12세 남아에서 상완골 소결절 견갑하건 부착부의 견열성 골절 - 증례 보고 -

An isolated avulsion fracture of the subscapularis from the lesser tuberosity of the proximal humerus is rare in the pediatric population and only a couple of cases have been described in literature. A 12-yearold right-handed boy, was admitted to the hospital with left shoulder pain after falling down the previous day. Physical examination revealed tenderness on the lesser tuberosity and intertubercular groove of the humerus and a limited range of motion. Ultrasonography, computed tomography, and magnetic resonance imaging showed an isolated avulsion fracture of the subscapularis tendon from the lesser tuberosity of the humerus, accompanied by medial subluxation of the long head of the biceps tendon. In this case, a subscapularis avulsion fracture which is rare in this age group was detected at an early stage, and surgical treatment resulted in positive outcomes. Subscapularis avulsion fractures in children are difficult to diagnose in the early stages, and if not treated on time, the symptomatic improvement may be delayed. Timely and accurate diagnosis and treatment can help faster return to normal activities. We would therefore like to report this case with a literature review. 소아에서 상완골 소결절의 단독 견열 골절은 매우 드물며문헌상에서 몇 가지 사례만 보고되고 있어 증례에 대하여 보고하고자 한다. 오른손잡이인 12세 소년이 내원 하루 전 운동하다가 넘어진 후 발생한 좌측 어깨 통증으로 내원했고 시행한 이학적 검사상 소결절 부위 및 결절간구 쪽에 압통이 있었으며, 운동 범위는 내회전 S1, 저항성 내회전 강도가 감소된 소견을 보였다. 추가적으로 belly press test와 lift-off test, bear hug test에서 양성 소견을 보였으며, Yergason test와speed test에서도 양성 소견을 보였다. 초음파, CT 및 MRI상에서 상완골 소결절의 견열 골절 및 상완 이두장건의 내측 아탈구가 관찰되어 수술적 치료를 시행하였다. 이 연령대에서드문 외상성 견갑하건 부착 부위의 소결절의 견열 골절을 조기에 발견하여 수술적 치료를 통하여 좋은 결과를 얻었다. 소아에서 견갑하건 견열 골절은 초기 진단이 어렵고 치료를 하지 않았을 시에 증상 개선이 되지 않아 정확한 진단과 치료가필요하여 이에 대해 문헌 고찰과 함께 보고하고자 한다.
급성 일산화탄소 중독 환자에서 급성 심혈관계, 신경학적 독성의 위험요인과 노출 경로의 역학적 특성

Purpose: This study evaluated aggressive hyperbaric oxygen therapy (HBOT) by understanding various exposure routes of acute carbon monoxide (CO) poisoning, the risk factors causing acute cardiovascular, and neurological toxicity caused by poisoning. Methods: A retrospective study was conducted based on the medical records of 417 acute CO poisoning patients who visited the emergency care unit from March 2017 to August 2019. The exposure routes, HBOT performance, age, sex, medical history (hypertension, diabetes mellitus, ischemic heart disease, heart failure), intentionality, loss of consciousness (LOC), intake with alcohol or sedatives, and initial test results (carboxyhemoglobin (COHb), troponin- I, electrocardiography, echocardiography, brain MRI) were examined. Comparative analysis of the clinical information was conducted between the groups that showed acute cardiovascular toxicity and neurological toxicity, and groups that did not. Results: Among 417 patients diagnosed with acute CO poisoning, 201 cases (48.2%) were intentional, and charcoal briquette was the most common route (169 patients (40.5%)). Two hundred sixteen cases (51.8%) were accidental, and fire was the most common route (135 patients (32.4%)). The exposure route was more diverse with accidental poisoning. Three hundred ninety-nine patients were studied for acute cardiovascular toxicity, and 62 patients (15.5%) were confirmed to be positive. The result was statistically significant in intentionality, LOC, combined sedatives, initial COHb, HTN, and IHD. One hundred two patients were studied for acute neurological toxicity, which was observed in 26 patients (25.5%). The result was statistically significant in age and LOC. Conclusion: Active HBOT should be performed to minimize damage to the major organs by identifying the various exposure routes of CO poisoning, risk factors for acute cardiovascular toxicity (intentionality, LOC, combined sedatives, initial COHb, HTN, IHD), and the risk factors for acute neurological toxicity (age, LOC).

Compressive deformation behavior of Al-10Si-5Fe-1Zr (wt%) alloy preform fabricated by SPS(spark plasma sintering) of gas atomized powder was investigated in the temperature range from 380 to 480℃ and at strain rates from 1.0×10?3 to 1.0×100 s?1. Stress-strain curves showed a peak stress (σp) during initial stage of deformation, followed by a steady state flow at all temperatures and strain rates tested. The σp decreased with both increase in temperature and decrease in strain rate. Nearly full densification was found to occur in the compressively deformed specimens irrespective of test condition. TEM observation revealed a restricted grain growth during steady state flow.
Variable Suspension Design for Active Pantograph

There are a lot of traffic jams in the metropolitan area and the commuting time has been longer nowadays. So the urban people has been interested in the GTX(Great Train Express) project in Korea. The GTX is the train which runs at 200km/h speed in underground tunnels. If the train also operates at high speed in tunnel section, the pressure wave will happen and the uplift force of pantograph may vary abruptly. If the rigid trolley bar system is used in tunnel section, it is difficult to improve the commercial speed of train. In order to improve the train speed in tunnel section, this paper presents the new pantograph concepts which can change the suspension stiffness and deals with the dynamic behavior characteristics of pantograph according to the parameter variation.

Objective: This study was performed to identify factors related with use of hearing protectors for prevention of noise-induced hearing loss among some manufacturing workers through health promotion model. Methods: The data were collected from 116 workers who were exposed to the noise level above 85dB, and interviewed them using organized questionnaire from April 19 to 27, 2004. Results: The subjects of 48.0 percent used hearing protectors for last one week. The factors associated with the use of hearing protectors for the last one week were the perceived barriers in respect to hearing protectors were lower (p<0.001), the perceived self efficacy was higher (p<0.001) and the perceived emotion in respect to hearing protectors was higher (p<0.05). However, a significant relation was not observed from the factors such as perceived health status, immediate competing demands and preferences, and interpersonal influences that showed significant relation in a simple analysis (p>0.05). Conclusion: It was concluded that as the perceived factors such as self-efficacy and perceived barrier and associated emotion influenced significantly the hearing protectors use of the worker, they should be carefully considered in developing a program to increase hearing protectors use.
레이저 디스플레이 기반 오감만족 스마트 스크린 엘리베이터 시스템 제안

최근, 현대인들의 필수품인 ‘스마트 폰’이 상용화 되면서 곳곳에 ‘스마트’ 한 생활을 위한 기술들이 개발되고 있다. 이에 본 논문은 레이저를 이용한 디스플레이 및 스피커와 방향제를 엘리베이터 내부에 도입하여 내부 화면의 다 양화를 연출하고, 핸드오버기술로 지속적인 와이파이 호환이 가능한 이용자 단말기 상의 엘리베이터 조절 화면 출력기 능 탑재로 이용자의 스마트폰을 통해 엘리베이터를 조작할 수 있으며 승강기 외부 상단에 장착된 키넥트로 이용자의 동작을 인식하여 작동되는 기존 엘리베이터 시스템과 차별화된 새로운 스마트스크린 엘리베이터 시스템을 제안하고자 한다. 마지막으로 엘리베이터 제조사, 유통사, 소비자 각 집단 200명을 대상으로 레이저스크린 엘리베이터의 상용화에 대한 설문조사를 진행한다. Recently, most of people wants to have smart life for themselves like smart phones. In this paper, we propose the smart elevator based on the laser screen, speakers and air fresheners. Using this elevator, we could enjoy the various screen inside, and we could keep contacting with manager for wifi in the emergency situation. Also, the elevator could be operated with user's motion for Kinect. Finally, we did the survey for understanding preference of smart elevator. The grops are elevator manufacturing company, elevator distribution corporation and consumers, each group have 200 people.

Energy Saving Methods in existing railway vehicle are considered by active approach such as regenerative energy storage and utilization, eco driving, etc. On the other hands, energy saving measures in railway station are operated by passive method such as reduction of operating time in ventilation system, cooing system and power equipment. To reduce energy and for independence in railway system, it requires an active energy saving measures. It needs to its own power source besides the power source of electric supply company such as renewable energy and regenerative energy and take the advantage of power storage system and stored power are used in optimum time. This paper deal with 3-level NPC inverter and T-type NPC inverter that used in various multi-level topology applicable to the railway system.
