        Orbital Apex Syndrome in a Patient with Sphenoid Fungal Balls

        Orbital apex syndrome (OAS) is a rare disease that presents with a complex of symptoms, including ophthalmoplegia, ptosis and visual loss. Due to the poor prognosis, making a prompt diagnosis and administering the appropriate treatment must be initiated without delay if OAS is suspected. We report here on a case of a patient with sphenoid fungal balls, and he presented with acute visual loss and ophthalmoplegia.         만성부비동염에서 전산화단층촬영과 병리조직학적 소견과의 관계:만성염증의 중증도에 대한 지표

        Background and Objectives:Chronic sinusitis is a comon disease in otolaryngology, but its pathologic mechanism has not been clearly known. Also, the evaluation method for the severity of chronic sinusitis is not established. The aim of this study was to analyze possible factors associated with the correlation betwen the radiological and pathological severity of chronic sinusitis. In addition, we assesed the profiles of inflammatory cells in the sinus mucosa and peripheral blood eosinophils in relation to the overall pathologic grades and OMU CT findings. Subjects and Method:Fifty specimens of pathologic sinus mucosa, obtained during endoscopic sinus surgery were stained with hematoxylin and eosin. Total inflammatory cels, plasma cells, neutrophils, lymphocytes and eosinophils were quantified. The preoperative OMU CT scans were scored by the staging system of Lund-Mackay. Also, the preoperative percentage of eosinophils in peripheral white blood cells were obtained with the complete blood count with differentiation. Results:The count of total inflamatory cells, lymphocytes and eosinophils infiltrated in the diseased sinus sed by Lund-Mackay system corelated significantly with the severity of the pathologic grades. Conclusion:The important in-dicators of the severity of the chronic inflamation in chronic sinusitis were OMU CT scores, overal pathologic grades, and total inflamatory cells, lymphocytes and eosinophils infiltrated in sinus mucosa.

        BTCA에 의한 실크/라미 교직물의 DP 가공 - FT-IR 및 HPLC에 의한 에스테르 가교 평가-

        Silk and ramie union fabric, DP finished with BTCA, were analyzed by FT-IR and HPLC and the physical properties were evaluated. Through FT-IR analysis, we could identify cross-linking by the presence of carbonyl peak in ramie and silk appearing at 1725 $cm^{-1}$ /. The peak intensity increased as the concentration and the temperature increased. With the use of HPLC, we could measure the amount of BTCA cross-linked using a calibration curve. The amount of BTCA detected increased in a straight line as the concentration and temperature increased. Crease resistance was higher than that of untreated silk and ramie union fabric under each processing condition and it increased in a straight line as the concentration and temperature of BTCA finishing increased.

      • A Case of Nasal Involvement of Sarcoidosis

        Sarcoidosis is a chronic systemic disease of unknown etiology characterized by non-caseating granulomatous inflammationof various organs. Nasal involvement of sarcoidosis is rare. Typical symptoms are nonspecific for sinonasal sarcoidosisand include nasal obstruction, postnasal drip, headache and recurrent sinus infection. We experienced a woman who had sarcoidosisinvolving the inferior turbinate. She was treated with en block resection, systemic steroids and immunosuppressiveagents. So, we report this case with the review of literature.

      • 개구리에서 산-염기 평형의 계절적 변동에 관한 연구

        The influence of environmental temperature on the acid base balance of frog(Rana Temporaria Chosenica) blood was investigated at 25℃ all the year round. pH of blood was measured by means of Artrup method and Sigaard- Anderson's nomogram using pH meter(Corning) and tonometer. Buffer value, actual bicarbonate concentration and OH^-/H^+ ratio were calculatedd by the measure buffer line, pH and CO2 gas tension which was used(low CO_2 was 4-5% and high C0_2 was 8-9% balanced with 0_2). Results obtained are as follows: 1. Buffer line was shift to the upper and-right side during winter season and the differences was statistically significant. 2. Buffer value was higher during winter season than summer value and the difference was significan (p<0.001). 3. The actual blood pH was 7.42±0.01 in September and 7.78±0.03 in January. Generally the pH was higher during winter season than that of summer. The difference between two seasons was statistically significant(p<0.05). 4. The blood PCO_2 value has a seasonal changes with low PC0_2 during winter season and the differences were statistically significant. 5. The plasma bicarbonate concentration was 17.00±1.07mM/L in September and the value were decreased with cold weather of 12.33±1.29mM/L in January. Seasonal changes were significantly different(p<0.05). It is concluded from this findings that the acid-base changes of blood in frog were related to the temperature of living habitat and the corresponding changes in the tissue energy store and buffers.

        OTT 서비스의 VOD와 실시간 방송 이용 의도 분석

        본 연구는 기대가치이론을 기반으로 OTT 서비스의 이용행태 및 의도를 분석하고자 했다. OTT 서비스를 하루 평균 30분 이상 이용하는 400명을 대상으로 하여 기대가치이론의 주요변인인 추구충족 및 획득충족의 요인을 연구하였다. 추구충족 요인으로는 메뉴 및 품질 우수성, 시청능동성, 기기다양성 등 3개 요인이 확인되었으며, 획득충족 요인으로는 편재성 및 기기다양성, 메뉴우수성, 품질우수성, 시청능동성 등 4개 요인이 확인되었다. 또한 VOD와 실시간 주 이용자를 분석한 결과, VOD 주 이용자는 편재성 및 기기다양성과 시청능동성이 VOD 이용의도에 영향을 미쳤으며, 편재성 및 기기다양성이 실시간 이용의도에 직접적인 영향을 미치는 변인으로 나타났다. 실시간 주 이용자는 편재성 및 기기다양성과 품질우수성이 실시간 및 VOD 이용의도에 모두 직접적으로 영향을 미치는 변인으로 나타났다. 한편, 실시간 주 이용자에게 메뉴우수성은 성별과 연령에 의해 조절되어 실시간 이용의도에 영향을 미쳤고, 실시간 주 이용자의 VOD 이용의도에는 연령에 의해 조절되어 영향을 미치는 것으로 나타났다. This study analyzed users’ usage and intention to use OTT service based on the Expectancy Value Theory. 400 participants using OTT services over half an hour per day on average responded to questions regarding gratification sought and gratification obtained as the main factors of the Expectancy Value Theory. Results showed that gratification sought involves menu and quality excellence, watching activeness, and device diversity, and gratification obtained is a function of ubiquity and device diversity, menu excellence, quality excellence, and watching activeness. According to the gratification obtained by main VOD users, ubiquity, device diversity and watching activeness directly influenced the participants’ intention to use VOD, while ubiquity and device diversity had a direct effect on their intention to use real-time broadcasting. According to the gratification obtained by main real-time users, their intentions to use real-time and VOD OTT service both depended on the ubiquity and device diversity, and quality excellence. The influence of menu excellence on main real-time users’ intentions for real-time use was mediated by users’ gender and age, while its effect on users’ intentions for VOD was mediated by age

        방송클립동영상 이용이 방송 시청의도에 미치는 영향

        본 연구에서는 방송클립동영상 이용이 VOD 구매의도와 본방송 시청의도에 영향을 줄 수 있는지를 살펴보고자 하였다. 이를 위해 방송클립동영상 이용자를 대상으로 설문조사를 통해 이용동기와 이용시간, 이용시점, 장르, 이용패턴 등의 이용경험 중심으로 살펴보았다. 연구결과를 요약하면, 방송클립동영상 이용동기 변인에서는 대인관계와 전체방송 대비이점이 높을수록, 방송클립동영상 이용경험 변인에서는 교육 장르, 게시된 영상보기, 게시된 영상 공유 및 추천을 이용하는 정도가 높을수록 VOD를 구매하고자 하는 의도가 높은 것으로 나타났다. 그리고 방송클립동영상 이용동기 변인에서는 이용유연성이 높을수록, 방송클립동영상 이용경험 변인에서는 드라마/영화 장르와 게시된 영상에 대한 댓글 읽기 및 작성 등과 같은 이용이 높을수록 본방송을 시청하고자 하는 의도가 높은 것으로 나타났다. The purpose of this study is to search shot clip using variables influencing on the VOD purchase intention and watching intention of real-time broadcasting. This research focused on usage motivation, usage experience such as use time, time of utilization, genre, utilization pattern. Controlling demographic factors, results show that personal relation, benefit against whole broadcast, education genre, watching video, video sharing and recommendation were statistically influential to VOD purchase intention. And, results show that flexibility of use, drama/movie genre, read and write comments were statistically influential to watching intention of real-time broadcasting.

      • Clinical Diagnosis and Treatment of Olfactory Dysfunction

        Olfactory dysfunction is a relatively common disorder that is often under-recognized by both patients and clinicians. It occurs more frequently in older ages and men, and decreases patients’ quality of life, as olfactory dysfunction may affect the emotion and memory functions. Three main causes of olfactory dysfunction are sinonasal diseases, upper respiratory viral infection, and head trauma. Olfactory dysfunction is classified quantitatively (hyposmia and anosmia) and qualitatively (parosmia and phantosmia). From a pathophysiological perspective, olfactory dysfunction is also classified by conductive or sensorineural types. All patients with olfactory dysfunction will need a complete history and physical examination to identify any possible or underlying causes and psychophysical olfactory tests are essential to estimate the residual olfactory function, which is the most important prognostic factor. CT or MRI may be adjunctively used in some indicated cases such as head trauma and neurodegenerative disorders. Functional MRI (fMRI) and psychophysiological tests (olfactory event-related potential, OERP) are also used in the research setting. Compared to rapid progress that has occurred in fields of basic science and diagnostic tools for the therapy of other diseases and disorders, treatments for olfactory loss are still in a state of unmet need. In most olfactory dysfunctions, there has been no well-designed randomized controlled study to justify or prove effective treatment modalities. Therefore, with more attention to the problem and further research we can expect breakthroughs in the treatment of smell loss in the near future.
