Comparison of the Spiritual Needs of Terminal Cancer Patients and Their Primary Family Caregivers

Purpose: This study was conducted to examine differences in Spiritual Interests Related to Illness Tool (SpIRIT) scores and the degree of spiritual needs (SNs) between patients with terminal cancer and their primary family caregivers and to compare spiritual needs between them. Methods: The study participants were inpatients with terminal cancer and their primary family caregivers at 40 national hospice centers. The final analysis included 120 SpIRIT surveys from patients and 115 from family members, and 99 SNs questionnaires from patients and 111 from family members. Data analysis was conducted using descriptive statistics, the t-test, one-way analysis of variance, and Pearson correlation coefficients. Results: There were no significant between-group differences in SpIRIT scores or SNs. The SpIRIT sub-dimensions that ranked high for both patients and primary family caregivers were "maintaining positive perspective", "loving others", and "finding meaning". The SNs sub-dimensions were ranked identically in both groups, in the order of "love and connection", "hope and peace", "meaning and purpose", respectively. In both groups, the recognition of the importance of spiritual matters and religion were major factors influencing SpIRIT scores and SNs. Conclusion: The SpIRIT scores and degree of SNs of patients with terminal cancer and their primary family caregivers were found to be very closely related, and the needs for coherence and meaning were greater than religious needs. When providing spiritual care for patients with terminal illness, family members should also be considered, and their prioritization of spiritual needs and the importance of spiritual matters and religion shall be taken into account.
Smart Learning 환경에서 GBS 를 활용한 역사수업 설계 연구

최근의 사회 환경과 정보통신 기술의 발달은 과거에 비해 그 속도와 발전방향을 가늠할 수 없을 정도로 빨리 변화하고 있으며, 더욱더 언제(anytime), 어디서나(anywhere) 학습자 중심의 맥락적 상호작용적 학습이 가능한 ‘스마트한’ 학습이 이루어지고 있다. Smart Learning 은 학습자들이 학교에서 벗어나 박물관, 과학관, 미술관 등 비형식적인 학습과 실제적 환경에서의 상황적 학습을 할 수 있는 기회를 제공하고 있다. 따라서 본 연구에서는 먼저 다양한 쓰이고 있는 스마트러닝의 개념과 특징을 규명하고 GBS 의 특징과 학습프로세스의 7 가지 핵심요소를 파악하여 초등학교 역사수업을 박물관과 교실수업을 병행한 실제적 맥락에서 학습할 수 있도록 스마트러닝 환경에서 GBS 을 활용한 역사수업을 설계하였다. 이러한 설계연구는 스마트러닝 환경에서 학습자들은 물리적인 공간을 벗어나 언제 어디서나 뿐만 아니라 이동하면서도 정보를 습득 및 제공할 수 있게 될 것이며, GBS 를 통해서 학습자중심의 맥락적, 상황적, 경험적인 학습 환경을 잘 반영할 수 있게 될 것이다. In these days, social environment and information technology are being developed faster than ever before in an unpredictable way. Under such circumstances, the pattern of study is also changing. Interactive way of learning so called smart learning which is led by the learner and which enables the learner to study anytime and anywhere is popular. Smart Learning provides the learner an opportunity to learn in untraditional places such as museums, science centers and art galleries as well as to conduct situated learning under practical environment. This study identified the concept and characteristics of smart learning and the features of GBS and seven core elements of the learning process and designed a history session for elementary students using GBS under the smart learning environment to apply such findings to sessions both in classrooms and museums. With smart learning, this study will contribute to enabling learners to acquire and provide information anywhere anytime not being restricted by physical spaces and to fully utilizing GBS to create learner-centered environment based on context, situation and experience.
가정 호스피스완화의료 서비스 현황 조사: 실무자 포커스 그룹 인터뷰를 중심으로

The aim of this study was to understand home hospice care status and problem in Korea, and ultimately to develop the home hospice standard. This study was conducted as a part of a study on the institutionalization of the home hospice in Korea. A focus group interview with representatives of seven home hospice agency where have provided home hospice service for years was conducted. All of the participants agreed to the essential components for home hospice service including 24 hour on call service, multidisciplinary team visiting, and periodical team meeting. Visiting frequency was 1-3 times per week mostly by nurses. And they agreed requisitely to fulfill an office for home visiting nurses, storage space, and home visiting bags. The obstacles of providing home hospice were 1) no reimbursement system, 2) difficulties to change medication at home, 3) lack of inpatient beds for symptom control. Standardization of home hospice is critical to improve service quality and to develop reimbursement system. The findings of this study could be used as a basic data to develop home hospice standards and guidelines. 본 연구는 가정 호스피스완화의료 제도도입방안 연구의 일부로 가정호스피스 기관들의 실태를 파악하여 가정 호스피스완화의료 표준지침 개발을 위한 기초자료로서 활용하기 위해 시행되었다. 별도의 인력을 배정하고 지속적으로 가정호스피스를 운영하고 있는 7개 기관의 실무자들을 대상으로 개방식 서면조사와 포커스그룹 인터뷰를 실시하였다. 인터뷰에 참여한 기관들은 24시간 on call 서비스, 의사와 간호사, 사회복지사, 사목자를 포함한 호스피스팀 구성, 정기적 팀회의, 사별가족관리 등을 가정 호스피스완화의료의 필수사항으로 여기고 있었고, 필수 시설은 방문간호사 사무실 및 물품준비 공간, 필수 장비는 방문용 가방으로 파악되었다. 방문을 가장 많이 하는 인력은 간호사였고, 가정방문서비스를 위한 전담인력을 두어야 한다는 데에 합의하였다. 가정호스피스 활성화의 장애요인으로 가정 호스피스에 대한 보험수가보상이 없어 팀원의 방문이 제한적이고, 집에서 처방변경이 어려우며, 24시간 주 7일 입원연계가 어려운 점, 가정 내 돌봄제공자가 없는 경우가 늘어나고 있는 점 등이 제시되었다. 가정호스피스 제도화를 위해 가정호스피스 서비스의 표준 개발은 매우 시급하고도 중요하며, 이에 본 연구의 결과는 서비스 표준안의 초안 마련에 근거자료로 활용될 것이다.
