        A Case of Hidradenitis Suppurativa in a Genetically Confirmed Lowe Syndrome Patient

        Lowe syndrome (LS), also known as oculocerebrorenal syndrome, is an X-linked multisys- temic disorder caused by mutations in OCR L1, which encodes a member of the inositol- 5-phosphatase family. As implied by its name, congenital cataracts, defects in the central nervous system, and renal manifestations are the main symptoms. Early hidradenitis sup- purativa (HS) occurrence in Dent disease 2 (DD2), which is a mild variant of LS and shares the OCR L1 gene mutation, has been reported, although not in LS patients. Here, we report a case of HS in a 17-year-old boy with genetically confirmed LS, which suggests that defects in the OCR L1 gene may contribute to HS pathogenesis.

        Domain Variables as Modifiers

        According to Stanley and Szabó’s Domain Restriction Theory, the phenomenon of quantifier domain restriction occurs due to unpronounced domain variables in the syntactic structure of a sentence. In the literature, these variables have been mostly associated either with a determiner or with a noun. I argue that both claims face serious problems, and propose what I call the Adjunct Restriction Theory, according to which the domain variable is a modifier that acts like a prepositional phrase. My proposal not only avoids all the problems raised for other theories, but also enjoys the following two advantages: it provides a unified analysis of both domain restriction over individuals and domain restriction over events; and it helps to avoid the problem of over-generation of variables, which has been an intractable problem for Stanley and Szabó’s Domain Restriction Theory.

        조갑진균증에서 Potassium Hydroxide (KOH) 도말검사의 진단적 유용성 및 이에 영향을 미치는 임상 및 진료 환경 요인

        Background: Onychomycosis, a common fungal nail infection, particularly affects the elderly. The diagnostic accuracy of a potassium hydroxide (KOH) smear, a primary diagnostic method, can vary based on the examiner’s proficiency. Objective: To evaluate the diagnostic utility of KOH smears in a real-world setting and the influence of clinical and practical factors on onychomycosis. Methods: This study included 381 patients who underwent KOH smear and histopathological staining for nail diseases at Seoul National University Hospital between March 2020 and February 2023. The study evaluated the diagnostic utility of the KOH smear, including its sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic odds ratio. We investigated the influence of factors such as training duration of examiners, prior history of antifungal treatment, and lesion location on diagnostic utility. Results: Of the 381 patients, 139 were male (36.5%) and 242 were female (63.5%). Onychomycosis was the most common diagnosis (59.3%), with 34.1% of patients reporting prior antifungal therapy within the past 6 months. The sensitivity of the monthly KOH smear varied from 16.7% to 70.0% with an annual sensitivity of 48.1%. The specificity ranged from 83.3% to 100.0%, with an annual specificity of 90.3%. While prior antifungal therapy decreased the sensitivity and specificity, no statistical significance was observed. Examiner's experience, measured by training duration, was not associated with sensitivity or specificity. Among patients with a history of antifungal treatment, an increasing number of KOH smear tests showed reduced sensitivity. Conclusion: Variations in the diagnostic utility of the KOH smear test, influenced by various factors, highlight the necessity for cautious interpretation in clinical settings.

      • eLoran Signal Strength and Atmospheric Noise Simulation over Korea

        GPS is the most widely-used Positioning, Navigation, and Timing (PNT) system. Since GPS is an important PNT infrastructure, the vulnerability of GPS to signal jamming has received significant attention. Especially, South Korea has experienced intentional high-power jamming from North Korea for the past three years, and thus realized the necessity of a complementary PNT system. South Korea recently decided to deploy a high-power terrestrial navigation system, eLoran, as a complementary PNT system. According to the plan, the initial operational capability of the Korean eLoran system is expected by 2016, and the full operational capability is expected by 2018. As a necessary research tool to support the Korean eLoran program, an eLoran performance simulation tool for Korea is under development. In this paper, the received signal strength, which is necessary to simulate eLoran performance, from the suggested Korean eLoran transmitters is simulated with the consideration of effective ground conductivities over Korea. Then, eLoran signal-to-noise ratios are also simulated based on atmospheric noise data over Korea. This basic simulation tool will be expanded to estimate the navigation performance (e.g., accuracy, integrity, continuity, and availability) of the Korean eLoran system.

      • 유효대지도전율을 고려한 한국 eLoran 시스템의 신호감쇠 시뮬레이션

        eLoran 시스템은 전파 교란에 강하다는 장점 때문에 GPS가 무력화 되었을 때의 백업시스템으로써 이용될 수 있는 지상파항법시스템이다. 현재 영국 등의 국가에서 실용화를 위한 성능 시험을 하고 있는 단계이며 한국에서도 도입을 추진하고 있다. eLoran 시스템을 효율적으로 구축하기 위해서는 정확도, 가용성, 무결성, 연속성을 예측할 수 있는 시뮬레이션이 필수적이며, 이는 시스템을 도입하는 지역의 고유한 환경과 변수를 반영할 수 있어야 한다. 본 연구에서는 한국에 설치될 eLoran 송신국에 의한 신호의 세기를 예측하는 시뮬레이션을 진행하였고 이를 분석한 결과를 정리하였다. 신호의 감쇠는 Log-distance 경로 손실 모델을 기반으로 유효대지도전율의 영향을 고려하여 시뮬레이션 하였다. 또한 이를 이용해 3개 이상의 송신국으로부터 eLoran 신호를 수신할 수 있는 지역을 예측하여 도시화하였다. eLoran system is a ground-based navigation system which can be used as a back-up system when the GPS signal is unavailable. The UK is now testing the performance of their first eLoran transmitter in Anthorn, and Korea is also in progress to deploy their own eLoran system. An eLoran performance simulation tool is useful to predict accuracy, availability, integrity and continuity of eLoran services. In this paper, the received signal strength from future eLoran transmitters in Korea is simulated, which is the first step to develop a complete eLoran performance simulation tool for Korea. The signal attenuation simulation is based on the log-distance path loss model, and the influence of effective ground conductivity is also considered.

        모호성과 맥락고정논변

        나는 이 논문에서 먼저 모호성에 대한 경계이동이론(또는 맥락주의)을 소개하고 이에 대한 가장 강력한 비판인 맥락고정논변이 무엇인지 설명할 것이다. 맥락고정논변의 기본 아이디어는 맥락을 고정해도 모호성은 남는다는 것이다. 우리는 이런 아이디어를 이용한 많은 반론들을 여러 문헌에서 발견할 수 있다. 만약 경계이동이론의 옹호자들이 맥락고정논변에 대한 설득력 있는 대답을 제시하지 못한다면 경계이동이론은 곤경에 처하게 될 것이다. 그런데 특이할 정도로 경계이동이론의 지지자들은 이 논변에 대해 거의 대답을 하지 않고 있었다. 이런 상황에서 에커만과 그리너프는 맥락고정논변에 대한 여러 대안들을 제안한다. 나는 이 논문에서 이들의 제안을 비판적으로 검토하고, 맥락고정논변에 대한 새로운 대안을 발전시킬 것이다. In this paper, I introduce what Boundary-Shifting theories of vagueness (or Contextualism about vagueness) are and what the Context-Fixing Argument is, which is the most powerful and plausible objection to Boundary-Shifting theories. The basic idea of the Context-Fixing Argument is that vagueness still remains even if we hold the context fixed. Many objections based on this simple idea can be found in the literature on Boundary-Shifting theories. It seems that, without a convincing response to the Context-Fixing Argument, Boundary-Shifters must be in trouble. But, oddly, defenders of Boundary Shifters have said very little about this matter. In this situation, Akerman and Greenough try to defend Boundary-Shifting theories of vagueness by suggesting several answers. In this paper, I critically examine their suggestions and develop a new answer to the Context-Fixing Argument.
