        완전무치악 환자에서 디지털 가이드 수술 방식을 이용한 무피판절개 임플란트 식립증례: 증례보고 및 5년 추적관찰

        One of the fastest growing segments of implant dentistry is the utilization of computed tomography (CT) scan data and treatment planning software in conjunction with guided surgery for implant reconstruction cases. Computer assisted planning systems and associated surgical templates have established a predictable, esthetic, functional technique for placing and restoring implants. Especially, a philosophy of restoratively driven implant placement has been generally adopted. Recently, a variety of commercial dental fields have released their scanning and fabricating protocols and methods for restorations. This process is still being investigated and developed for the most precise and predictable outcome. This case report describes a female patient who wanted dental implants in fully edentulous areas. Restoratively driven implant placements were performed with surgical guide and the patient was fully satisfied with the clinical results, and at 5-year post restorative follow-up assessment, both implant and prosthesis were proved clinical success. 디지털 방식을 이용한 치과 임플란트 진료에서 가장 주목받는 분야 중 하나가 디지털 소프트웨어와 컴퓨터 단층촬영을 이용하여 수술용 스텐트를 제작, 임플란트를 식립하는 분야이다. 컴퓨터를 이용한 디지털 임플란트 치료계획 시스템과 이를 이용한 수술용 가이드는 예지성과 심미성, 기능성을 갖춘 임플란트의 식립과 수복을 가능하게 한다. 특히 이러한 가이드 수술은 임플란트 수복물의 형태와 기능을 고려한 임플란트의 식립 시 더욱 유리하다. 최근 여러 제조사에서 보철물 제작을 위해 치과용 스캔부터 제작방법까지 다양한 디지털 방식의 제작과정을 제시하고 있다. 이러한 방법들은 정확하고 예지성 있는 예후를 위해 여러 연구자들에 의해 연구되어지고 있으며, 점차 발전하고 있다. 이번 증례는 완전무치악의 여성 환자에서 임플란트 고정성 보철물을 이용한 수복증례이다. 하악은 all-on-four 방식을 적용하여 4개의 임플란트를 식립하였으며, 상악은 이에 기초하여 6개의 임플란트를 식립하였다. 최종보철물에 기초한 임플란트의 식립을 위해 임플란트 수술용 스텐트를 이용하였으며, 환자는 치료 결과에 만족하였으며, 5년 추적관찰 결과, 임플란트와 보철물 모두 안정적으로 유지되고 있다.

        손상 받은 각막 상피세포가 중배엽 줄기세포의 분화에 미치는 영향

        Purpose: To identify the effects of microenvironmental changes caused by human corneal epithelial damages to characteristics or differentiation of human mesenchymal stem cells (hMSCs). Methods: Artificial corneal damage was induced onto a cultured monolayer of human corneal epithelial cells. hMSCs were then co-cultured with damaged human corneal epithelial cells (dIHCE). Morphological changes in the co-cultured hMSCs were observed. To elucidate the differentiation of hMSCs into corneal keratocytes or epithelial cells, the expressions of α-smooth muscle actin, keratin-3/-12, and E-cadherin were confirmed by immunofluorescence. Results: hMSCs co-cultured with dIHCE showed enhanced adherence in the neighborhood of dIHCE and morphological change into dendritic shapes at 6 days post-seeding. Although the expression of α-smooth muscle actin, known as hMSCs marker, significantly decreased at the dIHCE-contacted site of hMSCs; there were no expressional changes on keratin-3/-12 and E-cadherin, the markers of corneal epithelial cells. Interestingly, positive expression of corneal epithelial marker keratin-3/-12 was observed in dIHCE co-cultured hMSCs. hMSCs co-cultured with normal human corneal epithelial cells (nIHCE) were unable to attach, and showed no change in the expression of α-smooth muscle actin. Conclusions: It is proposed that dIHCE causes a morphological change in hMSCs, and decreased expression of α-smooth muscle actin. These results suggest that dIHCE can affect a change in the characteristics and differentiation of hMSCs.

        Replica technique과 Triple-scan protocol을 이용한 지르코니아 전부도재관의 변연 및 내면 적합도에 관한 비교 연구

        목적: 본 연구는 보철물의 적합도를 계측하는 두 가지의 측정방법을 평가하기 위해 지르코니아로 전부도재관을 제작하고 replica technique과 triple-scan protocol을 이용하여 전부도재관의 변연 및 내면 적합도를 비교 연구하고자 하였다. 재료 및 방법: 타이타늄의 지대치 모형을 제작하여 20개의 지르코니아 전부도재관을 제작하였다. 제작된 지르코니아 전부도재관 10개는 replica technique을 이용해 복제하고 복제된 시편을 협설, 근원심으로 절단하여 변연 및 내면 적합도를 측정하였고, 다른 10개의 전부도재관은 triple-scan protocol을 이용하여 협설과 근원심으로 단면을 형성하고 변연 및 내면 적합도를 계측하여 통계 분석하였다 (${\alpha}=.05$). 결과: 변연과 내면 적합도는 replica technique $49.86{\pm}29.69{\mu}m$, triple-scan protocol $75.35{\pm}64.73{\mu}m$로 유의한 차이가 있었고 (P < .001), 내면 적합도만을 측정한 결과 각각 $58.38{\pm}31.77{\mu}m$, $64.00{\pm}46.43{\mu}m$로 유의한 차이가 없었다 (P > .343). 결론: Replica technique과 triple-scan protocol을 이용하여 전부도재관의 변연 및 내면 적합도를 비교 측정한 결과 triple-scan protocol에서의 변연 적합도가 크게 측정되었고, 내면 적합도만 비교해 본 결과 통계적인 유의한 차이가 없었다. This study was conducted to compare the marginal and internal fit of all ceramic crown using the replica technique and the triple-scan protocol. Materials and methods: Twenty zirconia ceramic crowns were fabricated using titanium abutment model. All crowns were divided into two groups of 10 each, depending on the replica technique and the triple-scan protocol. The internal and marginal fit of 10 zirconia ceramic crowns were measured at 17 points for each specimen using the replica technique. The other 10 ceramic crowns were measured using the triple-scan protocol. Statistical analysis was performed by t-test (${\alpha}=.05$). Results: The mean and standard deviation of marginal and internal fit were significantly different between the replica technique ($49.86{\pm}29.69{\mu}m$) and triple-scan protocol ($75.35{\pm}64.73{\mu}m$, P<.001). The mean and standard deviation of internal fit except marginal fit were $58.38{\pm}31.77{\mu}m$ and $64.00{\pm}46.43{\mu}m$, respectively (P>.343). Conclusion: There was a statistically significant difference in the marginal fit measured by the two methods. However, there was no statistically significant difference in the internal fit between the two methods.

        단일구조 지르코니아 크라운의 소결 후 추가 소성 과정이 변연 및 내면 적합도에 미치는 영향

        목적: 본 연구는 단일구조 지르코니아 크라운의 소결 후 소성 과정이 변연 및 내면 적합도에 미치는 영향에 대하여 3차원으로 평가해 보고자 하였다. 재료 및 방법: 타이타늄 지대치 모형을 제작하여 10개의 단일구조 지르코니아 크라운을 제작하였다. 제작된 단일구조 지르코니아 크라운을 소결 한 상태를 대조군으로, 소결 후 광택을 위해 추가적인 소성단계를 거친 후를 실험군으로 설정하였다. 각 군에서 triple-scan protocol을 이용하여 협설과 근원심으로 단면을 형성하고 변연 및 내면 적합도를 계측하여 통계 분석하였으며, 삼차원 표면 비교를 시행하였다 (${\alpha}=.05$). 결과: 변연과 내면 적합도를 분석한 결과 근심 축벽에서 대조군($32.0{\pm}24.3{\mu}m$)과 실험군 간($17.0{\pm}10.8{\mu}m$)의 통계적 유의한 차이가 있었고 (P < .020), 원심 축벽에서 대조군($60.2{\pm}24.3{\mu}m$)과 실험군($71.8{\pm}21.5{\mu}m$)간의 통계적으로 유의한 차이가 있었다 (P < .01). 나머지 측점지점에서는 통계적으로 유의한 차이가 없었다. 결론: 단일구조 지르코니아 크라운에서의 소결 후 추가적인 소성은 내면의 변형에 큰 영향을 미치지 않았고 임상적으로 허용 가능한 범위에 있었다. Purpose: To evaluate an effect of additional firing process after sintering of monolithic zirconia crown on marginal and internal fit through three-dimensional analysis. Materials and methods: Ten monolithic zirconia crowns were fabricated using titanium abutment model. Monolithic zirconia crowns were designed, milled, and sintered as a control group, and additional firing with coloring was performed as a test group. Three dimensional analysis were performed by using triple-scan protocol, and cross-section analysis on mesio-distal and disto-lingual section was evaluated to measure marginal and internal fitness. Then, three-dimensional surface difference on between two groups was evaluated (${\alpha}=.05$). Results: There was statistically significant difference between the control group ($32.0{\pm}24.3{\mu}m$) and the test group ($17.0{\pm}10.8{\mu}m$) in the mesial axial wall (P < .02) and the control group ($60.2{\pm}24.3{\mu}m$) and the test group ($71.8{\pm}21.5{\mu}m$) in the distal axial wall (P < .01). There was no statistically significant difference at the remaining point. Conclusion: There was no statistical significance on the deviation of inner surface of crown according to firing number, and the results of both group were considered clinically acceptable.

        만성 C형 간염 환자에서 알파 인터페론 치료 중발생한 후천성 반응성 천공성 교원증

        Reactive Perforating Collagenosis (RPC) is a kind of perforating dermatosis, which shows transepidermal elimination of altered collagens. Acquired form is developed in an adult without family history, is favorably developed in scratched region due to pruritic sense. Acquired RPC has been reported to be associated with various systemic disorders, such as diabetes mellitus, liver disease, chronic renal failure, lymphoma, AIDS, hypothyroidism, hyperparathyroidism, and neurodermatitis, accompanied by itching & scratching. We report a case of acquired RPC in a patient with HCV hepatitis who was treated with interferon alpha. Skin lesions showed multiple erythematous centrally umbilicated, crusted papules of variable sizes on the extensor surfaces of both shins. (Korean J Dermatol 2008;46(6):788∼791)

        환경심리행태를 고려한 청소년문화의집 공간환경 개선방안에 관한 연구

        The 5-day schoolweek system at the present has increased spare time of the youth. The society should use this opportunity to lead the youth to grow as a responsible community member as offering them programs to fulfill their desires and potential abilities. The purpose of this study is to have the improvements to give the youth appropriate and comfortable space. As the scope of the study, I selected a total of 5 places as each rating (the best, good, moderate, insufficient) 4 places in the comprehensive assessment conducted by youth facilities association among the independent buildings located in Seoul and 1 place after the assessment. As the methods of the study, with regard to environmental psychology and behavior and Youth cultural center, I investigated them theoretically as literature survey method, organized and analyzed, put them together through facilities research analysis and the survey. The research results are as follows : First, a static individual space should be placed in well accessibility due to high affinity for the facilities. Second, a dynamic individual space needs the place and equipment for physically and mentally healthy leisure culture. Third, a dynamic group space needs to provide ensuring service hours, setting proper size of space, comfortable indoor environment considering the relationship between physical activity and others. Fourth, a static group space needs to vary the programs to develop potential abilities by a variety of experiences to maintain the correlation the layout adjacent to leader"s office.

        신장이식 후 발생한 Parvovirus B19에 의한 재발성 범혈구부전증 1례

        Pure red cell aplasia due to parvovirus B19 infection after renal transplantation has been rarely reported, and few have been described about pancytopenia due to parvovirus B19. We report a case of recurrent pancytopenia due to parvovirus B19 infection in a renal transplant recipient. Ten days after transplantation, the patient developed severe pancytopenia (WBC 400/mm3, platelet 29,000/mm3, hemoglobin 6.8 g/dL) and his blood sample was positive for parvovirus B19 DNA PCR. Two weeks after 5-day administration of IVIG 400 mg/kg/day, pancytopenia resolved and tacrolimus was converted to cyclosporine A for reducing immunosuppressant potency. However, recurrent pancytopenia developed two months after IVIG treatment. Both blood and bone marrow samples were positive again for parvovirus B19 DNA PCR. Although pancytopenia persisted after another 5-day administration of IVIG 400 mg/kg/day, excellent hematological response has been achieved with single dose of IVIG 1 g/kg/day. Our case suggested that parvovirus B19 infection should be considered in renal transplant recipients with unexplained severe pancytopenia. High dose IVIG would be an effective therapeutic option, if the infection is recurrent or refractory to the usual dose of IVIG.
