      • Poster Session : PS 0831 ; Upper GI Tract : Clinical Factors to Predict Angiographically Detectable Non-Variceal Upper Gastrointestinal Bleeding in Patients Refractory to Endoscopic Treatment

        Background: Non-variceal upper GI bleeding (NVUGIB) is a common medical problem that has signifi cant association with morbidity and mortality. Angiographic detection and subsequent transarterial embolization (TAE) is a primary treatment option when medical and endoscopic treatments fail. We investigated clinical factors that could affect the success of the angiographic detection and prognosis after TAE in patients with NVUGIB refractory to endoscopic therapy. Methods: A retrospective analysis of the clinical data was done in patients with failed endoscopic treatment who underwent angiography for the treatment of acute NVUGIB between May 2002 and May 2013. Patients were divided into detection or non-detection groups according to the presence of bleeding stigmata in angiographic fi nding. Rebleeding defi ned as subsequent bleeding event within 7 days and mortality within 30days were analyzed as outcome parameters after TAE following detection in angiography. Results: A total 45 patients 37 (male, mean age, 65.9±14.9 years) were analyzed and classifi ed as a detection group (n=25, 55.5%) and non-detection group (n=20, 44.6%). Peptic ulcers were the most common cause of refractory NVUGIB. Larger transfusion amount (5.7±3.9 unit vs. 3.5±2.8 unit; P=0.03), prolonged aPTT level (34.2±17.3 sec vs. 21.8±13.8 sec; P=0.01) and short time interval between last endoscopy and angiography (17.5±25.9 hours vs. 34.3±59.5 hours; P=0.04) were found to be signifi cant factors for predicting angiographic detection. TAE was performed in all patients detected in angiography. Rebleeding (44%) was significantly associated with higher Rockall score (8.3±1.5 vs. 6.6±2.4; P=0.046) and mortality (12%) was signifi cantly associated with higher Rockall score (9.3±0.6 vs. 7.1±2.2; P=0.002) and higher level of BUN (55.3±47.4 vs. 27.6±17.4; P=0.01). Conclusions: Clinical characteristics associated with angiographic detection in patients with NVUGIB refractory to endoscopic therapy were severe bleeding, bleeding tendency and early angiographic intervention. The Rockall score is useful parameter for predicting rebleeding and mortality after TAE.

        The purpose of this study was to analyze the educated teachers' satisfaction of the in-service education programs of industrial technical and technology teachers in Chungnam National University. This study was carried out through survey to accomplish the purpose. The number of questionnaires collected was 86. This study used the SPSS program to conduct a descriptive analysis of the data. The results of this study were as follows: (1) The satisfaction about 'teachers' participation', 'utilization of audiovisual materials and instruments' and 'level of professor and lecture' was relatively high. (2) The satisfaction about 'environments of in-service education programs', 'evaluation of achievement', and 'efficacy of lectures' was relatively low.

      • 적층압축에 의한 접합재의 경계면 성장거동에 대한 수치 시뮬레이션

        Under plane strain compression, of two blocks with sawtoothed interface, the sawtooth growth occurred. We presumed in the previous paper that such growth was caused by the shear-band formation starting from the tip of sawtooth and its degree depended on the material properties of the block. In this paper, Compression in the case where similar blocks are twinned and also compression in the case where dissimilar blocks are twinned variously are carried out in the condition of lubricated interface. Further, in the latter, compression where the interface is degreased is also performed. The degree of growth is experimentally investigated. Moreover, numerical simulations are carried out by the elastic-plastic FEM for the case of the dissimilar blocks with the initial sawtooth angle of 60℃ . In the experiment, the harder material yields larger growth than the softer one, and the growth in the case of initial sawtooth angle 60℃ is greater than that of 90℃ . Larger ratio of total height to sawtooth height gives larger growth in comparison with the previous experimental result. In the case where the dissimilar blocks are twinned, larger difference between material properties leads smaller growth, and the degreased interface leads smaller growth than that in the lubricated one. Furthermore, by the simulation of compression where dissimilar blocks are twinned, it is confirmed that the tendency of the general deformation pattern is very similar to the experiment.

        Rhabdomyolysis may be defined as a clinical and laboratory syndrome which releases muscle cell contents into the plasma, such as creatine phosphokinase, due to skeletal muscle injuries. It can be the result of many diverse causes including muscle disease, ischemia, genetic disorder, metabolic disorder, infectious disease, drugs, comatose state, trauma or vigorous exercise that can lead to acute renal failure as a complication. Since first reported in 1941, rhabdomyolysis-induced acute renal failure has been documented many times, and the major causes of this disease are carbon monoxide poisoning, snake bites, and etc in Korea Reports of acute renal failure after exercise-related rhabdomyolysis with myoglobinemia are rare. We report a case of acute renal failure caused by exercise-related rhabdomyolysis in a 20 year old man.

        수증기 활성화법으로 제조된 영월 석탄계 활성탄의 특성 연구

        Activated carbons were prepared from Youngwall coal by steam activation in this study. The feasibility of the Youngwall coal to commercial activated carbon was examined. The variation of pore structures and the development of porosity in activated carbons were investigated by changing activation conditions in batch type apparatus. The values of BET surface area and adsorption capacity of iodine and methylene blue of the resulting activated carbons were obtained as high as 1,000㎡/g, 900㎎/g, 150㎖/g, respectively. Youngwall activated carbon prepared in this study showed much higher pore volume in pore diameter over l0Å than that of commercial reference activated carbon(Ningxia Taihua ZJ-15C) produced from China anthracite.

        The Report on the Medical Voluntary Activities for Cleft Lip and Palate in Vietnam by the Korean Association of Maxillofacial Plastic and Reconstructive Surgeons

        대한악안면성형재건외과학회는 2004년도부터 매년 인도적 차원에서 의료 환경이 낙후된 Vietnam의 cleft lip and palate 환자들을 치료하기 위해 무료로 해외 진료 봉사 활동을 해왔다. 초창기 해외 진료봉사 활동 당시 베트남 의료 기술 발달이 미비하여 준비해야 할 것들이 많았으나 이 후 베트남의 경제발전과 함께 의료 시설 및 의료 기술이 발달함에 따라 준비 품목 등이 간단명료해지고 현지 지원 및 수술 준비도 원활히 진행되었다. 이러한 무료 의료 봉사 활동을 계기로 개발도상국의 낙후된 의료 시설 및 의료 기술 발전, 의료 혜택을 받지 못한 구순 구개열 환자의 정상적 안모 및 기능 회복, 조건 없는 의료 봉사 활동을 통한 민간외교 효과 및 양국간의 우호 증진 등의 성과가 있었다. 대한악안면성형재건외과학회에서는 2009년도 역시 베트남 National Institute of Odonto-stomatology 에서 11월17일부터 26일까지 cleft lip and palate 무료수술진료단(단장 신효근)을 보내 수술봉사 활동을 펼치고 돌아 왔다. 일반적인 구순구개열 뿐만 아니라 양측성 안면열(Bilateral Facial Cleft), 사경(Torticollis) 등의 희귀 기형을 포함하여 수술을 진행하였으며 총 31례의 수술을 성공적으로 마쳤으며 지난 6년 동안의 성과와 함께 이에 대한 보고를 하고자 한다.

      • Prominin-like regulates longevity through insulin signaling in Drosophila

        CD133, also called Prominin-1, is a penta-span transmembrane protein which is known as a biomarker for human hematopoietic stem cells and mouse neuroepithelial cells. Recent studies show that CD133 is also involved in cell growth, development and tumor biology. However, the function of CD133 at the organism level is not yet investigated. Here, we found that prominin-like, a Drosophila homolog of CD133, regulates longevity and metabolism by controlling insulin signaling. prominin-like mutant flies showed extended life span and metabolic defects such as increased glucose, triglycerides levels and starvation resistance. The mRNA expression level of Drosophila insulin-like peptides (Dilps) were reduced in prominin-like mutants. Consistently, the phosphorylated AKT level, the downstream component of insulin signaling, was also decreased in prominin-like mutants. Importantly, prominin-like protein is predominantly expressed in the pars intercerebralis region with insulin producing cells of the adult brain. These results indicate that the prominin-like gene regulates longevity and glucose metabolism by controlling insulin signaling in Drosophila.

        I-123 IPT SPECT를 이용한 주의력결핍 과잉행동장애 아동에서의 methylphenidate 투여 전후의 기저 신경절 도파민 운반체 밀도 변화 측정

        목적 : 주의력결핍 과잉행동장애 (Attention Deficit Hyperactivity disorder : 이하 ADHD)는 도파민계의 이상 기능으로 생기는 대표적 소아정신과 질환이다. Methylphenidate는 dopamine transporter(DAT)를 차단함으로써 ADHD 증상을 호전시키는 약물로 널리 알려져 있다. 따라서 본 연구에서는 ADHD 아동들을 대상으로 I-123 IPT SPECT를 이용하여 methylphenidate 투여 전후의 DAT density 양상을 비교해 보고자 한다. 대상 및 방법 : 연구대상은 9명의 ADHD 아동과 7명의 정상 대조군이었다. ADHD 아동군과 정상 대조군에 모두 약물 비노출 상태에서 [^(123)I]IPT를 정맥 주사후 2시간이 경과한 상태에서 SPECT를 촬영하였다. 이후 ADHD 아동군을 대상으로 methylphenidate 0.7mg/kg/d을 투여한 후 약 8주가 경과한 상태에서 [^(123)I]IPT SPECT를 재활영 하였다. Methylphenidate 투여 전화 후의 좌,우측 기저 신경절 DAT 특이결합/비특이결합 비율을 구한 뒤 비교하였고, 약물 투여 후의 ADHD 증상 척도로 측정한 증상 호전도와 좌,우측 기저 신경절 DAT 특이결합/비특이결합 비율 변화사이의 상관관계를 알아보았다. 결과 : ADHD 아동군에서 약물 비노출 상태와 정상 대조군 사이의 좌, 우측 기저 신경절 DAT 특이결합/비특이결합 비율을 비교한 결과 DAT 결합 비율이 정상 대조군에 비해서 유의하게 증가되었다(Right : z=2.057, p=0.041 ; Left : z=2.096, p=0.032). 또한 ADHD 아동들에세 methylphenidate 투여 전과 후 상태에서의 기저 신경절 DAT 특이결합/비특이 결합 비율을 비교한 결과, methylphenidate 투여 후 상태가 methylphenidate 투여 전 상태에 비해 좌, 우측 DAT 결합비율이 유의하게 낮아진 것을 관찰 할 수 있었다 (우측 : t=3.239, p=0.018 : 좌측 : t=3.133, p=0.020). ADHD 증상의 호전도와 좌,우측 기저 신경절 DAT 특이결합.비특이결합 비율 변화사이에는 유의한 상관관계를 보이지 않았다. 결론 : 이러한 결과는 주의력결핍 과잉행동장애의 치료제인 methylphenidate가 작용하는 기전을 이용하여 주의력결핍 과잉행동장애의 병태생리와 연관된 도파민계 기능이상 가능성을 지지한다고 생각된다. Purpose : attention deficit hyperactivity disorder (ADHD) has been known as psychiatrio disorder in childhood associated with dopamine dysregulation. In present study, we investigated changes in dopamine transporter (DAT) density of the basal ganglias using [-123 N-(3-iodoproper-2-yl]-2-carbomethoxy-3beta-(4-chlorophenyl) tropane [l-123 IPT] SPECT in children with ADGD before and after methylphenidate treatment. Materials and Method : Nine drug-naive children with ADGD and seven normal children were included in the study. We performed brain SPECT two hours after the intravenous administration of l-123 IPT and made both quantitative and qualitative analyses using the obtained SPECT data, which were reconstructed for assessment of specific/nonspecific DAT binding ratios in the basal ganglia. All children with ADGD reperformed [123]IPT SPECT after treatment with methylphenidate(0.7mg/kg/d) during about 8 weeks. SPECT data reconstructed for the assessment of specific/nonspecific DAT binding ration of the basal ganglia were compared between before and after treatment methylphenioate. We investigated correlation between the change of ADHD symptom severity assessed with ADHD rating scale-IV and specific/nonspecific DAT binding ratio of the basal ganglia comparing to normal children (Right : z - 2.057, p = 0.041 : Left : z = 2.096, p = 0.032). Under treatment with methylphenidate in all chkildren with ADHD, specific/nonspecific DAT binding ration of both basal ganglia decreased significatnly greater than before treatment with methyphenidate (Right 1 = 3.239, p = 0.018, Left : t =3.133, p = 0.020). However no significant correlation between the change of ADHD symptom severity scores and specific/nonspecific DAT binding ratio of the basal ganglia ere for support the complex dysregulation of the dopaminergic neurotransmitter system in children with ADHD.

        The prominin-like Gene Expressed in a Subset of Dopaminergic Neurons Regulates Locomotion in Drosophila

        CD133, also known as prominin-1, was first identified as a biomarker of mammalian cancer and neural stem cells. Previous studies have shown that the prominin-like (promL) gene, an orthologue of mammalian CD133 in Drosophila, plays a role in glucose and lipid metabolism, body growth, and longevity. Because locomotion is required for food sourcing and ultimately the regulation of metabolism, we examined the function of promL in Drosophila locomotion. Both promL mutants and pan-neuronal promL inhibition flies displayed reduced spontaneous locomotor activity. As dopamine is known to modulate locomotion, we also examined the effects of promL inhibition on the dopamine concentration and mRNA expression levels of tyrosine hydroxylase (TH) and DOPA decarboxylase (Ddc), the enzymes responsible for dopamine biosynthesis, in the heads of flies. Compared with those in control flies, the levels of dopamine and the mRNAs encoding TH and Ddc were lower in promL mutant and pan-neuronal promL inhibition flies. In addition, an immunostaining analysis revealed that, compared with control flies, promL mutant and pan-neuronal promL inhibition flies had lower levels of the TH protein in protocerebral anterior medial (PAM) neurons, a subset of dopaminergic neurons. Inhibition of promL in these PAM neurons reduced the locomotor activity of the flies. Overall, these findings indicate that promL expressed in PAM dopaminergic neurons regulates locomotion by controlling dopamine synthesis in Drosophila.
