,R. Sharma,S.K. Bhattacharyya Techno-Press 2023 Ocean systems engineering Vol.13 No.4

Herein, we present the design and development of an efficient finite element analysis model for thermal plate forming in shipbuilding. Double curvature shells in the ship building industries are primarily formed through the thermal forming technique. Thermal forming involves heating of steel plates using heat sources like oxy-acetylene gas torch, laser, and induction heating, etc. The differential expansion and contraction across the plate thickness cause plastic deformation and bending of plates. Thermal forming is a complex forming technique as the plastic deformation and bending depends on many factors such as peak temperature, heating and cooling rate, depth of heated zone and many other secondary factors. In this work, we develop an efficient finite element analysis model for the thermo-mechanical analysis of thermal forming. Different simulations are reported to study the effect of various parameters affecting the process. Temperature dependent properties are used in the analysis and the finite element analysis model is used to identify the critical flame velocity to avoid recrystallization of plate material. A spring connected plate is modeled for structural analysis using spring elements and that helps in identifying the resultant shapes of various thermal forming patterns. Finally, detailed simulation results are reported to establish the efficacy, applicability and efficiency of the designed and developed finite element analysis model.

Nanoemulsions loaded Carbopol® 934 based gel for intranasal delivery of neuroprotective Centella asiatica extract: in–vitro and ex–vivo permeation study

        Maneesh Jaiswal,Arun Kumar,Shivani Sharma 한국약제학회 2016 Journal of Pharmaceutical Investigation Vol.46 No.1

The objective of the present study was to develop a formulation for sustained release of phytotherapeutics through nasal membrane for brain targeting (anti- Alzheimer) in order to overcome their low residence time and poor nasal permeability issues. A dual approach containing nanoemulsions within gel (NE-gels) was used to achieve the objectives utilizing biocompatible biomaterials. Nanoemulsions were characterized for particle size and zeta potential. NE-gels loaded with Centella asiatica (CA) extract were developed using Carbopol® 934 and were characterized for rheological properties (spreadability, viscosity), drug release and prolong anti-oxidant profile. In vitro nasal permeation and free radical scavenging activity were performed to investigate their expected sustained pharmacological responses. With increasing Carbopol ® content, viscosity of NE-gels was found to increase up to 6237.3 ± 1.1 cp which adversely influenced the spreadability (3.06 ± 0.98) of the NE-gels. Enhancement in sustained release of the CA extract across the nasal membrane (four fold; up to 48 h) as well as retention time in nasal cavity (four fold; i.e. 48 h) were observed with NEgels in comparison to CA solution (i.e. 12 h). In vitro nasal permeation study showed 10 folds enhancement of percentage drug permeation across the nasal mucosa in comparison to extract solution while three folds increase in free radical scavenging activity was observed by NE-gel (i.e.48 h) in comparison to nanoemulsion (i.e. 24 h), respectively. Extensive in vitro investigation established these nanocarriers as suitable nanoformulations for brain targeting of water insoluble phytochemicals.
