      • 성인 엉덩관절을 이루는 구조에 대한 형태학적 연구

        With 77 hip joints of Korean adults available, anatomical study was carried out to clarify morphological variations of the acetabular labrum, the acetabular articular cartilage, the penetrating sites, the purchasing lengths of transacetabular screws including dangerous neurovascular structures, and the theoretical safe space of the acetabular fossa. The results were as follows; 1. The posterior superior region of the acetabulum through which the screw could be inserted more than 50 ㎜ in length was the safest region. However, depending on the insertion angle, the screw could penetrate the psoas muscle with an average 30 ㎜ of intervening space. 2. The average purchasing length of the screws penetrating the mid point (B) of the articular surface which was located on the inferior portion of the line (AB) connecting the anterior superior iliac spine and the center of the acetabulum: the mid points (C and D) of the articular surface which was located on the anterior and posterior portion of the line (CD) perpendicular to the line AB; and the mid point (G) of the articular surface which was located on the posterior inferior portion of the acetabular articular surface, was more than 20 ㎜. The screws penetrating the point "C" had a risk to damage the bladder and the screws penetrating the points "D" and "G" had a risk to endanger gluteal neurovascular bundles. 3. The screw placed in the anterior superior region of the acetabulum could be directed toward the external iliac vessels. The perpendicular bony thickness of this region averaged 13 ㎜. The topographical relationship of the external iliac artery and vein was variable and classified into 6 types. 4. At the center of the acetabulum, the most vulnerable structure was the obturator neurovascular bundle and the average thickness of bone at this point was measured as 6.5 ㎜. 5. The thickness of the compact bone became wider from the inner side to the outer side of the lunate surface. 6. The posterior inferior portion of the acetabular fossa would be a danger area when we used the screws for infernal fixation of the acetabular fracture through the acetabular fossa. 7. The morphological variations of the acetabular labrum which were measured differently at different locations should be considered during reading magnetic resonance images. 8. The anterior, anterosuperior and posterosuperior portion of the acetabular labrum might be easily torn because the labrum was attached to the bone-like "seating on a flat bony surface." 9. The shape of the articular cartilage was classified into 5 types according to the position of both ends. The above data in this study revealed new findings which should be considered at the times of diagnosis and operation of the hip joint together with morphological characteristics of Korean adult hips.

      • 폐주변부 병변 진단 시 기관지솔질에 의한 출혈이 병소확인에 주는 유용성 연구

        Background : Diagnosis of peripheral pulmonary lesion by conventional bronchoscopy is still challenging. The sheathed bronchial brush has been used to collect samples from the visible endobronchial lesion. Recently developed endobronchial ultrsonography (EBUS). can obtain image of peripheral pulmonary lesion at bronchus suspected by bleeding sign of sheathed brush. This study evaluated availibility of detected lesions by bleeding after bronchial brushing in pheripehal pulmonary lesions before transbronchial lung biopsy Methods : Between August 2006 and November 2008, 50 patients(38 men and 12 women, median age, 65 yrs) whose peripheral pulmonary lesions could not be detected with flexible bronchoscopy were included in this study. Among 50 patients, 40 cases were malignant lesion(non-small cell carcinoma 35, small cell carcinoma 5), and 10 cases were benign lesions(inflammatory disease 7, others 3) Results : Of the 50 patients, lesions detected bleeding after bronchial brushing was 34 cases(68%) and lesions detected by EBUS was 33 cases(66%). Sensitivity was 90.0-95.4%, Specificity was 83.3-80%, Positive predictive value(PPV) was 83.3-95.4% and Negative predictive value(NPV) was 80-90.9%. Diagnostic rates in visualized lesion by EBUS and otal peripheral lesions were 75%(25/33), 58%(29/50) respectively. There was no significant complication related with bronchial brushing. Conclusions : Bleeding sign by bronchial brushing was safe and effective method to predict location of peripheral pulmonary lesion with sufficient diagnostic yield

      • Fogarty Catheter를 이용한 소아 기도 이물의 제거 1례 : 증례 보고

        Foreign body aspiration is a common problem in children necessitating prompt recognition and early treatment to minimize the potentially serious and sometimes even fatal conseqeunce. We report the case of a 7-year-old boy with a button lodged in his right main bronchus. Under general anesthesia, ultrathin bronchoscopy identified the foreign body which was then removed by the combined use of a Fogarty catheter and extraction forcep. This case differs from previous reports, in that the forcep extraction through working channel of ultrathin scope was used simultaneously with Fogarty catheter even in relately small airway of children.

        김숨 소설에 나타난 공백의 숭고 연구

        In this paper, we well see Kim Soom's world of the succinct and non-representational narrative structure and the passive and quiet subject, especially the short-story collection 『Chim-dae(the Bed)』 and the novel 『No-ran gae-leul beorireo(to abandon the yellow dog)』 in connection with Alain Badiou and Slavoj Žižek의 ‘the sublime of the void(empty set, null set)’. In aesthetic categories, while the beautiful is realated to balance, order, and symmetry, ‘the sublim' mostly refers to disharmony, indeterminate forms, chaos, or exceptions. Badiou and Žižek has linked the sublime to the idea of ’the void‘ and by adding political and ethical features, emphasized the practical aspect of the sublime as an aesthetic term. We can find the similarities in Kim Soom's works which are backing away from existing narrativity and patterns of the subject, practicing the state of "void" based on passive and quiet actions like "staying" and "abandoning" through non-representative and non-deterministic writing. There are subjects of fidelity who practice non-representative "subtraction" writing and "keeping" in 『the bed』. Even though『the bed』 is a novel, it represents extremely compact situation and character types like the theatre language which centers around dialogues. As the repetitive pattern depicts identical structure and details, the possibility of abundant enunciation is obstructed and the whole narrative easily becomes impoverished. These non-representative and compact enunciation lead to the language of ‘substraction’ which Badiou once commented on the work of Beckett. In 『the bed』, the subjects exceedingly passive like old people shows the attitube of "keeping", trying to pursuing their aim in life against the invasion of others, which reminds of Badiou's argument which insists that ‘accidental encounter with others' can be the starting point of an event. To Badiou, the sublime means the string of process in which the event which deviates off the regular path is the starting point of the truth which can be found through the fidelity of the subject. Like the way 『the Bed』 can be related to Badiou’s concept of the sublime which moves from the set of void to the stage of truth. In 『To abandon the yellow dog』 is shown the similar narrative and the character types in 『the bed』, but moreover it represents the subjects which practice the writing of non-deterministic ‘addition’ and ‘discarding’ to confront the nothingness, In this paper, we will see the language of "addition" which highlights the void in real world by repeatedly narrating unknowable and indeterminable situations in 『the Bed』. The curse which represents the effect of a prophetic enunciation can be categorized as an enunciation of 'addition' which exposes the lack of the reality in which the language exceeds the reality. Furthermore, a meaningless signifier, ‘the yellow dog’ and the objectless action that abandon it, ‘to abandon the yellow dog’ expose the nothingness or the void of symbolic order by transforming the escape from the cruel reality into spontaneous renunciation. For Žižek the sublime is an action to see the actual nothingness by persisting maintaining the gap between the symbolic order and the subject. Like this way, 『to abandon the yellow dog』can be related to Žižek's concept of the sublime that the void which enters into the stage of action. By reading Kim's works through the view of the sublime, we can present new possible aesthetic approach beyond the discussion of grotesque imagery and objection to the existing representational narrative. Moreover, Kim's protagonists who has been mostly assumed as isolated people would be interpreted as active ones. There week moves against the viloent world is not merely unavoidable passive reaction to the others. On the contrary, Kim's subjects represent the will of the sublime which sticks to the aesthetic attitude to overcome the numerable objects which overwhelm and displease the subject, As they do not stop to pursuing their fidelity for the truth and step away from the confrontation with the reality, we can assume them as the subjects of the sublime who undergo the burden of the void. 본고는 김숨의 작품들 중 간결하고 비재현적인 서사구조 및 수동적이고 정적인 주체의 양상이 두드러지는 단편집 『침대』와 장편소설 『노란 개를 버리러』를 중심으로 바디우와 지젝의 ‘공백의 숭고’를 살펴보았다. 미적 범주에서 볼 때 아름다움이 조화와 질서, 균제, 정형과 연관된다면 ‘숭고(the sublim)'는 보통 부조화와 부정형, 혼돈 또는 파격을 가리킨다. 바디우와 지젝은 이러한 숭고를 ’공백‘의 개념과 연관시켰으며 여기에 정치적 ㆍ윤리적 면모를 더함으로써 미학적 용어인 숭고를 보다 실천적인 개념으로 확장시켰다. 이는 김숨의 작품들에서 기존의 소설로서의 서사성이나 주체의 양상을 지양하고 비재현적ㆍ비결정적 글쓰기를 통해 ’지키기‘나 ’버리기‘와 같은 수동적이고 정적인 행위를 하는 주체를 형상화하며 공백의 상태를 실현하려는 시도와 연관된다. 『침대』에서는 비재현적 ‘빼기’의 글쓰기와 ‘지키기’를 실천하는 충실성(fidelity)의 주체가 나타난다. 『침대』에서는 소설임에도 불구하고 무대화를 전제한 연극의 언어처럼 지극히 간결한 상황 설정 및 인물 구성 등의 서사성을 나타낸다. 그 가운데 나타나는 ‘반복’의 패턴은 동일한 내용과 구조의 서술을 되풀이함으로써 풍부한 언술의 가능성을 가로막으며 전반적인 서사를 빈곤하게 만든다. 이러한 김숨 소설의 비재현적 간결한 언술은 바디우가 베케트의 작품을 해석하며 언급한 ‘빼기(substraction)’의 언어와 연관된다. 또한 『침대』에서 노인들과 같이 지극히 수동적인 주인공이 타자의 침입에 대하여 자신의 입지를 묵묵히 지켜내려는 ‘지키기’의 태도를 보여주는데 이 또한 ‘타자와의 우연한 조우’를 사건의 출발점으로 본 바디우의 논의와 연관될 수 있다. 바디우에게 숭고는 현실의 법칙을 벗어나는 사건이 진리의 출발점이 되고 이에 대한 주체의 충실성을 통해 진리가 가능해지는 일련의 과정을 의미한다. 『침대』는 이렇듯 공백에서 진리로 나아가는 바디우의 숭고와 연관시킬 수 있다. 『노란 개를 버리러』에서도 『침대』에서와 유사한 서사와 인물 구성이 나타나지만 여기에서는 비결정적 ‘더하기’의 글쓰기와 ‘버리기’를 실천하며 무(無)와 대면하려는 주체가 나타난다. 여기에서는 『침대』에서의 ‘빼기’의 언어가 아니라 불확정적이고 불가지적인 상황을 끊임없이 서술함으로써 현실의 공백을 오히려 두드러지게 하는 공백의 언어인 ‘더하기’의 언어가 나타난다. 예언적 언술로서의 효과를 드러내는 저주 또한 언어가 현실을 압도하는 현실의 결핍을 드러낸다는 점에서 ‘더하기’의 언술에 속하게 된다. 또한 의미가 텅 빈 기표로 이해되는 ‘노란 개’와 이를 버리는 무목적적 행위인 ‘노란 개를 버리러’는 가혹한 현실로부터의 도피를 자발적 이탈로 탈바꿈시킴으로써 상징 질서의 공백, ‘무(無)’를 대면케 한다. 지젝에게 숭고는 상징 질서와 주체 사이의 간극을 메우려 하지 않고 끝까지 고수함으로써 실재의 무(無)를 목도하는 행위라 할 수 있다. 『노란 개를 버리러』는 이렇듯 공백에서 행위로 나아가는 지젝의 숭고와 연관시킬 수 있다. 숭고를 통해 김숨의 소설을 읽어냄으로써 기존에 그로테스크한 이미지, 기존의 재현 법칙을 벗어난 서사 등 다소 산발적이었던 논의들에 대해 새로운 미학적 가능성을 제시할 수 있을 것이다. 또한 주로 소외된 자들로서 인식되어 온 김숨 소설의 주인공들을 새로운 측면에서 능동적으로 해석할 수도 있을 것이다. 폭력적 세계에 대한 그들의 미약한 몸짓은 단지 타자의 불가피한 수동성이라기보다는, 형언할 수 없는 압도적이거나 불쾌한 대상을 심미적으로 극복하려 하는 숭고에서의 의지와 맞물리는 것으로 볼 수 있다. 이들은 진리에 대한 충실성을 멈추지 않으며 실재와의 대면을 회피하지 않는다는 점에서 공백을 감당하는 숭고의 주체로 볼 수 있다.

        초등학교 무용감상 프로그램 개발 연구

        The purpose of this study was to establish a model of Dance Appreciation Program for improvement of dance education in elementary schools. In order to achieve the aforementioned purpose, this study analysed contents of dance appreciation in the current 7th physical Education curriculum and developed Dance Appreciation Program. This program was suggested in purposes, contents, methods and evaluations. The result of this study are as follows: First, the purpose of this program was suggested in artistic, aesthetic, cultural aspects for intent values as an art education. Second, the content of this program was suggested with 3rd steps. Those were the understanding of dance aspects and forms, explanations of meaning, evaluations of the value of dance. Third, the method of this program was suggested in performing tasks, creating tasks, historical, culcural tasks and cross curriculum links. Forth, the evaluation of this program was suggested in artistic, aesthetic, cultural aspects, too.

        방과후 교육을 위한 통합형 문화예술교육에서의 무용 교육과정 개발

        The purpose of this study was to examine the role and possibility of Dance in integrated cultural-artistic education Model for After-school programs and to propose the dance Cumculums. In order to this, the concepts, functions, the goals and the system of contents in Integrated cultural-artistic education were investigated. The Dance Program(20hours) were proposed according to the principles and procedures of Integrated cultural-artistic education model. The Dance Programs were proposed in three spheres, Experience curriculums, Comprehension curriculums and Communication curriculums. Each curriculums were composed with activities of Dance, Music, Art, Animation, Drama, Korean Music, Media, Reading. The Experience curriculum was composed with contents for senses and languages based on Arts. The Comprehension curriculum was composed with contents for understanding for common spheres and each sphere. Commiinication curriculum was composed with contents for planning, creation and presentation of Arts.

        16세기 사화기에 있어서 호남학문의 형성과 전개양상

        Under the assumption that 'Honam Saryu' has a distinctive nature, which differs from that of Yeongnam Sarim(groups of Confucian scholars) or Kiho Sarim in their formation and development, this study aims to demonstrate validity of the independence of Honam Sarim from Kiho Sarim. First, noticing that the academic vein led by Kim Gweng-Pil and Jo Gwang-Jo played an important role in formation of the Sarim(groups of Confucian scholars), this study examines their academic characteristics and academic relations between Youngnam Sarim and Honam Sarim. Kim Gweng-Pil inherited moral philosophy to the Honam Saryu in Suncheon, his exiled place. Choi San-du and Yu Gye-rin who had been educated by Kim Gweng Pil educated Kim In-who and Yu Hee-chun, main members of Honam Sarim, and then Ki Dae-Seung followed them. Though the scholars of Honam Sarim were suffered in the Sahwa(massacre of scholars) period, they actively participated in the reformative politics led by Jo Gwang-Jo, which resulted in their exile or retirement. However, they participated in enlightenment of country people actively, proposing realization of principles presented in Sohak. And they made a great contribution to improvement in the Confucianism through hard study of the Confucian theories. Therefore, this study suggests their practice of village education and serious consideration of Sohak as characteristics of Honam study.

      • 미세알부민뇨를 동반한 인슐린 비의존성 당뇨병 환자에서 혈장 Endothelin의 변화

        목적 : 혈관내피세포에서 생성되는 endothelin(ET)는 강력한 혈관수축작용을 가진 펩타이드로 당뇨병 환자에서 혈관내피세포의 손상여부에 따라 혈중 농도가 증가할 수 있으며 혈관합병증과 관련이 있다는 보고가 있다. 그러나. 혈중 ET치의 상승과 당뇨병성 합병증의 상관성에 대해서는 논란이 많다. 이에 저자들은 인슐린 비의존성 당뇨병 환자에서 미세알부민뇨의 동반유무에 따른 혈중 ET치의 변화 및 혈장 ET치와 요중 알부민 배설률과의 상관성을 검토하였다. 대상 및 방법 : 인슐린 비의존성 당뇨병 환자중 미세알부민뇨를 보인 22명과 정상알부민뇨를 보인 43명을 대상으로 하였으며 혈장 ET를 측정하여 요중 알부민 배설률, 당뇨병성 망막증 및 당뇨병 관련인자들과의 관계를 비교하였다. 결과 : 당뇨병성 망막증이 합병된 군에서 망막증이 합병되지 않은 군에 비하여 혈장 ET치가 유의하게 높았던 반면 (각각 1.9±0.66, 1.5±0.76pg/㎖, p<0.05) 미세알부민뇨를 보인 군과 정상알부민뇨를 보인 군사이에 혈장 ET치는 통계학적으로 유의한 차이가 없었다(각각 1.9±1.14, 1.7±0.69pg/㎖). 또한 요중 알부민 배설률과 혈장 ET치 사이에 유의한 상관관계는 없었으며 미세알부민뇨를 동반한 인슐린 비의존성 당뇨병 환자군에서 혈장 ET치는 당뇨병의 관련인자들과도 유의한 상관관계가 없었다. 결론 : 인슐린 비의존 당뇨병 환자에서 혈장 ET치가 초기 당뇨병성 신증의 합병여부나 미세알부민뇨의 정도를 반영하기는 어려울 것임을 시사한다. Background : Endothelin(ET), a potent vasoconstritor peptide released from vascular endothelial cells, is known to be increased in diabetic patients with vascular damage and related with diabetic angiopathy. However, whether plasma ET levels may be related to diabetic complications is controversial. We studied the change of ET levels in non-insulin dependent diabetics with microalbuminuria and relation between ET and urinary albumin excretion rate. Methods : Plasma ET levels of 22 non-insulin dependent diabetics with microalbuminuria and 43 patients with normoalbuminuria were measured and relations of plasma ET to diabetic complications such as urinary albumin excretion rate, retinopathy and other associated factors were studied. Results : Plasma ET levels were significantly higher in patients with diabetic retinopathy than those without retinopathy(1.9 ± 0.66 vs 1.5 ± 0.76 pg/㎖. p<0.05). The difference, however, between diabetics with and without microalbuminuria was not significant(1.9 ± 1.14 vs 1.7 ± 0.69 pg/㎖). There were no correlations of plasma ET to urinary albumin excretion rate and to the other associated factors in non-insulin dependent diabetics. Conclusion : These results suggest that plasma ET levels may not seem to act as a predictor of diabetic nephropathy or microalbuminuria in non-insulin dependent diabetics.

        창작무용 학습이 초등학교 아동의 자아개념에 미치는 영향

        The above-mentioned findings suggested that creative dance had a favorable impact on the self-concept of the elementary schoolers. If systematic creative-dance education programs are developed and scientific, long-term research efforts are dedicated to this field, creative- dance education will take root as major part of dance education to help establish children's self-concept and facilitate their well-rounded growth.

        日本 初中高 학교의 漢字·漢文 敎育의 특징에 관하여

        일본의 국어생활은 원칙적으로는 '和漢混用'이다. 소고는 일본의 현행 초·중·고의 국어교육에서 한자·한문이 어떻게 교육되고 있는가를 살펴보고,우리 교육과의 상대적인 면을 생각해본다. 2002년도부터 실시된 현행의 학습지도요령에서 제시하고 있는 초 · 중등학교의 한자 · 한문교육은 1006자의 학습용 한자와 그것을 포함한 1945자의 상응한자의 학습을 중심으로 구성되어 있다. 적어도 고등학교를 마치게 되면1945자의 상용한자는 자유롭게 읽고 사용할 수 있어야 한다는 것이 한자 · 한문교육의 최종목표가 된다. 여기에 생활의 필요에서 익히세 되는 인명용 한자284자를 포함하면 2229자를 일본어 생활의 기본 베이스로 삼게된다. 일본 초등학교의 한자교육은 국어의 어휘력 신장에 맞추어서 구성되어있고, 언어의 원활한 의사소통의 목적에 기여할 것을 의도하고 있다. 1006자의한자를 학년별 한자배당표로 학년별로 배당하여 놓고 있다. 중학교의 한자교육은 초등학교 과정처럼 독립되어 있지 않고, 국어교과서안에서 학습하게 되어있다. 중학교과정에서는 한자어 어휘의 확대에 최대의 목표를 두면서도 거기에 필요한 한자어의 이론 및 일본·중국 고전과의 만남을 준비하고 있다. 고등학교의 한문교육은 역시 독립교과로 운영되지 않고 국어과의 한 과목으로 되어있다. 고전 교육의 특징은 음독, 낭독, 암송이 있다는 것이다. 고문과 한문의 분위기(리듬)을 익히기 위함이다. 그리고 또 한가지는 한문 교육을 통하여 중국 등 외국 문화와의 관계에 대해 이해의 깊이를 더해 가는데 도움이 되게 하려는 것이다. 그런데 실제로 교과서에 인용된 내용은 중국한문이 대부분이고 일본 한문은 극히 적은 양이 인용된다. 일본의 고등학교 한문교육은 대학입시와 밀접한 관계를 갖고 실시된다. 이유는 대학입시국어과 시험의 25%를 차지하는 위치에 있다. 실제 대학입시를 준비하는 고등학교 한문학습은 한문의 중요어휘의 암기와 한문의 基本句形의 학습에 힘을 기울이고 있다. 이러한 일본 고등학교의 한문학습은 입시준비를 위한 수업형태라고 말하여도 과언이 아니다. 실제로 수업시간에는 교과서가 아닌 입시 문제집을 주요 교재로 사용한다는 것에서도 여실하게 나타난다. 이상에서 일본의 한자 · 한문교육을 살펴보았다. 우리가 일본의 한문교육에서 참조할 것은 한자 · 한문의 기초교육을 통하여 질적으로 높은 국어생활을 영위하고, 전통유산으로 남겨준 고전의 세계에 친숙할 수 있는 계기를 마련하는 점이다. 한글과 한문교육은 한쪽의 쇠퇴가 한쪽의 영광이 될 수 없음이 자명한 사실이다. 서로의 도움으로 국어생활은 보다 내실감있게 꾸려가야 한다고 생각된다. Japanese language adheres to the principle that Chinese characters are adjusted to harmony with Japanese. This study examined how Chinese characters and writing are educated in Japanese language in the current Japanese elementary, middle and high schools to identify difference between Japanese education and Korean. The Chinese characters and writing education in elementary, middle and high schools suggested by the current teaching and learning guide since 2002 requires learning 1006 Chinese characters and 1945 Chinese characters for common use. The final goal of the education is that students are able to freely read and use 1945 Chinese characters for common use, if students complete at least the courses in high schools. And if students learn additional 284 characters that are used for names, they are well prepared for Japanese language learning. Chinese characters education in Japanese elementary schools is constructed in accordance with the expansion of Japanese vocabulary, and intends to contribute to achievement of the purpose of fluent verbal communication. 1006 Chinese characters are presented grade by grade in the Chinese character table. As Chinese characters education in middle schools is not independently seperated as in elementary schools, students learn Chinese characters through Japanese education. The primary goal of middle school curriculum is the expansion of the Chinese vocabulary, and to achieve the goal, students are to learn the theories of Chinese characters, Japanese and Chinese classics. High school Chinese writing education is also not independently separated and belongs to one division of Japanese language courses. The classics education consists of reading Chinese characters phonetically, reading aloud and recitation. The courses intends to help students to be familar with the mood and rhythm of Chineses classics and writing. And Chinese writing education intends to help students understand relations with foreign cultures including Chinese culture. However, most of the contents in textbooks are composed of original Chinese characters but, little sino-Japanese characters is found. Chinese characters learning in Japanese high schools has a close relation with university entrance. It is because it occupies 25% of the Japanese language examination. High school students who are preparing for university entrance examination focus on learning major vocabulary and basic compositions of Chinese writing. It can be said that such learning is designed to be prepared only for entrance examination. In actual lessons, learing is given through exercise books for entrance examination, not through textbooks. This study examined Chinese characters and writing education in Japan as above. Through the results, it was understood that the basic education of Chinese characters promotes the improvement of Korean language, and provides opportunities to be familar with the inherited classics. Korean characters and Chinese characters will coexist and help each other in various ways. It was thought that through the mutual help, Korean language should be improved.
