Pleural complication is extremely rare among the complications of lung following transcatheter arterial chemoembolization (TACE) for hepatocellular carcinoma. We report a case of hepatopleur al fistula complicated by TACE. A 50-year-old male was admitted to the hospital because of aggravation of cough and chest pain. Fifty-five days before admission, a liver abscess had developed and treated by percutaneous drainage. A hepatocellular carcinoma had been diagnosed as a cause of liver abscess. Forty-five days before admission, TACE had been performed for the hepatocellular carcinoma. Thirty days after TACE, dry cough and right pleuritic chest pain developed. After admission, serial CT scans of thorax and abdomen showed the pleural effusion containing the lipiodol from the liver through hepatopleural fistula. Chest tube insertion with a urokinase instillation was performed for the treatment of empyema caused by hepatopleural fistula. (Korean J Med 62:278- 283, 2002)

(1) This investigation had been made by the writers, KIM and REE, from the 30th of Jul. in 1972 till the 25th of Aug. in 1975. (2) Investigated fauna of fish and water insects in the mountain streams of the Han river. (3) Water temperature was as follows; in the spring 5∼10℃, in the summer 26∼27℃ C, in the winter 2∼3℃. (4) Collected 16 species of mountain stream fishes in Dong River(east side river) and 20 species in Suh River(west side river). (5) These fishes were separated into those living in the abyss or rapids and some are recognized living in both the abyss and rapids. (6) In winter rapids fish is more abundance than in summer, above all Liobagrus andersonii` Gobiobotia brevitarta and Coreoperca herzi is much more abundance. (7) In opposition to it, in summer rapids, Microphysogobio longidorsalis is much more abundance than in winter rapids. (8) Coreoleuciscus splendidus were not able to be collected in summer. rapids but were collected in abundance in winter rapids. (11) In quantitive collection of water insects, both at the upper part of the streams individual number, body weight, and species number were much higher than at the lower part of the streams. 직무분석기법을 변용한 공학 교과과정 설계 (명지대학교 전기공학과의 예)

공학분야의 빠른 발전에 따라 교육이 뒤쳐지는 현상이 심화되고 있다. 이러한 시기에 시의 적절한 공학교육을 위해서는 교수들의 임의적인 교과과정의 개정이 아닌 적절한 시스템에 의한 교과과정의 개편이 요구되어진다. 본 논문에서는 기능직 교과에서 사용하던 직무분석법을 일반 교과과정 설계에 사용한 예를 보여준다. 직능원에서 제시한 교과과정 설계는 지나치게 세부적으로 작업에 필요한 competence(지식, 기능력, 태도 도구 등)를 제시하였다. 일반 교과과정에서는 이렇게 자세한 직무분석은 피하고 시대에 따라 달라지는 목표학습 성과를 설정하고 이것을 이루기 위한 교과과정을 개발하기 위해서 졸업생의 사회진출 추이를 조사하여 대표 직무를 선정하고 직무를 분석하여 필요한 competence를 도출하고 거기에 따라 교육목표도 수정하였다. 전문가 회의도 주체하여 졸업생 및 산업체의 의견도 수렴하여 실제 현장에서 필요한 교육을 실시하고자 한다. 이러한 교과과정의 개편은 명지대학교에서 수행하고 있는 순환적 자율 개선형 교육제도의 일환으로 실시되고 있다. Recently, developments of engineering technology are too fast change to educate the students in the college of engineering. The lag between the innovation of the technology and the education of the college of engineering becomes larger and larger. In this situation, the appropriate education update system is needed to change the curricula for the updated engineering education. In this paper, the job analysis method which is used in the technical service area is transformed to design the curriculum of the general higher education area. The job analysis method which is used in the technical service area derives the competence too detail and complex to used in the higher education area. for the higher education area, the social activity analysis of the alumni is needed to extract the representative jobs of the area. Also using this representative jobs, the job analysis and competence derivation is conducted. If needed, the regular expert meeting is held to converge the job-site opinions of the graduate and industry people. This curriculum changing process is provided as a part of the circular self-improve education system of the Electrical Engineering of the MyongJi University.
성인의 Monteggia 골절에서 기능적 결과에 영향을 미치는 인자

Purpose: Monteggia fracture is a combination of ulnar fracture and radial head dislocation. Despite of low incidence, Monteggia fracture has a problem in the treatment. So satisfactory results could not be achieved easily. The purpose of this study was to analyze factors affecting functional results in Monteggia fracture. Materials and Methods: From January 1990 to June 1998, we retrospectively reviewed 24 patients out of 40 patients who had been treated for Monteggia fracture. The functional assessment was performed with Broberg and Morrey system10). Results: Excellent and good results were achieved in 16 cases(66.6%), whereas fair and poor results in 8 cases(33.4%). Satisfactory results were obtained in 66.6% overall. According to the Bado classification, functional results were not different significantly between types. According to the mechanisms of injury, unsatisfactory results were obtained in 8 of 17 patients with high energy injury. Monteggia fractures with ulnar fracture at the diaphyseal level showed satisfactory results significantly compared to others. In terms of the methods of internal fixation, the plate and screw fixation showed satisfactory results in 14 of 16 patients whereas tension band wiring or intramedullary nailing showed unsatisfactory results in 6 of 8 patients. Conclusion: In Monteggia fractures, factors affecting functional result may be the mechanism of injury, the level of ulnar fracture and the method of internal fixation.
