Oral allergy syndrome (OAS) is a subtype of food allergy composing of itching sense and edema in the oral cavity, lips, throat, pharynx, and larynx following ingestion of some fresh fruits or vegetables. Although the major pathogenic mechanism of OAS is known to be IgE-mediated response, here we experienced a case of OAS due to crown daisy (CD) and sesame leaf (SL) mediated by a non-IgE antibody mediated mechanism. A 33-year-old female visited our clinic to evaluate numbness of the tongue and gingiva after eating fresh CD and SL for 2 years. The patient had suffered from allergic rhinitis and atopic dermatitis for 20 years and took medications intermittently. There had been a history of food allergy to crab and shrimp. The serum total IgE level was elevated (404 kU/mL). The skin prick test showed strong positive reactions to tree and weed pollens, but not to CD and SL extracts. Enzyme-linked immunosorbant assay for detecting serum specific IgE to crude extracts of CD and SL showed negative results. The basophil activation test performed with crude extracts of CD or SL showed significant up-regulation of CD63-positive basophils by both CD and SL. In conclusion, we report a case of OAS due to CD and SL, not associated with pollen allergy, which is shown to be mediated by a non-IgE mediated mechanism.
아스피린 과민성 천식 환자에서 TLR3 유전자 다형성에 따른 IgG 아형 결핍 및 천식 중증도의 연관성 연구

Purpose: Toll-like receptor 3 (TLR3) recognizes to viral double-stranded RNA and is involved in antiviral defenses. A probable role of TLR3 gene variants in the pathogenesis of aspirin-intolerant asthma (AIA) has been suggested. AIA patients present more frequent asthma exacerbations in which respiratory viral infections could be an exacerbating factor. IgG subclass deficiency was commonly present with bronchial asthma. Based on previous findings, we investigated whether TLR3 variants could affect IgG3 subclass deficiency in AIA.Methods: We enrolled 279 AIA patients, 403 aspirin-tolerant asthma (ATA) patients, and 315 normal healthy controls (NC) in this study. TLR3 polymorphism at the promoter region -299698G>T was genotyped. The serum levels of IgG subclasses were determined by the single radial immunodiffusion method. Expressions of IgG3 and TLR3 on Epstein-Barr virus transformed-B cells isolated from asthmatic patients were evaluated by flow cytometry to investigate B-cell functions.Results: The TLR3 -299698 T allele was significantly associated with severity and IgG3 deficiency in the AIA group (P=0.044 and P=0.010, respectively), but not in the ATA group. IgG3 expression on B cells from asthmatics with IgG3 deficiency was significantly lower compared to those without (P=0.025). There was a positive correlation between IgG3 expression levels on B cells and serum IgG3 levels (r2=0.434, P=0.002).Conclusion: These results suggest that the TLR3 -299698G>T polymorphism may be associated with IgG3 subclass deficiency and severity in AIA. 고유-전정감각 중심의 감각통합중재가 발달지연 아동의 침 흘리기와 자세조절에 미치는 효과 : 단일 실험 설계 연구

본 연구는 고유-전정감각 중심의 감각통합치료가 발달지연 아동의 침 흘리기와 자세조절에 미치는 효과를 알아보자 하였다. 연구 대상자는 전반적인 발달영역에서 지연을 보이는 만 36개월 된 남자로, 과도한 침 흘리기와 자세조절의 문제를 가진 아동이었다. 연구 방법은 개별 실험 연구 중 AB 설계를 사용하였다. 실험과정은 기초선 기간(A) 5회기, 중재 기간(B) 15회기로 진행되었다. 기초선 기간에는 고유-전정감각 중심의 감각통합중재를 하지 않았으며, 중재기간에는 고유-전정감각 중심의 감각통합중재를 실시하였다. 침 흘리기 정도는 전자저울을 사용하여 0.5g 단위로 측정하였으며, 자세조절은 Nintendo Wii Fit 균형판을 사용하여 신체중심의 좌우차를 %로 측정하였다. 본 연구의 결과는 다음과 같다. 첫째, 침 흘리기는 침의 양이 기초선 기간(3.10 ±0.42g)보다 중재기간(1.50 ±0.46g)에 평균 1.6g 감소하였다. 둘째, 자세조절은 기초선 기간(25.47 ±3.72%) 보다 중재기간(16.12 ±4.87%)에 신체중심의 좌우차이가 평균 9.35% 감소하여 자세조절 능력이 향상되었다. 본 연구는 고유-전정감각 중심의 감각통합중재가 발달지연 아동의 침 흘리기와 자세조절 능력향상에 도움을 주는 것으로 나타났다. The purpose of this study was to investigate the effect of proprioceptivevestibular based sensory integration intervention on drooling and postural control of the child with developmental delay. The subject of study was a boy who is 36-month-old with delayed development on the overall developmental areas. A single subject research AB design was used for this study. The procedure consisted 5 sessions of baseline phase (A) and 15 sessions of intervention phase (B). The baseline phase involved no intervention, but during the intervention phase proprioceptive-vestibular based sensory integration intervention was conducted. the amount of drooling was measured by the bib with an electronic scale and the postural control by the Nintendo Wii Fit Balance Board. The results of this study are as follows; First, the amount of drooling was decreased in intervention phase (1.50g ±0.46g) than in baseline phase (3.10g ± 0.42g). Second, the postural control of subject was improved in intervention phase (16.12 ±4.87%) than baseline phase (25.47 ±3.72%). This study showed that the proprioceptive-vestibular based sensory integration intervention provided positive effects on the improvement of drooling and postural control of the children with developmental delay.
