A Multi-Center Educational Research Regarding Breastfeeding for Pediatrics Residents in Korea

Purpose: Pediatricians have a significant responsibility to educate mothers about the importance of breastfeeding. However, there have been minimal efforts in the courses of resident training in Korea. The purpose of this study is to evaluate the change in knowledge and attitude before and after a 4-week breastfeeding educational intervention among multicenter residents. Methods: Prospective interventional educational research was designed for residents at eight training hospitals in Korea. Institutional reviews were obtained in each hospital. The education curriculum consisted of 14 courses regarding breastfeeding theory and practice. These materials were used to teach pediatric residents for 4 weeks. Knowledge-based tests were administered before the course, and re-tests were administered after the course using different test items of similar levels. Test scores and survey responses were compared before and after the intervention. Results: A total of 73 residents (1st year 20, 2nd year 23, 3rd year 16, and 4th year residents 14) from eight training hospitals completed the intervention. Their average age was 30.3±2.9 years, 17 (23.3%) were male, 22 (30.1%) were married, and eight had more than one child of their own. The mean pre-test score was 61.8±13.4 and the mean post-test score was 78.3±7.5 (P<0.001). The inter-grade difference in the score was significant in the pre-test (P=0.005), but not significant in the post-test (P=0.155). There were more responses of obtaining confidence after the intervention (P<0.001). Conclusion: In our study, pediatric residents showed improvement in their knowledge and confidence level after 4 weeks of the breastfeeding curriculum. Korea Aerospace Research Institute (KAKI) is developing a Korea Multi-Purpose Satellite I (KOMPSAT-1) which accommodates Electro-Optical Camera (EOC), Ocean Scanning Multi-spectral Imager (OSMI), and Space Physics Sensor (SPS). The satellite has the weight of about 500kg and will be operated on the 10:50 AM sun-synchronized orbit with the altitude of 685 km. The satellite will be launched in 1999 and its lifetime is expected to be over 3 years. The main mission of EOC is the cartography to provide the images from a remote earth view for the production of 1/25000-scale maps of KOREA. EOC collects 510 - 730 nm panchromatic imagery with the ground sample distance(GSD) of 6.6 m and the swath width of 17 km by push broom scanning. EOC also can scan ±45 degree across the ground track using body pointing method. The primary mission of OSMI is worldwide ocean color monitoring for the study of biological oceanography. It will generate 6 band ocean color images with 800 km swath width and 1km GSD by whiskbroom scanning. OSMI is designed to provide on-orbit spectral band selectability in the spectral range from 400 nm to 900 nm through ground command. This flexibility in band selection can be used for various applications and will provide research opportunities to support the next generation sensor design. SPS consists of High Energy Particle Detector (HEPD) and Ionosphere Measurement Sensor (IMS). HEPD has missions to characterize the low altitude high-energy particle environment and to study the effects of radiation environment on microelectronics. IMS measures densities and temperature of electrons in the ionosphere and monitors the ionospheric irregularities at the KOMPSAT orbit.

<P>Atopic dermatitis (AD) is an allergic disease that has recently shown a dramatic increase of incidence in developed countries. Eosinophilia, the accumulation of eosinophils, occurs in AD patients through an anti-apoptotic mechanism. To understand the target proteins involved in the anti-apoptotic signaling of eosinophilia, we used a proteomic approach to analyze eosinophil proteins from AD patients with eosinophilia and healthy donors. Protein spots in two-dimensional electrophoresis (2-DE) gels were identified with peptide mass fingerprinting (PMF) based on matrix-assisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF MS) and database searching. More spots were observed in the 2-DE proteome map from AD patient samples (1310 ± 58 spots) than in those from healthy donors (1121 ± 40 spots). We identified 51 proteins affected by eosinophilia: 19 related to signaling, 8 involved in regulation of metabolism, 4 related to apoptosis, and 3 involved in inflammation. The other identified proteins were associated with transcription, RNA processing, translation, the cytoskeleton, and unknown functions. Among the identified proteins, we observed prominent increases in the expressions of cyclinA2, voltage-dependent anion channel protein 2, and 38 kDa FK506 binding protein 8 in eosinophils from AD patients in comparison to healthy donors. PMF and immunoblotting of a single spot that was expressed in eosinophils from healthy individuals but not in AD patients identified the protein as phosphorylated growth receptor binding 7 (Grb7) adaptor protein. Increased phosphorylation of Grb7 and its upstream signaling protein, focal adhesion kinase (FAK), was detected in low viability eosinophils such as those from healthy donors or in cultured eosinophils (AML14.3D10 cells) treated with dexamethasone. These results suggest that phosphorylation of Grb7 and the expressions of cyclinA2, voltage-dependent anion channel protein 2, and 38 kDa FK506 binding protein 8 may be related with the anti-apoptosis mechanism of eosinophilia.</P>
Validation of non-invasive brain temperature estimation models during swine therapeutic hypothermia

<P> <I>Objectives</I>: This paper introduces a mathematical model that can estimate deep brain temperature during therapeutic hypothermia (TH) based on a double sensor method (DSM). Although the cerebral temperature is more important than the non-cerebral core temperature during TH, pulmonary artery (PA), rectal, and esophageal measurements (i.e. the typical core temperature measurement locations) have all been used for target temperature management. This is because there is no safe means of measuring the exact brain temperature. <I>Approach</I>: We applied a double sensor thermometer to the subject’s forehead to measure the cerebral temperature non-invasively. Invasive and non-invasive brain temperature readings were acquired for 11 pigs, seven of which were used to develop an optimal model using jackknife resampling and four of which were used to test the model. <I>Main results</I>: The logit model exhibited the best performance of 0.134 °C root mean square error and a 0.993 Lin’s concordance correlation coefficient (CCC). Each test dataset had acceptable results in that each 95% limit of agreement was within the range of clinical acceptance of [−0.5 °C, 0.5 °C]. Three of the four datasets yielded an ‘almost perfect’ score for Lin’s CCC. <I>Significance</I>: Only a small number of studies have compared invasively and non-invasively measured brain temperatures, while most previous studies have concentrated on comparison with the core temperature. Furthermore, the possibility of measuring the exact brain temperature safely during TH using a DSM is shown in this work.</P>
