Traditional symptom-based therapies of irritable bowel syndrome (IBS) are directed at the relief of individual IBS symptoms, but they are often of limited efficacy in addressing the entire symptom complex. Combinations of drugs to target bothersome symptoms are suggested as the first-line pharmacologic treatment. Increasing knowledge of the pathophysiology and molecular mechanisms of IBS has resulted in the development of several new therapeutic approaches. Thirteen consensus statements for the treatment of IBS were developed using the modified Delphi approach. Exclusion diets have modest efficacy in improving symptoms in some IBS patients. Symptom-based therapies with dietary fiber, bulking agents, laxatives, antispasmodics and laxatives are effective in the improvement of some individual symptoms, e.g. dietary fiber and bulking agents for constipation, laxatives for constipation, antispasmodics for abdominal pain and discomfort, antidiarrheals for diarrhea. 5HT3 receptor antagonists and 5HT4 receptor agonists are effective in the relief of global IBS symptoms and individual symptoms such as abdominal pain and abnormal bowel habits. A short term course of nonabsorbable antibiotics may improve global IBS symptoms, particularly in patients with diarrhea- predominant IBS. Some probiotics appear to have the potential benefit in improving global IBS symptoms. Selective C-2 chloride channel activator is more effective than placebo at relieving global IBS symptoms in patients with constipation-predominant IBS. Both tricyclic antidepressants and selective serotonin reuptake inhibitors are equally effective in relieving global IBS symptoms, and have some benefits in treating abdominal pain. Certain types of psychologic therapy may be effective in improving global symptoms in some IBS patients. Further studies are strongly needed to develop better treatment strategies for Korean patients with IBS. (Korean J Gastroenterol 2011;57:82-99)

본 연구는 체조 철봉종목의 기본 기술인 대차돌기 시 발생하는 표면 근활성도를 역학적으로 분석하고 이를 통해 철봉 기본기술의 교육자료 제공하기 위한 목적으로 수행되었다. 연구방법은 7명의 남자체조 선수들을 대상으로 완요골근, 상완이두근, 삼각근, 복직근의 표면 근전도를 무선 표면 근전도 시스템과 고속카메라를 이용해 측정하였다. 자료처리는 SPSS Ver. 24.0 통계패키지 프로그램을 이용하여 대응표본 t검정을 실시하였다. 연구 결과는 다음과 같다. 첫째, 상승구간에서 팔꿈치 굴곡근인 상완이두근과 어깨 회전근인 삼각근의 근활성도가 높았고 통계적으로 유의하였다. 둘째, 손목굴곡근인 완요골근과 몸통굴곡근인 복직근은 상승구간에서 높은 경향을 보였으나, 통계적 유의차는 없었다. 본 연구를 중심으로 철봉기본 기술인 대차돌기에서 상승구간과 하강구간의 차이점을 확인하였고, 상승구간에서 원심력을 조절하기 위한 전략이 팔꿈치와 어깨를 중심으로 이루어짐을 알 수 있었다. 따라서 경기력 향상을 위한 기본기술 교육에 유익한 자료로 활용될 것이다. 아울러 일선 체조현장 지도자들의 현장활용 가치가 높을 것으로 기대된다. The purpose of this study is to dynamically analyze the surface muscle activity generated by Giant swing, the basic technology of gymnastics horizontal bar, and provide educational data on the basic technology of the horizontal bar. Surface electromyography of the brachialis (a wrist flexor), biceps brachii (an elbow flexor), deltoid (a rotator cuff), and rectus abdominis muscles (a trunk flexor) was performed for seven male gymnasts using a wireless surface electromyography system and high-speed camera. A paired samples t-test was performed using the SPSS ver. 24.0 statistical package program for data processing. The results showed that the activities of the biceps brachii and deltoid were high during the ascending phase, with a statistical difference between the two phases. However, the brachialis and rectus abdominis muscles had high activities during the ascending phase, but there was no significant difference between the two phases. Therefore, this study found a difference in muscle activity between the ascending and descending phases of Giant swings. In addition, it was identified that the strategy for controlling the centrifugal force during the ascending phase was centered on the elbows and shoulders. Therefore, the data can be used for technical guidance to improve sports performance. In addition, it is expected that the field use value of front-line gymnastics field leaders will be high.
