        응급 의료체계용 이동 의료단위에 대한 기초연구

        Recently, the disaster situation has been diversified and compounded worldwide as well as in Korea. And the damage scale of each disasters have increased, and following economic loss is also increasing immensely. In Korea, especially the out break frequency of large-scale disasters has increased. Disasters caused by earthquakes are also very likely to occur. When a major accident such as Mauna resort collapse accident, Ferry sewol sinking accident occurred, systematic emergency medical care at the site at the time of disaster occurred in order to operate an emergency medical facility corresponding to the site, which is not systematic. The need for this service is urgent. If highly reliable emergency medical system function at appropriate times in the disaster site, secondary damage can be reduced and the whole situation would be back to normal shortly. Therefore, it is necessary to provide an emergency medical system capable of quickly and safely responding to disaster sites in relation to various damage scale. For the purpose, thorough analysis on mobile medical units of inside and outside the country was accomplished. As a result, Mobile Medical Unit of outside the country are trailer type, tent type, container type etc. At home, these types have been studied and will be proposed early next year as a temporary solution. Here we introduce suitable types for our country. 마우나 리조트 붕괴사고, 세월호 침몰사고 등 대형 재난사고들이 발생하였을 때 체계적이지 못한 현장대응과 응급의료시설의 운영으로 인해 재난발생 시 현장에서의 체계적인 응급의료서비스의 필요성이 대두되고 있다. 재난, 재해 발생 시 적절한 시간 내에 안정적인 응급의료서비스가 제공 된다면 2차적인 피해를 줄일 수 있으며 피해에 대한 복구도 신속하게 이루어 질 수 있다. 따라서 다양한 재난의 피해유형과 규모에 맞추어 신속하고 안전하게 재난현장에 대응 할 수 있는 응급의료체계가 구축될 필요가 있다. 이를 위해 현재 국내, 외 사례들을 조사하여 이동 의료단위의 유형을 파악하고, 규모별 분석을 실시한 결과 국외에는 트레일러유형, 텐트유형, 컨테이너유형 등의 이동 의료단위들이 실제 운용되고 있었으며 국내에서는 그러한 운용 사례들을 조사하고 종합하여 정부에 제안하는 단계에 있다. 국내에 적합한 이동형 의료단위의 개발과 제안을 위해 실제운용 사례들의 단위유형, 규모 및 조합적 배치에 대한 심층적인 분석을 토대로 국내에 적절한 유형을 제시한다.

        친수성 및 소수성 아크릴 재질의 인공수정체 삽입술 후 장기간 임상 결과의 비교분석

        Purpose: To assess long-term clinical outcomes and factors influencing refractive change after implantation of hydrophilic (Akreos AO) and hydrophobic (Acrysof IQ) acrylic intraocular lens. Methods: After phacoemulsification, intraocular lens was inserted in the bag (Akreos AO for 84 eyes and Acrysof IQ for 19 eyes). Uncorrected visual acuities (UCVA), refraction in both spherical equivalent (SE) and astigmatism were compared longitudinally in more than 2 years follow-up. Factors associated with SE change ≥0.5 diopter (D) after Akreos AO implantation were analyzed with logistic regression. Results: Although Akreos AO showed hyperopic change mainly within 6 months, UCVA and astigmatism change were not statistically significant during follow up (+0.15 ± 0.43 D, p = 0.027). UCVA, SE, astigmatism were not changed after Acrysof IQ implantation and not significantly different between two groups during follow up. Preoperative anterior chamber depth was a sole factor associated with SE change ≥0.5 D after Akreos AO implantation (p = 0.006).Conclusions: Possible hyperopic shift after Akreos AO implantation should be considered in setting target diopter, especially in eye with shallow anterior chamber.       • 건물공개공지의 유형화를 위한 기초적 연구

        The study tries to find out basic elements that make a public-open-space(POS) of building in proper use as intended in building code. Several previous studies haves mentioned POS's physical elements but were not enough to encourage the use of them; but imposed impractical regulations for the building axle. We made a field survey on POS in 13 buildings and found that the building itself acts an important role to encourage the use of POS. We classified the case study buildings into centripetal building and centrifugal building according to the use, spatial organization, and location of building in urban setting. With the cross combination of these two factors, ie., with centripetal building or centrifugal building, and with the level of abundance of POS's physical elements, we propose 4 types of POS.

        The aim of the study is to find children's responses, to verify the their rationality, and to check possibility of conversion to 'Open Classroom'. For the study. Songjung Primary School was selected and questionnaire survey were conducted to the pupil. Their like/dislike were answered on 1) classroom interiors, 2) building and outdoor environment. As a result, the children's reaction of spaces is architecturally appropriate but there were little difference between grade Levels. The existing facilities can be used as 'Open Classroom' on the condition that 1) class pupil size under 30 children, 2) reorganization of the building plan into 3 classroom and closed corridor.

        Deposition of Cr-doped SrZrO3 thin films on Si substrates and their resistance switching characteristics

        Cr-doped SrZrO3 perovskite thin films were deposited on SrRuO3 bottom electrode/SiO2/Si(100) substrates by pulsed laser deposition. The SrZrO3 : Cr perovskite and SrRuO3 bottom electrode showed a well controlled interface, as well as good resistive switching behavior with an ON/OFF ratio > 103. Resistive switching memory devices made from Cr-doped SrZrO3thin films deposited on Si substrates are expected to be more compatible with conventional Si-based electronics.

        실리콘 기판위에서의 Cr-Doped SrZrO<sub>3</sub> 박막의 저항변화 특성

        One of the weak points of the Cr-doped SZO is that until now, it has only been fabricated on perovskite substrates, whereas NiO-ReRAM devices have already been deposited on Si substrates. The fabrication of RAM devices on Si substrates is important for commercialization because conventional electronics are based mainly on silicon materials. Cr-doped ReRAM will find a wide range of applications in embedded systems or conventional memory device manufacturing processes if it can be fabricated on Si substrates. For application of the commercial memory device, Cr-doped $SrZrO_3$ perovskite thin films were deposited on a $SrRuO_3$ bottom electrode/Si(100)substrate using pulsed laser deposition. XRD peaks corresponding to the (112), (004) and (132) planes of both the SZO and SRO were observed with the highest intensity along the (112) direction. The positions of the SZO grains matched those of the SRO grains. A well-controlled interface between the $SrZrO_3$:Cr perovskite and the $SrRuO_3$ bottom electrode were fabricated, so that good resistive switching behavior was observed with an on/off ratio higher than $10^2$. A pulse test showed the switching behavior of the Pt/$SrZrO_3:Cr/SrRuO^3$ device under a pulse of 10 kHz for $10^4$ cycles. The resistive switching memory devices made of the Cr-doped $SrZrO_3$ thin films deposited on Si substrates are expected to be more compatible with conventional Si-based electronics.

        산소 분압의 변화에 따른 Cr-Doped SrZrO<sub>3</sub> 페로브스카이트 박막의 저항변화 특성

        A non-volatile resistive random access memory (RRAM) device with a Cr-doped $SrZrO_3/SrRuO_3$ bottom electrode heterostructure was fabricated on $SrTiO_3$ substrates using pulsed laser deposition. During the deposition process, the substrate temperature was $650^{\circ}C$ and the variable ambient oxygen pressure had a range of 50-250 mTorr. The sensitive dependences of the film structure on the processing oxygen pressure are important in controlling the bistable resistive switching of the Cr-doped $SrZrO_3$ film. Therefore, oxygen pressure plays a crucial role in determining electrical properties and film growth characteristics such as various microstructural defects and crystallization. Inside, the microstructure and crystallinity of the Cr-doped $SrZrO_3$ film by oxygen pressure were strong effects on the set, reset switching voltage of the Cr-doped $SrZrO_3$. The bistable switching is related to the defects and controls their number and structure. Therefore, the relation of defects generated and resistive switching behavior by oxygen pressure change will be discussed. We found that deposition conditions and ambient oxygen pressure highly affect the switching behavior. It is suggested that the interface between the top electrode and Cr-doped $SrZrO_3$ perovskite plays an important role in the resistive switching behavior. From I-V characteristics, a typical ON state resistance of $100-200\;{\Omega}$ and a typical OFF state resistance of $1-2\;k{\Omega}$, were observed. These transition metal-doped perovskite thin films can be used for memory device applications due to their high ON/OFF ratio, simple device structure, and non-volatility.

        국내외 재난대응 이동형 병원의 배치특성 비교분석 연구

        Purpose: The purpose of this study is to analyze the deployment characteristics of korean and foreign mobile hospitals and to draw out the advantages and disadvantages based on the movement of patients. Methods: The arrangement and user movement lines of foreign mobile hospitals which were actually utilized, and the arrangements and user movement lines of korean mobile hospitals are compared and analyzed. Based on the results of the comparative analysis, we suggest the optimal placement of mobile hospital in Korea. Results: The recently developed Korean mobile hospitals have made many improvements. However, there is no practical use case for the mobile hospital in Korea, so it is necessary to establish the basis for the mobile hospital development site and the site access considerations. In addition, there is no detailed analysis of the movement of the administrators other than the medical staff, and it is considered that research on safe waste disposal is further needed. Implications: It is highly likely to be used as a basic data to find out the combination method of mobile hospitals that can efficiently deal with disasters through the arrangement and movement analysis of mobile hospitals in Korea and abroad.

        후천성 면역 결핍증 환자에서 발생한 각결막의 편평세포암 1예

        Purpose: To describe the clinical and histopathological findings of corneal and conjunctival squamous cell carcinoma (SCC) in a patient with acquired immune deficiency syndrome (AIDS). Case summary: A 45-year-old Korean male who had been diagnosed of AIDS 20 years before presented with vascularized mass in the inferotemporal conjunctiva and in the cornea. The patient underwent an en bloc excisional biopsy of the mass, application of mitomycin C, and repeated cycles of freeze-thaw. Histopathology revealed dysplastic squamous epithelium in the full thickness of epithelial layer. Immunohistochemical staining for proliferation markers showed that the tumor had a high proliferation index: Ki-67 was positive throughout the full epithelial layer, and both p53 and p16 were positive. The patient had an adjuvant chemotherapy with topical interferon alpha 2b (IFNα 2b) for 8 months after surgery. Conclusions: Given the high proliferation index of the tumor in this patient, close observation and combined adjuvant chemotherapy will be required to prevent recurrence in a patient with AIDS.
