        대형 화재로 인한 사상자의 손상 유형과 합병증 : 동인천 라이브호프 화재를 중심으로

        Background: High risks of fire alway exist for buildings in urban areas, especially those in downtown. Crowds, as well as more complex and larger structures, may cause more victims in the event of fire; therefore, emergency medical service plans must be established for such disastrous events. Methods: On the evening of October 30,1999, a fire broke out in downtown, Dong-Incheon Live-Hof restaurant; 56 people were killed and 76 were injured. Most of them were teenagers. We retrospectively reviewed the medical records of the victims, Results: 1) Among the 56 dead, 54 died from smoke inhalation, one from extensive burns, and one from sepsis during treatment. 2) Among the 76 injured, 70 patients suffered from smoke inhalation, 53 from burns, and 9 from several types of trauma (sprains, contusions, lacerations, abrasions, fractures, etc.). 3) Later complications were laryngeal edema, pulmonary edema, scar contracture, and hypoxic brain damage, and so forth. 4) Post traumatic stress syndrome was unexpectedly more prevalent in mildly to moderately injured survivors and witnesses than in seriously injured survivors. Conclusion: Many complications exist after a fire. Some may be resolved in time, but others may result in permanent sequelae. Early rescue, early triage, and early management during transport by emergency medical service (EMS) personnel can result in fewer complications and a lower mortality rate. Therefore, we propose the establishment of plans to be followed during various major disasters.

      • 한국산 개불(Urechis unicinctus)의 순환적 생식주기(生殖週期)에 따른 체강세포(Coelomocyte)의 分化

        The coelomocyte is a free cell containing hemoglobin as a respiratory pigment in the large coelom of Urechis unicinctus. During the developmental stages of reproductive cells, especially the female generative cells, there are some evidences that the coelomocyte vary in its number and shapes. This phenomena suggest the fact that some substantial association is reinforced between the coelomocytes and the developing free reproductive cells. The structural changes of the coelomocyte observed by TEM and SEM during the seasonal reproductive cycle of the generative cells were investigated in this work attending to find a substantial relationship between differentiation of the coelomocyte and development of the free generative cells. And it was observed that the changes of coelomocyte, such as appearances of various inclusions and numerous exocytotic vesicles come from the increased activity of the coelomocyte during the reproductive cycle. The released exocytotic vesicles may contribute to the content of coelomic fluid, following changes of coelomic fluid constituents which come into directly contact with developing reproductive cells. Therefore, this work has paid attentions to the causal relationship between the structural changes of free somatic coelomocytes and the development of female reproductive cells in the coelom. The coincidences are interpreted.

        거대 청신경초종의 수술치료성적 : 수술전 안면신경전위의 예견 Preoperative Prediction of Facial Nerve Displacement by MRI

        A total of 22 patients(13 males and 9 females : mean age 49 years) with large acoustic neurinoma who underwent the retrosigmoid transmeatal approach from May 1993 to May )995 at the Chonnam university Hospital was reviewed retrospectively. The authors investigated the surgical results and accuracy in predicting the direction of displaced facial nerve which was determined by preoperative magnetic resonance(MRI) findings. In all cases the preferred method was the suboccipital transmeatal approach. The direction of displacement of the facial nerve could be predicted by preoperative axial and coronal MRI scans and verified intraoperatively. Gross total removal was performed in 82%, the accuracy rate for facial nerve displacement was 77% and anatomical preservation was accomplished in 82.3% of the totally removed cases. During follow-up good functional outcomes were achieved in 91% and fair in 45%. The authors conclude that such good surgical results can be attributed to advances in microsurgical technique and intraoperative facial monitoring. In addition preoperative prediction of the direction of displaced facial nerves has significantly reduced the incidence of severe facial nerve weakness.

      • Urechis unicinctus 난세포의 인공수정에 의한 구조적 변화

        The artificial insemination in Urechis unicinctus oocyte revealed some fine structural changes under electron microscope. The degradation of surface coat, retraction of microvilli, fusion of cortical granules with oocyte membrane are the events of fine structural changes after the fertilization. At fertilization, the cortical granules of oocyte are transported from the inner cortex toward oocyte membrane. After the fusion, the contents of cortical granules are discharged into the perivitelline space, the extracelluar space which in future develops into fertilization membrane. There are some indications that the fertilization occur on the microvillous tip in U. uicinctus. Under light microscope, one of the evidences on the fertilization was the formation of fertilization cone, which seemed to sperm entry site, on membrane of round mature oocyte.

      • Coevaporation에 의한 cd₁-xZn_(x)S 박막 제작 및 특성에 관한 연구

        In this paper, structural optical and electrical properties of Cd_(1-x)Zn_(x)S thin films prepared by coevaporation method were studied. Also, effects of ZnS mole ratio(x) to CdS on properties of thin film Cd_(1-x)Zn_(x)S/CdTe solar cells were investigated. When the ZnS mole ratio was less than 0.85, the crystal structure of Cd_(1-x)Zn_(x)S films was hexagonal with the c axis aligned perpendicular to the substrate. For x>0.85, however, the Cd_(1-x)Zn_(x)S films were grown with cubic zincblende structire. As the ZnS mole ratio increased the lattice constant of Cd_(1-x)Zn_(x)S films rapidly increased with ZnS mole ratio. The open circuit voltage of Cd₁xZnxS/CdTe solar cells increased with x due to reducing of the electron affinity difference between Cd₁xZnxS and CdTe films. However, the increase of series resistance due to the high resistivity of Cd_(1-x)Zn_(x)S films results on reducing conversion efficiency.

      • 冷間押出用 헬리컬기어 金型의 强度解析

        In forging die design, pre-stressing is useful in preventing excessive tensile circumferential stresses which may cause die failure during forming. The finite element analysis (F.E.A.) package ANSYS reversion 4.4 A was used to this study. F.E.A., that is, static analysis was accomplished for insert-dies of helical gear. Die model was constructed with 8 node isoparametric solid finite elements. Static load analysis was performed for internal and outer pressure. Also, stress distribution and deformed shapes are shown. To assure durability and reliability, failure analysis was calculated by maximum normal stress theory and distortion energy theory. Results of the F.E. analysis and failure analysis for Samanta and new process dies were compared. The results of F.E. Analysis were in good agreement with that of experimental results. It is expected that the results of this study can be applied to designing various kinds of extrusion gear die.

      • 표면 플라즈몬 공명 신호의 증폭을 위한 새로운 금 콜로이드 센서칩 제조와 특성 연구

        New gold colloidal film sensor chips were employed to enhance the sensitivity of the Surface Plasmon Resonce (SPR). Au collodal solutions were synthesized, and Au colloidal films were fabricated by immobilization of colloid on vacuum deposited Au film through the coupling agent of 1,8-diaminooctane. Specific interaction of human IgG antibody to human antigen was monitored by in situ SPR measurement from both a smooth and a Au colloidal films. Au colloidal film sensor chips resulted in 27 % enhancements effect for the same reaction on SPR angle shifts compared to a smooth Au film.

      • Diethyl 1-chloro-2-phenyl-2-(tosyl) aminoethylphosphonate 의 결정 및 분자구조

        The crystal and molecular structure of diethyl 1-chloro-2-phenyl-2-(tosyl)aminoethyl phosphonate has been determined from single crystal X-ray diffraction study ; C19H25NO5PCIS, Monoclinic, P2₁/a, a=10.8507(11)Å, b=16.1201(12)Å, c=12.7354(12)Å, β=90.192(7)°, V=2227.6(3)ų, T=293(2)K, Z=4, Cuk α(λ=1.54178Å). The structure was solved by direct method and refined by full-matrix least squares to a final R=5.17% for 2444 unique observed F0> 4σ(F0) reflections and 254 parameters.

        Background: This study evaluated the efficacy of administering Donepezil(Aricept^(?)) for improving cognitive function, activities of daily living, and quality of life in patients with post-stroke dementia. Methods: Patients(n=99; mean age, 69.89 years; 48.5% men) with post-stroke dementia, according to criteria of the DSM-IV and MRI, were treated with donepezil 5mg/day or 10mg/day during 24 weeks. They were examined using Mini-mental Status Examination(MMSE), Chinical Dementia Rating(CDR), Barthel Activities of daily living(B-ADL), Instrumental Activities of daily living(I-ADL), Short Form 36 Health Survey-Korean(SF-36-K), and Geriatric Depression Scale(GDS) at before treatment and 24 weeks after treatment. Results: The 97 patients completed the trial. At 24 weeks, they showed significant improvement in MMSE(p<0.001), CDR(p<0.001), B-ADL(p=0.002), I-ADL(p<0.001), and SF-36-K(p=0.003) except GDS(p=0.288). Donepezil was well tolerated. There was no withdrawal due to adverse events. Conclusion: This results demonstrate that donepezil was beneficial in improving the cognitive function, activities of daily living, and quality of life in patients with post-stroke dementia and was well toleraetd.

        Transthyretin의 Glutathione 자동산화 촉진작용

        When reduced glutathione(GSH) was incubated at neutral pH and at 37°, its concentration decreased slowly with formation of oxidized glutathione(GSSG). Autooxidation of GSH was accelerated by Cu^(2+) and Hg^(2+), but not by other common mono-, di-, and tri-valent cations. Transthyretin was found to stimulate autooxidation of GSH in the presence or absence of Cu^(2+) and Hg^(2+). EDTA inhibited perfectly the autooxidation of GSH regardless of the presence of transthyretin. The stimulating activity of transthyretin was maximal at pH 7.0, declining progressively with increase or decrease of pH from 7.0. Sulfhydryl-blocking agents such as p-hydroxymercuribenzoic acid and Nethylmaleimide markedly inhibited the stimulating activity of transthyretin. Transthyretin stimulated autooxidation of other sulfhydryl compounds such as clithiothreitol and cysteine. However, it did not show a significant effect on autooxidation of sulfhydryl group of egg albumin and eye lens proteins. And transthyretin did not cause any oxidative change to thyroxine(T₄), 3, 5, 3Ltri iodo thyronine(T₃) and 3, 3: 5~triiodothpnine(rT₃) bound to it in the presence of GSH and Cu^(2+). The above results suggest that transthyretin may play a role in regulation of oxidized status of sulfhydryl groups in blood plasma and cerebrospinal fluid
