Objective : We assessed the surgical results of bipolar release in 31 adult patients with uncorrected congenital muscular torticollis (CMT) and more than 12 months of follow-up. Methods : Thirty-one patients underwent a bipolar release of the sternocleidomastoid muscle (SCM) and were retrospectively analyzed. The mean follow-up period was 14.9 months (range, 12-30). The mean age at time of surgery was 30.3 years (range, 20-54). Patients were evaluated with a modified Lee's scoring system, cervicomandibular angle (CMA) measurement, and a global satisfaction rating scale using patient self-reporting. Results : The modified Lee's scoring system indicated excellent results in 4 (12.9%) patients, good in 18 (58.1%), and fair in 9 (29.0%) at the last follow-up after surgery. The improvements in neck movement and head tilt were statistically significant (p<0.05). The preoperative mean CMA was $15.4^{\circ}$ (range, 5.4-29.0), which was reduced to a mean of CMA of $6.3^{\circ}$ (range, 0-25) after surgery (p<0.05). The global satisfaction rating scale was 93.7% (range, 90-100). A transient sensory deficit on the ipsilateral lower ear lobe was noted in three cases. No significant permanent complications occurred. Conclusion : Bipolar release of the SCM is a safe and reliable technique for the treatment of CMT in adults.

Since the first report by Tansini in 1896, the latissimus dorsi muscle free flap has been widely used for various types of soft tissue defect due to reliable anatomy with a sufficient diameter of neurovascular pedicle and a sizable muscle. However, for relatively small soft tissue defect, latissimus dorsi free flap offers several distinct disadvantages of donor site including loss of the posterior axillary fold and flattening of the posterolateral chest wall, weakness of upper arm strength in extension, adduction and internal rotation. We treated three patients having various types of soft tissue defect using segmental latissimus dorsi muscular free flap depending on its descending branch of thoracodorsal neurovascular pedicles. There were no serious complications during 18 months of mean follow-up. We concluded that this method has some advantages such as no weakness of strength of the upper arm including walking on crutches, preserving the posterior axillary fold, preventing winging of the scapula and increased chance of using a flow-through technique. Here we present our cases of reconstruction of soft tissue defect using segmental latissimus dorsi free flap with a review of the literature.

윈드서핑선수 체력 트레이닝을 위한 프로그램개발에 관한 연구

The purpose of this research is to investigate training methods and physiological responses according to the indensity through analyzing researches in this area and develope program models for physical training in order to find an effective training method for windsurfing players. 1. Training for Muscular Strength Muscular strength is an important component for windsurfing player to move fast with less fatigue. Weight training is commely used to imporve muscular strength. It is proper for beginners to use loads in the range of 60% to 70% of one repetition maximum, and for athletes in the range of 80% to 100% of 1RM. It is required to do 1 rep at 12-15 RM for 1-2 seconds, and 1 rep at 5-10RM for 2-3 seconds. Moreover, it is adequate for beginners to do 3-5 sets, and for athletes to do 5-7 sets. Handling heavy weights in the pursuit of strength will require a recovery of 3-5 minutes between sets. As 48 hours should elapse between sessions, it is necessary to exercise at least three times a week. Finally, it is important to increase repetitions before increasing the load. The load should be increased by 3-5% of one repetiton maximum after 4 weeks. 2. Training for Muscular Endurance When these ways of training are conducted in good order, it has a good effect on windsurfing players: Chin→up→Hanging-out →Bar →Climbing the rope →Jumping in a crouch →Hanging the rope →Jump over the bench →lifting a sandbag on the top side of the foot →Throwing the ball in the position of lying down 3. Whole body endurance Whole body endurance is usually regarded as cardiovascular endurance or stamina. To improve the whole body endurance, players are trained by the methods such as long-distance run, Country cross, rope jumping, and swimming. Long-distance running is an essential item for windsurfing players to build up their physical strength, especially cardiovascular function. It is effective to run 4-5km at 150-170 heart rates. For rope jumping, exercise 3-5 rounds and do 3 minutes for one round. The significant components of physical strength for windsurfing players are muscular strength, muscular endurance, and whole body strength. The training for these factors would have an important influence on windsurfing players.

Thunderclap headache is a hyperacute, severe headache that is so named because the pain strikes suddenly and severely, and is sometimes a sign of a medical emergency. We report a case of pheochromocytoma in the urinary bladder in a 48-year-old woman who presented with recurrent thunderclap headache after voiding and exercise. So we think thunderclap headache during or immediately after micturition may be an important diagnostic clue of bladder pheochromocytoma.

The syndrome of hemolysis, elevated liver enzymes and low platelet count during pregnancy (HELLP syndrome) is recognized as a serous, multisystemic disease and associated with poor maternal and perinatal outcome. The purpose of this study is to elevate the clinical appearance and the maternal and perinatal outcome with HELLP syndrome. It was a retrospective study that maternal outcome and perinatal variables and the clinical course of eight cases HELLP syndrome in 234 cases with pregnancy induced hypertension who visited the Department of Obstetrics and Gynecology, Chungnam National University Hospital from January 1995 to December 1998. The results were as follows; 1. The incidence of the HELLP syndrome among patient with pregnancy induced hypertension was 3.4%(8/234) and mean age, mean gestational age was each 29.7 year-old, 36.7 weeks and there was 3 primigravidas and 5 multipara(38% vs 62%). 2. In the abnormal elevated laboratory finding on admission, mean platelet count, sGOT, sGPT, total bilirubin, LDH, BUN, and creatinine was each as 74,000/㎣, 2231U/ℓ, 1941U/ℓ, 1.7mg/㎗, 10951U/ℓ, 22.3mg/㎗ and 2.0mg/㎗. 3. About the coagulation system, PT and aPTT were all within the normal limit but in 38% of patient with HELLP syndrome fibrinogen was decreased, in 50% FDP was increased and in 25% patient was made diagnosis of DIC. 4. The 6 cases were perceived as immediate delivery, one case was vaginal delivery only and others cesarean section and maternal complication was placenta abruption, severe pulmonary edema, acute renal insufficiency, acute toxic hepatitis and cardiomyopathy but there was no maternal mortality in all cases. 5. The mean birth weight 2,374g, 50% of fetus were delivered before 38 weeks, the IUGR was found in 25% and the perinatal mortality rate was 25%. 6. Neonatal complication was neonatal asphyxia, RDS(> 2 grade), sepsis, hyperbilirubinemia, hypermagnesemia, TTN and major handicaps of PVL. These findings suggest that infants born to HELLP syndrome mother have an increased need for resuscitation at delivery and aggressive management for mothers. Thus obstetricians should be identified promptly and need for appropriate management of nowborn infant and mothers with HELLP syndrome.
