성범죄에 따른 의사면허제제 제도 논의 -각국 의료법 비교 및 국내 자체규제제도 분석을 통하여-

[연구배경] 의료인에 의한 성범죄가 최근 사회적 화제로 대두됨에 따라 성범죄자의 의사면허취득자격 박탈 및 성범죄를 저지른 의료인의 의사면허제제에 대한 논의들이 활발히 전개되고 있다. 본 연구는 의료인의 성범죄에 대한 주요 국가들의 현행법을 비교하고, 우리나라 관련단체들의 규제제도 등을 살펴 봄으로서 국내현황 분석 및 발전방향 모색을 목적으로 한다. [연구목적] 성범죄자의 의사면허 취득자격 박탈 및 성범죄를 저지른 의료인의 의사면허제제가 논의되어야 하는 이유는 효과적인 의료전달을 위하여 의사와 환자 간의 관계에 신뢰성이 전제되어야 하기 때문이다. 하지만 국내의 경우 성범죄를 저지른 자의 계속적인 의료행위를 불허하는 제도적 장치가 존재하지 않는다. 이러한 제도적 장치의 부재는 환자들이 의사가 성범죄전과자인지를 확인할 수 있는 방법이 없으므로, 의사에 대한 환자들의 신뢰를 하락시킨다. [연구결과] 성범죄 전과자의 의사 면허에 대한 규정으로, 미국 일부 주와 독일의 경우 성범죄 전과자가 의사 면허를 취득하는 것을 제도적으로 제한할 수 있도록 하고 있다. 이에 더하여 위의 국가들과 일본은 성범죄를 저지른 의사의 의사 면허를 취소할 수 있음을 의료법에 명시하고 있으며, 면허 정지 기간의 탄력적 적용 또한 가능하다. 반면 한국의 경우 성범죄 전과자의 의사 면허 취득을 제한하거나 성범죄를 저지른 의사의 면허를 취소하는 규정은 존재하지 않으며 면허 정지도 1년 범위로 한정되어 있다. 또한 국내에 법제도적인 면 외에 의사협회, 병원협회, 병원윤리위원회와 같은 단체 및 내부규제가 존재하나 규제력은 낮은 수준이다. 따라서 본 논문에서 이루어질 성범죄 전과자의 면허 제제 규정에 대한 논의는 의료계 전반에 대한 불신을 해소하여 효과적인 의료체계를 보장하기 위한 방안 모색에 의의를 둔다.
19세 여성에서 Chlamydia 감염에 의해 발생한 Fitz-Hugh-Curtis Syndrome 1예

Fitz-Hugh-Curtis syndrome (FHCS)은 여성 생식기 감염과 관련하여 간 피막에 염증(간 주위염)이 발생하는 것으로, 골반 내 감염 환자의 4∼14%에서 발생할 수 있는 것으로 알려져 있다. 우상복부에 날카로운 통증 또는 늑막염과 유사한 통증이 특징적이며, 항상 난관염의 증후가 동반되는 것은 아니기 때문에 담낭염이나 신우신염과 같은 질환과 감별이 필요하다. 정확한 발병 기전은 알려져 있지 않지만, Neisseria gonorrhoeae와 Chlamydia trachomatis가 주요 원인균으로 알려져 있다. 저자들은 최근 성경험이 있었던 19세 여성 환자에게서 Chlamydia 감염에 의해 FHCS가 발생된 증례를 경험하였기에 보고하는 바이다. Generally presenting as sharp, pleuritic right upper quadrant pain, usually but not always accompanied by signs of salpingitis. It needs differential diagnosis such as cholecystitis, pyelonephritis. The pathogenesis of FHCS is incompletely understood. But Neisseria gonorrhoeae and Chlamydia trachomatis are thought to be the primary causative germs of FHCS. D-Amphetamine Causes Dual Actions on Catecholamine Release from the Rat Adrenal Medulla

The present study was designed to examine the effect of d-amphetamine on CA release from the isolated perfused model of the rat adrenal gland, and to establish its mechanism of action. D- amphetamine (10~100μM), when perfused into an adrenal vein of the rat adrenal gland for 60 min, enhanced the CA secretory responses evoked by ACh (5.32×10-3 M), excess K+ (5.6×10-2 M, a membrane depolarizer), DMPP (10-4 M, a selective neuronal nicotinic Nn-receptor agonist) and McN-A-343 (10-4 M, a selective M1-muscarinic agonist) only for the first period (4 min), although it alone has weak effect on CA secretion. Moreover, d-amphetamine (30μM) in to an adrenal vein for 60 min also augmented the CA release evoked by BAY-K-8644, an activator of the dihydropyridine L-type Ca2+ channels, and cyclopiazonic acid, an inhibitor of cytoplasmic Ca2+ ATPase only for the first period (4 min). However, in the presence of high concentration (500μM), d-amphetamine rather inhibited the CA secretory responses evoked by the above all of secretagogues. Collectively, these experimental results suggest that d-amphetamine at low concentrations enhances the CA secretion from the rat adrenal medulla evoked by cholinergic stimulation (both nicotininc and muscarinic receptors) as well as by membrane depolarization, but at high concentration it rather inhibits them. It seems that d-amphetamine has dual effects as both agonist and antagonist at nicotinic receptors of the isolated perfused rat adrenal medulla, which might be dependent on the concentration. It is also thought that these actions of d-amphetamine are probably relevant to the Ca2+ mobilization through the dihydropyridine L-type Ca2+ channels located on the rat adrenomedullary chromaffin cell membrane and the release of Ca2+ from the cytoplasmic store
