Twenty-three patients with pleural effusion of unknown etiology were selected because by means of radiological and cytologic examination, smear of the sputum and pleural fluids for tubercle bacilli and eggs of the lung fluke had failed to provide a diagnosis, from September, 1974 to October, 1975 at the Department of internal Medicine, National Medical Center in Korea. Needle biopsy of the pleura using Vim-Silverman needle, culture of the sputum and pleural fluids for tubercle bacilli, red blood cell, lymphocyte, monocyte and mesothelial cell counts in the pleural fluids, and measurement of the pleural fluid glucose and pH levels were done to provide definite or suspected diagnosis. The results were as follows: 1) neeedle biopsies were done on 23 patients and 11 of these showed tuberculous findings, 2 of these showed lymphoma and the remaining 10 showed nonspecific chronic inflammation (43.5%). In one patient of the nonspecific group, an open pleural biopsy which showed paragonimiasis. 2) culture of the pleural fluid was positive for tubercle bacilli in only 2 of the tuberculous group and 1 of the nonspecific group. 3) Used in conjunction with pleural fluid red blood cell, lymphocyte, monocyte, rresothelial cell counts and glucose, pH levels, most provable tuberculous etiology could be established in 6 of 8 nonspecific group, and remaining 2 were as idiopathic pleural effusion. 이 연구의 목적은 제대군인에 대한 효율적인 전직지원을 위해서 현행 전직지원 프로세스에 대한 개선 안과 이에 따른 국방부, 국가보훈처 등 제대군인 전직지원관련 기관 간의 역할분담 및 연계방안에 대해서 제안하는 것이다. 연구는 크게 세 가지 과정을 거쳤다. 첫째, 국내 군 전직지원 실태를 분석하여 문제점과 시사점을 도출하였다. 둘째, 이러한 문제점과 시사점, 군 전직지원 선행연구를 종합하여 효율적인 군 전직지원 프로세스 안을 제시하였다. 셋째, 구축한 군 전직지원 프로세스 안에 따라 주요 군 전직지원 관련 기관인 국방부, 국가보훈처의 역할 분담 안을 제안하였다. This study aims to suggest an improvement plan for the support process of effective occupation change for discharged soldiers and then role assignments among related institutions for supporting discharged soldiers such as Ministry of Defense, Ministry of Patriots and Veterans Affairs, Ministry of Labor and the cooperation plan among the institutions. The study was made in three steps. First, after analyzing currents support facts and status for discharged soldiers, it induced its problems and implications. Second, integrating the findings of previous studies related to support plan on occupation changes for the discharged soldiers, Problems and implications mentioned above, it constructed effective occupation change process for discharged soldiers. Then, according to the established process, it suggested role assignments such as Ministry of Defense, Ministry of Patriots and Veterans Affairs, Ministry of Labor.
Chemical Synthesis of 15-Ketosterols and their Inhibitions of Cholesteryl Ester Transfer Protein

Described herein are the chemical syntheses of 3β-hydroxy-5α-cholest-8(14)-en-15-one and 3β-hydroxy-5α-cholest-8(14),16-dien-15-one from diosgenin and the examinations of their ability to inhibit the cholesteryl ester transfer proein (CETP). Clemmensen reduction of diosgenin gave cholest-5-ene-3β,16β,26-triol. Tosylation of the latter compound gave cholest-5-ene-3β,16β,26-triol 26-tosylate which, upon reduction with LiAlH₄, gave cholest-5-ene-3β,16β-diol. Hydrogenation-benzoylation of the latter to 5α-cholest-3β,16β-diol 3β-benzoate followed by mesylation-elimination gave 5α-cholest-16-ene-3β-ol 3β-benzoate. Controlled oxidation of the latter with CrO3-dimethylpyrazole gave 3β-hydroxy-5α,14α-cholest-16-en-15-one 3β-benzoate. Oxidation of △^(16)-15-one with SeO₂ gave 3β-hydroxy-5α-cholest-8(14),16-dien-15-one 3β-benzoate along with 3β-hydroxy-5α,14βcholest-16-en-15-one 3β-benzoate. Selective hydrogenation of the △^(8(14),16)-15-ketosteryl ester, followed by base hydrolysis gave 3β-hydroxy-5α-cholest-8(14)-en-15-one. Hydrolysis of 3β-hydroxy-5α-cholest-8(14),16-dien-15-one 3β-benzoate in basic media gave 3β-hydroxy-5α-cholest-8(14),16-dien-15-one. The effects of the 15-ketosterols on the CETP activity were studied m vitro by incubating cholesteryl reset donor (HDL), cholesteryl ester acceptor (LDL) and human plasma as a CETP source at 37℃. 3β-Hydroxy-5α-cholest-8(14)-en-15-one was found to be active in supression of the levels of CETP activity in human plasma.
Velocity Profile Generation Methods for Industrial Robots and CNC Machine tools

We propose software algorithms which provide the characteristics of acceleration/deceleration essential to high dynamic performance at the transient state where industrial robots or CNC machine tools start and stop. The path error, which is one of the most significant factors in performance evaluation of industrial robots and CNC machine tools, is analyzed for linear, exponential, and parabolic acceleration/deceleration algorithms in case of circular interpolation. The analysis shows that the path error depends on the acceleration/deceleration routine and the servo control system. In experiments, the entire control algorithm including the proposed acceleration/deceleration algorithms is executed on the motion control system with a floating point digital signal processor(DSP) TMS320C30 as a CPU. The experimental results demonstrate that the proposed algorithms are very effective in controlling axes of motion of industrial robots or CNC machine tools with the desired characteristics.

Purpose This study was performed to assess the usefulness of $^{18}F$-fluorodeoxyglucose ($^{18}F$-FDG) positron emission tomography (PET) or PET/computed tomography (CT) for distinguishing thymic epithelial tumors according to World Health Organization (WHO) classifications. Methods We analyzed a total of 45 patients (range, 29. 75 years of age; mean, 55 years) with pathologically confirmed thymic epithelial tumors who underwent pretreatment $^{18}F$-FDG PET or PET/CT between November 2003 and October 2009. The size, visual grading of uptake value, peak standardized uptake value (SUVpeak), uptake pattern, and contour of each tumor, and associated findings on PET or PET/CT, were analyzed relative to the three simplified WHO subgroups: less-invasive thymomas (types A and AB), more-invasive thymomas (types B1, B2, and B3) and thymic carcinomas. We statistically assessed the relationship of $^{18}F$-FDG PET or PET/CT findings with these simplified subgroups. Results Of the 45 patients, ten had less-invasive thymomas, 23 had more-invasive thymomas, and 12 had thymic carcinomas. The SUVpeak of the less- and more-invasive thymomas were significantly lower than those of thymic carcinomas (p<0.000), but there was no difference in SUVpeak between less- and more-invasive thymomas. The visual grading scale (p<0.000), uptake pattern (p=0.001), and contour (p<0.000) of the tumors differed significantly among the three simplified subgroups. Conclusion The image findings of $^{18}F$-FDG PET or PET/CT differed significantly by histologic subgroups. Pretreatment evaluation with $^{18}F$-FDG PET or PET/CT might be helpful in differentiating subgroups of thymic epithelial tumors.
규칙적인 걷기와 고강도 신체활동 참여에 따른 대사증후군 위험요인 및 고혈압과의 관계

The purpose of this study was to examine the association of walking activity, vigorous physical activity, metabolic syndrome (MS) risk factors, and hypertension among Korean adult population. We analyzed data from The Fifth Korea National Health and Nutrition Examination Survey V-3 in 2012. A total of 5,217 participated in the cross-sectional study. MS was ascertained using criteria of the National Cholesterol Education Program Adult Treatment Panel-Ⅲ. Results showed that 1) participants in walking activity significantly decreased body mass index (BMI), waist circumference, fasting glucose, and total cholesterol compared with those in non-participants. 2) participants in vigorous physical activity significantly decreased systolic blood pressure and number of participants with MS and hypertension compared with those in non-participants. 3) Non-participants in walking and vigorous physical activity (Odds ratio (OR): 1.40, 95% CI: 1.01-1.95) had 40 percent higher risks for prevalence of metabolic syndrome compared to participants in walking and vigorous physical activity (Ref OR: 1) after adjustment for age, and body mass index. In conclusion, This study showed that walking activity and vigorous physical activity are important factors for the prevention of metabolic syndrome and hypertension among Korean adult population.
