        정신과 입원환자에서 Olanzapine의 처방경향에 대한 연구 : 한국과 미국의 일 대학병원에서 사용비교

        Objective : Recently, domestic use of olanzapine and the studies for olanzapine have been increased. However, naturalistic studies for the practical use of olanzapine are rare. Therefore, this study was designed to evaluate the prescription trend for olanzapine and its related pharmacological variables at a university hospital in Korea, and to compare with those of university hospital in USA, simultaneously. Methods : Data of 33 Psychiatric inpatients who had been firstly treated with olanzapine at st. Mary's Hospital from October 1999 to March 2000 and of 109 at McLean Hospital, Hanard Medical School, USA from March 1998 to June 1998 were collected, respectively. Data on patient's age, sex, number of past admission, diagnosis distribution, duration of hospitalization, antipsychotic combination, combined psychotropics, combined mood stabilizer, and dosage of olanzapine were analyzed. Results : The mean age and number of past admission were significantly higher at McLean hospital than those of St. Mary's hospital. In terms of diagnosis, olanzapine was most widely prescribed to psychotic disorder, nextly to mood disorder at St. Mary's hospital, but in order of mood disorder, psychotic disorder, and other psychiatric disorder at McLean hospital and, these diagnostic distribution were significantly different. The mean initial dose was similar between two hospitals, but maximal and discharge dose of olanzapine were significantly higher at St. Mary's hospital than those of McLean hospital. In aspects of psychotropic combination , anxiolytic was highly used with statistical significance at St. Mary's hospital but antidepressant and mood stablizer were highly used at McLean hospital. Conclusion : Prescription trend of olanzapine for psychiatric inpatient between two hospital was different, olanzapine was prescribed mainly by diagnosis at St. Mary's hospital but by symptomatic management at McLean hospital. In future, further systematic study should be conducted including various clinical variables to refine these differences.

        주요 우울장애에서 Cytotoxic T Lynphocyte Antigen(CTLA-4) 유전자의 다형성

        연구목적 : 세포의 면역 기능 조절에 관여하는 CTLA-4의 유전자 다형성을 분석하여 주요 우울장애와의 면역 유전학적 관련성을 알아보고자 하였다. 방 법 : DSM-Ⅳ진단 기준에 따라 주요 우울장애로 진단된 환자 77명을 선정하였으며 가톨릭대학교 의과대학 조혈모세포은행에서 보유하고 있는 149명의 정상 한국인 자료를 대조군으로 사용하였다. 전혈에서 DNA를 추출하였으며 중합효소반응으로 CTLA-4유전자 exon 1 부위를 증폭한 후 SSCP방법으로 유전?? 형별 분석하였다. 결 과 : 대조군과 주요 우울장애 환자군 사이의 CTLA-4 유전자형과 대립유전자의 발현 빈도를 비교시 유의한 차이가 없었다. 결 론 : 본 연구 결과 exon 1 CTLA-4 유전자형 및 대립유전자의 발현빈도는 주요 우울장애 환자군과 정상 대조군 사이에 차이가 없어, exon 1 영역은 후보 유전자로서의 가능성이 없었다. 향후 연구에서는 다양한 변인들에 따라 대상군을 임상적으로 동질성을 지닌 집단으로 세분하여 조사대상 유전자와의 관련성에 대한 연구가 이루어져야 하겠다. Objectives : This study was carried out to explore the relationship between major depressive disorder and CTLA-4 which is related to the immunologic function such as T cell regulation. Methods : Among the korean patients diagnosed as major depressive disorder according to DSM-Ⅳ, 77 patients without neurological illness, hormonal disorder, or comorbid mental illness were selected. The stored data of 149 normal Koreans from the Catholic Hemopoietic Stem Cell Bank of Korea, were used as a normal control group. The data of Korean control group were compared with those of the studies of different ethnics. DNA was extracted from whole blood and the exon I region of CTLA-4 gene was amplified by polymerase chain reaction. Gene typing was performed by using SSCP and then, the results were assessed. Results : There were no significant differences in genotype frquencies of CTLA-4*G/G, CTLA-4*G/A, and CTLA-4*A/A between the patients with major depressive disorder and the control group in Korean population(48.1% vs 46.3%, 41.6% vs 39.6%, 10.3% vs 14.1%, respectively).There were no significant differences in allelic frequencies of CTLA-4*G and CTLA-4*A between the patients with major depressive disorder and the control group in Korean population(68.8% vs 66.1%, 31.2% vs 33.9%, respectively). Conclusion : Considering negative result for the association of the exon I polymorphism of CTLA-4 gene with major depressive disorder in this study, the exon I polymorphism does not appear to be possible candidate gene for major depressive disorder. Moreover, further systematic researches including diverse clinical variables would required.

      • Anti-inflammatory and Anti-oxidant Effects of Ledebouriella seseloides produced by Different Cultivation Method

        Ledebouriella seseloides (LS) is a medicinal plant with high functional activity in anti-inflammatory and anti-stroke effects. Recently, LS has been cultivated widely in plant factory with artificial light (LS-A). We evaluated the effects of LS-A and LS cultivated in the open field (LS-F) on anti-oxidative and anti-inflammatory activities. In DPPH and ABTS radical scavenging tests and total polyphenol analysis, LS-A and LS-F extracts showed similar antioxidant activity. They significantly reduced lipopolysaccaride (LPS, 1 μg/mL)-induced nitric oxide (NO) concentration in a dose-dependent manner without cytotoxicity in RAW264.7 cells cultured with the extracts. In the western blot analysis, they decreased the expression of iNOS, COX-2, and NF-κB proteins in the RAW.264.7 cells. The results suggests that LS-A has anti-oxidative activity and anti-inflammatory effects like as LS-F extracts. LS-A may be used as a potential therapeutic candidate for the prevention and treatment of macrophage-derived chronic immune diseases.

      • 은 이온과 아연 이온으로 치촨한 제올라이트 A (Ag_(2.8)Zn_(4.6)-A)의 탈수한 결정구조와 이것에 에틸렌을 흡착시킨 결정구조

        탈수한 Ag_(2.8)Zn_(4.6)-A의 구조와 이 결정에 에틸렌 기체가 흡착된 구조를 X-선 단결정 회전법으로 입방공간군인 Pm3m을 사용하여 구조를 해석하고 정밀화 시켰다. Ag+ 이온 및 Zn^(2+)이온으로 이온교환시킨 두개의 결정을 400℃, 2 ×10^(-6) Torr의 진공하에서 2일간 탈수시킨 후, 그 중 하나의 결정에는 25(1)℃에서 250 Torr의 에틸렌 기체를 1시간 동안 처리하였다. 탈수한 Ag_(2.8)Zn_(4.6)A(a=12.137(2)Å)의 결정구조는 I>3σ(l)인 237개의 회절점을 사용하여 R_(u) 값이 0.044까지 정밀화시켰고, 에틸렌을 흡착시킨 Ag_(2.8)Zn_(4.6)A 5.6C_(2)ll_(4) (a=12.106(2)Å)의 결정구조에서는 301개의 회절점을 사용하여 R_(u) 값이 0.050까지 정밀화시켰다. 2.8개의 Ag^(+)이온은 에틸렌 분자와 п착물을 형성하며, 6-링 평면에서 큰 동공쪽으로 0.922(2)Å 이동한 위치에 골조의 산소와 2.240(5)Å에서 결합하고 있었고, 에틸렌 분자의 탄소원자와 2.290(5)Å에서 결합하고 있었다. Zn^(2+)이온은 단위세포당 두 개의 서로 다른 3회 회전축상에 위치하고 있으며, 이 중 2.8개의 Zn^(2+) 이온은 에틸렌분자와 п 착물을 형성하며, (111) 평면에서 큰 동공쪽으로 0.408(2)Å 이동한 위치에 골조의 산소와 결합하고 있었다. Zn2+ 이온과 에틸렌 분자의 탄소원자간 결합거리는 2.78(4)Å 이며, 이는 비교적 약한 결합임을 나타낸다. Dehydration of two crystals studied were archieved at 400℃ and 2×10^(6) Torr for 2 days and one ctystal was treated with 250 Torr of ethylene at 25(1)℃. The structures of dehydrated Ag_(2.8)Zn_(4.6)-A (a= 12.137(2) A)and of its ethylene sorption complex (a=12.106(2) A)were refined to final error indices, R(weighted)=0.044 with 237 reflections and R(weighted)=0.050 with 301 reflections, respectively, for which I>3σ(I). 2.8 Ag^(+) ions are recessed 0.922(2) A from (111) plane of three 6-ring oxygens into the large cavity shere each forms a lateral πcomplex with an ethylene molecule. These Ag^(+) ions are in 2.240(5) A from three framework oxide ions and 2.290(5) A from each carbon atom of an ethlene molecule. The Zn^(2+) ions occupy two different threefole axis positions of the unit cell. 2.8 Zn^(2+) iions are recessed 0.408(2) A from (111) plane of the 6-ring oxygens and each Zn^(2+) ion forms a π complex with an C2H4 molecule. The distances between Zn^(2+) ions and carbon atom of ethlene molecule, Zn(2)-C=2.78(4) A are long. This indicates that this bond is relatively weak.

        Two Crystal Structures of Dehydrated Sr$^{2+}$ and Tl$^+$ Exchanged Zeolite A, $Sr_xTl_{12-2x}$-A (x=1.6 and 5.45)

        Two crystal structures of dehydrated $Sr^{2+}\;and\;Tl^+$ exchanged zeolite A, $Sr_xTl_{12-2x}-A$ (x = 1.6 and 5.45), have been determined by single-crystal X-ray diffraction techniques. Their structures were solved and refined in the cubic space group Pm3m at $21(1)^{\circ}C.$ Both crystals were ion exchanged in flowing streams of mixed $Sr(NO_3)_2\;and\;TlNO_3$ aqueous solution, followed by dehydration at $360^{\circ}C\; and\; 2${\times}$10^{-6}$ Torr for 2 days. Full-matrix least-squares refinements of the dehydrated $Sr_{1.6}Tl_{8.8}-A (a = 12.214(2){\AA})\; and\;Sr_{5.45}Tl{1.1}-A (a=12.291(2){\AA})$ have converged to final error indices, $R_1=0.055\; and\;R_2=0.061$ with 286 reflections, and R1 = 0.072 and R2 = 0.090 with 217 reflections, respectively, for which$\;I\;{>}\;3{\sigma}(I)$. In both structures, all Sr(II) ions are coordinated by three framework oxygens; Sr(II) to O(3) distances are $2.21(2){\AA}\;for\;Sr_{1.6}Tl_{8.8}-A \;and\;2.31(1){\AA} \;for\;Sr_{5.45}Tl_{1.1}-A,$and Tl(I) to O(3) distances are $2.657(6){\AA}\;for\;Sr_{1.6}Tl_{8.8}-A\;and\;2.845(8){\AA}\;for\;Sr_{5.45}Tl_{1.1}-A,$ respectively. In each structure, the angle subtended at Sr(II), O(3)-Sr(II)-O(3) is $118.7(4)^{\circ}\;for\; Sr_{1.6}Tl_{8.8}-A \;and\;120.0(4)^{\circ}\;for\;Sr_{5.45}Tl_{1.1}-A.\;Sr^{2+}$ ions prefer to 6-ring sites and $Tl^+$ ions to 8-ring sites when total number of ions per unit cell is more than 8.
