XPC polymorphisms and lung cancer risk

<P>Polymorphisms in DNA repair genes may be associated with differences in the capacity to repair DNA damage and thereby influence an individual's susceptibility to smoking-related cancer. To test this hypothesis, we investigated the potential association of 7 XPC polymorphisms (–449G→C, –371G→A, –27G→C, Val499Arg, PAT–/+, IVS11-5C→A and Lys939Gln) and their haplotypes with lung cancer risk in a Korean population. XPC genotypes were determined in 432 lung cancer patients and 432 healthy controls frequency-matched for age and sex. XPC haplotypes were predicted using a Bayesian algorithm in the Phase program. The combined –27CG+CC genotype was associated with a significantly increased risk for overall lung cancer compared to the –27GG genotype (adjusted OR = 1.97, 95% CI 1.22–3.17, p = 0.005). The other 6 polymorphisms were not significantly associated with overall risk of lung cancer. When lung cancer cases were categorized by tumor histology, the –371AA genotype was associated with a significantly increased risk of squamous cell carcinoma compared to the combined –371GG and GA genotype (adjusted OR = 2.08, 95% CI 1.09–4.00, p = 0.03). The PAT–/+, IVS11-5C→A and Lys939Gln polymorphisms were associated with a significantly decreased risk of small cell carcinoma (SM) under a dominant model for the polymorphic allele (adjusted OR = 0.49, 95% CI 0.29–0.82, p = 0.006; adjusted OR = 0.60, 95% CI 0.36–1.00, p = 0.05; and adjusted OR = 0.58, 95% CI 0.35–0.97, p = 0.04, respectively). Consistent with genotyping analyses, haplotype 4 (1112222) containing the PAT+/IVS11-5A/939Gln alleles was associated with a significantly decreased risk of SM (adjusted OR = 0.56, 95% CI 0.37–0.85, p = 0.007 and Bonferroni-corrected p = 0.049), whereas haplotype 5 (1122111) containing the –27C allele was associated with a significantly increased risk of SM (adjusted OR = 2.88, 95% CI 1.41–5.87, p = 0.004 and Bonferroni-corrected p = 0.028). These results suggest that XPC polymorphisms/haplotypes may contribute to genetic susceptibility for lung cancer. © 2005 Wiley-Liss, Inc.</P>

<P><B>Abstract</B></P> <P>Advanced Power Reactor of 1400 MWe (APR1400) adopts a Direct Vessel Injection (DVI) system that injects Safety Injection (SI) water directly into the reactor vessel downcomer during accidents. As the DVI nozzles are directly attached to the reactor vessel downcomer, complex thermal-hydraulic phenomena occur in the downcomer region. Recently, in the International Standard Problem (ISP) No. 50 exercise, the phenomena of Emergency Core Cooling (ECC) water mixing in the upper downcomer during the DVI line break accident observed in Advanced Thermal-hydraulic Test Loop for Accident Simulation (ATLAS) test were highlighted in terms of the prediction capability of the system analysis codes. Thus, to validate the results from the ATLAS study independently, an additional experimental study was performed with an integral effect test facility, Seoul National University Facility (SNUF), to observe the mixing behavior in the downcomer during a DVI line break accident. According to the SNUF test results, the ECC water mixed vigorously in the downcomer annulus. However, the temperature difference in the azimuthal direction was predicted by a system analysis code, Multi-dimensional Analysis Reactor Safety (MARS). In the MARS calculation, the momentum flux terms are set to zero for the junction between the one-dimensional volume and three-dimensional cell of the MultiD component because the axial and radial velocities are marginal in the large three-dimensional region. However, if the nozzles are attached to the downcomer with a thin gap, the axial and radial velocities are significant when the incoming orthogonal flow through the nozzles impinges against the downcomer wall. It was necessary to consider the momentum flux terms induced by the impinging flow, and to do so, an appropriate jet impingement model, for incorporation in the system analysis code, MARS, was developed in this study. To develop the jet impingement model, Computational Fluid Dynamics (CFD) calculations were carried out, and the jet impingement model was formulated based on the CFD calculations under various conditions. The momentum flux term resulting from the jet impingement phenomenon was correlated with the diameter of the nozzle, downcomer gap size, and incoming flow velocity. This model was applied to MARS by considering the momentum flux term for the junctions connected to the cell of the MultiD component. The modified MARS incorporating the jet impingement model was validated with the test results from the SNUF and ATLAS, and the analysis results exhibited reasonable agreement with the test data.</P>

Lupus vulgaris is the most common form of cutaneous tuberculosis showing various clinical manifestations. Because AFB staining and culture are often negative, diagnosis can be difficult. Polymerase chain reaction with high sensitivity and specificity may improve the diagnostic panel for cutaneous tuberculosis. We report a case of lupus vulgaris in a 5 year-old girl. The presence of Mycobaterium tuberculosis DNA in a skin biopsy demonstrated by polymerase chain reaction led to the diagnosis of lupus vulgaris.
유지 혈액투석 환자에서 에리트로포에틴의 반응성에 관한 연구

목 적 : 혈액투석 중인 만성 신부전 환자에서 빈혈은 흔히 접하게 되는 중요한 임상적인 문제로 에리트로포에틴이 치료제로 널리 사용되면서 많이 개선되었으나 약제가 고가이고 환자마다 반응성에 큰 차이가 있어 이에 영향을 미치는 인자를 알아보기 위해 연구를 시행하였다. 방 법 : 2001년 1월부터 2003년 3월까지 고려대학교 구로병원과 개인 인공신장실에서 3개월 이상 유지 혈액투석 요법을 받는 환자들 중 에리트로포에틴 투여 후 혈색소가 2.0 g/dL 이상 상승한 70명의 환자를 대상으로 하였다. 혈색소가 2.0 g/dL 이상 상승하는 동안 투여된 주당 건체중 당 에리트로포에틴의 용량에 따라 저용량 군과 고용량 군으로 나누어 이 두 군간에 나이, 성별, 만성 신부전의 기저 질환, 혈액투석 기간, 안지오텐신 전환 효소 억제제의 사용 빈도, 혈색소, 헤마토크리트, 혈청 철, 총 철분 결합능, Trasferrin 포화도, Ferritin, 알부민, 콜레스테롤, 부갑상선 호르몬 농도, C-반응성 단백질, CO₂content, BUN, 크레아티닌, Kt/V 등에 관해 비교 분석하였다. 결 과 : 두 군간에 나이, 성별, 만성 신부전의 기저 질환, 안지오텐신 전환 효소 억제제의 사용 빈도, 혈청 철, 총 철분 결합능, Trasferrin 포화도, Ferritin, 콜레스테롤, C-반응성 단백질, CO₂content, BUN의 유의한 차이는 없었다. 저용량 군이 고용량 군보다 혈액투석 기간이 짧고 (40.9개월 vs. 66.1개월, p=0.036), 혈청 알부민이 높고 (3.93 g/dL vs. 3.72 g/dL, p=0.011), 부갑상선 호르몬 농도가 낮은 (94.97 pg/mL vs. 218.52 pg/mL, p=0.013) 특징을 보였다. 저용량 군이 고용량 군보다 크레아티닌과 (10.53 mg/dL vs. 9.40 mg/dL, p=0.053) Kt/V가 높은 (1.69 vs. 1.38, p=0.080) 경향을 보였으나 통계적으로 유의한 차이는 없었다. 결 론 : 혈색소를 증가시키기 위해 투여된 주당 건체중 당 에리트로포에틴 용량이 적은 저용량 군의 환자는 에리트로포에틴 용량이 많은 고용량 군의 환자에 비해 혈액투석을 받은 기간이 짧았으며 혈청 알부민이 높고 부갑상선 호르몬 농도가 낮았다. Background : Hyporesponsiveness to erythropoietin is an important issue in the treatment of the anemia of chronic renal failure. We tried to identify the factors of erythropoietin responsiveness in chronic renal failure patients undergoing maintenance hemodialysis for the effective treatment of anemia. Methods : Seventy hemodialysis patients with hemoglobin increment over 2.0 g/dL during erythropoietin treatment were divided into two groups by median erythropoietin dose, 120 IU/㎏/week (the low-dose group vs. the high-dose group). We compared age, gender, cause of renal failure, duration of hemodialyis, use of angiotensin-converting enzyme inhibitor, hemoglobin, hematocrit, serum iron, TIBC, transferrin saturation, ferritin, albumin, cholesterol, parathyroid hormone (iPTH), CRP, CO₂content, BUN, creatinine and Kt/V between the two groups. Results : The low-dose group had significantly shorter duration of hemodialysis (40.9 months vs, 66.1 months, p=0.036), higher serum albumin level (3.93 g/dL vs. 3.72 g/dL, p=0.011) and lower iPTH level (94.97 pg/mL vs. 218.52 pg/mL, p=0.013) compared with the high-dose group. Serum creatinine level and Kt/V showed a tendency to be higher in the low-dose group but there were no significant differences (10.53 mg/dL vs. 9.40 mg/dL, p=0.053 and 1.69 vs. 1.38, p=0.080). Othere clinical and laboratory parameters were not different between the two groups. Conclusion : Adequate nutritional support and prevention of secondary hyperparathyroidism may be helpful to enhance the responsiveness of erythropoietin in chronic renal failure patients undergoing maintenance hemodialysis.
