Optimal duration of preoperative antibiotic treatment prior to ureteroscopic lithotripsy to prevent postoperative systemic inflammatory response syndrome in patients presenting with urolithiasis-induced obstructive acute pyelonephritis

Purpose: There is no consensus on the optimal duration of preoperative antibiotic treatment prior to ureteroscopic lithotripsy in patients presenting with urolithiasis-induced obstructive acute pyelonephritis (APN). We aimed to identify surgeon-modifiable, preoperative risk factors associated with postoperative systemic inflammatory response syndrome (SIRS) in these patients. Materials and Methods: This multicenter retrospective study evaluated 115 patients who presented with urolithiasis-induced obstructive APN between January 2008 and December 2019. All patients were administered intravenous third-generation cephalosporin until culture sensitivity confirmation or until ureteroscopic lithotripsy. Data were collected for age, sex, diabetes mellitus, performance status, stone features, hydronephrosis grade, preoperative renal collecting system drainage, laboratory data, operative time, and duration of preoperative antibiotic treatment. Sensitivity analysis using Youden's index and logistic regression analysis were used to assess risk factors of postoperative SIRS. Results: Postoperative SIRS was identified in 32 (27.8%) patients. The incidence of postoperative SIRS was higher in patients who received preoperative antibiotic treatment for fewer than 14 days (38.8% vs. 12.5%; p=0.001). Backward variable selection logistic regression analysis revealed maximal stone diameter ≥15 mm, duration of preoperative antibiotic treatment <14 days, and preoperative C-reactive protein (CRP) level ≥6.0 mg/L to be associated with higher risk of postoperative SIRS. Conclusions: Patients with urolithiasis-induced obstructive APN planned for ureteroscopic lithotripsy should be administered at least 14 days of preoperative antibiotic administration and achieve a serum CRP level ≤6.0 mg/L to minimize the risk of postoperative SIRS.

민영교도소 평가와 제도적 개선방안에 관한 연구

Korea's first private prison, Somang Correctional Institution opened on December 1, 2010. As it was the case that the field of correction as one of the national core authority was entrusted to the private, the public interest has focused until now. Operating performance over the past six years was positively evaluated and the Ministry of Justice renewed the contract for 20 years with Agape last February 5, 2015. Somang Correctional Institution currently reach a turning point to improve their weaknesses in order to make a growth and development as a personality-education-specialized private prison. It's also time for the Ministry of Justice to review the privatization policy that the private prison system on the ballast continue desired result. Accordingly, this study made some suggestions so that a private prison can be properly established hereinafter through the analysis of structural contradiction and operational problems of the private prison system, which have been exposed through actual operational experience in Somang Correctional Institution for the past five years. First, there’s the necessity of having to relax the regulations related to Somang Correctional Institution, and to guarantee the status as a correctional institution. Second, it’s necessary to establish the complementary cooperative relations between correct between the Korea Correctional Service and a private prison correct. Third, it’s necessary to get a private prison settled as an education- specialized correctional institution by intensifying an intensive personality education which could be called a private prison’s own specialization. Fourth, it’s necessary to let a private prison get approval from a superintendent in relation to protective gear, use of weapons, restriction on a right, inmate treatment, and conditional release business, etc., which fall under authority-based business, and to secure autonomy for private prison officers so that they can perform jobs without special regulations with regard to the business like general affairs, accounting, maintenance, vocational training, education and classification review, etc. which fall under non-authority-based business. Fifth, at present, the Classification & Examination Division of the Korea Correctional Service is taking charge of the business generalization of a private prison. However, actual business is broken up into each division, and the window for related business is not unified, causing necessary administrative measures to be unnecessarily delayed. To solve it, there is the need to inquire into the plan to utilize subject assignments for correctional officers. Sixth, Article 32 of the Private Prison Act prescribes the support officer system. It’s worthwhile to make use of the method of getting a similar effect by unifying the communication window for the business of the Ministry of Justice through a support officer dispatched to a private prison by easing off & expanding requirements in a consignment agreement. Lastly, it’s important to rationalize the calculation standard of working expenses. A correctional corporation has to pay taxes and the public utilities’ charge, such as property tax, etc. It’s because there is no virtual method of covering necessary expenses for managing corporation but contributions. Therefore, there is the necessity of examining the revision of a contract, through which financial compensation equivalent to profits is included into profits by reflecting such an aspect. 국내 최초의 민영교도소인 소망교도소가 2010년 12월 1일 개소하였다. 국가권력의 핵심인 교정분야가 민간에 위탁되는 사례였기 때문에 개소초반부터 현재까지 많은 관심이 집중되고 있다. 법무부는 6년간의 소망교도소 운영성과를 긍정적으로 평가하고, 지난 2015년 2월 5일 (재)아가페와의 교정업무 위탁계약 기간을 20년 갱신하였다. 현재 소망교도소는 인성교육 특화형 민영교도소로서 성장발전 해 나가기 위해 지금까지 드러난 운영상 취약점들을 개선해 나가야 할 전환점에 서 있다. 또한 법무부로서도 안착기에 접어들고 있는 민영교도소 제도가 향후 지속적으로 성과를 거둘 수 있도록 그간의 민영화 정책을 생산적으로 재검토 해 볼 시점에 있다. 따라서 이 연구에서는 지난 5년간의 소망교도소 실제 운영경험을 통해 드러난 민영교도소 제도의 구조적 모순과 운영상 문제점을 분석하여, 향후 민영교도소가 올바르게 정착될 수 있도록 몇 가지 제언을 하였다. 첫째, 소망교도소 관련 규제를 완화하고 교정기관으로서의 지위가 보장될 필요가 있다. 둘째, 교정본부와 민영교도소간 상호보완적 협력관계 정립될 필요가 있다. 셋째, 민영교도소만의 특성화라 할 수 있는 집중인성교육을 강화시켜 교육특성화 교정기관으로 자리매김 시켜야한다. 넷째, 권력적 업무에 해당하는 보호장비, 무기사용, 권리제한, 수형자 처우, 가석방 업무 등은 감독관의 승인을 받도록 하고, 비권력적 업무에 해당하는 서무, 경리, 유지관리, 직업훈련, 교육, 분류심사 등의 업무는 민영교도소 직원에 의해 특별한 규제 없이 업무 수행할 수 있도록 자율성을 확보해줄 필요가 있다. 다섯째, 현재 민영교도소 업무 총괄은 교정본부 분류심사과에서 맡고 있다. 그러나 실제 업무는 각 부서에 분산되어 있고 관련 업무 창구가 일원화되어 있지 않아 필요한 행정조치가 불필요하게 지연되는 경우가 일어난다. 이를 해결하기 위해 교정공무원 교과제를 활용하는 방안을 모색해 볼 필요가 있다. 여섯째, 「민영교도소 등의 설치ㆍ운영에 관한 법률」 제32조는 지원관 제도를 규정하고 있다. 위탁계약서상 요건을 완화ㆍ확대하여 민영교도소에 파견되는 지원관을 통해 법무부 업무 소통창구를 일원화하여 유사한 효과를 거두는 방법도 활용해 볼 만하다. 마지막으로, 사업비 산출기준을 합리화하는 것이다. 교정법인은 재산세 등 제세공과금을 납부하여야 하며 법인의 운영에 필요한 경비를 기부금 이외의 방법으로 충당할 방법이 사실상 없기 때문이다. 이를 반영하여 이익금에 상당하는 재정적 보상을 계약서에 포함시키는 계약서 개정이 검토될 필요가 있다.

교도소 안의 관계회복 : 소망교도소 법률고충상담 운영을 통한 회복적 정의 원리의 한국화 시도

○ 회복적 정의는 북미 메노나이트 기독교 공동체가 400년 넘게 자신들의 생활문화로 유지해 온 기독교의 권징(교정적 갈등관리) 교리를 1970년대부터 주로 형사사법 분 야에서 응용하며 실천적으로 형성해 온 대안적 분쟁해결‧갈등관리 원리이다. 2000 년대 초반 국내 형사사법계에 소개된 이후 현재 교육계 등으로 확대되어 다양한 응 용 프로그램들이 활발히 시도‧ 운영되고 있다. ○ 그러나, 한국과 법문화적 토양이 상이한 미국에서 ‘대화’와 ‘존중’을 핵심가치 로 하여 형성된 원리가 국내에서 지속적인 성공을 거두기 위해서는, 정책담당자들이 그 원리의 구조를 이해하고 그것이 구현될 수 있는 실질적 여건을 조성하는 등 현지 화가 필요하다. ○ 이러한 맥락에서 본고는 필자가 현지화(한국화) 시도의 일환으로 운영 중인 소망교도 소 법률고충상담 경험을 소개하고, 이를 통해 회복적 정의 원리의 한국화에 필요한 조건들을 살펴본다. ○ 회복적 정의 원리는 대화와 존중의 실천을 통해 ‘죄의 인식’, ‘화해’, ‘원상회 복’으로 이어지는 자발적이고 종국적인 갈등해소를 목표로 한다. 이런 면에서, 한 국화의 가장 큰 난제는 그 핵심가치인 ‘대화’와 ‘존중’(승복) 문화의 부재에 있 다. 특히, 승복(존중) 부재 문화는 회복적 정의 원리 구현의 첫 단계인 죄 및 그에 따라 공동체와 상대방이 입은 피해에 대한 책임을 회피하는 양태(을질)로 드러난다. ○ 필자는 소망교도소 법률고충상담에서 ①범죄의 상호성에 대한 인식 (책임 인정), ②팩 트베이스(facts-based) 대화, ③정확한 법률정보의 제공 등을 통해 내담자의 을질 심 리를 극복하고 화해를 이루는 과정을 통해 회복적 정의 원리의 한국화를 추구하고 있다.

확장성 심근증 환아에서의 부분 심실 절제술의 적용 -1례 보고-

심한 울혈성 심부전으로 Dopamine을 1개월간 사용해온 10세의 확장성 심근증 환아에서 심장 이식을 시행하려 하였으나, 적절한 크기의 공여 심장이 없었고, 환아의 증상이 악화되어 부분 심실 절제술과 승모판 성형술을 시행하였다. 수술후 환아의 임상 증상은 현저히 호전되었고 수술전과 수술후 3개월, 6개월 1년째의 심장 초음파 검사 소견을 비교하여 추적 관찰한 결과, 좌심실의 심박출 계수는 수술전 17 %에서 수술후 각각 29%, 35%, 36%로 개선되었고 좌심실 확장기말 직경은 수술전 72 mm에서 수술후 각각 59 mm, 61 mm, 61 mm로 변화를 보였다. 부분 좌심실 절제술과 승모판 성형술의 동반 시행은 심장 이식의 기회가 상대적으로 적은 소아의 확장성 심근증에 있어 심장의 부하를 감소시켜 증상 완화를 시킬 수 있는 외과적 방법으로 심장 이식을 시행할 때까지 유지시켜주는 고식적 가치가 크며 아울러 장기적 추적 관찰이 필요하다고 사료 된다. Heart transplantation was planned for a 10-year old boy who had dilated cardiomyopathy with severe congestive heart failure and had been on dopamine for 1month. However, partial left ventriculectomy and mitral annuloplasty were performed instead, because there was no donor heart of the adequate size and the symptoms were aggravated. The clinical symptoms were markedly improved after the surgery. Comparing the postoperative echocardiographic results with the preoperative results, there were remarkable changes in the left ventricular ejection fraction(preoperative LV EF 17% to postoperative 3 months 29%, 6 months 35%, 1 year 36%) and the left ventricular end-diastolic dimension(preoperative 72 mm to postoperative 3 months 59 mm, 6 months 61 mm, 1 year 61 mm). Partial left ventriculectomy and mitral annuloplasty reduced the cardiac loading in the dilated cardiomyopathy. Partial left ventriculectomy and mitral annuloplasty may be considered as one of the alternative surgical metho s to carry over until a heart transplantation can be performed, especially for children.

부산지역 신성장 동력산업으로서 의료관광산업 전략 업종 도출

The object of this study is to objectively analyze and present bases about the derivation of the main strategic types in health tourism industry in Busan. AHP(analytic hierarchy process) method, relatively comparing classes and items in the each class and then deducing priority by evaluating suitability and potential growth in Busan at the same time, was conjugated for this object. The computation of weight value based on evaluation items was in the form of a relative evaluation through pairwise comparison, and very high consistency was shown under 0.1 C.R. as a result of verifying reliability. According to the results, since an economical guarantee was estimated as the most important item in the health tourism industry, it seemed that continual efforts are required. Though regional suitability was found to be a key experts in the field related with tourism industry because the importance of the items was thought differently depending on each class category considering every specialist group, dissimilar evaluation results were produced as economics was valued highest by experts in the field medically related. 본 연구는 부산지역 의료관광산업을 신성장 동력산업으로 육성하기 위해 의료관광 분야 주요 전략 업종을 도출하는데 그 목적이 있다. 본 연구를 위해 현재 부산지역 적합성과 미래성장성을 동시에 평가함으로써 계층간 그리고 계층내 항목간의 중요도에 따른 상대적 비교를 수행하였다. 우선순위를 도출하여 경쟁력을 분석하기 위해 계층분석과정(AHP: analytic hierarchy process)이라고 하는 의사결정모형을 사용하였으며 전문가 집단에 대한 신뢰성 검증 결과 C.R.값이 0.1이하로 일관성이 매우 높은 것으로 나타났다. AHP 분석 결과 의료관광산업은 기본적으로 경제성 확보가 가장 중요한 항목으로 평가되었으므로 경제성 확보를 위한 지속적인 노력이 필요한 것으로 나타났다. 전문가 그룹별로 살펴보면 관광산업관련 전문가의 경우 지역적합성을 가장 중요시하게 판단하고 있으나, 의료관련 전문가는 경제성을 가장 중요하게 판단하고 있는 것으로 나타나 상이한 평가결과를 보였다.
