      • $Fraxion^{(R)}$ System을 이용한 뇌 정위적 방사선 수술 유용성 평가

        목 적 : $Fraxion^{(R)}$ system과 s-thermoplastic mask을 사용하여 뇌 전이(Brain metastasis)가 있는 환자에게 정위적 방사선 수술(Stereotactic Radiosurgery, SRS)을 시행하면서 발생 되는 환자의 셋업 오차(Setup Error)를 비교하고 오차가 선량에 미치는 영향을 측정하여 $Fraxion^{(R)}$ system의 유용성을 평가하고자 한다. 대상 및 방법 : 2014년 5월부터 2014년 10월까지 본원에 내원하여 정위적 방사선 수술을 받은 뇌 전이(Brain metastasis) 환자 6명을 대상으로 하였다. 머리를 고정하기 위해 3명은 s-thermoplastic mask와 mouthpiece를 이용한 그룹과 3명은 $Fraxion^{(R)}$ system을 이용하여 제작 한 2그룹으로 나눈 뒤 방사선 수술 당시 얻어진 3D 콘빔CT(Cone Beam Computerized Tomography, CBCT) 영상과 전산화 단층모의치료 영상의 오차 값을 기록하고 환자별 최대치(Max), 최소치(Min), 평균치(Mean), 표준편차(standard deviation, SD) 구하여 비교하였다. 또한 StereoPHAN Phantom 및 Pinpoint 3D cylindrical chamber를 이용하여 환자별 정도관리(Patient Specific Quality Assurance, PSQA)와 같은 방법으로 선량을 측정하여 선량값을 비교하였다. 결 과 : 정위적 방사선 수술 전 콘빔CT 영상과 전산화단층모의치료영상을 비교하여 얻어진 셋업 오차의 경우 $Fraxion^{(R)}$ system이 s-thermoplastic mask와 mouthpiece를 함께 사용한 경우보다 셋업 오차가 평균을 기준으로 X축 83% 감소, Y축 40% 감소, Z축 92% 감소하였으며 X, Y, Z 회전성분인 Pitch 64% 감소, Roll 88% 감소, Yaw 87% 감소로 모든 방향에서 상대적으로 이동 값이 적었으며 선량 측정을 시행한 결과는 평균을 기준으로 s-thermoplastic mask와 mouthpiece를 사용한 경우가 $Fraxion^{(R)}$ system보다 셋업 오차 보정 전은 83% 감소, 보정 후는 1.9% 감소된 선량측정을 보였다. 결 론 : $Fraxion^{(R)}$ system은 개인 치열(dentition)에 맞춘 구강고정기구(mouthpiece)와 Fraxion frame, Frontpiece, thermoplastic mask nose을 사용하여 s-thermoplastic mask와 mouthpiece를 가지고 머리를 고정하는 기존의 방식보다 높은 재현성을 보였으며 이는 기존의 치료 방법보다 1회 치료로 많은 선량이 조사되는 뇌 정위적 방사선 수술에서 안정된 고정 효과를 나타낼 것으로 사료된다. Purpose : We evaluated the usefulness of $Fraxion^{(R)}$ system and s-thermoplastic mask by analyzing setup error when stereotactic radiousurgery (SRS) was treated for brain metastasis. Materials and Methods : 6 patients who received definite diagnosis as brain metastasis between May 2014 and October 2014 were selected. 3 patients were immobilized s-thermoplastic mask and mouthpiece (group1), while $Fraxion^{(R)}$ system was used for the other 3 patients (group2). Cone Beam Computerized Tomography (CBCT) scan was acquired to register planning CT scan. The registration offset was compared for each group. We compared and reported the errors using maximum, minimum, mean, and standard deviation of registration offsets. Furthermore, We used the same method as patient specific quality assurance to verify absorbed dose of PTV. Results : The setup error which is registration offset was reduced 83% in x, 40% in y, and 92% in z-direction when $Fraxion^{(R)}$ system was used compared to the case of using s-thermoplastic mask and mouthpiece. In addition, using $Fraxion^{(R)}$ system showed improved results in rotational components, pitch (rotation along x-axis), roll (y), and yaw (z) which were reduced 64, 88, and 87% respectively compared to the case of using s-thermoplastic mask and mouthpiece. In dosimetry results, when s-thermoplastic mask and mouthpiece used, absorbed dose was reduce 83% compared to before and after registration. However, using $Fraxion^{(R)}$ system showed only 1.9%. All percentage were calculated with respect to average value. Conclusion : Using $Fraxion^{(R)}$ system including mouthpiece, Fraxion frame, frontpiece, and thermoplastic mask, showed better repeatability and precision compared to using s-thermoplastic mask and mouthpiece, which is consequently considered as more improved immobilization system.

        최근 중국은 양산항을 개장하여 선사 유치를 위한 다양한 혜택을 제공하고, 일본은 "수퍼중추항만 육성계획“을 세워 항만의 효율성을 바탕으로 비용을 낮추려는 노력을 하고 있다. 또한 선박의 대형화에 의한 중심항만(Hub Port) 위주의 기항과 북중국의 대량 물량 발생지역에 대한 직기항 경향으로 동북아 항만간의 경쟁이 더욱더 치열해 지고 있으며 동북아 역내 피더네트워크 강화의 필요성이 제기되고 있다. 따라서, 본 연구에서는 유럽 피더네트워크 전략 사례를 바탕으로 동북아 피더시장 현황을 파악하고 동북아 피더네트워크 구조를 분석함으로써 동북아 역내 항만 및 선사간의 과당경쟁을 지양하고 서로 Win-Win할 수 있는 터미널의 전략적 활용, 선사간 제휴 확대, 항만간 협력 강화를 위한 구체적인 전략을 제시하였다. Recently, China opened Yangshan port and provides a variety of benefits for shipping companies. Japan plans to reduce logistics costs and improve efficiency through the established "super major ports" project. According to the "hub and spokes" system, the number of direct calls toward North-Chinese ports are increasing. This changing environment has negative effects on the amount of transshipment cargos in Korean ports. In order to make a hub port of a domestic port, it is necessary to develop northeast Asian feeder networks. Therefore, this paper analyzes the structure of northeast Asian feeder networks through the case study of European feeder networks strategy. Finally, we suggests the plans to reinforce feeder networks, which are strategic terminal operation, strategic alliances between shipping companies and port co-operation, for win-win strategy in the competitive northeast Asian market.

        청색고무수포모반증후군으로 인한 안구돌출 1예

        Purpose: Blue rubber bleb syndrome (BRBNS) is a rare disorder characterized by multiple, distinctive cavernous hemangiomas of the skin, and gastrointestinal tract. We investigated the surgical treatment and clinical findings for multiple hemangiomas in the orbit of a patient who had BRBNS on the skin and liver. Methods: A 33-year-old white woman visited our clinic with the chief complaint of continuous exophthalmos of one year duration. She did not complain of ocular pain or decreased visual acuity. Nine years previously, her medical history showed the removal of a mass from her left arm, the result of histopathologic examination was multiple hemangiomas. Check-up for gastrointestinal lesions by colonoscopy was negative and all hematological parameters were normal. The orbital mass was surgically removed. Histopathological finding showed it to be the same as hemangioma. Results: Multiple bluish nodules on the skin, visceral hemangioma, multiple hemangiomas in orbit led to the diagnosis of BRBNS. Conclusions: Multiple hemangiomas in orbit should be suspected as BRBNS, and therefore systemic evaluation is required to consider the association with BRBNS.

        패킷 카운팅을 이용한 DoS/DDoS 공격 탐지 알고리즘 및 이를 이용한 시스템

        Currently, by using the Internet, We can do varius things such as Web surfing, email, on-line shopping, stock trading on your home or office. However, as being out of the concept of security from the beginning, it is the big social issues that malicious user intrudes into the system through the network, on purpose to steal personal information or to paralyze system. In addition, network intrusion by ordinary people using network attack tools is bringing about big worries, so that the need for effective and powerful intrusion detection system becomes very important issue in our Internet environment. However, it is very difficult to prevent this attack perfectly. In this paper we proposed the algorithm for the detection of DoS attacks, and developed attack detection tools. Through learning in a normal state on Step 1, we calculate thresholds, the number of packets that are coming to each port, the median and the average utilization of each port on Step 2. And we propose values to determine how to attack detection on Step 3. By programing proposed attack detection algorithm and by testing the results, we can see that the difference between the median of packet mounts for unit interval and the average utilization of each port number is effective in detecting attacks. Also, without the need to look into the network data, we can easily be implemented by only using the number of packets to detect attacks.
