      • Plasma zinc's alter ego is a low-molecular-weight humoral factor

        <P>Mild dietary zinc deprivation in humans and rodents has little effect on blood plasma zinc levels, and yet cellular consequences of zinc depletion can be detected in vascular and other tissues. We proposed that a zinc-regulated humoral factor might mediate the effects of zinc deprivation. Using a novel approach, primary rat vascular smooth muscle cells (VSMCs) were treated with plasma from zinc-deficient (<1 mg Zn/kg) or zinc-adequate (35 mg Zn/kg, pair-fed) adult male rats, and zinc levels were manipulated to distinguish direct and indirect effects of plasma zinc. Gene expression changes were analyzed by microarray and qPCR, and incubation of VSMCs with blood plasma from zinc-deficient rats strongly changed the expression of >2500 genes, compared to incubation of cells with zinc-adequate rat plasma. We demonstrated that this effect was caused by a low-molecular-weight (∼2-kDa) zinc-regulated humoral factor but that changes in gene expression were mostly reversed by adding zinc back to zinc-deficient plasma. Strongly regulated genes were overrepresented in pathways associated with immune function and development. We conclude that zinc deficiency induces the production of a low-molecular-weight humoral factor whose influence on VSMC gene expression is blocked by plasma zinc.         보건관리자의 직능별 직업 및 조직 헌신도에 대한 비교 연구

        This study was carried out in order to confirm the reliability and validity of the commitment inventor developed by Meyer and Allen, and to investigate commitment level of health manager, to compare two different professionals of health manager such industrial health nurse and industrial hygienist to find out some characteristics that have effect on commitment. This survey was done by self -administered questionnaire to 227 respondents as the trainees of Industrial Safety Training Center, KISCO from June to December 1996. Authors classified commitment into two categories : professional commitment and organizational commitment, and these two types of commitments have three different aspects affective, continuance, and normative commitment based on Meyer's inventory. The results were as follows: 1. Items of whole scales of the both type of commitments have three factors that represent three aspects of commitment ; affective, continuance, and normative commitment. This means that each type of commitment was reliable to use as measurement tool of three different aspects of commitment. Classification of items by factor analysis was more consistent in professional commitment than organizational commitment, Among 16 items of organizational commitment, four items were classified into different aspects of commitment with similar factor loading. 2. Commitment level of industrial health nurse was higher than that of industrial hygienist in affective, continuance professional commitment controlled by other characteristics. These differences can be due to other characteristics of specific task and background of health managers that were not used in this study. 3. The level of affective professional commitment was statistically different in age, sex, education level, and that of organizational commitment was statistically different in only two variables such as age, and job tenure. The level of continuance professional and organizational commitments were different in sex group, but past history of employ went have effect on continuance organizational commitment. The level of normative organizational commitment was affected by only age. As the above results, the tools of measurement of commitment developed by Meyer and Allen can be useful to measure the level of commitment of health manager. Three aspects of two types of commitments were influenced by different characteristics of health manager. Authors suggested future study on the affecting variables to the commitment such as background, task of health manager and organizational characteristics.

      • A deep and multiscale network for pavement crack detection based on function-specific modules

        Using 3D asphalt pavement surface data, a deep and multiscale network named CrackNet-M is proposed in this paper for pixel-level crack detection for improvements in both accuracy and robustness. The CrackNet-M consists of four function-specific architectural modules: a central branch net (CBN), a crack map enhancement (CME) module, three pooling feature pyramids (PFP), and an output layer. The CBN maintains crack boundaries using no pooling reductions throughout all convolutional layers. The CME applies a pooling layer to enhance potential thin cracks for better continuity, consuming no data loss and attenuation when working jointly with CBN. The PFP modules implement direct down-sampling and pyramidal upsampling with multiscale contexts specifically for the detection of thick cracks and exclusion of non-crack patterns. Finally, the output layer is optimized with a skip layer supervision technique proposed to further improve the network performance. Compared with traditional supervisions, the skip layer supervision brings about not only significant performance gains with respect to both accuracy and robustness but a faster convergence rate. CrackNet-M was trained on a total of 2,500 pixel-wise annotated 3D pavement images and finely scaled with another 200 images with full considerations on accuracy and efficiency. CrackNet-M can potentially achieve crack detection in real-time with a processing speed of 40 ms/image. The experimental results on 500 testing images demonstrate that CrackNet-M can effectively detect both thick and thin cracks from various pavement surfaces with a high level of Precision (94.28%), Recall (93.89%), and F-measure (94.04%). In addition, the proposed CrackNet-M compares favorably to other well-developed networks with respect to the detection of thin cracks as well as the removal of shoulder drop-offs.

        Profound trigeminocardiac reflex from lingual nerve stimulation: a case report

        Trigeminocardiac reflex (TCR) is a well-known brainstem reflex that manifests as hypotension, bradycardia, dysrhythmia, and asystole when stimulation is applied to a branch of the trigeminal nerve. Most commonly associated with ophthalmic, orbital, and neurologic surgeries, mandibular division and oral cavity variants occur far less frequently. Here, we describe a case of asystolic TCR elicited by lingual nerve stimulation. This case highlights the role of specific anesthetic medications in modulating this phenomenon and reinforces the need for early recognition and clear communication in case of its occurrence. Anesthesia providers must consider discontinuing or avoiding certain medications when clinically appropriate, even during low TCR-risk procedures.

        Forcing the Genie Back in the Bottle: Sociological Change, Institutional Reform, and Health Policy in Thailand

        In 2007, those behind the 2006 coup drafted a new constitution specifically aimed at turning back the political and policymaking clock to the pre-1997 era. However, in the preceding decade a significant transformation of Thai politics had taken place. Specifically, social cleavages had become politicized and particized in ways we have not seen before, and policy-focused, popular party programs had become part and parcel of serious party campaign strategies. Focusing on health policy, we thus argue in this article that institutional reforms have had predictable and observable implications for policymaking in Thailand, but only when considered in the context of changes to the broader social structure and other political conditions. While the 1997 reforms brought about a well-documented shift toward a more centralized, coordinated, and nationally focused policymaking environment, the 2007 reforms have been less successful at reversing that impact. In short, the coup makers are finding it harder than they supposed to force the genie back into the bottle.

        Two-Blade Guillotine Technique for Nipple Graft Harvest

        The nipple-sharing technique for nipple reconstruction offers excellent tissue matching. The method used for nipple graft harvesting determines the quality of the graft and hence, the success of nipple sharing. Here, we described a guillotine technique wherein the nipple is first transfixed with 2 straight needles to stabilise it. Two No. 11 blades are then inserted in the center and simultaneously swept outwards to amputate the distal portion of the nipple. This technique provides good control, resulting in a very evenly cut base. The recipient bed is deepithelialized thinly, and the nipple graft is inset with interrupted 8-0 nylon sutures under magnification. Being a composite graft, it is protected with splint dressings for 6 weeks, and the dressing is regularly changed by the surgeon. The height of the nipple grafts ranges from 4 to 8 mm. This technique was performed in 9 patients with an average follow-up of 2.9 years (range, 1–4.5 years). Apposition between the nipple graft and its bed is crucial for the success of this technique. When correctly applied, we observed rapid revascularization of the graft.
