        The main purpose of this study is to find out whether current English textbooks for middle school students in Korea are properly leveled or graded. The researchers utilized Fry Readability Graph, Flesch Readability Formula, and Dale-Chall Readability Formula for the analysis of eight English textbooks which are being used for second graders. The results of the analysis are as follows. 1. The texts are at least one or two levels higher than the formula suggest. 2. Most of the English textbooks are not properly graded in the light of readability. 3. Dale-Chall's readability formula has more precise prediction about readability analysis among them.

        Self-etching primer를 이용한 교정용 브라켓 부착시 전단결합강도와 파절양상에 관한 비교연구

        본 연구의 목적은 산부식과 전처리 과정을 결합하여 접착 단계를 단순화시킨 self-etching primer의 임상적인 유용성을 판단하고자, self-etching primer로 브라켓을 접착하는 방법과 기존의 37% 인산으로 부식 하여 접착하는 방법을 사용하여, 광중합시 사용되는 광원 및 브라켓 종류에 따른 전단결합강도와 접착파절양상에 관하여 비교 연구하는 것이다. 사람의 상하악 소구치를 포매하여 만든 시편을 부식 및 전처리 방법에 따라 각각 37%인산으로 산부식 후 Transbond XT primer를 사용하여 접착한 군과 Transbond Plus self-etching primer를 사용하여 접착한 군으로, 광원 종류에 따라 가시광선과 plasma arc light을 이용하여 중합한 군으로 나누었고, 브라켓 종류에 따라 금속브라켓과 세라믹 브라켓을 사용하여 접착한 군으로 분류하여 각 군간의 전단결합강도와 접착파절양상을 관찰하여 다음과 같은 결과를 얻었다. 1. 광원과 브라켓 종류가 동일한 조건일 때, self-etching primer를 사용하여 접착한 군과 XT primer를 사용하여 접착한 군간의 전단결합강도는 통계적으로 유의한 차가 없었다. 2. 금속브라켓을 접착한 경우, 광원과 부식 및 전처리 방법에 따른 전단결합강도는 통계적으로 유의한 차가 없었다. 3. Self-etching primer와 XT primer를 사용하여 접착한 군 모두에서 세라믹 브라켓을 사용하여 접착한 군의 전단결합강도는 금속브라켓을 사용하여 접착한 군보다 통계적으로 유의하게 컸다(p<0.001). 4. 접착제 잔류지수는 self-etching primer를 사용하여 접착한 군과 XT primer를 사용하여 접착한 군에서 통계적인 유의한 차가 없었으며, 세라믹 브라켓을 사용하여 접착한 군에서 금속브라켓을 사용하여 접착한 군보다 유의하게 커서, 법랑질-레진 접착 계면 부위의 파절이 더 많은 것으로 나타났다. 이상의 결과로 미루어 보아 Transbond Plus self-etching primer를 사용하여 브라켓을 접착하는 것은 적절한 결합강도를 얻으면서도 사용이 간편하고 시술 시간을 단축시킬 수 있으므로 임상적으로 유용할 것으로 판단된다. A self-etching primer that combines the etchant and primer in one chemical compound saves time and should be more cost-effective to the clinician and patient. The purpose of this study was to evaluate the clinical effectiveness of a self-etching primer by measuring shear bond strengths according to various conditions and observing adhesive failure patterns. For this investigation, 120 upper and lower premolars extracted for orthodontic purposes were used and randomly divided into six groups of twenty teeth each. Human premolars were embedded in a metal cylinder with orthodontic resin. Metal brackets and ceramic brackets were bonded with XT primer and self- etching primer by means of XT adhesive. Upon curing, plasma arc light and visible light were used. After bonding, the shear bond strength was tested by Instron universal testing machine, and the amount of residual adhesive that remained on the tooth after debonding was measured by stereoscope and assessed with an adhesive remnant index. The results were as follows: 1. When brackets were bonded, if other conditions remained the same, there was no significant difference in shear bond strength due to the type of primer - either self-etching primer or XT primer. 2. When metal brackets were bonded, there was no significant difference in shear bond strength according to the source of light - plasma arc light or visible light - and type of primer. 3. There was a very significant difference in shear bond strength according to the type of brackets - metal or ceramic brackets. The shear bond strength of ceramic brackets was stronger than metal brackets. 4. When the adhesive failure patterns of metal brackets bonded with self-etching primer were observed by using the adhesive remnant index, the bond failure of the metal bracket occurred more frequently at the bracket-adhesive. The failure of the ceramic bracket, however, occurred more frequently at the enamel- adhesive interface. The adhesive failure patterns of metal brackets bonded with XT primer observed the same patterns. The above results suggest that self-etching primer can be clinically useful for bonding the brackets without fear of a decrease in shear bond strength.

      • 자가 말초혈액 조혈모세포 이식술의 임상적용

        Responses to chemotherapy correlate with the increased dose of chemotherapeutic agents in some cancers, e.g. breast cancer, Ewing's sarcoma, Hodgkin's disease(HD) and non-Hodgkin's lymphoma(NHL), then for the improvement of the chemotherapy responsiveness, high-dose chemotherapy(HDCT) was proposed. But, it's application was limited due to complications, especially bone marrow suppression. HDCT and succeeding autologous peripheral blood stem cell transplantation(APBSCT) was introduced to overcome this problem. This study was designed to determine the clinical pictures including clinical parameters and the responsiveness of HDCT followed by APBSCT in Hanyang University Hospital. Ten patients were enrolled. They were 8 adults (3 breast cancers, 3 multiple myelomas, 1 HD, 1 NHL) and 2 children (1 acute myelogenous leukemia (AML), 1 neuroblastoma). Mobilization chemocherapy(MCT) followed by Granulocyte Colony Stimulating Factor(G-CSF) was administered to mobilize stem cells from bone marrow to peripheral blood. The stem cells were collected by using CS-3000 and cryopreserved at -196℃ with programmed controlled rate freezer as the mixture with 10% dimethyl sulfoxide(DMSO). HDCT was administered and cryopreserved peripheral stem cells were infused on day 0 as planned. Patients' mean age was 41 years old. After induction chemotherapy three patients achieved complete response and 5 partial response. The median time for bone marrow recovery after MCT was 11.5 days for neutrophil(〉500/㎣) and 4.8 days for platelet(〉20,000/㎣). There were three cases of neutropenic fever and one case of gingivitis. After HDCT with APBSCT, one patient was converted partial response to complete response, and one patient achieved complete response after HDCT without induction chemotherapy. The median time for bone marrow recovery was 12.7 days for neutrophil(〉500/㎣) and 14.7 days for platelet(〉20,000/㎣). Eight patients developed neutropenic fever and required systemic antibiotics. Transfusion was required 1.6 pints for packed red cell and 23.4 pints for platelet concentrates. Toxicity consisted mainly of vomiting, diarrhea, and mucositis which were mild. HDCT with APBSCT can be performed safely with minimal complications. For the evaluation of the exact role of HDCT with APBSCT in malignant diseases, further studies are required as a large scale of patients and lung-term follow up.

      • Thermus caldophilus GK24로부터 내열성 alkaline phosphatase의 특성

        Thermostable alkaline phosphatase was found from Thermus caldophilus GK24, extreme thermophile. T caldophilus GK24 alkaline phosphatase was found to be inducible. When starved of inorganic phosphate, T caldophilus GK24 produces the enzyme alkaline phosphatase. The addition of inorganic phosphate to growth medium had a repressive effect on enzyme synthesis. T caldophilus GK24 was tested for the production of alkaline phosphatase by the addition of various concentrations of sodium glutamate, bactotryptone, glucose and yeast extract to basal salts. Sodium glutamate was found to be effective for the alkaline phosphatase induction. The enzyme was stable in the pH range of 7-12. The optimum temperature, pH of the enzyme were 75-85℃ and 11.0-11.5, respectively.

        적층충전법이 복합레진의 중합수축에 미치는 영향

        본 연구는 복합레진 중합 시 발생되는 중합수축이 C-factor에 의해 어떻게 영향 받는지 그리고 적층충전이 중합수축에 어떻게 영향을 미치는지 알아보고자 시행되었다. 세 가지 다른 깊이의 시편을 비교하였으며, 충전방법에 따른 비교를 위해 5가지 다른 충전방법에 따른 수축응력을 스트레인 게이지 (Strain guage)를 사용하여 측정하였다. 이상의 실험을 통해 얻은 결과는 다음과 같다. 1. 와동깊이에 따른 수축응력을 측정한 결과 2mm와 3mm 시편에서는 800초 후 수축응력이 2.18±0.23MPa과 2.38±0.07MPa로 유의한 차이가 없었다(P>0.05). 2. 4mm 시편에서 800초 후 수축응력은 1.99±0.24MPa로 가장 낮은 수축응력을 보였다(P<0.05). 3. 중합정도를 확인하기 위한 미세경도 실험결과 2mm와 3mm 시편에서는 상면과 하면의 미세경도 차이가 20% 내외로 양호한 중합상태를 보여주었으나, 4mm 시편에서는 상면과 하면의 미세경도가 현저한 차이를 보여 불완전한 중합이 이루어졌음을 알 수 있었다(P<0.05). 4. 충전방식에 따른 수축력의 차이를 비교한 결과 flowable resin을 이장하고 bulk로 충전한 제 5군에서 가장 낮은 수축응력을 보였으며, 제1군(bulk충전)과 4군(oblique 적층충전)이 유사한 정도의 수축응력을 보였고, horizontal과 vertical하게 적층충전한 제2군과 3군에서 가장 큰 수축응력이 관찰되었다(P<0.05). The aim of this study was to investigate the relationship between the C-factor and shrinkage strain values of composite resin and examine the strain values in different incremental filling techniques. The strain gauge method was used for measurement of polymerization shrinkge strain. Experiment was divided two step. In a first experiment, we compared with strain value in three different depth (2mm, 3mm, 4mm) and microhardness of each samples after 24hours were measured. In a second experiment, we examined the strain values in five different filling techniques(Group 1: bulk filling, Group 2: oblique incremental filling, Group 3: horizontal incremental filling, Group 4: vertical incremental filling, Group 5: lining of flowable resin and bulk filling) The results of the present study can be summarized as follows: 1. Composite resin in acrylic molds showed the initial expansion at the early phase of polymerization. 2. Contraction stress was not revealed significant difference between depth of 2mm and 3mm(P>0.05). 3. Contraction stress in sample of 4mm was showed the lowest value(P<0.05). 4. Microhardness of specimen was revealed more difference between upper and lower surface in depth of 4mm than 2 and 3mm(P<0.05). 5. Lining of flowable resin and bulk filling (Group 5) was showed the lowest contraction stress, Group 2 and 3 was showed the highest contraction stress(P<0.05). On the basis above results, the stress that result from the polymerization shrinkage, when incremental curing techniques are used, showed that there is no advantage in incremental placement and curing.

        In case of the occurrence of osteonecrosis at three or more anatomical location, it is defined as multifocal osteonecrosis, but still any king of study with references hasn't been reported in Korea. The patient who had admitted due to the pain in both knee joints, both hip joints and left ankle joint and we diagnosed as multifocal osteonecrosis by using bone scan and magnetic resonance imaging. To relieve right knee joint and left hip joint pain, we perfomed arthroplasty. Here we present one case report of multifocal osteonecrosis through analysis by patient's history and hematologic examination and provide a review of the literature.

      • 카본블랙의 종류 및 배합제의 혼합방법에 따른 NR/HNBR 블렌드의 특성

        It was studied how physical properties, heat resistance, oil resistance, ozone resistance of NR(natural rubber)/HNBR(hydrogenated acrylonitrile butadiene rubber) blends influenced by different mixing methods and various carbon black types. Of the various mixing methods, masterbatch mixing method was better than others in considering the overall stabilities of the compounds. The most important parameter which influenced on heat, oil and ozone resistance was rubber material than sorts of carbon black. The resistance for heat, oil and ozone of NR compounds were improved by blending HNBR and these effects were revealed clearly at elevated temperatures.

      • 고관절 전 치환 성형술 후 탈구에 관한 치험 및 연구

        Dislocation is one of the serious complications in total hip replacement arthroplasties. The purpose of this study is to evaluate the risk factors of dislocation, including age, sex, preoperative diagnosis and bilaterality, history of previous hip surgery, positioning of component, soft tissue tension, treatment after dislocation or redislocation. Authors reviewed 190 patients of total hip replacement arthroplasty, which were performed from Jan. 1990 to Dec. 1996. 15 cases developed dislocation after surgery. Incidence of dislocation was 7.89%(15 of 190 cases). Dislocation after revision was not happened(0 of 18 cases). Inclination of acetabular cup was average 37.5° (range: 31° - 43° ). Average anteversion of the acetabular component was 18.1" by Woo and Morrey method, 11.4° by Hassan method. They were normal range except 3 cases. Dislocation was not prevented even though inclination and anteversion of acetabular cup were within normal range. Soft tissue tension is determined by change of limb and femoral neck length. Average limb length difference was -0.2mm(range; -l0mm - +10mm) and average offset difference was +1.7mm(range;-4mm - +18mm). Revision must be accomplished as soon as possible when limb shortening happen by upward migration of the acetabular component. In conclusion, anteversion of acetabular cup may be most important factor in our cases, so cup position must be fixed in proper position during operation.

      • 호알칼리성 Bacillus sp. No. 4의 Cyclodextrin Glycosyltransferase에 의한 Glycosyl Sucrose의 생산과 저충치성 당으로서의 응용

        Action of a cyclodextrin glycosyltransferase (CGTase) produce from alkalophilic Bacillus sp. No. 4 was studied in a solution containing starch and sucrose to prepare glycosyl sucrose syrup with good sweetness and antidecaying properties of teeth. In the initial stage of the reaction the CGTase produced cyclodextrin, however, the cyclodextrin disappeared and glycosyl sucrose was formed with the lapse of reaction time. The best proportion of sucrose to starch for prodution of glycosyl sucrose was about 1 : 1. The optimum pH and temperature of the coupling reaction was pH 6.0 and 60℃, respectively. Main composition of glycosyl sucrose syrup prepared with 20% starch and 20% sucrose was sucrose 18%, glucosyl sucrose (G_2F) 15.3% and amltosyl sucorse (G_3F) 11.3%. And glucose, maltose and maltotriose were produced very little. Smaller amounts of acid and insoluble glucan were formed in the syrup by Streptococcus mtans OMZ176 than in the sucrose. Therefore, the prepared glycosyl sucrose sucrose syrup is expected to prevent teeth from decaying.
