        부신피질 자극호르몬 단독결핍 1례

        부신피질 자극호르몬 단독결핍은 국내에서 7례만이 보고된 희귀 질환이며 병인 및 경과 등 아직 불분명한 측면이 많다. 이는 질환지체의 희귀성도 있지만 치료후 자연경과에 대한 장기적인 추적검사가 이루어진 경우가 드물기 때문이기도 하다. 저자들은 비특이적인 전신증상을 주소로 내원한 폐경기후의 56세 여자 환자에서 자가면역질환의 증거없이 고프로릭틴혈증을 보인 부신피질 자극호르몬 단독결핍 1례를 경험하고 1년간의 부신피질호르몬 대치요법후 추적검사를 실시하였기에 문헌 고찰과 함께 보고하는 바이다. Isolated ACTH deficiency is a rare disorder which shows secondary adrenal insufficiency with normal states of other pituitary hormones. A 56-year-old postmenopausal woman complaining general weakness and intermittent mental confusion presented negligible serum cortisol and ACTH level with hyperprolactinemia and mild hyponatremia. Rapid ACTH stimulation test did not show significant increase of serum cortisol level. Combined pituitary stimulation test showed negligible response of cortisol and ACTH, and slightly decreased response of prolactin and gonadotropin. But the responses of TSH and GH were normal. There was no evidence of organic pituitary of hypothalamic lesion in brain MRI and antithyroid autoantibody was not detected in serum. After receiving 12 months of glucocorticoid replacement therapy, she skipped the medicine for 3 weeks and we performed follow-up endocrinologic tests. Serum ACTH and cortisol level was also negligible and prolonged ACTH simulation test showed normal response of serum cortisol. CRF stimulation test showed no response of serum ACTH. Basal prolactin level was decreased to near normal and combined pituitary stimulation test showed normal response of prolactin with slightly blunted response of TSH and GH. We concluded that the case was nonautoimmune origined isolated ACTH deficiency which was accompanied by hyperprolactinemia and resultant deficiency of gonadotropin, so we report this case with review of literatures.

      • Beam形 Load Cell의 FEM Simulation과 그 電氣的特性

        In this paper, we simulate and calculate the stress and output voltage of the beam structure load cells by using FEM as varing physical structure parameters and loading positions. It is proved that stress enhance as the increase of the notch pitch and radius of the load cell, but decrease as the increase of the notch thickness and beam width. The results are good matched for basic formulas of the single fixed beam, and are verified our simulation is correct. Also, it is found that the stress characteristics of the load cell is varied according to loading positions with structure parameters, and caculated output voltage of the load cell approximate to those of the real manufactured ones. As a result, this study will offer efficient design and analysis technique for making special and variety capacity of load cells.

        만성 위염전 2 례

        위염전은 위의 비정상적인 회전으로 생기는데 급성증에서는 진단이 늦어질 경우 치명적일 수도 있으나, 만성증의 경우는 비특이적 위장관 중상을 나타내며, 치료되지 않을 경우 수년 동안 증상이 지속될 수도 있다. 최근 저자들은 소화불량, 식후 상복부 동통을 주소로 내원한 환자에서 만성위염전 2 례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Gastric volvulus is an abnormal rotation of the stomach. Gastric volvulus may present as an emergency or as a chronic condition. Acute volvulus si potentially lethal if unrecognized, while a chronic volvulus may cause symptoms for years if not treated. Gastric volvulus may be more common than previously estimated. Approximately 700 cases have thus far been documented. In patients with vague upper abdominal pain, bloating, vomiting, early satiety and weight loss, chronic gastric volvulus are reported and the related literature reviewed. The clinical presentation, diagnosis, and treatment are treatment are discussed. Features of acute and chronic volvulus are compared.

        To make high speed spindle system work properly, sensors with outstanding resolution and dynamic characteristics are essential. An optical fiber displacement sensor is based on simple principles. Electrical signal responds to the optical flux change due to the displacement change between a target and a sensor probe. In this paper, the performance of optical fiber displacement sensor has been investigated according to properties of optical fiber. Firstly, optical loss has been measured before and after polishing optical fiber endface. Secondly, allowance of optical fiber bending has been tested. thirdly sensitivity and linear range of the sensor has been found out according to the shape of cross section of optical fiber.

      • 장거리선수의 운동시 심전도 파고 및 시간간격에 관한 고찰

        This study Attempts to that Heart Functional know Long-distance Athlete by Electrocardiographic Time Interval and Wave Amplitudes. Fourteen Male Subjects aged Between 21-23years were devided into Athete and Control Groups. Pre-execise, Execise and Recovery Mode was performed on a Bicycle-ergometer. Load of Execise Mode was 1Kp/3min, 2Kp/3min, 3Kp/3min. ECG's were traced in LoadⅡ,V₁ and V? to Pre-execise, Execise and Recovery. ECG interval were measured from LeadⅡ and ECG amplitudes from LoadⅡ, V₁ and ??. The results Obtained are Aummarized as Follow : 1. ECG interval ; 1) The R-R interval at Pre-exercise Mode was 1.121±0.181sec in Athlete group and 0.862±0.148sec Control group. Athlete group was Sinus Bradycardia. 2) The P-R interval at Pre-exercise Mode was 0.174±0.018sec in Athlete group and 0.487±0.048sec Control group. 3) The QRS interval at Pre-exercise Mode was 0.093±0.009sec in Athlete group and 0.018±0.009sec Control group. 4) The QRS interval at Pre-exercise Mode was 0.512±0.077sec in Athlete group and 0.409±0.019sec Control group. 2. ECG amplitude ; 1) The P wave Amplitude at Pre-exercise Mode was 0.871±0.304mm Athlete group and 0.801±0.162mm Control group. The Amplitude was Increaser the Athlete group than Control group. 2) The R wave Amplitude at Pre-exercise Mode was 26.61±4.63mm Athlete group and 19.84±3.87mm Control group. The Amplitude was Increaser the Athlete group than Control group. 3) The S wave Amplitude at Pre-exercise Mode was 13.32±2.14mm Athlete group and 10.61±1.03mm Control group. The Amplitude was Increaser the Athlete group than Control group. Exercise Mode was enlarged Change Form Time Interval and Amplitude in Control group and Recovery Mode Time Interval and Amplitude in Athlete group rapided Rahabilition and the Sum of Amplitude in ?? and SV₁ in Athlete group was 40.00mm suggesting a Left Ventricular Hypertrophy.

        This paper is to develop an optical fiber displacement sensor for monitoring high speed spindle. Proper magnitude of optical power as well as amplification of output signal are necessary to improve sensitivity of the sensor. In this paper, to meet the need of improvement of the sensor resolution, we choose proper optical power and amplification level through speculating on optical power of a laser diode.

      • 오스템퍼 처리한 구상흑연주철의 드릴가공 특성과 공구수명에 관한 연구

        Drilling tests were carried out austempered ductile cast iron(ADI) to clarify the factors influencing the drilling chracteristics of ADI material. The machinability of material was evaluated using high speed steel drill and cobalt contained drill of 6mm diameter. The spherodal graphite cast iron materials were austenized at 900℃ for in hour and then wear was kept at 375℃ for 2 hours. Austempered ductile cast iron contains a great deal of retained austenite which contributes to an improvement of impact strength. In this paper, machinability of ADI was investigated by drilling experimentation. The results obtained are as follows: a) Flank wear increases logarithmically with the increases of cutting time. b) Relation of flank wear and cutting force can be applied to FZ = 925VB + 820 for the cutting suggested condition. c) Drilling hole number of about 2 times can be reduced more step feed than ordinary feed due to the high hardness of ADI material and hardness increasing ascribed to the martensite of retained austenite.

        Compensation of temperature is very important to make high precision load cell. In this study, we developed a new type load cell. The structure of the load cell has our strain gauges on single side of the load cell beam. Also a new temperature compensation method was proposed and these characteristics were better than previous one. This study will offer application to other type of load cell and another sensors.

      • 마찰력과 열처리에 따른 아크용사 및 아크용접된 Fe-Cr-B 합금 피복층의 변화

        Variations of arc thermal sprayed and arc welded Fe-Cr-B alloy coating with frictional loading and heat treatment were investigated by means of X-ray diffraction, optical microscopy, scanning electron microscopy, electron probe microanalysis and hardness test. The as-coated layers were shown to be crystalline state but peaks with the frictional loading largely disappeared in X-ray diffraction profiles. The splat is relatively wavy with the boundary in arc thermal spray coating. After heat treatment, the amount of porosity was found to be decreased and the intersplat distance was increased. As annealing temperature increased, interface layer for arc welding coating increased by the diffusion of Cr atoms. The wear type observed in arc thermal spray coatings is splat delamination. The hardness of the arc thermal spray coation layer was superior to that of the arc welding layer. Hardness of coated layer is reduced and microstructures become to be uniformed by the heat treatment.
