        Development of New Endovascular Devices for Aneurysm Treatment

        Since the first use of the Guglielmi detachable coil system for cerebral aneurysm embolization in 1990, various endovascular methods have been developed to treat large numbers of aneurysms. The main strategic and technical modifications introduced to date include balloon-assisted coilembolization, stent-assisted coil embolization, flow diverters, and flow disrupters. The development and introduction of such devices have been so persistent and rapid that new devices are being approved worldwide even before the earlier ones become available in some countries. However, even if some patient populations may possibly benefit from earlier introduction of new devices, the approval authorities should balance the available evidence of the safety and effectiveness of novel devices. This review aims to provide an overview of the recent innovations in endovascular treatment of cerebral aneurysms and a brief review of market access policies and regulations for importing high-risk medical devices, such as those used for endovascular aneurysm management, which correspond to class III devices, as defined by the U.S. Food and Drug Administration. We focus on the current situation in Korea and compare it with that in other Asian countries, such as China and Japan.

        Parameter Estimation of 2-D Stochastic FM Model based on Two Step Estimation Procedure

        This paper considers the problem of estimating the parameters of two-dimensional (2-D) stochastic FM models. First, the estimators of the parameters of 2-D stochastic FM model are obtained using two step estimation procedures by Kronecker product and least square method. Then, the asymptotic properties of the estimators are given. It is shown that the estimators are to be consistent and to have a distribution which converges to that of a normally distributed random vector under fairly general conditions. Finally, the performance of the proposed methods is illustrated by examples.

        Asymptotical Stability for 2-D Stochastic Coupled FMII Models on Networks

        In this paper, computable stability criteria for two dimensional stochastic coupled FMII models on networks are studied based on Kronecker product and nonnegative matrices theory. Necessary and sufficient condition for two dimensional stochastic coupled FMII models on networks is presented, this is the first time that has been proposed. Then, computable mean-square asymptotic stability criteria are derived via Kronecker product and the nonnegative matrix theory. The criteria are only sufficient conditions. Finally, illustrative examples are provided.

      • Benefit of Post-mastectomy Radiotherapy of the Supra-/infraclavicular Lymphatic Drainage Area in Breast Cancer Patients

        Background: This study investigated the survival benefit of radiotherapy (RT) of the supra- and infraclavicular lymphatic drainage area in Chinese women with T1-2N1M0 breast cancer receiving mastectomy. Methods: A total of 593 cases were retrospectively reviewed from 1998 to 2007. The relationship between supra- or infraclavicular fossa relapse (SCFR) and post-operative RT at the supra-/infraclavicular lymphatic drainage area was evaluated. Results: The majority of patients (532/593; 89. 8%) received no RT while 61 patients received RT. The median follow-up was 85 months. Among patients without RT, 54 (10. 2%) developed recurrence in the chest wall or ipsilateral SCFR. However, none of the 61 patients who underwent RT demonstrated SCFR. One patient who received RT (1. 6%) experienced recurrence in the chest wall. Univariate analysis revealed that age and molecular subtype (both P < 0. 05) were two prognostic factors related to supraclavicular and infraclavicular fossa relapse-free survival (SFRFS). Multivariate analysis revealed that only Her-2 positive status (P = 0. 011) was an independent predictor of SFRFS. RT had no influence on distant metastasis (P = 0. 328) or overall survival (P = 0. 541). SCFR significantly affected probability of distant metastasis (P < 0. 001) and overall survival (P < 0. 001). Conclusion: Although RT was not significantly associated with SFRFS, postoperative RT was significantly associated with a lower locoregional (i. e., supraclavicular/infraclavicular and chest wall) recurrence rate. SCFR significantly influenced distant metastasis-free survival, which significantly influenced the overall survival of T1-2N1M0 breast cancer patients after mastectomy. Thus, prophylactic RT is recommended in T1-2N1M0 breast cancer patients, especially those who have Her-2 positive lesions.

        Isoindolin-1-ones from the stems of Nicotiana tabacum and their antiviral activities

        In previous studies, several isoindolin-1-oneanalogs that exhibited signifi cant anti-tobacco mosaic virus(anti-TMV) activities were isolated from Nicotiana tabacum . Since gene-editing mutants provide a new sample for thediscovery of active metabolites, we focused on the stems ofYN-18–23 (a mutant N. tabacum for gene editing with thealkaloid metabolic pathway cultivated by Yunnan TobaccoCompany), which led to the isolation of four new ( 1–4 )and four known ( 5–8 ) isoindolin-1-ones. To the best of ourknowledge, nicindole C ( 3 ) is the fi rst subclass of isoindolin-1-one bearing a pentacyclic ketone, while nicindole D ( 4 )is the fi rst example of isoindolin-1-one bearing a methylpyridin-2-(1 H )-one moiety. Compounds 1–4 were testedfor their anti-TMV activities, and the results revealed thatcompounds 1 , 3 , and 4 exhibited high anti-TMV activities atconcentrations of 20 μM with inhibition rates of 48.6, 42.8,and 71.5%, respectively. These rates are higher than the inhibitionrate of the positive control (33.2%). The mechanisticstudy of compound 4 , which had the highest anti-TMV activityrevealed that increased potentiation of defense-related enzyme activities and downregulation of expression of theNtHsp70 protein may induce resistance in tobacco againstthe viral pathogen TMV. Molecular docking studies alsorevealed that the isoindolin-1-one substructure is fundamentalfor anti-TMV activity. The methyl-pyridin-2-(1 H )-onemoiety in compound 4 and the 2-oxopropyl groups in compounds1 and 3 at the N -2 position may increase inhibitoryactivities. This study of the structure–activity relationshipis helpful for fi nding new anti-TMV activity inhibitors. Tostudy whether the isoindolin-1-ones have broader antiviralactivities, compounds 1–4 were also tested for their antirotavirusactivities. Compound 4 exhibited high anti-rotavirusactivity with a therapeutic index (TI) value of 20.7. This TI value is close to that of the positive control (20.2).

      • Serum Tumor Marker Levels might have Little Significance in Evaluating Neoadjuvant Treatment Response in Locally Advanced Breast Cancer

        Background: To determine the potential value of serum tumor markers in predicting pCR (pathological complete response) during neoadjuvant chemotherapy. Materials and Methods: We retrospectively monitored the pro-, mid-, and post-neoadjuvant treatment serum tumor marker concentrations in patients with locally advanced breast cancer (stage II-III) who accepted pre-surgical chemotherapy or chemotherapy in combination with targeted therapy at Fudan University Shanghai Cancer Center between September 2011 and January 2014 and investigated the association of serum tumor marker levels with therapeutic effect. Core needle biopsy samples were assessed using immunohistochemistry (IHC) prior to neoadjuvant treatment to determine hormone receptor, human epidermal growth factor receptor 2(HER2), and proliferation index Ki67 values. In our study, therapeutic response was evaluated by pCR, defined as the disappearance of all invasive cancer cells from excised tissue (including primary lesion and axillary lymph nodes) after completion of chemotherapy. Analysis of variance of repeated measures and receiver operating characteristic (ROC) curves were employed for statistical analysis of the data. Results: A total of 348 patients were recruited in our study after excluding patients with incomplete clinical information. Of these, 106 patients were observed to have acquired pCR status after treatment completion, accounting for approximately 30.5% of study individuals. In addition, 147patients were determined to be Her-2 positive, among whom the pCR rate was 45.6% (69 patients). General linear model analysis (repeated measures analysis of variance) showed that the concentration of cancer antigen (CA) 15-3 increased after neoadjuvant chemotherapy in both pCR and non-pCR groups, and that there were significant differences between the two groups (P=0.008). The areas under the ROC curves (AUCs) of pre-, mid-, and post-treatment CA15-3 concentrations demonstrated low-level predictive value (AUC=0.594, 0.644, 0.621, respectively). No significant differences in carcinoembryonic antigen (CEA) or CA12-5 serum levels were observed between the pCR and non-pCR groups (P=0.196 and 0.693, respectively). No efficient AUC of CEA or CA12-5 concentrations were observed to predict patient response toward neoadjuvant treatment (both less than 0.7), nor were differences between the two groups observed at different time points. We then analyzed the Her-2 positive subset of our cohort. Significant differences in CEA concentrations were identified between the pCR and non-pCR groups (P=0.039), but not in CA15-3 or CA12-5 levels (p=0.092 and 0.89, respectively). None of the ROC curves showed underlying prognostic value, as the AUCs of these three markers were less than 0.7. The ROC-AUCs for the CA12-5 concentrations of inter-and post-neoadjuvant chemotherapy in the estrogen receptor negative HER2 positive subgroup were 0.735 and 0.767, respectively. However, the specificity and sensitivity values were at odds with each other which meant that improving either the sensitivity or specificity would impair the efficiency of the other. Conclusions: Serum tumor markers CA15-3, CA12-5, and CEA might have little clinical significance in predicting neoadjuvant treatment response in locally advanced breast cancer.

        Polyethersulfone membrane modified by zwitterionic groups for improving anti-fouling and antibacterial properties

        Ultrafiltration (UF) membranes are commonly confronted with threats from pollutants during long-termoperation. The zwitterions with high hydrophilicity are expected to be the critical material to improve theanti-fouling and antibacterial abilities of membranes. Herein, a new zwitterionic polyethersulfone (PES)(zwitterionic PES - ZPES) was synthesized. The as-prepared ZPES was used to fabricate a zwitterionic UFmembrane via the non-solvent induced phase inversion (NIPS) method. Zwitterionic polymer skeletonenhanced the hydrophilicity, permeability and anti-fouling properties of the material. As a result, theZPES membrane exhibited doubled water flux growth (269.6 Lm2h1) compared with that of the PES(125.3 Lm2h1) and high protein rejection (98.6%). Besides, the ZPES membrane could maintain anexcellent flux recovery rate (94.1%) and antibacterial effect (more than 95%). Therefore, the excellentanti-fouling and antibacterial abilities of the ZPES membrane broaden its application scope further.

        Fluorescent Nanoparticles for Observing Primo Vascular System Along Sciatic Nerve

        The primo vascular system was found in the epineurium along the rat sciatic nerve following subcutaneous injection of fluorescent nanoparticles at the Zusanli acupoint (ST-36). Nanoparticles were injected into the primo-vessel near ST-36 and flowed along the sciatic nerve. Fluorescence revealed a structure in the epineurium that was hardly detectable. Images of the isolated sample stained with 4’,6-diamidino-2-phenylindole were captured using confocal microscopy. These images showed the distinctive nuclei distribution and multi-lumen structure of primo-vessels that differentiate them from lymphatic vessels, blood capillaries and nerves. This study demonstrates a new use for nanoparticles in fluorescence reflectance imaging techniques during in vivo imaging of primo-vessels.

        Neurotrophic Effects of Schwann Cell Secretions from Wallerian Degenerating Facial Nerves on Neonatal Rats Facial Motoneurons in vitro

        Regeneration and functional recovery of facial nerves after injury are still far from satisfaction. Studies on whether Schwann cells of injured facial nerves could secrete neurotrophic factors to promote nerve regeneration are rare, and that is the pbjective of this study. Schwann cells from Wallerian degenerating segments of rats facial nerves were isolated and cultured in conditioned culture medium, collected, concentrated and fractionated by ultra-filtration. The neurotrophic bioactivity of the soluble secretions from cultured Schwann cells were examined on an established neuron culture model. Dil fluorescence labeling facial motoneurons from neonatal rats, MTT assay, and image analysis for survival of facial motoneurons were used. The results show that facial motoneuron survival activity in Schwann cell culture medium containing a molecular weight larger than 30 kDa component, was significantly higher than in medium with and without serum (p<0.05 and p<0.01 respectively). The effect could be extinguished by either boiling or trypsinizing. At the 2nd, 4th and 7th days after conditioned culture, mean perikaryon area and neurite length of facial motoneurons in a medium containing molecular weight larger than 30kDa component, were significantly larger than in serum containing medium. The results show that Schwann cells from Wallerian degenerating facial nerves in serum-free and neurite re-growth of facial motoneurons in vitro. The results also suggest that the neurotrophic effect might be derived from protein(s) or peptide(s) with molecular weight larger than 30 kDa component of the soluble secretions. It may be potent for functional repair of facial verve and other neural disorders.
