Ciprofloxacin is one of synthetic fluoroquinolone antibacterial agents which has great bactericidal activity against Gram negative aerobic organisms (especially, Enterobacteriaceae and Pseudomonas aeruginosa) and Gram positive bacteria (including Penicillinase-producing and Methicillin-sensitive Staphylococcus aureus). However, the appearance of ciprofloxacin resistant to many different microorganisms limits the use of ciprofloxacin for patients who are really needed it. This sudy was performed to analyzed the appropriateness for ciprofloxacin use in St. Mary's Hospital setting. Forty-one charts of patients who used ciprofloxacin (Ⅳ) (M/F 24/17, Jan.-Aug. of 1998) underwent drug utilization evaluation (DUE). The criterias of DUE used in this study were based on the standards by the American Society of Hospital Pharmacy which modified to be appropriate in our hospital setting : 1) Justification of drug use, 2) Critical & process indications, 3) Complications, 4) Outcome measures. Justification of use showed high appropriate rate (87.8%). Most of the critical & process indications also showed high appropriate rates (82.9 thru 97.5%) except C&S test during ciprofloxacin use (60.9%). Complications in ciprofloxacin use were usually minor symptoms such as pruritis (2.4%) and GI problems (39.0%). Also these study datas demonstrated other factors such as drug-drug interactions and co-disease could influence the complications. In case of outcome measures, the total therapeutic response showed high rate (95.1%). Only two patients (4.8%) had no response to ciprofloxacin at all. This study shows DUE analyzed for the appropriateness and problems of ciprofloxacin use. Therefore, DUE to a specific drug can apply to improve the efficacy of drug use and quality of medical service.

<P>We investigated the electronic structure of [(SrIrO3)(m),(SrTiO3)](m = 1, 2, and infinity) superlattice (SL) thin films with optical spectroscopy and first principles calculations. Our optical results confirmed the existence of the J(eff) = 1/2 states in SL samples, similar to the bulk Ruddlesden-Popper series Sr Ir-n+ 1(n) O3n+1 iridates. Apart from this similarity, in the SL samples, we observed red shifts of the characteristic optical excitations in the J(eff) = 1/2 state and an enhancement of the low-energy spectral weight, which implies a reduction in the effective electron correlation for bands near the Fermi energy. The density functional theory plus Coulomb interactions (DFT + U) calculations suggested that the SrTiO3 layer intervened between SrIrO3 layers in the SLs activated additional hopping channels between the Ir ions, thus increasing the bandwidth and reducing the effective strength of the correlations. This paper demonstrates that fabrication of iridium-based heterostructures can be used to finely tune electronic structures via alteration of their local orbital environments.</P>

Background/Aims: Plasminogen activator inhibitor-1 (PAI-1) is important for tumor growth, Invasion, and metastasis. In this study, we investigated the relationship between plasma levels of PAI-1 and colorectal adenomas. Methods: We reviewed the medical records of 3,136 subjects who underwent colonoscopy as a screening exam. The subjects were classified into a case group with adenomas (n=990) and a control group (n=2,146). Plasma PAI-1 levels were categorized into three groups based on tertile. Results: The plasma levels of PAI-1 were significantly higher in adenoma cases than in controls (p=0.023). The prevalence of colorec-tal adenomas increased significantly with increasing levels of PAI-1 (p=0.038). In the adenoma group, advanced patho-logic features, size, and number of adenomas did not differ among the three groups based on tertiles for plasma PAI-1 levels. Using multivariate analysis, we found that plasma level of PAI-1 was not associated with the risk of colorectal adenomas (p=0.675). Adjusted odds ratios for colorectal ad-enomas according to increasing plasma levels of PAI-1 were 0.980 (95% confidence interval [CI], 0.768 to 1.251) for the second-highest plasma level and 1.091 (95% CI, 0.898 to 1.326) for the highest level, compared with the lowest levels. Conclusions: These results suggest that elevated plasma PAI-1 levels are not associated with the risk of colorectal neo-plasms. (Gut Liver 2013;7:519-523)

<P>We report a temperature-dependent optical spectroscopic study of the Heisenberg-Kitaev magnet alpha-RuCl3. Our measurements reveal anomalies in the optical response near the magnetic ordering temperature. At higher temperatures, we observe a redistribution of spectral weight over a broad energy range that is associated with nearest-neighbor spin-spin correlations. This finding is consistent with highly frustrated magnetic interactions and in agreement with theoretical expectations for this class of material. The optical data also reveal significant electron-hole interaction effects, including a bound excitonic state. These results demonstrate a clear coupling between charge and spin degrees of freedom and provide insight into the properties of thermally disordered Heisenberg-Kitaev magnets.</P>

<P>The prototypical correlated metal Sr2RhO4 was studied using optical and photoemission spectroscopy. At low energies and temperatures, the optical data reveal a complex, multicomponent response that on the surface points to an unconventional metallic state in this material. Via a comparison with photoemission, the anomalous optical response may be attributed to an unexpectedly strong interband transition near 180 meV between spin-orbit coupled bands that are nearly parallel along Gamma X. This spin-orbit coupling effect is shown to occur in a number of related metallic ruthenates and explains the previously puzzling optical properties reported for these materials.</P>
입원중인 암환자에 대한 통증관리의 적절성평가 - 한 3차 의료기관 내과 전공의를 대상으로 -

목적 : 암환자에서 가장 흔히 겪게 되고, 심각한 증세는 통증이다. 진행성 암인 경우 $60{\sim}90%$의 환자들이 통증으로 고상하게 되며, WHO통계에 따르면 전세계적으로 4백만 정도의 암환자들이 통증으로 고생하고 있다. 통증관리원칙에 따르면 환자 $70{\sim}90%$이상에서 적절한 통증조절이 이루어질 수 있음에도 불구하고 이들 중 $60{\sim}70%$의 암환자들이 적절한 통증치료를 받지 못하고 있다. 본 연구는 1) 암환자의 통증 빈도 2) 환자와 담당전공의에서 통증인지의 일치 여부 3) 암환자의 통증치료에서 진통제 투약의 적절성 등을 조사함으로써, 암환자 통증에 대한 이해와 통증관리에 대한 지침을 마련하는데 기초자료를 제공하고자 하였다. 대상 및 방법 : 서울대학교병원 내과병동에 입원한 암환자들의 통증 빈도, 담당전공의와 환자의 통증인지 일치도, 통증 관리의 적절성 등을 평가하기 위해서 환자와 담당전공의를 대상으로 설문조사를 시행하고, 진료기록을 평가하였다. 결과 : 7일 이상 내과병동에 입원해 있는 암환자 59명중 43명(62.8%)이 통증을 호소하였다. 전공의들은 담당 암환자들의 통증을 실제보다 과소평가하고 있었으며, 진통제 투약 원칙이 지켜지지 않은 경우도 상당수 있었고, 환자의 통증이 심해질수록 통증에 대한 치료가 더 불충분한 경향이 있었다. 결론 : 암환자에 있어서 통증은 아직까지 낮게 평가되고 있으며 통증 치료는 여전히 불충분하다, 이와 같은 점을 고려할 때, 통증의 평가 및 치료에 대한 더 많은 교육이 필요할 것으로 생각된다. Purpose : Pain is the most serious symptom that cancer patients experience. About $60{\sim}90%$ of advanced cancer patients and four million patients worldwide, according to the WHO statistics, are reported to suffer from cancer pain. Although about $70{\sim}80%$ of the pain could be controlled according to the pain control principles, to our regret, only $30{\sim}40%$ are managed appropriately. This research was aimed to (1) investigates the prevalence of pain among cancer patients, (2) compare patients' perception of pain with physician's recognition and (3) evaluate appropriateness of the doctor's prescription of analgesic. Materials and Methods : Patients with advanced or terminal cancers admitted at department of internal medicine of Seoul National University Hospital for at least 7 days were enrolled. A questionnaire for the patients and the physicians in charge were given and the answers were compared for each other. We also examined their medical records and the physician's orders. Results : Total 59 patients were enrolled. Among them, 43 patients answered the questionnaire, and 27 patients (62.8%) suffered hem cancer pain. The survey also showed that physicians underestimated the severity of pain, overlooked frequently analgesic prescription principle, and that as the patients' pain became severe, the less adequate was pain managements. Conclusion : For cancer patients, pain was frequently overlooked, and treatment still inadequate. Based on this evidence, it seemed that more active practice and education about evaluation and management of cancer pain are needed.
A Study on Practicality of R&D Outcomes from the Korean land Spatialization Program

본 연구의 목적은 지능형국토정보기술혁신사업 연구결과물들의 실용화 목표를 위해, "실용화를 위해 고려할 요소는 무엇이며, 어떻게 R&D과제를 관리해야할 것인가?"라는 문제에 집중하였다. 이 연구 사업은 5개의 핵심주관기관과 협력적인 작업을 통해 대량의 실용적 결과물들을 적용한 지능형국토를 지향점으로 하고 있다. 이를 위한 성공적인 R&D관리는 공동실험장, 실용적인 비즈니스 모델, 그리고 통합 서비스의 형태로서 유용성과 경쟁성에 집중하여 협동적인 역할을 수행한다. 또한, 이러한 역할정립은 국토해양부에 위탁받아 진행되는 정부주도의 형태이며, 5개의 핵심주관기관과 연계하여 높은 실용화/상업화 목표 달성을 위해 R&D과제를 수행하고 있다. 이 연구결과는 GIS 기술과 공간정보의 실용화방안과 상업화방안을 지능형 국토정보기술혁신사업의 전략 분석을 통하여 보여준다. This study investigates how Korean Land Spatialization Group (KLSG) has controlled and managed its own R&D effectively and what particular elements have made it practical. Korean Land Spatialization Program (KLSP) has produced large amounts of practical outcomes for the intelligent land by collaborative 5 core research projects. The well-harmonized cooperations between proving ground, practical business model, and services oriented integration and others have driven the KLSG to an succeedable management. The active role for practicality is prominent with its usefulness and competitiveness. KLSP is financially and administratively supported from Ministry of Land Transport and Maritime Affairs of Korea (MLTM). By investigating relationships between 5 core research projects involved in the R&D projects of KLSG, it has been found out that R&D projects of KLSG bring forth practicality and commercialization. The results of this study presents strategies of KLSP in conducting research for practicality and commercialization of GIS technology and integrated geospatial information.
