전신성 홍반성 루푸스 환자에서 발생한 Plasmodium vivax 감염

Malaria was rarely reported in patients with systemic lupus erythematosus (SLE). We report a case of P. vivax infection in a patient with SLE. A 42-year-old woman was admitted to the hospital because of fever, anemia, and severe thrombocytopenia and had transfusion of multiple blood products including 16 units of red blood cells (RBC). The patient was diagnosed as having SLE complicated with antiphospholipid syndrome. From the 96th hospital day, the patient had cyclic high fever for 4~5 hours, myalgia and shaking chills, follwed by generalized diaphoresis and resolution of fever. Examination of her peripheral blood smear showed gametocytes of P. vivax. She received hydroxychloroquine of a total dose of 25 mg/kg orally over 3 days and then oral primaquine 15 mg/day for 14 days. Blood donors were traced retrospectively. The 13 RBC products were negative for malaria but other three RBC products could not be checked on P. vivax. The patient was presumed to have acquired malaria from infected blood products. Relapse did not occur 1 year later. In conclusion, malaria should be considered in the differential diagnosis of persistent fever in a patients with SLE who received multiple blood products in Korea.

Objective: To estimate the prevalence of osteoarthritis (OA) and rheumatoid arthritis (RA) in Korean. Methods: Those who participated arthritis/allergy field study in Icheon, Kyunggi Province and Goesan, Chungbuk Province in 2001 were evaluated for the prevalence of arthritis. Diagnosis of OA and RA were made using American College of Rheumatology criteria. Results: Among the 983 participants (mean age±SD, 49.9±14.0 years), 498 participants were male (mean age 49.9±13.1 years) and 485 participants were female (mean age 49.9±15.0 years), and there was no difference of mean age between male and female. There were 153 cases of OA (15.6%, mean age, 61.6±9.5), which consisted of 44 male cases (mean age, 58.9±10.7) and 109 female cases (mean age, 62.7±9.5). Prevalence of OA was significantly higher in female compared to male (22.5% vs 8.8%, p<0.0001). Prevalence of OA in the 4th to the 8th decade was 1.1, 7.5, 19.4, 34, 37.2%, respectively and showed increasing trend in the ascending order of age in decade (p<0.001). Localization of OA was knees (77.1%), knees and hands (17.6%), and hands (5.2%). RA was detected in 14 cases (1.4%, mean age, 58.8±14.6, male to female 1:13). Prevalence of RA was significantly higher in female compared to male (2.7% vs 0.2%, p<0.01). Conclusion: The prevalence of knee or hand OA and RA were 15.6% and 1.4% in Korean, respectively. The prevalence of OA was significantly higher in female and aged individuals. The prevalence of RA was also significantly higher in female.
태권도 수련기간이 아동의 신체구성과 근력 및 최대산소섭취량에 미치는 영향

The purpose of this study was to investigate the difference of body composition, muscle strength and VO₂max according to the Taekwondo training periods. Our participants in the study consisted of twenty-four elementary school student(11yrs). They were separated into three groups a control group(n=8), a 8month taekwondo training group(n=10) and a 15month Taekwondo training group(n=8). We found that %fat was significantly difference between control and Taekwondo training group, also, lowest 15 month group. And muscle strength and VO₂max were high in the Taekwondo training group compared to the control group, but blood pressure was no significant difference among group. Especially VO₂max/weight was highest 15 month group. In conclusion, Taekwondo training was shown to positive influence on body composition, muscle strength and cardiorespiratory function in elementary school student. However, growing adolescents needed long-term Taekwondo training to improve their body composition and physical fitness.

Objective: To determine the effectiveness of pulsed electromagnetic fields (PEMF) in the treatment of knee osteoarthritis (OA). Methods: In this randomized, placebo-controlled, double-blind trial, 51 patients with knee OA, diagnosed according to the criteria of the American College of Rheumatology, were treated with PEMF or placebo. Eleven patients failed to attend after screening and were excluded from analysis. Treatment consisted of 3 half-hour periods of exposure per week over 6 weeks in a specially designed cylindrical device that emits low-frequency pulsed electromagnetic fields (25 gauss, 12 Hz). The primary outcome measure was reduction in pain on movement using a 10 cm visual analog scale. Secondary outcome measures included joint swelling and tenderness, the Lequesne index, and overall evaluations of improvement by the patient and examining physician. Evaluations were made at baseline, 3 week and 6 week during treatment and 4 weeks after finishing treatment. Results: There were no significant differences between PEMF and placebo groups in respect of any outcome measures after treatment. Range of motion and knee swelling tended to be improved in the PEMF group. There were no clinically relevant adverse effects attributable to PEMF treatment. Conclusion: These results suggest that the PEMF treatment has no clinically significant benefits in patients with knee OA resistant to conventional treatment. The larger studies are needed to confirm the efficacy of PEMF therapy in knee OA.

This study Attempts to that Heart Functional know Long-distance Athlete by Electrocardiographic Time Interval and Wave Amplitudes. Fourteen Male Subjects aged Between 21-23years were devided into Athete and Control Groups. Pre-execise, Execise and Recovery Mode was performed on a Bicycle-ergometer. Load of Execise Mode was 1Kp/3min, 2Kp/3min, 3Kp/3min. ECG's were traced in LoadⅡ,V₁ and V? to Pre-execise, Execise and Recovery. ECG interval were measured from LeadⅡ and ECG amplitudes from LoadⅡ, V₁ and ??. The results Obtained are Aummarized as Follow : 1. ECG interval ; 1) The R-R interval at Pre-exercise Mode was 1.121±0.181sec in Athlete group and 0.862±0.148sec Control group. Athlete group was Sinus Bradycardia. 2) The P-R interval at Pre-exercise Mode was 0.174±0.018sec in Athlete group and 0.487±0.048sec Control group. 3) The QRS interval at Pre-exercise Mode was 0.093±0.009sec in Athlete group and 0.018±0.009sec Control group. 4) The QRS interval at Pre-exercise Mode was 0.512±0.077sec in Athlete group and 0.409±0.019sec Control group. 2. ECG amplitude ; 1) The P wave Amplitude at Pre-exercise Mode was 0.871±0.304mm Athlete group and 0.801±0.162mm Control group. The Amplitude was Increaser the Athlete group than Control group. 2) The R wave Amplitude at Pre-exercise Mode was 26.61±4.63mm Athlete group and 19.84±3.87mm Control group. The Amplitude was Increaser the Athlete group than Control group. 3) The S wave Amplitude at Pre-exercise Mode was 13.32±2.14mm Athlete group and 10.61±1.03mm Control group. The Amplitude was Increaser the Athlete group than Control group. Exercise Mode was enlarged Change Form Time Interval and Amplitude in Control group and Recovery Mode Time Interval and Amplitude in Athlete group rapided Rahabilition and the Sum of Amplitude in ?? and SV₁ in Athlete group was 40.00mm suggesting a Left Ventricular Hypertrophy.

The purpose of this study was to evaluate the anaerobic thresold and EPOC(Excess post-exercise Oxygen Consumption) in treadmill exercise of taekwondo players(n=7),and group of controls college students(n=7). The started treadmill exercise with speed of 120m/min and within treadmill 5% slope. Especially the treadmill speed was increased with 20m/min per two minutes and subjects were finally led to exhaustion. The results obtained from this test were as follows: 1. The VO₂max( /min), VO₂ max(kg) showed significantly higher differences taekwondo group than control group in maximum exercise. 2. The ventilation volume showed significant difference taekwondo group than control group in maximum exercise. 3. The anaerobic threshold showed significant difference taekwondo group than control group in maximum exercise. 4. The EPOC showed significant difference taekwondo group than control group in maximum exercise.

In this paper, An I-PD control system with feedforward compensation is proposed. This control scheme makes the poles and zeros of closed-loop systems be assigned arbitrarily. As the result, the better dynamic performance of the output responses can be attained. A low-pass filter system with variable band width is also proposed in this paper for the robustness with respect to measurement noise and for the fast recovery against external load torque disturbance. This low-pass filter system shows some excellent dynamic characteristics as it is applied to the speed control systems for servo motors. When the speed commands of servo motors are low, the low-pass filter suppresses the influence of the speed measurement noise. Because the low-pass filter deteriorates the dynamic performance of the speed responses against external load torque disturbance, the band width of the low-pass filter is designed to be enlarged as the speed commands increase. Hence, this low-pass filter system with variable band width shows the robustness with respect to measurement noise at low speeds and the fast recovery characteristics against external load torque disturbance at high speeds. To illuminate the performance of the proposed controller further, some experimental results are presented.
