        Facial Nerve Paralysis in Patients With Chronic Ear Infections: Surgical Outcomes and Radiologic Analysis

        Objectives. The purpose of this study was to investigate the clinical features, radiologic findings, and treatment outcomes in patients of facial nerve paralysis with chronic ear infections. And we also aimed to evaluate for radiologic sensitivities on facial canal, labyrinth and cranial fossa dehiscences in middle ear cholesteatomas. Methods. A total of 13 patients were enrolled in this study. Medical records were retrospectively reviewed for clinical features, radiologic findings, surgical findings, and recovery course. In addition, retrospective review of temporal bone computed tomography (CT) and operative records in 254 middle ear cholesteatoma patients were also performed. Results. Of the 13 patients, eight had cholesteatomas in the middle ear, while two patients exhibited external auditory canal cholesteatomas. Chronic suppurative otitis media, petrous apex cholesteatoma and tuberculous otitis media were also observed in some patients. The prevalence of facial paralysis in middle ear cholesteatoma patients was 3.5%. The most common involved site of the facial nerve was the tympanic segment. Labyrinthine fistulas and destruction of cranial bases were more frequently observed in facial paralysis patients than nonfacial paralysis patients. The radiologic sensitivity for facial canal dehiscence was 91%. The surgical outcomes for facial paralysis were relatively satisfactory in all patients except in two patients who had petrous apex cholesteatoma and requiring conservative management. Conclusion. Facial paralyses associated with chronic ear infections were observed in more advanced lesions and the surgical outcomes for facial paralysis were relatively satisfactory. Facial canal dehiscences can be anticipated preoperatively with high resolution CTs.

        하악 중심위 유도방법에 따른 하악위의 재현성 및 변위량에 관한 비교연구(LEAF GAUGES의 사용을 중심으로)

        The objectives of this study were to compare the position of centric relation guided by means of leaf gauges, bilateral manipulation, chin-point guidance with Lucia jig and self-guided method and the reproducibility, respectively. A Veri-check (Denar Co., Anaheim, California) was employed for examining, and the displacement of position and reproducibility were verified. The following results were obtained. 1. On the sagittal plane, the centric relation guided by means of leaf gauges showed greater posterior displacement than that by means of bilateral manipulation and less than that by means of chin-point guidance with Lucia jig, and self-guided centric relation showed least posterior displacement. The centric relation guided by means of bilateral manipulation showed greater superior displacement than that by means of chin-point guidance with Lucia jig and less than that by means of leaf gauges and self-guided centric relation showed least superior displacement. 2. On the horizontal plane, the centric relation guided by means of chin-point guidance with Lucia jig showed greater posterior displacement than that of bilateral manipulation and less than that leaf gauges, however the self-guided centric relation showed slightly anterior displacement. 3. The anteroposterior displacement measured on sagittal plane and horizontal plane were highly correlated. (p<0.05). 4. The reproducibility of centric relation guided by means of leaf gauges, bilateral manipulation and chin-point guidance with Lucia jig were similar and more reproducible than self-guided centric relation.

        Holographic Interferometry를 이용한 하악 구치부 도재소부 전장관용 금속 구조물의 굴곡성향에 대한 연구

        Flexion of a metal/ceramic fixed partial denture(EPD) frameworks under function can cause fracture of porcelain or deterioration of the cement seal. This study evaluated the flexion characteristics of three-unit mandibular FPD frameworks, repacing the second pre-molar under compressive load(200g, 400g). Testing was accompished with real-time holographic interferometry, using 6 porcelain fused-to metal frameworks. Tested alloys were non-precious alloy(Heracles, Holland), semi-precious alloy(Degudent U, Germany) and precious alloy(Degudent H, Germany). Changes of the fringe patterns according to the heat treatment(porcelain firing cycle), various loads(200g, 400g), occlusal forms(occlusal porcelain veneering, facial porcelain veneering), various alloys and post-soldering units were compared. Dental study model(Nissan dental products, Inc. D51DP-500A, Japan) and six 3-unit metal/ceramic fixed partial denture frameworks were used as experimental materials. 36 holograms were taken on fixed dental study model by using the 10mW He-Ne laser and real-time holographic interferometry. On the basis of this study, the following conclusions can be drawn : 1. In the frameworks for facial porcelain veneering, the semi-precious alloy framework was least deformed and precious alloy framework, non-precious alloy framework orderly before heat treatment, and the deformation was not shown great difference among three alloys after heat treatment and post-soldering. 2. In the frameworks for occlusal porcelain veneering, the precious alloy framework was greatest deformed and the deformation was not difference between semi-precious alloy framework and non-precious alloy framework before, after heat treatment, and the deformation was not shown great difference among three alloys after post soldering. 3. In the non-precious alloy frameworks for facial porcelain veneering and occlusal porcelain veneering, the deformation was greatly decreased after heat treatment and conversely increased after post-soldering. 4. In the semi-precious alloy framework for facial porcelain veneering, the deformation was not detectable after heat treatment and increased after post-solder. And in the frame-work for occlusal porcelain veneering, the deformation was slightly decreased after heat treatment and increased after post-soldering. 5. In the precious alloy framework for facial porcelain veneering, the deformation was greatly decreased after heat treatment and increased after post-soldering, And in the framework for occlusal porcelain veneering, the deformation was greatly decreased after heat treatment and decreased after post-soldering.

        만성 비부비동염에서 Biofilm이 수술 후 경과에 미치는 영향

        Background and ObjectivesZZBacterial biofilm is thought to contribute to the progression and persistence of chronic rhinosinusitis. There are many studies which demonstrate these structures on chronic rhinosinusitis (CRS), but the majority of the studies just focus on the documentation of their presence and the method for identification. There are also a few researches that show the impact of biofilm on postoperative surgical outcomes. In this study, we demonstrated bacterial biofilm in CRS patients and determined whether they affect clinical courses and outcomes after surgery. Subjects and MethodZZThe mucosa of ethmoid bulla was obtained during endoscopic sinus surgery from 21 CRS patients. Fluorescent microscope and scanning electron microscopy (SEM)were used for the determination of biofilm. The outcomes of the surgery were evaluated by endoscope every two weeks for 6 months. Preoperative symptom scores, CT scores, and the post operative results were compared between patients with biofilm and patients without biofilm. ResultsZZBacterial biofilm were seen in 13 (62%) of the 21 CRS patients. A statistical association existed between the presence of biofilm and the worsened preoperative symptom scores,but the preoperative CT scores were not related to the presence of biofilm. Patients with biofilm had long-lasting postoperative mucosal inflammation and a prolonged healing time, and these differences have statistical significance. ConclusionZZThis study showed that the presence of bacterial biofilm were significantly correlated with worsening preoperative symptom scores and the prolonged postoperative recovery time. Therefore, bacterial biofilm may play an important role in the recalcitrant and resistant nature of CRS. Korean J Otorhinolaryngol-Head Neck Surg 2010;53:349-53

        Client Oriented Scale of Improvement의 한글 번역 및 표준화

        Background and Objectives Client Oriented Scale of Improvement (COSI) has been designed to identify client needs, changes in listening ability, and final listening ability in situations important to each client. The aim of this study was to translate COSI into Korean with subsequent linguistic validation and to determine the reliability of the Korean version of the COSI (K-COSI). Subjects and Method An expert panel translated the original version of COSI into the Korean language. A bilingual translator back-translated the translated version into English, which was subsequently compared with the original English version. K-COSI was administered at 14 referral hospitals, to 128 patients with hearing disability after cognitive debriefing. Reliability was assessed using correlation study. Results K-COSI showed an excellent test-retest correlation and high reliability in degree of change (Spearman correlation=0.89, interclass correlation coefficient=0.922). It also showed a fair test-retest correlation and high reliability in final hearing ability (Spearman correlation=0.49, interclass correlation coefficient=0.353) Conclusion K-COSI proved to be highly reliable. The results suggest that the adapted Korean version of COSI is a reliable and valid measure for Korean-speaking patients with hearing loss. Korean J Otorhinolaryngol-Head Neck Surg 2017;60(7):336-41

        This research is to make a decision making code of repair and strengthening for a reasonable decision through the case studies of overseas. First, to make decision making code, the guidelines and code for a decision making of repair and strengthening and facilities state assessment in United States and Japan were investigated and analyzed. According to the investigation and analysis, the guidelines and code to help on the decision of repair and strengthening were existed, and repair and strengthening are done reasonably. Therefore in Korea, decision making code or guidelines on repair and strengthening is needed.

        This paper presents an electrical feature analysis of hysteresis curves in memristor differential and intergral control circuit. After making macro model of the memristor device, electric characteristics of the model such as time analysis, frequency dependent DC I-V curves were performed by PSPICE simulation. Also, we made a circuit of memristor-capacitor based on nano-wired memristor device and analyzed the simulated PSPICE results. Finally, we proposed a memristor based differential or integral control circuit, analyzed hysteresis curve characteristic in the control circuit.
