        Gastritis cystica profunda (GCP) is a rare disease characterized by gastric foveolae elongation along with hyperplasia and cystic dilatation of the gastric glands extending into the tissue beneath the submucosa. GCP mainly develops at the gastroenterostomy stoma but can arise in a stomach that has not undergone surgery. The proposed pathogenesis is related to chronic inflammation, ischemia and the presence of a foreign body. GCP may present as a submucosal tumor, Polyp or a giant gastric mucosal fold. Endoscopic ultrasonography (EUS) is an effective diagnostic method for differentiating GCP from protruding and elevated gastric lesions. We report a case of GCP along with the endoscopic and EUS findings that were indistinguishable from a malignant submucosal tumor.

        변색 실활치에 대한 carbamide peroxide gel의 표백효과

        The bleaching of discolored nonvital teeth is conservative treatement that satisfy the cosmetic desire. The most common method for this treatement, walking bleaching, is using 30% hydrogen peroxide and sodium perborate. Many alternatives are suggested for preventing the external cervical root resorption that is the common complication of the nonvital teeth bleaching with 30% hydrogen peroxide. The same extent of oxidation reactions as that resulted by the bleaching with the application of 30% hydrogen peroxide and sodium perborate can also be acquired more safely by materials that contain 10% carbamide peroxide, used primarily for the bleaching of vital teeth. Therefore, this study was performed to evaluate the efficacy of 10% and 15% carbamide peroxide bleaching gel in nonvatal teeth bleaching. The internal bleaching of intentionally discolored teeth was performed in vitro with 10% carbamide peroxide (Group 1), 15% carbamide peroxide (Group 2), mixture of distilled water and sodium perborate (Group 3), and mixture of 30% hydrogen peroxide and sodium perborate (Group 4). The bleaching materials were refreshed following 3, 6, 9 and 12 days. To evaluate the bleaching effect, the color change of the crowns was measured at 1, 2, 3, 4, 7 and 15 days of bleaching using the colorimeter. The results were as follows : 1. L^* and ΔE^* values were increased with time in all bleaching agents(p<0.01). 2. There was no significant difference in L^* and ΔE^* value among bleaching agents. 3. Δ^* value higher than 3 was shown after 3 days of bleaching with 10% carbamide peroxide gel, 1 day with 15% carbamide peroxide gel, 4 days with mixture sodium perborate and distilled water and 4 days with mixture sodium perborate and 30% hydrogen peroxide, respectively. These results revealed that the use of 10% and 15% carbamide peroxide bleaching gel in non-vital teeth bleaching is as effective as mixture of distilled water and sodium perborate and mixture of 30% hydrogen peroxide and sodium perborate. Accordingly, carbamide peroxide could be used clinically to bleach discolored non-vital teeth.

        Inhibitory Effect of Clavicepitaceae on Serotonin Release out of Human Platelets and Human Platelet Aggregation

        We have investigated the effects of hypha-water extracts (HWE), fruit body-water extracts (FWE) and cordycepin from Cordyceps militaris on serotonin release out of human platelets and human platelet aggregation. HWE and FWE inhibited the release of [^(3)H]-serotonin from human platelet stimulated by thrombin (2 U/ml) or collagen (20 ㎍/ml) in a dose-dependent manner. Furthermore, cordycepin, a major component of Cordyceps militaris, inhibited the human platelet aggregation induced by collagen (10 ㎍/ml) in a dose-dependent manner. These results suggest that cordycepin containing in HWE and FWE may inhibit the serotonin release by suppressing the collagen-induced human platelet aggregation. Accordingly, our data demonstrate that HWE and FWE containing much cordycepin might have antithrombotic and antimigrainous functions.

      • Streptomyces albulus 세정균체에 의한 polylysine의 대량생산

        Streptomyces albulus 세정균체를 이용하여 항세균물질인 폴리리신의 대량생산 가능성에 대하여 조사하였다. 발효기 배양에서 배양액 pH는 6.8에서 3.2로 감소하는 동안에 pH 4.0에서 4.5사이에서 폴리리신이 최대로 축적되므로 대수말기의 균체를 멸균증류수로 세척하여 마련한 세정균체를 아미노산 생산을 향상할수 있는 구연산을 2%첨가하고 pH를 최적인 4.2로 조정한 배지로 옮겨 배양하였다. 이러한 2단계공정으로 세정균체를 배양한 경우 배양액내의 폴리리신은 일반 1단계 단순 배양에 비하여 최대 6배의 축적량을 나타내었다. The feasibility of mass production of polylysine, an antibacterial substance, employing a washed mycelium of Streptomyces albulus has been investigated. Since the maximum accumulation of polylysine had appeared between 4.0 to 4.5 of pH during fermentor cultivation of which the pH of the broth has declined to 3.2 from 6.8, the strain growing in the broth on late log phase had been washed with sterilized water, then the sole mycelium had been transferred to another medium containing 2% citric acid having pH adjusted with NaOH to optimum of 4.2 to improve the production of the amino acid. The total accumulation of polylysine by washed mycelium of Streptomyces albulus from this two step procedure has exhibited 6 times more than the normal production of the simple one step fermentation.

      • 혼합형 자가면역성용혈성빈혈과 동반된 Evans 증후군 1례

        Evans syndrome is defined as a simultaneous or sequential occurrence of autoimmune hemolytic anemia and autoimmune thrombocytopenia. Autoimmune hemolytic anemia (AIHA) is usually induced by IgG warm autoantibody or cold-active IgM antibodies reacting specifically with antigens associated with a patient's RBC. AIHA is a fairly uncommon disorder, with estimates of the incidence at 1 to 3 cases per 100,000 per year. Mixed-type AIHA is a relatively uncommon form of AIHA, with studies noting the incidence of 7-8% among cases of AIHA. We experienced a patient, 46-year-old woman who was diagnosed having a very rare clinical presentation of mixed warm and cold antibody mediated Evans syndrome. She was treated with corticosteroid therapy only and has been maintaining a complete response for 15 weeks. 저자들은 혼합형의 자가면역성 용혈성 빈혈과 자가면역성 혈소판 감소증이 동시에 발생한 혼합형 Evans 증후군으로 진단하고 스테로이드 요법 후 혈액학적으로 회복된 상태로 15주가 지난 현재 steroid 5 mg/일 까지 감량한 상태에서 추적 관찰 치료중인 46세 여자 환자를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

        복합레진의 조건등색에 관한 연구

        This study was done to present a criterion in selection of the most proper light sources and materials by measuring metamerism index(MI) of the light curing composite resins with spectrocolorimeter. Metamerism is defined when two objects appear to be the same color in one illuminant but different in another. This is due to the fact that they have different spectral curves that fail to match under the second illuminant. In this study, A1 & A3 shade of five light curing composite resins (Esthet-X, Filteck Z250, Filteck A110. Charisma. Vitalescence) were chosen based on Vita shade. Five samples were made for shade of each product with Teflon mold (diameter: 15mm, thickness: 2mm). Metamerism index of each samples on a Barium sulfate plate(L^(*)=96.54, a^(*)=0.19, b^(*)=0.01) prepared for sample fixation were measured with spectrocolorimeter(Miniscan XE plus. Model 4000s. Hunter Lab. USA) by applying standard light source D_(65), C. Fcw, TL84 and A. Standardization was done with reference standard(X=80.8, Y=85.7, Z=90.8) and light trap. The results were as follows. 1. Different resins with same Vita shade showed recognizable color difference (ΔE^(*)>2). 2. All composites had MI below accepted value 0.5 between standard illuminant(D_(65), C, & A) and below 1.5 under fluorescent condition (Fcw & TL84). 3. MI value between D_(65) and A showed higher value than MI value between other source of light(p<0.01). 4. All resins except Z250 showed MI value that A3 is higher than A1 between D_(65) and A(p<0.05).

      • 폐외소세포암

        Background: Small cell carcinomas (SCC) are most commonly seen in the lung, approximately 2.5-4% of all SCC occur in extrapulmonary sites. This study aimed to review the clinical features, therapy and natural course of patients with extrapulmonary small cell carcinoma (EPSCC) in single-institution series. Methods: We retrospectively reviewed the medical records of patients with SCC between December 2002 and November 2005. Study eligibility required that patients had pathologically proven SCC in sites other than lung with normal radiological findings of the chest and normal sputum cytology or negative bronchoscopic findings. Result: Eight patients with EPSCC were identified and their primary sites were various such as esophagus (4 cases), kidney (1 case), thymus (1 case), stomach (1 case) and unknown primary supraclavicular lymph node (1 case). The 4 patients were classified as limited disease (LD) and 2 patients were as extended disease (ED) at initial staging work-up. But 2 patients were undetermined. The 3 patients with EPSCC of LD were treated with operation and adjuvant chemotherapy or chemotherapy and radiotherapy and one received only supportive care. They showed favorable clinical courses with a median overall survival of 23 months. EPSCC of ED and undetermined group received only supportive care. They had aggressive clinical courses with a median overall survival of 3 months, Conclusions: EPSCC was identified in various sites with the most common primary site being the esophagus, It was predominantly occurred in old male patients and associated with smoking like as SCC of the lung. Since EPSCC is a relalively rare disease that mimics SCC of the lung in its response to treatment and survival pattern, it would appear advisable to follow similar treatment guidelines. Although recurrence within short terms was observed, a significant survival benefit was obtained in EPSCC of LD received treatment (chemotherapy, operation and radiation therapy). Thus, EPSCC is usually a fatal disease with meidan overall survival of 18 months. 연구배경: 소세포암의 가장 흔한 발생부위는 폐이며, 폐외소세포암은 전체 소세포암의 2.5~4%를 차지하는 매우 드문질환이다. 본 연구의 목적은 단일 기관에서 진단된 폐외소세포암의 임상적 특징, 치료와 자연경과를 보고하고자 하였다. 방법: 조선대학교부속병원에서 2002년 12월 부터 2005년 11월 사이에 소세포암으로 진단된 66명의 의무기록을 검토하였다. 폐외소세포암은 흉부 단순 몇 전산화단층 촬영, 객담세포 검사, 기관지경 검사에서 폐병번이 없으면서 폐와 병변의 조직학적 생검에서 소세포암으로 증명된 경우로 하였으며, 총 8명이였다. 결과: 본 연구에서 식도가 전체 폐외소세포 암 환자의 50%(4례)로 가장 많은 원발병소를 보였으며 그 외 흉선, 신장, 위가 1례씩 있었으며 쇄곧상부 림프절에서 소세포암이 발견되었으나 원발 병소를 발견 못한 경우가 1례 관찰되었다. 4명의 환자는 제한 병기를 보였으며 2명은 광범위 병기를 보였지만 나머지 2명은 위내시경으로 식도에서 소세포암이 조직학적으로 진단되었으나 보전적 치료만을 받길 원하여 병기결정을 위한 검사는 시행할 수 없었다. 제한 병기의 폐외소세포암 환자는 4명이었다. 2명은 수술적 절제후 보조항암화학요법을 받았으며, 1명은 항암화학요법후 방사션치료를 받았고, 나머지 한명은 보전적 치료만 시행하였다. 제한 병기의 폐외소세포암 환자틀은 양호한 임상 경과를 보였으며 중앙 생존기간은 23개월이었다. 하지만 광범위 병기의 폐외소세포암 환자와 병기를 분류할 수 없던 환자에 대해서는 항암화학요법 등의 적극적 치료는 시행하지 않고 보존적 치료만 시행하였다. 그들의 임상경과는 빠르게 악화되었으며 중앙 생존 기간은 3개월이었다. 결론: 본 연구에서 폐외소세포암은 다양한 장기에서 발견될 수 있으나 가장 흔한 곳은 식도였다. 소세포폐암과 유사하게 고령, 남성 및 흡연과 관련을 보였었다. 제한 병기에서 국소 치료, 항암화학요법은 생존기간의 연장을 보였으나 쉽게 다른 장기로 전이가 되거나 재발하는 매우 치명적인 질환임을 확인할 수 있었다. 전체 중앙 생존기간은 18개월 이었고 항암 화학요법등의 치료를 받은 군이 보존적 치료를 받은 군에 비해 의미있는 생존기간 (p=0.040)의 연장을 보였다. 본 연구는 단일기관의 상대적으로 적은 수의 소규모 연구이지만 폐외소세포암의 제한 병기에서 국소치료 및 항암화학요법은 생존기간의 연장을 보였고 쉽게 다른 장기로 전이가 되거나 재발하는 매우 치명적인 질환임을 확인 할 수 있었다.

        There were problems in selecting exact colors of composite resins currently used in clinic by examining with naked eyes. This study was done to present a criterion in selection of the most proper light sources and materials by measuring metamerism index of the light curing composite resins with spectrocolorimeter. Metamerism is present when two objects appear to be the same color in one illuminant but different in another. This is due to the fact that they have different spectral curves that fail to match under the second illuminant. In this study, five light polymerized composite resins; Esthet-X, Filteck Z250, Filteck A110, Charisma, Vitalescence which are currently used in clinic were chosen based on Vita shade. Five samples were made for shade of each product with Teflon mold(diameter: 15mm, thickness: 2mm). Compressed resins with glass slab were light polymerized with a visible light curing unit(Optilux 500, 13mm tip diameter, Demetron, USA) for 60 seconds. Polymerized samples were removed and those were grinded to 2.0mm in thickness with sand papers(#600 & #1000). Samples were carefully prepared without fingerprints and impurities in grinding procedure and kept in distilled water for a week at room temperature. Metamerism index of each samples on a Barium sulfate plate(L^(*)=96.54, a^(*)=0.19, b^(*)=0.01) prepared for sample fixation were measured with spectrocolorimeter(Miniscan XE plus. Model 4000s. Hunter Lab, USA) by applying standard light source D65, C, Fcw, TL84' and A. Standardization was done with Reference standard(X=80.8, Y=85.7, Z=90.8) and Light trap. Samples were kept in distilled water at room temperature in all procedures. The results are as follows. 1. Different resins using same Vita shade showed recognizable color difference(ΔE^(*)>2). 2. All composites had Ml below accepted value 0.5 between standard illuminant(D65, C, & A) and below 1.5 under fluorescent condition (Fcw & TL84). 3. Ml value between D65 and A showed higher value than Ml value between other source of light(p<0.01). 4. All resins except Z25O showed Ml value that A3 is higher than Al between D65 and A(p<0.05).

      • 개에서 발생한 편충 감염 증례

        A 4-year-old, intact male, Jindo-dog was admitted to the Veterinary Medical Teaching Hospital of Chungnam National University with the history of diarrhea several times a day the onest of which was a month ago and weight loss. She was not prevented against intestinal parasites and heartmorm since 1 year ago. On physical examination, she was emaciated and had abdominal tension. A Giardia kit test indicated negative result. Results from a serum biochemical profile and electrolytes demonstrated mild reduction in phosphate and sodium. Microfilaria was detected on direct blood smear. A Heartworm kit test indicated positive result. Thoracic radiographs revealed dilation of pulmonary vessels, increased opacity of caudal lung field, and reversed D sign. Abdominal radiographs revealed loss of abdominal serosal details. A abdominal ultrasonograph showed the following: dilation of the intestinal lumen, hyperechoic linear material with acoustic shadowing in bowel, and enlarged lymph node. Fecal examination using saturated saline flotation method indicated that she had trichuris vulpis infection. Anthelmintics were administered orally, and antibiotics were administered parenterally. In addition, heatworm treatment was advised. She had no significant clinical signs such as diarrhea after ten days and five months.

        Inhibitory Effect of Cordycepin on Human Platelet Aggregation

        Cordycepin separated from Cordyceps militaris is a major physiologic active component in Cordyceps militaris. The platelet aggregation is stimulated by Ca^(2+), which is either mobilized from intracellular endoplasmic reticulum or transported from extracellular space. cGMP antagonizes the actions of Ca^(2+). Based on these facts, we have investigated the effects of cordycepin on the mobilization of Ca^(2+) and the production of cGMP on collagen (10 ㎍/ml)-induced human platelet aggregation. Cordycepin potently stimulated the human platelet aggregation induced by collagen (10 ㎍/ml) in a dose-dependent manner. Cordycepin (500 μM) inhibited also the collagen-induced human platelet aggregation in the presence both 1 mM and 2 mM of CaCl_(2). These are in accord with the results that cordycepin inhibited the Ca^(2+)-influx on collagen-induced human platelet aggregation. These results suggest that cordycepin decrease the intracellular Ca^(2+) concentration to inhibit collagen-induced human platelet aggregation. Besides, cordycepin increased the level of cGMP on collagen-induced human platelet aggregation. This result is related with the decrease of intracellular Ca^(2+) concentration, because cGMP inhibits the mobilization of Ca^(2+). In addition, cordycepin inhibited the human platelet aggregation induced by LY-83583, inhibitor of guanylate cyclase. This result suggested that cordycepin inhibit the platelet aggregation by stimulating the activity of guanylate cyclase. In conclusion, we demonstrated that cordycepin might have the antiplatelet function by inhibiting Ca^(2+)-mobilization via the stimulation of the production of cGMP.
