Most thyroid nodules are cytologically benign and can be managed nonsurgically. Asymptomatic benign nodules require follow-up without treatment. Cosmetic problems and/or compression-related symptoms may be indications for treatment. Until now in clinical practices, nonsurgical image-guided therapy includes percutaneous ethanol injection, radiofrequency ablation, laser ablation, and high intensity focused ultrasound. Percutaneous ethanol injection should be used as the first-line therapy for recurrent symptomatic cystic nodules. Radiofrequency ablation is an effective procedure for obtaining shrinkage and improving symptoms of solid thyroid nodules. Better understanding of image-guided therapy makes physicians personalize the management of benign thyroid nodules according to a cost-benefit analysis.
인삼 사포닌이 일산화탄소중독 및 노화과정에서 흰쥐의 뇌에너지 대사물 함량 변화에 미치는 영향

This study was performed to investigate the effects of ginseng saponins on the cerebral energy metalbolite's contents influenced by carbon monoxide(CO) intoxication. Each experimental group was divided young (5~8 weeks) and aged (43~52 weeks) rats, and they were exposed at 5,000 ppm CO(72% HbCO) for 30 min. One of the other groups was pretreated with ginseng saponins for 5 days before CO intoxication. The contents of cerebral energy metabolites in cerebral cortex, stratum and hypothalamus were measured. In cerebral cortex of both young and aged rats, the levels of ATP and creatine phosphate were significantly decreased, while those of lactate were significantly increased. There was no difference between the levels of cerebral energy metabolites of young and aged rats. Pretreatment of ginseng saponins before CO intoxication lowered decrease of the levels of cerebral energy metabolites and ATP levels were significantly recovered. On the other hands, contents of lactate in stratum and hypothalamus of young rats were significantly increased and the levels of ATP and creatine phosphate in stratum and hypothalamus were completely recovered at 2 weeks after CO intoxication. The results suggest that ginseng saponins have an ameliorating action against disturbance of the cerebral energy metabolites by CO intoxication.
상피 화생성 유방암의 초음파 소견 : 병리 소견과의 비교

Purpose: We investigated the ultrasonographic (US) appearance of metaplastic squamous breast carcinoma with the pathologic correlation. Materials and Methods: During an 8-year period, the US appearances of 10 patients with metaplastic squamous breast carcinoma were retrospectively analyzed on the basis of the Breast Imaging and Reporting Data System (BI-RADS) - US lexicon. These 10 patients included 9 patients having invasive adenocarcinomas of the breast with more than 30% squamous metaplasia, and one patient had pure primary squamous cell carcinoma. We correlated the US findings with the pathologic findings. Results: On US, the majority of the tumors showed oval shapes (70%), indistinct margins (50%), parallel orientation (80%), echogenic halos (60%), complex echogenicity with solid and cystic components (60%), and posterior enhancement (70%). The findings of calcifications (20%) and metastatic axillary lymph nodes (10%) were rare. On pathologic examination, half of the tumors showed infiltrative microscopic margins. All six cases showing complex echogenicity with solid and cystic components on US were pathologically related to the cystic or necrotic portion, and three of these cases had hemorrhage. Conclusion: On US, metaplastic squamous breast carcinoma mainly manifested as oval, complex, echoic masses with indistinct margins and posterior enhancement that was pathologically related to the cystic or necrotic portions. 목적: 상피 화생성 유방암의 초음파 소견을 알아 보고 병리적 소견과 비교하고자 하였다. 대상과 방법: 8년간 10명의 상피 화생성 유방암을 가진 환자의 초음파 소견을 Breast Imaging and Reporting Data System (BI-RADS) - US lexicon 에 근거하여 후향적으로 분석하였다. 이들 10명의 환자들은9명의 30% 이상 의 상피 화생을 가진 화생성 유방암과 1명의 원발성 상피세포암으로 구성되었다. 초음파 소견과 병리적 소견을 비 교하였다. 결과: 초음파상에서 종괴의 대부분은 타원형(70%), 불분명한 경계(50%). 평행한 방향(80%), 에코성의 달무리 (60%), 고형과 낭성 성분을 가진 복합성 에코(60%), 그리고 후방 증가 음영(70%)을 보였다. 석회화(20%)와 전 이성 액와 림프절(10%)의 소견은 드물었다. 병리적 소견에서 종괴의 반은 침윤성의 미세현미경적 경계를 보였다. 초음파상에서 복합성 에코를 보인 6예는 병리적으로 낭성과 괴사 부분과 연관되어 있었고 그 중 3예는 출혈을 포 함했다. 결론: 초음파상 상피 화생성 유방암은 병리적으로 고형과 낭성 부분과 연관된 복합성 에코, 불분명한 경계, 후방 음 영증가를 주로 나타냈다.

The effects of ginseng saponins on the distribution of nerve cells in cerebral cortex of carbon monoxide (CO)-intoxicated mice were studied in the young (5~8 weeks) and aged (43~52 weeks) mice. Mice were exposed to 5000ppm of CO for 40 minutes (72% HbCO). After that, nerve cells in motor(area 4), somatosensory(area 3) and visual(area 17) area of cerebral cortex was observed. In young mice, the number of nerve cells in each area was significantly decreased on 1st, 7th and 14th day after CO intoxication. In aged mice, that was also decreased after CO intoxication. Especially the number of the nerve cells in motor and somatosensory area was significantly decreased on 1st and 7th day, while that in visual area was decreased only on 1st day. The number of nerve cells in young mice pretreated with ginseng saponins were significantly decreased less on 7th and 14th day than that of untreated mice. The number of nerve cells in each area of normal aged mice was larger than that of normal young mice. The results suggest that CO exposure causes local degeneration or disturbance of nerve cells and delayed neurologic sequelae, while ginseng saponins might play a role of protective action on the nerve cells which were damaged by CO.
