        Clinical Research Article : The effect on respiratory mechanics when using a Jackson surgical table in the prone position during spinal surgery

        Background: Respiratory dynamics may be monitored and evaluated indirectly by measuring the peak inspiratory pressure and plateau pressure. In this study, the respiratory dynamics of patients undergoing spinal surgery using a Jackson surgical table were observed with a device after converting their position from supine to prone. The effects of the dynamic compliance and airway resistance were observed from the changes in peak inspiratory pressure and plateau. Methods: Twenty five patients were selected as subjects scheduled to undergo lumbar spine surgery. After intubation, the patients were ventilated mechanically with a tidal volume of 10 ml/kg and a respiration rate of 10/min. Anesthesia was maintained with sevoflurane 1.5%, nitrous oxide 2 L/min and oxygen 2 L/min. The peak inspiratory pressure, plateau pressure, resistance, compliance, arterial oxygen tension, carbon dioxide tension, heart rate and arterial blood pressure were measured at 10 minutes after the induction of anesthesia. These parameters were measured again 10 minutes after placing the patient in the prone position. Results: The prone position did not significantly affect the arterial oxygen tension, carbon dioxide tension, blood pressure and heart rate, but significantly increased the peak inspiratory pressure and resistance and decreased the dynamic compliance. Conclusions: The peak inspiratory pressure was increased using a Jackson surgical table to minimize the abdominal pressure when converting from the supine to prone position. This might be due to a decrease in lung and chest compliance as well as an increase in airway resistance. (Korean J Anesthesiol 2010; 59: 323-328)

        A Dynamic Cell Clustering Algorithm for Maximization of Coordination Gain in Uplink Coordinated System

        <P>We propose a dynamic clustering algorithm for maximizing a coordination gain in the uplink coordinated system. The dynamic clustering algorithm configures clusters periodically to adopt the change of the channel environment. Since the greedy-search clustering algorithm does not guarantee cell-edge users' performance and the full-search clustering algorithm (FSCA) is highly complex, we define a coordination gain between the coordinated communication system and the single-cell communication system, as a new parameter, to maximize the benefit of the coordinated communication system. We develop a maximum coordination gain (MAX-CG) clustering algorithm to maximize the coordination gain and an interference weight (IW) clustering algorithm to reduce complexity and guarantee the data rate of cell-edge users. Simulation results show that the MAX-CG clustering algorithm improves the average user rate and the 5% edge user rate. The IW clustering algorithm improves the 5% edge user's rate, guarantees the fairness among the cells, and reduces the complexity to only half of the existing algorithm.</P>

        A TR-MISI Serial Prefilter for Robustness to ISI and Noise in Indoor Wireless Communication System

        <P>We propose a time reversal and minimum inter-symbol interference (TR-MISI) serial prefilter for indoor wireless communication system. Proposed TR-MISI serial prefilter minimizes inter-symbol interference (ISI) and makes received peak power be robust to noise. Since the TR prefilter does not minimize the ISI and the joint zero-forcing and TR prefilter does not guarantee the performance in low signal-to-noise ratio (SNR) environment, we consider these two facts in designing an optimization problem for proposed TR-MISI serial prefilter. We use the semidefinite relaxation to solve the optimization problem and get a near-optimal solution. The simulation results show that proposed TR-MISI serial prefilter improves the bit error rate performance at overall SNR.</P>

        인공와우이식 아동과 건청 아동의 음성 분석 비교

        The purpose of this study was to compare the acoustic voice outcomes of children with cochlear implant to those of children with normal hearing. Participants were 41 children using unilateral cochlear implant (18 males and 23 females), and children with normal hearing from the same age and sex. Mean age of implantation was approximately 3 years old, mean duration of implant use was 4 years in CI group. Acoustic analyses were performed using MDVP of CSL. Speech samples were 3 sustained vowels, /a, i, u/. 9 parameters (F0, Fhi, Flo, Jitter, Shimmer, vF0, vAm, NHR, and SPI) were analyzed. Children with CI did not show the significant differences in those parameters after the vowel /a/ phonation. Meanwhile, there were significantly different results in F0, Fhi, vF0, and SPI after /i, u/ phonation. These results revealed that differences of voice characteristics in children with CI compare to children with NH persist regarding vowel context. It suggests that high vowels would recommend as speech samples for acoustic evaluation. Futhermore perceptual analysis and speech therapy for phonation control would be necessary for children with CI.

        청각장애아동의 말명료도에 영향을 미치는 화자 요인

        본 연구는 청각장애 아동의 말명료도에 영향을 주는 화자 변인(생활연령, 청각장애의 진단시기, 언어능력, 조음능력, 청력손실의 정도, 교육기간)들과 말명료도의 관계를 살펴보고, 이들 중 말명료도에 가장 큰 영향을 주는 변인을 찾아보는 것을 목적으로 하였다. 연구의 대상은 5세에서 10세 사이의 중고도(70 dB 이상) 청각장애 아동 20명으로, 이들에게 낱말명료도검사를 실시하여 말명료도를 측정하였다. 연구결과 생활연령, 조음능력, 언어능력, 청력손실의 정도가 말명료도와 상관관계가 높게 나타났으며, 그 중 자음정확도로 측정한 조음능력이 말명료도에 가장 큰 영향을 주는 것으로 나타났다. 이러한 결과에 대해 청각장애 아동의 말명료도에 영향을 주는 요인들에 관련하여 논의하였다. This study aimed to evaluate the relationship between the speech intelligibility and related factor s of speaker in hearing impaired children using hearing aids. For the purpose of the study, the following 6 factor s were selected as the related factor s of speaker based on the previou s studies : (1) chronological ages, (2) the fir st diagnostic date of hearing los s, (3) language ability , (4) articulation ability , (5) the degree of hearing los s, and (6) duration of education . T he subject s were 20 prelingually hearing impaired children between the ages of 5 and 10 who have been using oral communication mode everyday . T heir hearing losses ranged from 70 dB to 130 dB in better ear . Speech intelligibility , articulation ability and language ability were measured by u sing word intelligibility test , PCC (Percent of Con sonant Correct ) and two test s (reception : Korean Peabody Picture Vocabulary T est ; expres sion : Korean Boston Naming T est ), respectively . T he result s of the study were as follows : (1) the speech intelligibility had a strong positive correlation with three related factor s of speaker s : the articulation ability (r = .87), the receptive language ability ( r = .66), the expres sive language ability factor (r = .62); (2) the speech intelligibility had a negative correlation with the degrees of hearing loss (r = - .51) at the level of .01; and (3) the articulation ability alone explained the 76 % of speech intelligibility . T hese result s suggest that the articulation ability factor appeared to be most influential among the 6 related factor s of the speaker , which affect s the speech intelligibility of prelingually hearing impaired children with hearing aids on . From the clinical per spective, the result s also suggest that the treatment focused on improving the articulation ability could help to enhance verbal communication ability of hearing impaired children .

        Pseudohypoplasia of Right Coronary Artery in a Korean Female Cadaver

        심장동맥 협착이 임상적으로 중요한 지표로 사용되고 있지만, 형성부전은 선천적인 요소를 갖고 있어 의미가 있다. 혈관에서 거짓형성부전은 반대쪽 혈관 직경에 비해 작은 직경을 갖지만 어떤 형태학적 검사에서도 형성부전의 증거를 보이지 않는 것으로 정의할 수 있다. 온창자간막동맥, 왼척추동맥 형성부전이 동반된 82세 여자 시신의 해부과정에서 오른심장동맥 거짓형성부전이 관찰되었다. 맨눈으로 볼 때 형성부전으로 생각되었지만, 오른심장동맥은 정상적인 크기와 조직 구성을 보였다. 반면 왼심장동맥 주 가지는 동맥경화로 인한 확장을 보였다. 결과적으로 이 시신에서 관찰된 오른심장동맥 형성부전은 왼심장동맥의 동맥경화성 동맥류에 기인한 거짓형성부전임을 알 수 있었다. 많은 임상 상황들이 맨눈에 의존하여 판단하게 되므로 심장동맥에 대한 보다 정확한 해부학 지식을 통해 그릇된 접근을 예방할 수 있어야 할 것으로 생각한다. Stenotic or hypoplastic coronary arteries occupy vital features in the clinical situations. The pseudohypoplasia might be defined as a macroscopic hypoplasia based on the diameter of an artery compared with contralateral artery but is not hypoplasia both anatomically and histopathologically. During a routine dissection course, a pseudohypoplasia in right coronary artery was recognized in an 82-year-old Korean female cadaver who had a common mesenteric trunk and a fibromuscular dysplasia in the left vertebral artery. Although macroscopic hypoplasia was discovered, the right coronary artery had normal anatomical features and the left coronary artery showed atherosclerotic changes on the main trunk. The right coronary artery was misread to have hypoplasia due to atherosclerotic aneurysm on left coronary artery. Since macroscopic hypoplasia in radiological and surgical situations may lead to confusion to interpret its pathophysiology just like this case, the better anatomical knowledge on the coronary artery can help to prevent misleading approaches.

        대두부산물의 지방세포분화 유도유전자의 발현저해 및 전지방세포 분화 억제 효과

        대두는 여러 종류의 파이토케미컬을 함유하고 있으며 이에 의한 항산화, 항염증, 항비만 효능 등이 있음이 잘 알려져 있다. 대두 부산물 성분의 비만억제효능을 확인하기 위하여 Oil-Red O 염색법과 정량 PCR을 통하여 지방세포분화억제 및 지방생성억제 효능을 분석하였다. 분화유도물질인 isobutylmethylanthine (IBMX), dexamathasone 및 insulin 처리에 따른 지방세포 분화유도와 함께 대두부산물-침지수, 순물, 두유-의 처리는 세포손상 없이 지방세포분화 연관 유전자-PPARγ, Fabp4, Scd1, adipsin, apolipoprotein (APOE), adiponectin (ADIPOQ)-의 발현을 감소시키는 효능을 보였다. 또한 대두에서 잘 알려진 isoflavone -daidzein과 genistein- 분석을 통하여 대두 부산물에 포함된 두 종류의 isoflavone의 함량을 확인하였다. 결과적으로 대두부산물에는 지방세포분화유도 유전자의 발현억제를 통한 전지방세포의 분화 및 지방세포의 지방생성을 억제하고 있음을 확인하였다. 따라서 두부생산 시 발생하는 대두부산물은 지방세포분화 억제 효능을 바탕으로 이를 이용하는 건강기능식품제조에 활용될 수 있을 것으로 판단된다. Soybean is known to contain various phytochemicals that are related to anti-oxidant, anti-inflammatory and anti-obesity effects in mice and humans. The anti-obesity effect of by-product extracts from soybean on the differentiation of 3T3-L1 pre-adipocytes to adipocytes was investigated by suppressing adipocyte differentiation and lipid accumulation with Oil Red-O assay and quantitative PCR. In inducing differentiation of 3T3-L1 pre-adipocytes in the presence of an adipogenic cocktail, isobutylmethylanthine (IBMX), dexamathasone, and insulin, treatment with filtrated soybean soaked water, soybean milk, and soycurd residue from soybean curd processing significantly decreased mRNA expression of obesity-related gene such as PPARγ, Fabp4, and Scd1, adipsin, apolipoprotein (APOE) and adiponectin (ADIPOQ) without any significant cytotoxicity. We also determined the well-known isoflavones in soybean, such as daidzein and genistein, in the by-product extracts. Taken together, we suggest that soybean by-product extract showed anti-obesity effect by suppressing adipocyte related gene expression, and that by-products collected during soybean curd processing may be a good candidate as an ingredient in health care products.

        Newborn Hearing Screening Test: A Comparison between Infants in Neonatal Intensive Care Unit versus Nursery

        Objective: The purpose of this study is to compare the clinical characteristics of newborn hearing test results between infants in well-baby nursery (WBN) versus neonatal intensive care unit (NICU) and to determine the optimal strategies of hearing screening for each group. Methods: A total of 2,161 newborns admitted to WBN and NICU were enrolled in this retrospective cohort study. Automated audio brainstem response was used for hearing screening test. The screening rates, referral rates, prevalence rates, false positive rates and predictive positive value (PPV) were analyzed for infants in WBN and NICU. Results: The referral rates of the screening test were 2.2% and 5.0% in WBN and NICU groups (P<0.01); the prevalence rate of congenital hearing loss (HL) were 0.5% and 4.6% in the WBN and NICU groups (P<0.01). The false positive rates and PPV of screening test were 0.9% and 36.8% in WBN; 0.8% and 80% in NICU. Infants with bilateral refer results in both WBN and NICU were at the higher risk of congenital HL, compared with infants with unilateral refer results in WBN who were mostly confirmed to have false positive. Conclusion: The infants in WBN showed high unilateral referral rate with low prevalence rate of HL. However, the infants with bilateral refer results in WBN and uni- or bi-lateral refer results in NICU had higher prevalence rate of HL, compared with those with unilateral refer results in WBN. Different strategies for hearing screening might be considered in infants in WBN and NICU.
