중환자 분류도구(WMSCN)의 신뢰도, 타당도 및 환산지수 검증

Purpose: The purpose of this study is to establish reliability and validity and to identify the conversion index. Method: The WMSCN for ICU was revised from Workload Management System for Nurses(WMSN) of Walter Reed Army Hospital. Reliability of the WMSCN was evaluated interrater reliability between head nurses and staff nurses at 124 patients in April 2008. Validity through the correlation between direct nursing care hours and WMSCN score was conducted at 20 ICUs of 10 hospitals. Finally the conversion index was identified by total nursing hours and it divided by WMSCN score. Results: The scores by nurses were highly correlated with head nurses’(p=.967), and also scores of the WMSCN were highly correlated with the direct nursing care hours(p<.001). The distribution of patient classification ranks into class V(61.3%), class IV(24.2%) and class VI(11.3%). The scores of the WMSCN were no differences between MICU and SICU. Finally, the conversion index was 8.2 minutes. Conclusion: WMSCN is available to classify the nursing workload for critical care patients. The repeated evaluation of validity and reliability are requisite to use WMSCN effectively. And the conversion index should be adjusted to estimate the appropriate staffing in Korea.

Purpose: This study was done to suggest the standard ICU nursing procedure and standard ICU nursing practice time carried out according to the standard ICU nursing procedure. Methods: Through the literature review, 35 standard ICU nursing activities were selected and classified under 6 categories. Standard nursing procedures were described by research team and 4 clinical practice experts. Content validity of descripted nursing procedure was performed by 6 clinical practice experts. After that standard ICU nursing procedures were identified, standard nursing practice times were checked through 1:1 observation. Results: 35 ICU nursing activities were identified and categorized into 6 domains of activities. Also, their standard nursing procedures were identified, and finally 33 standard nursing practice times were measured and analyzed. The result of the measurement, 1 ICU nursing activity was spent above 30 minutes, 3 activities done within 10-20 minutes, 4 activities done within 5-10 minutes, and 25 activities were spent below 5 minutes. Conclusion: Through this study, 35 standard ICU nursing procedures were described and 33 standard ICU nursing practice times which carried out according to standard ICU nursing procedures were checked. The result can be as fundamental data for the study of nursing activity time in the ICU.

Purpose: This study was conducted to explore and compare the level and type of nursing ethical values according to career and nursing units of clinical nurses, Methods: The subjects were 944 nurses working in General Hospital in Seoul and the data were collected from March 8 to August, 2007 using the nursing ethical values questionnaire, The collected data were analyzed using SPSS WIN 10.0 program. Results: The results were as follows; The level of clinical nurses' ethical values were significantly different according to career and nursing units (p<.01) and it was found that most clinical nurses had deontological ethics in the areas of professional nursing domain ($2.77{\pm}.31$), cooperative relations domain ($2.90{\pm}.26$) and customer relation domain ($3.03{\pm}.32$). but that they had utilitarian ethics in human life domain ($2.53{\pm}.32$). Conclusion: Different approaches are needed to develop to establish clinical nurses' ethical values based on career, characteristics of nursing unit and various clinical situations.
중환자실 흡인간호 및 인공호흡기관리 표준화를 통한 인공호흡기 관련 폐렴발생 감소효과에 관한 연구

Background : This study aimed at identifying the effect of the standardized protocol on lowering the incidence of the ventilator associated pneumonia(VAP). Methods : The standard protocol focusing on decreasing VAP was made and applied at 5 ICUs (Medical ICU, surgical ICU, Respiratory ICU, Neonatal ICU, Pediatric ICU) in a university affiliated tertiary hospital, from April 1, 2000 to Oct 31, 2000. The protocol involved 3 parts : hand washing, the suctioning method and ventilator circuit management. All the nursing personnel received intensive education which was consisted of lecture, video film and demonstration. 176 nurses reported the performance of handwashing pre and post intervention. And randomly selected 15 nurses were observed by charge nurse and the handwashing practice was analyzed pre and post intervention. The incidence of VAP was compared with the former year incidence. Results : The self reported frequency of hand washing increased. In the direct observation of handwashing, the frequency, time, thoroughness of hand washing during 8 hours day duty was found to be improved. The frequency was increased from 1.1 time to 4.1 times; the time was improved from 1.7 seconds to 5.7 seconds and the thoroughness of the washing practice was from 0.2 times to 3.0 times respectively (p<0.001). The incidence of VAP decreased from at a rate of 15.63 number of case per 1,000 ventilator-day (April 1~August 31, 1999) to 7.23 number of case per 1,000 ventilator-days(April 1~Oct 31, 2000)(P<0.001). Conclusion : We developed the protocols which included hand washing, the suctioning method, and ventilator circuit management. Through the implementation of the protocol, the performance of hand washing improved and the VAP incidence rate in ICU was decreased.

Purpose: The purpose of this study was to investigate the present status of falls among inpatients in general hospitals and to identify the fall risk factors that reflect the characteristics of domestic hospitals. Methods: Data were collected between December 15, 2016 and January 15, 2017 from 32 Korean hospitals having 500 or more beds. First, 42 risk factors were extracted based on literature review and expert opinions. Then the importance of each factor was evaluated by 223 nurses from medical and surgical adult ward and intensive care units in 40 hospitals. Results: The incidence rate of falls in 18 hospitals was 3.87 per 1,000 total discharged patients and 0.55 per 1,000 patient-days, and the rate of injury-related falls was 40.5%. Major risk factors for falling were identified as the following: being over 65 years of age, history of falls during admission, physical mobility disorders requiring assistance, physical factors (dizziness or vertigo, unstable gait, general weakness, walking aids, visual problems), cognitive factors (delirium, lack of understanding on limitations), neurological disease, CNS medications Conclusion: The findings provide information that can be in the development of a fall risk assessment tool for inpatients in general hospitals in Korea.

Purpose: This study was conducted to investigate the extent of diffusion of Intravenous infusion nursing practice guideline among nurses in advanced general hospitals and general hospitals in Korea. Methods: The subjects were 234 nurses who practice the intravenous infusion in 24 advanced general hospitals and general hospitals. Data were collected between October 5 and November 2, 2015 by mail (return rates: 97.5 %). Data were analyzed using descriptive statistics, t-test, and ANOVA with SPSS/ WIN 21. Results: The average guideline diffusion score and levels of diffusion was 3.39±0.58 (level of “use sometimes”). 44 recommendations (46%) were in level of “use always” and 50 recommendations (53%) were in “use sometimes”. Extent of diffusion were significantly different according to present status (F=2.81, p=.040) and education (F=4.35, p=.014). The facilitating factors to use the guideline were education by department of nursing service, convenient composition of guideline and barrier factors were “no time to use the guideline”, “don’t know the guideline” and “there is no guideline at ward”. Conclusion: Extent of diffusion of Intravenous infusion nursing practice guideline among nurses was moderate level. The strategies for promoting the implementation of guideline in clinical practice is needed. Further study of outcome evaluation of guideline implementation will be needed.

Purpose: This study was conducted to investigate the level of attitudes to and diffusion of Intravenous infusion nursing practice guideline in advanced general hospitals and general hospitals in Korea. Methods: The subjects were 41 nurses who were in charge of guideline diffusion in 41 advanced general hospitals and general hospitals. Data were collected between September 25 and November 2, 2014 by mail (return rates: 68.3 %). Data were analyzed using descriptive statistics, t-test, and ANOVA with SPSS/WIN 18.0. Results: The average attitude score of guideline among nurses in charge of guideline diffusion were 3.98±0.38 (range 1~5). The average guideline diffusion score and levels of diffusion was 2.26±1.39 (level of “persuasion of nursing knowledge”) for staff nurses, 3.05±0.86 (level of “use sometimes”) for nurses in charge of guideline diffusion, 2.87±0.78 (level of “use sometimes”) for nurse directors. 41.4 % of nursing divisions in 41 hospitals reviewed the intravenous infusion nursing practice guideline, educated the guideline to nurses, and implemented the guideline in clinical practice for guideline diffusion. Conclusion: Study findings indicate that nurses in charge of intravenous infusion nursing practice guideline in hospitals had positive attitude for guideline. The levels of guideline diffusion in hospitals was “persuasion of nursing knowledge” to “use sometimes”. Therefore, the strategies for promoting the diffusion and implementation of guideline in clinical practice in the level of nursing division is needed. Further study of outcome evaluation of guideline implementation will be needed.
