Superior mesenteric artery (SMA) syndrome is a rare entity characterized by external compression of the third portion of the duodenum by the SMA, resulting in proximal duodenal obstruction. SMA syndrome is manifesting with nausea, anorexia, epigastric pain, bilious vomiting, and postprandial discomfort severe weight loss in catabolic states, external and intra-abdominal compression, mesenteric tension or postoperative states can lead to SMA syndrome. A 36-year-old man developed sudden-onset epigastric pain, bilious vomiting. He was diagnosed with hyperthyroidism 8 months ago and he was treated with antithyroid drugs for 6 months, but he stopped taking the medicine 2 months ago. Abdominal computed tomography revealed typical findings of SMA syndrome. His symptoms subsided shortly after treatment of hyperthyroidism including antithyroid drugs, beta blocker therapy. Then he took radio-iodine treatment. He has taken synthyroid as hypothyroidism after radio-iodine therapy and maintained euthyroid state.

The control of PWM Inverter-Fed SLIM(Single-sided Linear Induction Motor) is achieved with Vector control and PI control. In the manufacture line. SLIM used to the conveyance machine has required to the quick response and the constant operating without trembling when the goods convey. So, the speed control and the thrust control must be achived, and the trembling of air gap length which is occured between the primary winding core and the secondary structure of the SLIM must be restricted. First, voltage equations of SLIM is calculated from the proposed equivalent in this paper and the exciting current and the thrust component is gotten from its. Its modeling and analysis are able to control robust transient thrust from the current regulation when SLIM drive. These results exemplified a capacity by the direct drive of SLIM used to the conveyance.

A case of Reyes syndrome in a 18-year-old male patient, who was transferred to the hospital because of bizarre behvior, is reported. There were respiratory tract infection and ingestion of aspirin preparation prior to the illness. Serum transaminase levels and serum ammonia levels were elevated. In microscopir examintion of the liver, diffuse microvesicular fatty infiltration was observed. We think that nonpediatricians should consider the possibility of this disease in adult patients with altered consciousness.

The objective is to develop the complex coating technique with high quality and homogeneity of coating for spherical parts with diameter of 2 ~ 4 mm and stable mass production shall be secured with consideration of coating material, process and jig design. Silicon oxide-diamond like carbon (SiO-DLC) complex coating chemistry is expected to enhance the high temperature stability as opposed to DLC. Currently, the spherical part is first welded and then assembled with other parts, finally they are coated with DLC. However in this manner, the maximum charging in one coating batch is limited due to the very big size of the whole component which increases cost. Therefore the spherical part is to be coated before welding. Overall, reactive sputtering with high precision fixturing and masking technique resulted in the best overall properties for modern powertrain fuel system. The jig design was optimized with consideration of coating properties (height, width, thickness, inserting distance) and spherical parts were fixed at the jig with magnet plate. Coating material and process were determined with several trials. In conclusion the PVD+PACVD coated parts is developed and coatings with high quality, homogeneity and 30,000 inserting amount are secured. Coated parts are minimum 100 % more durable than non-coated parts.

From September to November 1g85, 16 cases of Leptospirosis were observed in Chuncheon area, and the following results were obtained. 1) There was no significant differences in age and sex. 2) The outbreak mainly occured in arable fields in Autumn and majority were Agricultural workers. 3) Clinical pictures of Leptospirosis were characterized by fever and chill(100%), myalgia(81.3%), headache(60.3%), and dyspnea(56.3%). 4) On phisical exam, there were fever(100%), tachycardia(68.8%), moist rale(56.3%), pitting edema(43.8%). 5) Abnormal laboratory findings were thrombocytopenia, proteinuria, hypoalbuminemia, and abnormal LFT. 6) On chest X-ray, almost all showed pulmonary edema and congestion. 7) There was a close relationship between total duration of fever and the onset of illness.

Background : Antimicrobial resistance is considered as the primary reason for eradication failure of Helicobacter pylori. Resistance to clarithromycin is mostly due to the point mutation in H. pylori 23S rRNA gene. The aims of this study were to determine the primary resistance rate to clarithromycin and metronidazole and to examine the mechanism of clarithromycin resistance in H. pylori isolates. Methods : Seventynine strains were isolated from 73 patients within about five years. The susceptibility of H. pylori isolates to clarithromycin and metronidazole were tested by E-test and broth dilution test. To detect point mutations in the 23S rRNA gene, PCR-RFLP (restriction fragment length polymorphism) was performed. Mutations in clarithromycin-resistant strains also were analyzed by direct sequencing. Results : The resistance rate to clarithromycin (> 1 mg/L) and metronidazole (> 8 mg/L) were 5.1% and 54.4%, respectively. Annual metronidazole-resistant rates were 43.7% (7/16) in 1996-1997, 61.1% (11/18) in 1998, 55.6% (5/9) in 1999, and 55.6% (20/36) in 2000. Annual clarithromycin-resistant rates were 6.3% (1/16) in 1996-1997, 0% (0/18) in 1998, 11.1% (1/9) in 1999, and 5.6% (2/36) in 2000. Two of 4 clarithromycin-resistant isolates contained the A2144G mutation. One isolate contained A2143G mutation. One isolate possibly contained T2183C mutation. Different strains, isolated separately from antrum and body in 6 patients, showed same susceptibility to clarithromycin. However, different strains in two patients showed different susceptibility to metronidazole. Conclusion : No significant increase of resistantce rate to both clarithromycin and metronidazole were found within recent five years. Resistance of H. pylori to clarithromycin was caused by A2144G and A2143G mutation mainly and by T2183C mutation possibly.(Korean J Med 61:470-478, 2001)
