Background and Objectives Cervical necrotizing fasciitis is a fulminant disease associated with necrosis of connective tissue, spread along the fascial plane, and high mortality. We analyzed the clinical characteristics and treatment outcome of this rare fatal disease. Materials and Methods We retrospectively reviewed the medical records of 19 patients treated for cervical necrotizing fasciitis from January 1999 to January 2009. Mean age was 53.7 years. Results The most common predisposing illness was tonsillitis (36.8%), followed by odontogenic infection (15.7%). Diabetes mellitus was most common underlying disease. Liver cirrhosis and chronic renal failure were found in 2 patients each. All patients were treated with combination of parenteral antibiotics and wide surgical debridement by transcervical and/or thoracotomy approach. Multiple surgical debridements were performed in 7 patients. Tracheotomy was performed in most of the patients (88.8%). Period of total hospitalization and Intensive care unit was 23 days and 10.1 days. Two patients died of disease and overall survival rate was 89.4%. Conclusion Early surgical management and care in intensive care unit are essential for cervical necrotizing fasciitis. Possible complications such as respiratory failure, mediastinitis or sepsis should be carefully evaluated.

Purpose: To report a case of iatrogenic Horner’s syndrome after video-thoracoscopic surgery for primary pneumothorax. Case summary: An 18-year-old man with ptosis in the right eye was referred to our clinic. The patient had undergone wedge resection via video-thoracoscopic surgery for primary pneumothorax three weeks previously. On ocular examination, the palpebral fissure width was 7 mm in the right lid and 8 mm in the left lid, the marginal reflex distance 1 (MRD 1) was 2 mm in the right lid and 3 mm in the left lid, and the bilateral levator muscle function was good. Anisocoria was present, and pupil size in a dark room was 2.5 mm in the right eye and 4 mm in the left eye. The patient complained of facial anhidrosis on the right side of the face. Conclusions: Although iatrogenic Horner’s syndrome is rare complication of video-thoracoscopic surgery for primary pneumothorax, diagnosis after surgery of the thoracic cavity should be made carefully. Purpose: To assess the clinical features associated with the direction of deviation in sensory strabismus and postoperative stability of strabismus angles. Methods: The authors retrospectively reviewed the medical records of 98 patients diagnosed with sensory strabismus who were treated surgically. Among the factors analyzed were sex, age, direction of strabismus, preoperative visual acuity, age at onset of visual loss, cause of primary sensory deficit, refractive error, age at operation, deviation angle at 1 and 6 months postoperatively and after at least 5 years. Results: At baseline, 20 patients (20.4%) had esotropia and 78 (79.6%) had exotropia. Common causes of primary sensory deficit were corneal opacity (25.5%), vitreoretinal disorder (23.5%), and congenital cataracts (16.3%). The proportion of esotropia increased significantly as the refractive error of the sound eye became more hyperopic. The older the patients were when they lost their vision, the higher the proportion of exotropia. Patients with longer duration of vision loss were more likely to have a larger deviation angle with exotropia, but the association was not significant with esotropia. Conclusions: Patients with sensory strabismus tend to develop exotropia. Refractive error of the sound eye and age at onset of vision loss were associated with the direction of strabismus. Despite poor sensory fusion even after surgery, more than 50% of patients maintained a stable eye position. J Korean Ophthalmol Soc 2012;53(8):1138-1142

Laser is a relatively recent addition to laryngeal surgery. Since their invention, laser use and applications have expanded rapidly. The use of lasers in surgery has offered a time- and cost-efficient alternative to cold surgical techniques and has been used in the treatment of numerous laryngeal pathologies, including stenoses, recurrent respiratory papillomatosis, leukoplakia, nodules, malignant laryngeal disease, and polypoid degeneration (Reinke's edema). Despite the notable benefits, laser surgery is not without disadvantages. Laser heat can increase scarring and cause damage to adjacent tissue. With laser laryngeal microsurgery, there is potential for airway fire, endolaryngeal bleeding, perichondritis, chondritis, granuloma, surgical emphysema, laryngeal stenosis and web formation, postoperative edema and swallowing problem. Surgeons should be known about these complications and could manage properly.

The morphology of a bone is closely associated with its biomechanical response. Thus, much research has been focused on analyzing the effects of variation of bone morphology with subject-specific models. Subject-specific models, which are generally achieved from 3D imaging devices like CT and MRI, incorporate more of the detailed information that makes a model unique. Hence, it may predict individual responses more accurately. Despite these powerful characteristics, specific models are not easily parameterized to the extent possible with statistical models because of their morphologic complexities. Thus, it is still proven challenging to analyze morphologic variations of subject-specific models across changes due to aging or disease. The aim of this article is to propose a generic and robust parametric morphing method for a subject-specific bone structure. We demonstrate this by using the proposed method on a model of a human proximal femur. Automatic segmentation algorithms are also presented to parameterize the specific model efficiently. A total of 48 femur models were evaluated for defining morphing vector fields. Also, several anatomical and mechanical functions of femur were considered as morphing constraints, and the NURBS interpolating technique was applied in the method to guarantee the generality of our morphed results.

본 고에서는 최근 대용량 고압 전동기 구동 시스템에 새로운 적용이 가능한 모듈러 멀티레벨 컨버터 토폴로지의 기술적 특징을 확인하고, 전동기의 기동 및 저속 영역에서 영향을 미치는 모듈러 멀티레벨 컨버터의 주파수에 따른 서브 모듈 커패시터 전압의 변동폭을 이론적으로 분석하고, 이러한 문제를 해결하기 위한 기존 연구 결과의 고찰을 통해 향후 MMC 적용 고압 전동기 드라이브 기술의 개발 전략들을 제언하고자 한다. In this article, the technological characteristics of the modular multi-level converter(MMC) topology, which can be newly applied to the high-power medium-voltage motor drive system, has been confirmed. The voltage fluctuation of the sub-module capacitor, which affects the motor start-up and low-speed region according to the operating frequency of the modular multi-level converter, was analyzed by theoretical equation. This article seeks the future strategies for MMC-applied Medium-voltage motor drive technology through consideration of the results of previous research.

In this paper, the fault tolerant system for BLDC motor has been proposed to maintain control performance under open phase fault of inverter. Fault detection and diagnosis method is proposed to two different types which are the method by differences between reference and actual current, and using additional voltage sensor across lower switch of inverter leg. The reconfiguration consists of four switch topology connecting faulty leg to DC-link middle point by bidirectional switch. The proposed fault tolerant system quickly recover general performance by short detecting time and reconfiguration of system topology. The feasibility of proposed system is improved by simulation.

In this paper, a novel sensorless brushless DC (BLDC) motor drive method using the fuzzy back-EMF observer is proposed to improve the performance of conventional sensorless drive methods. Most existing back-EMF sensing methods need additional circuit and have a low performance in transient state or low speed range. Therefore, this paper proposes the fuzzy back-EMF observer and an algorithm using this observer to estimate a speed and a position of the rotor. The robustness of the proposed algorithm is proved through the simulation compared with other sensorless drive methods.
