        도파민 길항제에 치료 불응성인 기능성 소화불량증 환자에서 시사프리드의 효과

        N/A Background/Aims : Prokinetics are commonly used for the treatment of functional dyspesia, but their methods of action are different. First, we compared the efficacy and safety of the dopamine receptor antagonists, which were domperidone maleate and levosulpiride, in a 2 week treatment in functional dyspepsia, then investigated the efficacy and safety of cisapride tartrate in a 2 week treatment in those who were resistant to domperidone maleate or levosulpiride. Methods : One hundred Forty-nine patients, who were diagnosed with functional dyspepsia, were selected. The subjects were randomly divided into two groups, domperidone maleate (75) and levosupiride (74). Daily they took 30mg of domperidone maleate (DOM) or 75mg of levosulpiride (LEV) for 2 weeks. Then the subjects who didn't respond to these treatments took 30mg of cisapride tartrate for the following 2 weeks. Results : At week 0, the total symptom scores of the DOM group and LEV group were 8.01±2.57 and 8.14±2.65 respectively, which were not statistically different. At week 2, the total symptom scores of the DOM and LEV groups were significantly reduced to 4.28±3.30 and 4.85±3.53(p=0.0001). The efficacy rates of the DOM and LEV groups at week 2 were 50.8% vs. 44.1%. The rate of adverse events in the LEV groups was much higher than in the DOM group(17.7% vs. 8.0%). In addition the rate of change from normal to abnomal in prolactin level was markedly higher in the LEV than that of the DOM group(80.0% vs. 8.3%). After 2 weeks of treatment with cisapride tartrate, the total symptom score was significantly reduced to 3.77±2.49(p=0.0001), and the efficacy rate was 75.0%. The satisfaction of the resistant subjects in efficacy of cisapride compared with the previous treatment was 73.3%. The rate of adverse events of cisapride tartrate was 5.0%. Conclusions: Considering efficacy and safety of domperidone maleate and levosulpiride, domperidone maleate was the safer drug for the treatment of functional dyspepsia, and cisapride tartrate can be a useful drug in those patients who are resistant to dopamine antagonists like domperidone maleate and levosulpiride.

        오령산 및 가미오령산이 가토이뇨작용에 미치는 영향

        In order to determine the effect of Oryeongsan reputed to have diuretic action since Han Dynasty and possible synergetic action of Dianthi Semen, Polygonum avicularis Herba, Kochiae Fructus and Akebiae Lignum, herbs with similar reputation, when added to the above prescription, their decoction powders were solved into distilled water and injected into rabbits through the ear vein. Upon the treatment, the excretion of sodium, potassium and chloride ion together with urine volume was kinetically determined. At the same time the clearance of plasma creatinine and sodium ion was determined and the following results were obtained. Every experimental group demonstrated diuretic action, though feable, of relatively long duration. The diuretic mechanism in the case of Oryeongsan made up by mixing the seperate extracts of individual components and Oryeongsan(A) plus Dianthi Semen was found to be inhibitory effect of renal tubular reabsorption in contrast to the case of Oryeongsan (A) plus Polygonum avicularis Herba, Kochiae Fructus or Akebiae Lignum in which case the diuretic mechanism was found to be glomerular vascular dilatation. The urinary excretion of potassium ion was increased in the case of Oryeongsan (A) plus Dianthi Semen, Kochiae Fructus of Akebiae Lignum whereas in the other cases no similar change was registered. The diuretic action was most remarkable in the case of Oryeongsan (A) plus Polygonum avicularis Herba followed by Oryeongsan (A) plus Kochiae Fructus, Oryeongsan (A) plus Dianthi Semen, extract mixture of individual component of Oryeongsan, Oryeongsan(A) and Oryeongsan(A) plus Akebiae Lignum decreasing order. The duration of diuretic action was found to be 90 minutes in the case of Oryeongsan and mixture of individual component of Oryeongsan, and 60 minutes in the case of Oryeongsan(A) plus Dianthi Semen, Kochiae Fructus or Akebiae Lignum in contrast to the case of Oryeongsan(A) plus Polygonum avicularis Herba which lasted up to 120 minutes.

        원발병소 및 전이병소에서 배양된 위선암세포의 체외 항암제 감수성 비교

        연구배경 : 세균에 대한 항생제 감수성검사와 같은 개념으로 항암제 감수성 검사법이 다양하게 개발, 발전되어 왔으나 아직까지 임상응용에는 많은 제약이 있다. 그러나 감수성검사 결과와 생체내에서의 항암효과 사이에 연관성이 높다는 여러 보고와 함께 실제 생존율도 증가 시킨다는 보고가 있어 항암제 감수성검사의 중요성 및 필요성이 더욱 강조되고 있다. 또한 원발병소 및 전이병소의 암세포는 세포형태, 핵형, 면역성, 항원성, 침습력, 암유전자 발현율 및 항암제 감수성에 있어서 차이가 있는 것으로 알려져 있다. 본 연구의 목적은 원발병소 및 전이병소의 암세포가 항암제 단일 또는 병합에 대하여 감수성에 차이가 있는 지를 알아보고, 체외 항암제 감수성검사를 통하여 가장 감수성이 높은 단일 또는 병합 항암제를 선별할 수 있는지를 알아보는데 있다. 방법 : 위암환자 158예중 138예는 원발병소 (A군)에서, 26예는 전이병소(B군)에서 채취된 위선암세포를 단기간 시험관내에서 배양하였다. 단기간 배양된 암세포를 3-{4, 5-dimethylthiazol-2-yl}-2-5-diphenyl-tetrazolium bromide(MTT) 검사방법을 이용하여 adriamycin(ADR), cisplatin(DDP), 5-fluorouracil(5-FU), mitomycin C (MMC) 및 etoposide (VP-16)의 5가지 단일 또는 병합 항암제에 대한 체외 항암제 감수성검사를 시행하였다. 결과 : 1) 위선암세포의 단기간 배양성공율은 A군에서 138병소 중 20병소로 14.5%, B군에서는 27병소중 18병소로 66.7%로 양군간에 의의있는 차이를 보였다(p<0.001). 2) 배양된 암세포의 위상차 현미경 소견이나 병리학적 도말검사 소견은 양군간에 유의한 형태학적 차이를 관찰할 수 없었다. 3) 각 항암제 각각에 대한 감수성은 환자 개개인에 따라 차이가 많았으며 ADR과 MMC의 IC_50은 B군에서 A군에 비하여, VP-16의 IC_50은 A군에서 B군에 비하여 유의하게 높았다(p<0.05). 4) Additivity model을 이용한 항암제 병합에 대한 감수성검사 결과, 함암제 2제 병합시 A군에서는 ADR+MMC과 ADR+DDP에서, B군에서는 ADR+VP-16, 5-FU+VP-16 그리고 MMC+VP-16에서 유의한 상승효과를 보였으며 (p<0.05), 항암제 3제 병합시 A군에서는 5-FU+ADR+DDP와 MMC+DDP+5-FU에서, B군에서는 5-FU+ADR+VP-16와 VP-16+ADR+DDP에서 유의한 상승효과를 보았다(p<0.01). 5) Isobologram model을 이용한 병합 함암제에 대한 감수성검사 결과, 항암제 2제 병합시 A군에서는 ADR+DDP와 ADR+MMC, B군에서는 ADR+VP-16, MMC+VP-16, DDP+VP-16이, 항암제 3제 병합시 A군에서는 5-FU+ADR+DDP와 MMC+DDP+5-FU, B군에서는 VP-16+ADR+DDP와 5-FU+ADR+VP-16이 대상환자중 70% 이상에서 높은 감수성을 보였다. 6) A군과 B군 사이에 감수성이 높은 병합 항암제는 서로 달라 5-FU+ADR+MMC과 5-FU+ADR+DDP는 A군에서는 상승효과를 보였으나 B군에서는 오히려 길항작용을 보였으며, 5-FU+ADR+VP-16와 VP-16+ADR+DDP는 A군에는 준첨가효과를 보였으나 B군에서는 상승효과를 보였다. 결론 : 원발병소 및 전이병소 위선암세포간에 단일 또 는 병합 항암제에 대한 감수성에 있어서 유의하게 차이가 있었다. 따라서 원발병소 또는 전이병소의 위선암세포를 시험관내에서 단기 배양하여 체외 항암제감수성검사를 시행함으로서 환자 개개인의 위암세포에 가장 감수성이 있는 항암제 선별할 수 있고, 감수성검사의 결과에 따라 선택된 단일 또는 병합 항암제로 화학요법을 시행하면 현재의 경험적인 함암제 투여에 비하여 치료성적이 향상될 것으로 생각된다. Background : Recently, attention has been focused on the possibility of predicting the effectiveness of anticaner drugs against individual tumors through chemosensitivity tests. The MTT assay is a preferable, accurate, and convenient colorimetric assay which is based on the premise that the mitochondria of living cells reduce the tetrazolium salt to formazan. It has been suggested that malignant neoplasms contain multiple cell populations exhibiting tremendous biologic heterogeneity. Particularly the detailed differences in chemosensitivity between cells from primary and from metastasis lesions remain unsolved. This study was designed to assess the usefulness of in vitro chemosensitivity tests in the tratment of gastric cancers and differences in chemosensitivity between gastric cancer cells cultured from primary and metastatic lesions. Method : Primary cancer cells of 138 patients with gastric cancer (group A), obtained through the endoscopic biopsy or surgical open biopsy, and metastatc cancer cells of 26 patients (group B : 27 lesions), obtained through the paracentesis or pleural centesis, were cultured using culture midium supplemented with 20% fetal bovine serum. The colorimetric MTT assay was applied for in vitro chemosensitivity tests. Results : 1) The success rates of short-term culture of gastric cancer cells were 14.5% (20/138) in group A and 66.7% (18/27) in group B, respectively. 2) The morphology of the cultured cells observed through phase-contrast and light microscopy showed no significant difference between group A and B. 3) The mean IC_50 of ADR and MMC were significantly higher in group B than group A (p<0.05) and that of VOㆍ16 significantly higher in group A (p<001) 4) The two-drug combination regimens having thd statistically significant synergistic effects were ADR+MMC (p<0.05) and ADR+DDP (p=0.01) in group a, ADR+VPㆍ16 (p<0.05), 5ㆍFU+VPㆍ16 (p<0.05), and MMC+VPㆍ16 (p<0.05) in group B, and the three-drug combination regimens were 5ㆍFU+ADR+DDP (p<0.05) and MMC+DDP+5ㆍFU (p=0.01) in group A, 5ㆍFU+ADR +VPㆍ16 (p<0.01) and VPㆍ16+ADR+DDP (p<0.01) in group B. 5) The two-drun combination regimens having the supraaddictive effects over 70% of the cases were ADR+MMC and ADR+DDP in group A, ADR+VP-16, 5-FU+VP-16, and DDP+VP-16 in group B, and the three-drug combination regimens 5-FU +ADR+DDP and MMC+DDP+5-FU in group A, 5-FU +ADR +VP-16 and VP-16 +ADR+DDP in group B. 6) The combination regimens having synergistic effects were different between group A and group B. 5-FU +ADR+MMC and 5-FU +ADR+DDP combination had synergistic effects in group A, but antagonistic effects in group B. 5-FU +ADR+VP-16 and VP-16+ADR+DDP combination synergistic effects in group B, but antagonistic effects in group A. Conclusion : The chemosensitivities of gastric cancer cells were significantly different between the primary and metastatic lesions from which the cultured cells were derived. Thus, the individual chemosensitivity test using short-term culture system may be useful for the selection of the most effective anticancer drugs.

      • 알레르기성 비염 흰쥐모델에서 理中湯合敗毒散이 비염치료에 미치는 영향

        Background and Objectives: Recently the incidence of allergic rhinitis has increased but treatment in most cases has only dealt with the symptoms. Medicine has been developed that shows fewer side effects. However, some side effects and the psychological stress over taking medicine have remained. There have been no studies so far performed on the effect of Lizhongtang plus Baidusan Extract. This study aimed to find out the therapeutic effects of its exclusive use in rats with Allergic Rhinitis. Materials and Methods : Thirty Sprague-Dawley rats were divided into three groups: the control group, the cetirizine HCI group and the sample group. To induce allergic rhinitis in the control group, the cetirizine HCI group and the sample group, rats were sensitized intraperitoneally with 0.1$\%$ ovalumin solution 3 times at an interval of I week. Then intranasal sensitization was performed by diffusing 0.1$\%$ ovalumin solution 3 times at an interval of 2 days. After that time, rats of the cetirizine HCI group were orally administered with cetirizine HCI. Rats of the sample group were treated with Lizhongtang plus Baidusan Ex. for 28 days. We observed changes in nasal mucosa and submucosa. Also we found changes in the segment of neutrophil and lympocyte in Leukocyte. We used the statistical methods of ANOVA test(p 〈0.05). Results: The loss of the cilium and the secretion of mucus in the treated group was rare when compared to control group. Effects of Lizhongtang plus Baidusan Ex. on the liver function were also studied in rats. Treatment of Lizhongtang plus Baidusan Ex. did not affect AST and ALT. The segment of neutrophil was significantly increased in the treated group when compared with the control group and the cetirizine HCI group(p 〈0.05). The segment of lympocyte was significantly decreased in the treated group when compared with the control group and the cetirizine HCI group(p 〈0.05). Conclusion: This study shows that Lizhongtang plus Baidusan Ex. decreases the inflammatory response in rats with Allergic Rhinitis.

        천연물 / 유기화학 : 고지방식이로 유도한 비만 흰쥐에서 Monascus pilosus 균사체 추출물의 간 손상 예방효과

        Hepatoprotective effects of Monascus pilosus mycelial ethanol extract (MPME) were examined in high-fat diet induced-obese rats. The rats were randomly divided into 2 groups; normal control (NC) and a high-fat and high cholesterol diet group (HFC). The HFC diet group was fed a 5L79 diet supplemented with 15% lard and 1% cholesterol for 3 weeks for induction of obesity. And then, the rats were divided into 4 groups (n=5); the NC, a HFC diet obesity control group (HF), 0.5% MPME supplemented HFC diet group (MPM), and 2% conjugated linoleic acid (CLA) supplemented HFC diet group for 7 weeks. Whereas the daily weight gain of NC and HFC groups were 3.48 g and 4.48 g, respectively, those of MPM and CLA were 3.09 g and 4.38 g, respectively. Furthermore, activity of serum alanine and aspartic aminotransferase in HF was markedly higher than those of NC group, but, the activity in MPM and CLA was significantly lower than HF. Hepatic reduced glutathione content in MPM and CLA was higher than HF. On the contrary, hepatic lipid peroxide content in MPM and CLA was significantly lower than HF. In conclusion, although the precise mechanisms of the hepatoprotective effects of the MPME in this study are unknown, our study provides experimental evidence that MPME may prevent obesity and hepatic damage by high-fat and high cholesterol diet via inhibition of lipid absorption and induction of reactive oxygen spices scavenging enzyme such as superoxide dismutase.

        Anti - HBc 단독 양성자에 대한 B 형간염백신의 면역효과

        N/A 1mmune responses with hepatitis R vaccine were studied in 13 indivisuals with isolated anti-HBc and 35 indivisuals with negative hepatitis B marker. All healthy volunteers in Severance Hospital were no evidence of hepatitis, history of hepatitis, immunocom-promised state, transfusion, HB vaccination, diabetes mellitus or heavy use of any kinds of drugs. Three doses of Hepavax® 1 ml (HBsAg, 20 mg) were administered intramuscularly in the deltoid muscle at 0, 1 and 2 months, and sera were collected from the subjects at monthly intervals for 3 months after the first injection. All sera were tested for HBsAg, anti-HBs and anti-HBc by radioimmunoassay, and serum aminotransferase. The results were as follows: 1) The anti-HBs response rates in 35 seronegative controls were 60.0% at 1 month after the first vaccination, 74.3% at 2 months and 94.3% at 3 months. Four subjects (11.4%) responded 1 month after vaccination with anti-HBs titers of 50RI.J or greater. 2) The anti-HBs response rate in 13 indivisuals with isolated anti-HBc was 100% from 1 month after the first vaccination. Six subjects (46.2%) responded 1 month after vaccination with antiHBs titiers of 50RU or greater. 3) The isolated anti-HBc group had a significantly higher anti-HBs response rate when compared with seronegative controls at 1 and 2 months but not at 3 months. Anti-HBs titers of isolated anti-HBc group were greater than seronegative controls. 4) There were no abnormal elevation of aminotransferase, no change of HBsAg and anti-HBc for 3 months follow-up. The side effect after vaccination were negligible. In conclusion, there was a tendencv of more rapid anti-HBs response and higher anti-HBs titers in the isolated anti-HBc group than the seronegative group.

        Monascus pilosus로 발효시킨 비지의 항비만 효과

        본 연구에서는 고지방식이로 비만을 유도한 마우스를 이용하여 비지 (SCR)와 Monascus pilosus로 발효비지의 항비만 및 항산화 효능을 조사하였다. 마우스는 비지와 발효비지를 2% (wt/wt)가 되도록 고지방식이에 첨가하여 8주간 사육하였다. 비지 및 발효비지 투여군은 고지방식이 대조군과 비교할 때 모두 유의한 수준의 체중 감량, 체중 증가율, 부고환주변의 지방 함량이 낮았다. 8주째의 초기체중에 대한 체중증가율은 정상식이군19.54%, 고지방식이군 45.40%, 비지군 33.39% 그리고 발효비 지군 31.42%로 발효비지군에서 가장 낮았다. 또한 혈청의triglyceride, 총 콜레스테롤, LDL-콜레스테롤 함량 그리고 간 조직의 trigrlyceride 함량도 고지방식이로 증가된 수준에서 크게 줄어들었다. 한편 비지와 발효비지의 급여는 glutathione 함량을 높이고, 지질의 과산화, 혈청의 aminotransferase 및 xanthineoxidase의 활성을 낮추는 것으로 나타났다. 반면, superoxide dismutase, glutathione S-transferase, glutathione peroxidase의 활성은 비지 및 발효비지군 모두에서 고지방식 이군에 비하여 높았다. 마지막으로, 비지군과 비교할 때 발효비지군은 고지방식이로 비만을 유도한 마우스에서 비정상적인 비만 관련 생화학적 지표들을 좀 더 효과적으로 정상식이군의 지표들로 회복시켜주는 것을 확인할 수 있었다. 이상의 결과, 고지방 식이로 인한 체중 증가와 복부지방의 축적 및 고지혈증과 간 조직의 지방 축적현상이 M. pilosus로 발효시킨 비지의 2% 식이로 항비만 효과와 동시에 혈액 및 간 조직 지방의 함량을 저하시켜 고지혈증으로 인해 야기될 수 있는 심혈관계 질환을 예방할 수있을 것으로 생각된다. 또한 활성산소 생성계의 활성 억제와 활성산소 소거계 활성 증가를 유도함으로서 비만 상태에서 나타날 수 있는 과잉의 활성산소에 의한 간 조직 손상을 예방 혹은 경감시켜줄 수 있을 것으로 사료된다. This study investigated anti-obesity and antioxidant effects of dietary non-fermented soybean crud residue (SCR) and fermented SCR by Monascus pilosus (FSCR) in high-fat inducedobese mice. SCR and FSCR were supplemented with high-fat diet at 2% (wt/wt) dose for 8 weeks. Both SCR and FSCR significantly lowered body weight, epididymal fat weight and weight gain rate compared to high-fat diet control (HC) group and FSCR group showed lowest weight gain rate. In addition, it was observed that serum and hepatic lipid profiles including triglyceride, total cholesterol, LDL-cholesterol and HDL-cholesterol were significantly improved by supplementing SCR or FSCR. Furthermore, SCR and FSCR administration showed increase of glutathione content and decrease of hepatic lipid peroxide content, serum aminotransferase activity, and hepatic xanthine oxidase activity. On the other hand, activities of reactive oxygen species scavenging enzyme such as superoxide dismutase, glutathione S-transferase and glutathione peroxidase in two test groups were higher than those of HC. Lastly, in comparison with SCR, FSCR was more effective in restoring obesity-related biomarkers to normal level in high-diet induced obese mice. In conclusion, the present study indicates that FSCR could have not only anti-obese effects such as inhibition of abdominal fat accumulation, but also protective effects of cardiovascular disease such as atherosclerosis by decreasing serum and hepatic lipid contents. Furthermore, these results suggest that experimental diets in this study could alleviate hepatic damage caused by overproduction of reactive oxygen spices (ROS) due to obesity via inhibition of ROS generating activities and induction of ROS scavenging activities.

        대망에 발생한 원발성 평활근육종

        Although the greater omentum is frequently the site of metastatic neoplasm, primary omental tumors are exeedingly rare. The authors report here, a case of primary omental leiomyosarcoma presented initially as a palpable abdominal mass and epigastic discomfort for one year in a 66 year-old rnale patient. At laparotomy, 21*15*2.5 cm rnass weighing 600 gram was found in the omentum which was adherent to the anterior wall of the lower body of the stomach and liver surface. In the histologic findings presence of tumor necrosis, increased cellularity, cellular atypia and pleomorphism as well as large size of mass were indicative of leiomyosarcoma. Surg cal resection has been known to be the principle treatment for this disease, In this case, excision of the tumor with omentectomy, radical subtotal gastrectomy and wedge resection of the liver were performed. Eight months later, a recurrence was detected and chemotherapy with doxorubicin, dacarhazine, vincris- tine, and cyclophosphamide was done.

        The purpose of this study is to estimate the characteristics and pollutant loadings of nonpoint pollutants that flowed in the streams on precipitation for pollutant loading reduction of Hwa-sung lake inflow streams. Although it has been made an effort to improve the water quality of Hwa-sung basin through the strategies for the preservation of water quality, it is shown that the water quality is not greatly improved. Because it has been industrialized and urbanized near Hwa-sung basin so that it is difficult to reduce the water pollution due to the increase in pollutant loadings of point and non-point sources. In this study, it is investigated the outflow characteristics of non-point pollutants that discharged with storm runoff and estimated the effect of runoff on Hwa-sung basin. The final goal of this study is to utilize the basic information for proper management and strategies of non-point sources on Hwa-sung basin. At the result of inflow streams, Ja-an stream that has the greatest pollutant loadings on precipitation is strongly influenced on the water quantity of Hwa-sung basin. On the other hand, it is shown that Nam-yang stream is strongly influenced on the SS concentration of Hwasung basin among them. Also, all streams; Nam-yang, Ja-ahn, Ah-eun stream; has the degree of slope more than or near 1 in the correlation results so that they have strong pollutant loading impact and the concentration of SS is the highest among other pollutants. So, specific studies on initial rain phenomena are more necessary to manage the pollutants economically. Also, the proper control of SS concentration is required to manage the effluent pollutants effectively on precipitation. So, it is necessary to consider the strategies for non-point pollutants as well as point pollutants when the new management is imposed to reduce the pollutant load for improvement of Hwa-sung basin.
