복막 가성점액종은 원발병소에 관계없이 복막표면과 대망 등에 점액성 물질이 침착되면서 점액성 복수가 복강 내에 축적되는 상태를 말하는 임상적 질환이다. 원인질환으로 매우 드물지만 췌관내 유두상 점액종양이 보고되고 있으며, 시술과 관련된 점액의 누출, IPMN 파열 및 샛길 형성에 의한 복강내 점액누출 등이 유발기전으로 제시되고있다. 저자들은 췌장의 악성 IPMN 점액누출에 의해 발생한 복막 가성점액종 1예를 경험하였기에 문헌고찰과 함께보고하는 바이다. Pseudomyxoma peritonei (PMP) is a rare clinical entity characterized by gelatinous tumor implants within the peritoneal cavity and a large amount of mucinous ascites. Although most PMP cases originate in the appendix, few are associated with pancreatic intraductal papillary mucinous neoplasm (IPMN). IPMNs secrete thick mucin; therefore, they usually present with pancreatic duct dilatation. The presence of IPMN with extra-pancreatic mucin could be a marker of the eventual seeding of tumor outside the primary site. PMP associated with mucin leakage from pancreatic IPMN has been rarely reported. Here, we report the case of a patient with PMP that originated from pancreatic IPMN.

Optimal atomic configurations in a nanoparticle were predicted by genetic algorithm. A truncated octahedron with a fixed composition of 1 : 1 was investigated as a model system. A Python code for genetic algorithm linked with a molecular dynamics method was developed. Various operators were implemented to accelerate the optimization of atomic configuration for a given composition and a given morphology of a nanoparticle. The combination of random mix as a crossover operator and total_inversion as a mutation operator showed the most stable structure within the shortest calculation time. Pt-Ag core-shell structure was predicted as the most stable structure for a nanoparticle of approximately 4 nm in diameter. The calculation results in this study led to successful prediction of the atomic configuration of nanoparticle, the size of which is comparable to that of practical nanoparticls for the application to the nanocatalyst.
난치성 전신성 홍반성 루푸스 환자에 대한 자가조혈모세포이식

Objective: To determine the safety and efficacy of immune ablation followed by autologous hematopoietic stem cell transplantation (HSCT) in refractory systemic lupus erythematosus (SLE). Methods: Three patients who had been refractory to steroid and one or more immunosuppressive drug were included. Peripheral blood stem cells were mobilized with cyclophosphamide (CTX, 3 g/m2), followed by granulocyte-colony stimulating factor (5μg/kg/day). T lymphocytes were depleted from the graft by selection of CD34 positive cells. Conditioning regimens composed of high dose CTX (total dose 200 mg/kg) and anti-thymocyte globulin (total dose 90 mg/kg) in 2 patients and BEAM (BCNU 300 mg/m2, etoposide 200 mg/m2, cytarabine 200 mg/m2, melphalan 200 mg/m2) in 1 patient. Stored stem cells were reinfused 48 hours after conditioning. Results: Among 3 patients, first patient had lupus nephritis, second patient had overlap syndrome combined with rheumatoid arthritis (rhupus) and the last patient had severe thromobocytopenia and intractable polyserositis. No life threatening complications were observed during the treatment. All patients demonstrated rapid and marked improvement in organ function and quality of life just after HSCT. Previous massive proteinuria in first patient was decreased to less than 500 mg/day with 6 months follow up. Rhupus patient satisfied American college of rheumatology 70% response criteria with 3 months follow up. But third patient improved polyserositis dramatically 1 month after HSCT and relapsed at 9 weeks. Conclusion: In refractory SLE patients, HSCT can be performed safely with marked improvement and sustained withdrawal of all immunosuppressive medication. A further randomized trial is needed to confirm the efficacy and durability of remission.

Pernicious anemia is the most common cause of vitamin B12 deficiency and is caused by autoimmune destruction of parietal cell containing gastric mucosa region. This disease is significantly associated with autoimmune endocrinopathies and anti-receptor autoimmune disease. We report here a case of 26-year old man who was diagnosed pernicious anemia with high level of autoantibody titer (antithyroglobulin antibody 1:25,600 and antimicromosomal antibody 1:6,400) indicating subclinical Hashimoto's thyroiditis. Since other autoimmune diseases are frequently associated with pernicious anemia even without evidence of clinical symptoms, screening test for the presence of autoimmune disease should be evaluated in these patients.

Background/Aims. Epidermal growth factors (EGF) is known to activate mitogen activated protein kinase (MAP kinase) in hepatocytes by the route of both Raf-dependent and Raf-indefendent pathways. And this is likely to play important role in normal liver cell growth and regeneration. EGF is also reported as a potent mitogen and one of the angiogenic factors. To elucidate the dynamic changes of the serum concentration of epidermal growth factor in chronic liver disease and its correlation with role of EGF and mechanism of tumor development, this study is intended to employ an ELISA in 38 biopsy-proven cases. Methods '. Sera taken out of 5 patients with chronic persistent hepatitis. 4 patients with chronic active hepatitis, 19 patients with liver cirrhosis, 10 patients with hepato- cellular carcinoma that pathological diagnosis was proven later were tested for EGF employing Quantikine ELISA Kits (R & D Systems Inc. Minneapolis, MN). The statistical analysis was evaluated by student's t-test. Results . EGF concentration was 253.33±69.5pg/ml(Mean±SE) in hepatocellular carcinoma, 246.60±91.19pg/ml(Mean±SE) in chronic active hepatitis, 222.71±115.97pg/ml (Mean±SE) in chronic persistent hepatitis, 141.15±23.12pg/ml(Mean±SE) in liver cirrhosis in orders. Serum EGF concentration in hepatocellular carcinoma was significantly higher than that in liver cirrhosis(p value=0.021695). However, comparing to the remaining other groups, no significant difference was found. Conclusion .' These results support that the reconstruction of the capillary networks in liver cirrhosis resplts in down-regulation of the EGF in comparison to chronic hepatitis. But it is suggested that revaluation of EGF stimulates MAP kinase activity eventually playing in tumorigenesis of the liver with neoangiogenesis.
