Introduction: The liver-specific hexokinase isoenzyme, referred to as glucokinase, is thought to play a key reglulatory role in hepatic glucose metabolism. The glucokinase gene is, therefore, of interest both because of its tissue-specific expression and because of the several regulatory processes that can be analyzed. The level of hepatic glucokinase activity appers to be determined essentially by regulation of the rate of enzyme sythesis, with insulin playing a leading role as an inducer. We investigated the role of insulin for the induction of glucokinase in the liver of diabetic rats. Methods: Experimental diabetes was induced by injection of streptozotocin 7 days before the experiment. Regular insulin was given by three days intraperitoneal injection at 8-h interval. The glucokinase mRNA in the liver was estimated by Nothern blot assay, as well as by fluorometric enzyme activity assay. Results: Glucokinase activity was not reduced in the liver of normal fasting rats as compared to normal fed rats. But glucokinase activity was recuced in the liver of daibetic rats as compared to normal rats. In diabetic rats treated with insulin, glucokinase enzyme activity were increased. But glucokinase mRNA expression was only increased in normogycemic diabetic rat with treated with insulin as compared to hyperglycemic rat. Conclusion: These data indicate that insulin stimulates hepatic glucokinase enzyme activity and mRNA expression. But other hormonal or metabolic factors may be contribute to regulation of glucokinase mRNA expressiom.

The mass spectrometer, being an essential scientific instrument for uncovering the origin of the solar system and life, has been used since the early 1970s on board spacecraft to obtain information of neutral and ionized elements in the atmosphere and surface of the moon, planets, asteroids, and comets. According to the 4th Basic Plan for the Promotion of Space Development (2023-2027), Korea plans to conduct lunar landing in 2032 and Mars landing in 2045 as the core goals of the plan and focuses on developing the technologies required for unmanned robotic exploration missions. In this regard, it is crucial to develop the technology of a mass spectrometer, which is the most fundamental payload for space exploration for maximized scientific achievements, however never tried before in any domestic space missions. We describe in this paper the principle of a domestically developed quadrupole mass spectrometer, its prototype model, and the test results of its performance. We conclude this paper with intended future improvements.

Perforated peptic ulcers are frequently sealed spontaneously by omentum or by adjacent organs, and the associated peritonitis resolves without operation. Several surgeons, nearly 40 years ago, have applied their knowledge of the phenomenon of spontaneous seal to the treatment of the perforated duodenal ulcer and their reported mortality rate was almost half that generally recorded mortality rate for surgical treatment at that time. But despite these encouraging reports of successful non-op treatment of the perforated duodenal ulcer, the conservative treatment had not been gained widespread acceptance. Instead, much of the recent surgical literature concerns not whether patients with perforated duodenal ulcers should have an operation, but whether a definitive operation to heal the ulcer should be performed at the time of perforation. But recently there was a significant advances in ulcer treatment and antibiotic therapy. And also there was a few promising reports of non-op treatment of the perforated peptic ulcers by means of a prospective randomized or selective trial. In this study, we compared the clinical charateristics of emergency operative treatment group with non-op group retrospectively to offer the opportunity to reap- praise the role of the non-op treatment of the perforated duodenal ulcer. Our data suggest that perforated duodenal ulcer can be safely treated non-op when spontaneous seal of the ulcer perforation had occured. Therefore, efforts to induct and maintain the spontaneous sealing by using the initial observation period should be made positively.

우리나라의 첫 달 탐사선 다누리호가 달 궤도에 진입을 성공하면서 이후 화성 탐사와 같은 우리나라의 새로운 우주탐사 계획을 기대할 수 있게 되었다. 우리나라의 우주탐사 탑재체는 한정적인 분야에서만 개발되어 새로운 우주탐사 탑재체를 개발할 필요성이 있다. 국외에서는 우주탐사에 거의 기본적으로 탑재되는 장비로 질량분석기가 있으며, 탐사 대상의 대기와 휘발성 물질 관측과 더불어 유기물 분석을 통한 생명체 탐사까지 아우르는 매우 유용한 탑재체이다. 하지만 우리나라에선 우주탐사용으로 질량분석기 탑재체를 개발한 전적이 없으므로 앞으로의 우주탐사를 대비해 기술을 확보해야 할 필요가 있다. 그에 앞서 국외의 우주탐사용 질량분석기 탑재체의 과학적 성과를 살펴보고 동향을 파악해본다.

Background : Peroxisome proliferator activated receptor-gamma (PPAR-γ) is a nuclear receptor that regulate adipocyte differentiation and modulate intracellular insulin-signaling events. As such, PPARγ is a candidate gene for several human disorders including obesity and type 2 diabetes mellitus. The objective of our study was to examine the relationship between genetic variation of PPARγ2 and diabetes and obesity in Korean subjects. Methods : We studied 99 subjects with type 2 diabetes mellitus, 128 obesity patients and 97 controls. Screening for mutation at codon 12 and 115 of PPARγ2 were carried out by PCR-RFLP analyses. Statistical significance was evaluated by Chi-square test. Results : The allele frequency of the Pro12Ala PPARγ2 variant were 0.05 in controls, 0.06 in type 2 diabetes group, and 0.07 in obesity group (p=0.47). Pro115Gln variant were only proline homozygote in all groups. Genotype frequencies were also similar and conformed to expectations of the Hardy-Weinberg rule. The presence of PPARγ2 gene variant was no associated with concentrations of total cholesterol, triglyceride, HDL-cholesterol, and also with fasting glucose. Conclusion : We concluded that the Pro12Ala and Pro115Gln PPARγ2 missense mutation may not be associated with type 2 diabetes mellitus and obesity in Korean patients.(Korean J Med 59:132-141, 2000)

Background/Aims: The incidence of duodenal tumors has increased by health surveillance. However, preoperative diagnosis of subepithelial duodenal tumors remains difficult because of the wide variety of pathologies and the location of the tumors. We analyzed endoscopic, radiological, and pathological features of subepithelial benign duodenal tumors (BDTs), which were treated by surgical resection. Methods: Five patients with subepithelial BDTs treated by surgical resection were analyzed retrospectively. We compared the preoperative and postoperative diagnosis and evaluated the clinical presentations, endoscopic and radiological findings, surgical treatments, pathological results, and outcomes of these patients. Results: All the patients underwent successful surgical resection. There were two cases of gastrointestinal stromal tumors (GISTs) treated with segmental duodenectomy, one case of carcinoid tumor treated with antrectomy, one case of gangliocytic paraganglioma treated with ampullectomy, and a lipoma removed by mass excision. The two GISTs were in the duodenal third and fourth segment close to the pancreas, and it was difficult to exclude pancreatic tumors by imaging studies. All the patients remained healthy for more than three years. Conclusions: Subepithelial BDTs are rare and difficult to diagnosis. Awareness and preoperative diagnosis of subepithelial BDTs can lead to minimally invasive treatment, including endoscopic or local surgical resection. Korean J Pancreatobiliary 2014;19(1):18-25
