Background: Synchrotron small-angle X-ray scattering (SAXS) is a very useful technique for experimental study of the nano-structure of the nervous system of animals. The study was designed to evaluate nerve preservation methods for the measurement of SAXS patterns. Methods: Normal sciatic nerves extracted from male Sprague- Dawley rats were preserved in saline (N = 2), formalin (N = 2) or liquid nitrogen (N = 2) for 1 day, followed by measurement of SAXS patterns. SAXS patterns of normal sciatic nerves (N = 3) extracted just before the initiation of the experiment were used as controls. The study was carried out using the 4C1 beamline at Pohang Accelerator Laboratory in Korea. Incoming X-rays were monochromatized at 11 keV using a double multilayer (WB4C) monochromator with beam size of approximately 0.5 (V) × 0.8 (H) mm2. The exposure time was set at 60 sec, and 8 to 12 images per sample were acquired at a 0.5 mm interval. Results: The periodic peaks of interfibrillar space between collagen fibrils were undetectable. The periodic peaks of the myelin sheath and collagen fibers were weakly detected or undetected in the nerves preserved in normal saline or formalin. The periodic peaks and intensity of the myelin sheath, collagen fibers, and interfibrillar space between collagens in the nerves preserved in liquid nitrogen were comparable to those of nerves in the ex vivo state. Conclusions: The study results indicated that preservation of nerves in liquid nitrogen is adequate for measurements with SAXS. However, saline and formalin preservation techniques were inadequate for SAXS measurement.

아산만 중부 해역의 여름철 대형저서동물군집의 구조

A study on the community structure of macrobenthos was accomplished at 10 stations in the Asan Bay, western coast of Korea. Sampling was performed for times using a Smith-McIntyre grab (0.05㎡) at ten stations to August 1996. The macrobenthos comprised of 105 species, annelids (47 spp. 44.8%), arthropods (36 spp. 31.4%), molluscs(12 spp. 11.4%) and others (13 spp. 12.4%). Mean density was 913 ind./㎡ and biomass was 101.2 gwwt./m2. Annelids was a density-dominant group with a mean density of 733 ind./m2 which occupied 80.3% of the total individual benthic animals. The major density-dominant species were four polychaetes, Capitella capitata (251 ind./㎡), Prinospio japonicus (111 ind./㎡), Hemipodus yenourensis (72 ind./㎡) and Tylorrhynchus heterochaetus (51 ind./㎡). Clustering analysis showed that the study area could be divided into two stational groups and three stations.

Effect of neuromuscular blockade reversal by pyridostigmine on spectral entropy values during recovery from desflurane anesthesia: a prospective, randomized, double-blind, controlled trial

Background: According to several studies investigating the relationship between muscle activity and electroencephalogram results, reversal of neuromuscular blockade (NMB) may affect depth of anesthesia indices. Therefore, we investigated the effect of pyridostigmine on these indices via spectral entropy. Methods: Fifty-six patients scheduled for thyroidectomy or parotidectomy were included in this study and randomized into two groups. At the start of skin suturing, the desflurane concentration was adjusted to 4.2 vol% in both groups. Following this, the pyridostigmine group (group P, n = 28) was administered pyridostigmine 0.2 mg/kg mixed with glycopyrrolate 0.04 mg/kg, while the control group (group C, n = 28) received normal saline. Entropy values (response entropy [RE] and state entropy [SE]), train of four (TOF) ratio, and end-tidal desflurane concentration were recorded from point of drug administration to 15 minutes post-drug administration. Results: Mean RE values at 15 minutes, when the maximum effect of pyridostigmine was anticipated, showed a statistically significant difference between groups (53.8 ± 10.5 in group P and 48.0 ± 8.8 in group C; P = 0.030). However, mean SE at 15 minutes showed no significant difference between the two groups (P = 0.066). At 15 minutes, there were significant differences in the TOF ratio between the two groups (P < 0.001). Conclusions: NMB reversal by pyridostigmine significantly increased RE values but not SE values. This finding suggests that spectral entropy may be a useful alternative tool for monitoring anesthetic depth during recovery from anesthesia in the presence of electromyogram activity.

강박성향을 가진 여아의 예술치료 사례연구

본 연구의 목적은 우울증을 가진 어머니로 인하여 강박적인 방어기제를 가지게 된 여아를 대상으로, 충분히 좋은 모성적 돌봄을 제공받지 못했던 초기 대상관계 경험을 가진 아동에게 Winnicott의 “안아주기” 환경으로서의 예술치료가 적절하게 다가가는 과정을 살펴보는 것이다. 연구대상자는 서울시에 소재하고 있는 초등학교 2학년 여아이며, 단일사례연구이다. 본 연구의 주요 결과는 다음과 같다. 대상아동에게 “안아주기” 환경으로서의 예술치료 경험은 첫째, 충분히 좋은 환경으로써 스스로 양육하고 다시 자라게 하였으며 둘째, 강박적인 방어기제와 정서적인 문제가 점차 완화되는데 도움이 되었다. 본 연구는 강박성향을 가진 아동에게 예술치료를 적용하여 문제행동의 변화과정을 살펴봄으로써 아동기에 발생할 수 있는 강박장애에 대한 효과적인 개입방법을 살펴보고 논의하였다는 데 그 의의가 있다.

집단미술치료 프로그램이 미술치료사의 자기성찰에 미치는 효과

본 연구는 집단미술치료 프로그램이 미술치료사의 자기성찰에 미치는 효과에 대해 알아보았다. 연구대상은 임상경력 3년 이하의 초보미술치료사 13명으로, 실험집단 7명, 통제집단 6명 선 착순 배치한 후 실험집단은 2020년 7월 8일부터 8월 5일까지 총 9회기의 집단미술치료 프로그 램을 주 2회 110분씩 진행하였다. 연구도구는 자기성찰 척도, 미술치료사 발달수준 척도를 사 용하였으며, 프로그램 과정 내 자기성찰 변화를 단계별로 살펴보았다. 연구 결과는 첫째, 실험 집단의 자기성찰 전체 척도와 하위영역에서 유의미한 향상을 보였다. 둘째, 집단미술치료 프로 그램 과정에서 자기성찰과 관련된 긍정적인 변화가 나타났다. 미술치료사의 자기성찰에는 담는 것과 담기는 것의 상호작용을 경험하는 집단미술치료 프로그램이 필요하며 이는 미술치료사의 전문성 발달에 영향을 주었다. 따라서 본 연구는 집단미술치료 프로그램이 미술치료사의 자기 성찰에 긍정적인 효과가 있으며 경험으로부터 의미를 알아가는 과정임을 확인할 수 있었다. In this study, the effect of group art therapy program on self-reflection of art therapist was investigated. The subjects of this study were 13 beginner art therapists within 3 years of clinical experience. Among them, 7 were grouped into the experimental group and 6 were grouped into the control group. Group art therapy programs were provided to the experimental group for total of 9 sessions for 110 minutes twice a week from July 8 to August 5, 2020. The research tool used the self-reflection scale and the art therapist development level scale and the changes in self-reflection that occurred within the program process were examined step by step. The results are follows: First, the experimental group showed significant improvement in the overall self-reflection scale. Second, positive changes related to self-reflection showed during the group art therapy program. For the art therapist's self-reflection, a group art therapy program that can experience the interaction of container-contained is needed. It also influenced the professional development of art therapists. Therefore, this study confirmed that the group art therapy program has a positive effect on self-reflection of art therapists and is a process of learning meaning from experience.

여름철 보현천의 저서동물군집의 구조

The benthic macroinvertebrates in Bohyeon stream occured 61 species, 32 families, 13 orders, 7 classes in 4 phyla. Among them, aquatic insects containing 52 species were the major group and occupied 85.2% of the whole. In particular, Diptera containing 15 species was the most dominant. Second, Ephemeroptera containing 14 species was dominant. Number of species were similar at each station. Ephemeroptera were a density-dominant group with a mean density of 920 ind./㎡ which occupied 85.0% of the total individual of benthic animals. The major density-dominant species were five species of Ephemeroptera, Choroterpes (E.) altioculus (231 ind./㎡), Ecdyonurus kibunensis (194 ind./㎡), Serratella setigera (152 ind./㎡), Caenis KUa (109 ind./㎡), Ecdyonurus levis (69 ind./㎡). Clustering analysis showed that the study area could be divided into two stational groups. Number of species and species diversity (H') were more abundent Group A than Group B. Group A located in the upper stream.

Target-controlled infusion of remifentanil for conscious sedation during spinal anesthesia

Background: The aim of this study was to define the optimal target concentration of remifentanil which effectively achieves conscious sedation without significant vital sign changes and side effects during spinal anesthesia. Methods: Sixty patients underwent spinal anesthesia with 0.5% hyperbaric bupivacaine (8-16 mg), and were infused with a target controlled infusion (TCI) of remifentanil at 1.0 ng/ml (group R10, n = 15), 2.0 ng/ml (group R20, n = 15),3.0 ng/ml (group R30, n = 15), and 3.5 ng/ml (group R35, n = 15). Observer’s assessment of alertness/sedation (OAA/S)scale, the bispectral index (BIS), anxiety levels and infusion rate of remifentanil were monitored during the operation. Results: OAA/S scale was significantly lower in groups R30 (3.96) and R35 (3.34) than groups R10 (4.31) and R20 (4.26). Incidence of intraoperative respiratory depression events, post operative nausea and vomiting were significantly higher in group R35 than the other groups. There were no significant differences in BIS, anxiety level and incidences of recall of the operative procedure among the groups. Conclusions: We conclude that the TCI of remifentanil at 3.0 ng/ml produces an effective sedation and anti-anxiety effects without significant side effects during spinal anesthesia.

Effects of small tidal volume and positive end-expiratory pressure on oxygenation in pressure-controlled ventilation-volume guaranteed mode during one-lung ventilation

Background: The purpose of this study was to investigate whether tidal volume (TV) of 8 mL/kg without positive end-expiratory pressure (PEEP) and TV of 6 mL/kg with or without PEEP in pressure-controlled ventilation-volume guaranteed (PCV-VG) mode can maintain arterial oxygenation and decrease inspiratory airway pressure effectively during one-lung ventilation (OLV). Methods: The study enrolled 27 patients undergoing thoracic surgery. All patients were ventilated with PCVVG mode. During OLV, patients were initially ventilated with TV 8 mL/kg (group TV8) without PEEP. Ventilation was subsequently changed to TV 6 mL/kg with PEEP (5 cmH2O; group TV6+PEEP) or without (group TV6) in random sequence. Peak inspiratory pressure (Ppeak), mean airway pressure (Pmean), and arterial blood gas analysis were measured 30 min after changing ventilator settings. Ventilation was then changed once more to add or eliminate PEEP (5 cmH2O), while maintaining TV 6 mL/kg. Thirty min after changing ventilator settings, the same parameters were measured once more. Results: The Ppeak was significantly lower in group TV6 (19.3±3.3 cmH2O) than in group TV8 (21.8±3.1 cmH2O) and group TV6+PEEP (20.1±3.4 cmH2O). PaO2 was significantly higher in group TV8 (242.5±111.4 mmHg) than in group TV6 (202.1±101.3 mmHg) (p=0.044). There was no significant difference in PaO2 between group TV8 and group TV6+PEEP (226.8±121.1 mmHg). However, three patients in group TV6 were dropped from the study because PaO2 was lower than 80 mmHg after ventilation. Conclusion: It is postulated that TV 8 mL/kg without PEEP or TV 6 mL/kg with 5 cmH2O PEEP in PCV-VG mode during OLV can safely maintain adequate oxygenation.
